Purpose: To report a successful eyelid reconstruction performed using the residual tarsus after excision of a sebaceous gland carcinoma. Case Report: An 86-year-old woman presented with a sebaceous gland carcinoma of the upper eyelid margin. After excision of the tumor, she underwent reconstructive surgery of the superior eyelid performed using the residual tarsus. Because the tumor was localized on the upper lid margin, a tarsus of about 4–5 mm in height remained after the excision; we therefore used the residual tarsus to reconstruct the upper eyelid. No functional or cosmetic problems arose as a result of this method. Conclusions: Reconstructive surgery of the upper eyelid using the residual tarsus may be a viable option provided that surgeons can ensure a sufficient safety margin, and that more than 4 mm of the tarsus remains.

ABSTRACT Introduction Sebaceous gland carcinoma (SGC) is a rare tumor affecting the elderly, with a predisposition for females arising from the meibomian glands and occurring more commonly on the upper eyelid. Diagnosis is difficult because the tumor mimics chalazion or blepharitis. Sebaceous gland carcinoma has a mortality rate of about 5 to 10%. Aim To present a modified Cutler-Beard technique of lid reconstruction to manage a case of extensive SGC of upper lid. Materials and methods A 65-year-old male presented with a rapidly growing extensive mass of right upper eyelid (size 4.2 × 4 × 2.1 cm) causing mechanical ptosis. Histopathology confirmed the diagnosis as SGC. Wide excision of the lesion was performed sacrificing the whole upper eyelid. Lid reconstruction was done employing lower eyelid as per the bridged flap technique with the use of 4 mm silicon band to enhance lid stability. Patient achieved a satisfactory functional and cosmetic result following the second stage of the procedure. Conclusion Total loss of upper eyelid is often dealt with classical lid sharing technique of reconstruction first described by Cutler-Beard. For lid stability, use of tarsus from contralateral eye, ear cartilage has their attendant problems. A 4.0 silicon band was used to replace the sacrificed tarsus, achieving good results. Sebaceous gland carcinoma of ocular adnexa is a highly malignant but relatively rare condition. It has an aggressive course with tendency for local invasion and systemic metastasis resulting in poor outcome and increased mortality. Here, we describe the clinical course of recurrent sebaceous gland carcinoma of the upper eyelid in a 55 year old female patient who presented with complaint of growth over right upper eyelid from 10 years.

Sebaceous gland carcinomas are tumours of the skin adnexa which occur around the periocular region of elderly females. It is a rare malignancy usually involving the upper eyelid and has an indolent course. Treatment is surgical excision of the lesion with lid reconstruction. Here authors discuss a case of swellings involving both the eyelids of an elderly female diagnosed as sebaceous gland carcinoma on biopsy for which wide local excision and primary eyelid repair was done. This case has been presented as the lesion masquerades as a benign one but is malignant with multifocal spread.
