Prevalence of Overweight and Obesity in The Netherlands in Relation to Sociodemographic Variables, Lifestyle and Eating Behavior: Starting Points for the Prevention and Treatment of Obesity

Background: Several drugs have been currently approved for the treatment of obesity. The pharmacokinetic of liraglutide, as well as the treatment of type 2 diabetes mellitus, have been widely described. Objective: To analyze the published systematic reviews on the use of liraglutide for the treatment of obesity. Methods: Systematic reviews were found out through MEDLINE searches, through EBSCO host and the Cochrane Library based on the following terms: "liraglutide" as major term and using the following Medical Subject Headings (MesH) terms: "obesity", "overweight", "weight loss". A total of 3 systematic reviews were finally included to be analyzed. Results: From the three systematic reviews selected, only two included the randomized clinical trials, while the third study reviewed both randomized and non-randomized clinical trials. Only one review performed statistical tests of heterogeneity and a meta-analysis, combining the results of individual studies. Another review showed the results of individual studies with odds ratio and confidence interval, but a second one just showed the means and confidence intervals. In all studies, weight loss was registered in persons treated with liraglutide in a dose dependent form, reaching a plateau at 3.0 mg dose, which was reached just in men. Most usual adverse events were gastrointestinal. Conclusion: More powerful and prospective studies are needed to assess all aspects related to liraglutide in the overweight and obesity treatment.

Overweight and obesity are an increasingly common problem, not only among the healthy population, but also in adolescents with type 1 diabetes (T1DM). Excess body weight is related to many cardiometabolic complications as well as a high risk of metabolic syndrome (MetS). The purpose of this systematic review is to provide a concise and critical overview of the prevalence of MetS in children and adolescents with T1DM and, ultimately, to discuss prevention and treatment options. The study was conducted in accordance with PRISMA guidelines. This review shows that, apart from the growing percentage of overweight and obese children and adolescents with T1DM (on average 20.1% and 9.5%, respectively), the problem of the increasing incidence of MetS (range from 3.2 to 29.9%, depending on the criteria used) is one of the most important phenomena of our time. One of the methods of prevention and treatment is a combined approach: changing eating habits and lifestyle, but there are also reports about the beneficial effects of the gut microflora.

A túlsúly és az elhízás bizonyítottan a jövő egyik legjelentősebb egészségügyi problémája. A fejlett országokban a túlsúlyos és elhízott emberek aránya ötévente megduplázódik. Egy európai vizsgálat adatai szerint minden második felnőtt túlsúlyos, és a becslések szerint 2000 és 2030 között az elhízottak száma megduplázódhat. Ahogy növekszik világszerte az elhízottak és a cukorbetegek száma, úgy nő az érdeklődés a természetes alapú készítmények iránt. Az elhízás terápiájában több száz gyógynövény ismert, de a forgalmazott és alkalmazott teakeverékeket szájhagyomány vagy más információforrás alapján alkalmazzák. A nagyszámú növény botanikailag rendkívül sokféle, számos rendszertani egységből kerül ki, így az sem meglepő, hogy a hatóanyagok kémiailag szintén sokfélék. A különböző hatóanyagok más-más hatásmechanizmuson keresztül fejtik ki hatásukat. Az elhízás, a diabetes kezelésére engedélyezett növényi gyógyszerkészítmény még nincs forgalomban, így a lakosság vagy engedély nélküli, vagy ismeretlen minőségű, vagy más célra engedélyezett termékeket fogyaszt. Ez a helyzet sok országra jellemző, ami visszaélésekre ad lehetőséget, valamint a betegek számára sem ad kellő biztonságot a gyógynövények alkalmazásában.

Abstract The objective of the study is a critical analysis of the selected alternative diets used in the treatment of obesity inconsistent with the recommended standards, from the perspective of clinical dieticians, based on up-to-date guidelines for dietary management of obesity. Attention was paid to the assumptions of the selected alternative diets, some physiological mechanisms related with their use, as well as the deficit of data pertaining their distant effectiveness and safety. In the context of the current epidemiological situation concerning the prevalence of obesity, it is justifiable to undertake actions aimed at the professionalization of dietary management in obesity, consisting in the application of the treatment methods based on data generated in the EBM paradigm, with a simultaneous indication and criticism of dietary pseudo-therapies with unconfirmed curative value which, in addition, do not guarantee the reduction of the risk of metabolic complications of obesity.
