Gender inequalities in perceived stress and the factors affecting it in infertile couples in Iranian society: a case in one of the provinces of Iran

Purpose The aim of this study is to investigate the gender inequalities in perceived stress and the influencing factors in infertile couples in Iranian society. Design/methodology/approach This cross-sectional study was conducted on infertile couples who were referring to Kurdistan Infertility Diagnosis and Treatment Medical Center in 2019. Demographic and clinical information questionnaire, Newton's Infertility perceived stress questionnaire, Rosenberg's standard self-confidence questionnaire and the multidimensional scale of social support were used. Multiple linear logistic models were also used. Findings A total of 560 couples (1,120 people) participated in the study. The average perceived infertility-related stress, self-esteem scores and social support and social-emotional loneliness were 173.95 ± 41.87, 13.99 ± 2.29 and 27.81 ± 7.33, respectively, which were significantly different scores across infertility cause and sex (P < 0.05). Males compared to females had lower perceived infertility-related stress (169.93 ± 42.51 vs 177.97 ± 40.86, P = 0.001) and self-esteem scores (14.33 ± 2.29 vs 13.66 ± 2.24, P < 0.001) and social support and social-emotional loneliness (32.92 ± 9.31 vs 30.94 ± 9.04, P < 0.001). The partners who reported themselves as infertile, compared significantly higher in perceived infertility-related stress than those who reported their spouse being infertile (194.24 ± 35.33 vs 141.90 ± 39.28), lower self-esteem scores (12.77 ± 2.21 vs 13.94 ± 1.56) and social support and social-emotional loneliness score (27.81 ± 7.33 vs 30.11 ± 7.70). Also, after taking potential confounders into account with increase in each score of self-esteem, 12.19 units of stress decreases (P < 0.001, 95% CI: 11.40–12.99) and with increase in each score of social support and social-emotional loneliness, 3.45 units of stress decreases (P < 0.001, 95% CI: 3.28–3.63). Originality/value There is perceived stress among infertile couples, and this rate is higher among infertile people and women. Therefore, it seems that specific intervention programs for infertile couples should be implemented based on the results of this study, and their stress levels in a way that self-esteem and support for both partners be increased and the perceived stress among women and infertile individuals be decreased.

The fighter controllers (FC) are an occupational group who deal with a hectic, difficult and highly demanding job. Their occupation entails high levels of responsibility in terms of shouldering the Air Defence operations. These kinds of duties require them to perform optimally at all times and thus impose high grade of an unambiguous occupational stress. In the present study 38 Fighter Controllers of 4 IAF Stations were administered Organisational Role Stress (ORS) scale, Stress Buster Tool Kit and Holmes & Rahe Stress Scale. It was found that 66 per cent of the participants felt Inter Role Distance as a reason for high graded stress, similarly Role Stagnancy (55 %), Role overload (61 %) and Role Isolation (40 %) have severely affected the sample studied. Stress buster tool kit showed that 34 per cent participants perceived severe service related and workplace related stress. Holmes & Rahe questionnaire reported that a total of 31.6 per cent participants were at 90 per cent risk of developing systemic co-morbidities due to stress. This study reports that Fighter Controllers perceive their role to be less important with little opportunities for growth. A significant finding that emerged and that needs attention is the possibility that 1/3rd of the participants are prone to stress induced illnesses. The role of Aerospace Medicine Specialists in this context is paramount in alleviating stress and enhancing self-esteem in Fighter Controllers.

Abstract Background: Anxiety and depression are the most common occurring psychological distress in infertile patients. Our study aimed to evaluate the prevalence of depression and anxiety, and investigate the association of infertility-related stress with them among Chinese infertility patients. Meanwhile, the role of self-esteem in this association was explored as a positive psychological resource.Methods: The investigation was conducted between December 2017 to May 2018. Questionnaires that measured infertility-related stress, self-esteem, depression, and anxiety were distributed to 536 female infertility patients in Shenyang, China. Hierarchical linear regression analyses were used to examine the mediating and moderating role of self-esteem. Baron and Kenny’s technique, asymptotic, and resampling strategies were used to confirm the mediating role of self-esteem on the associations of infertility-related stress with depression and anxiety.Results: The prevalence of depression and anxiety was 27.9% and 42.2% among Chinese female infertility patients. Infertility-related stress was positively associated with depression and anxiety, whereas self-esteem was negatively associated with depression and anxiety. Besides, the effect of infertility-related stress on depression and anxiety gradually decreases with the increase of self-esteem. Meanwhile, self-esteem partially mediated the association of infertility-related stress with depression and anxiety.Conclusions: There was a high prevalence of psychological distress. Infertility-related stress and self-esteem were associated with psychological distress, which helped to explain why infertile women suffered higher prevalence of depression and anxiety. Besides, self-esteem mediated and moderated the effects of infertility-related stress on depression and anxiety. Interventions that focus on self-esteem may be effective and available resource to combat mental problems.

Introduction: Oncoplastic breast-conserving surgery (OBCS) is considered a cornerstone in the management of locally invasive breast cancer. We evaluated patient-reported outcomes of OBCS with contralateral balancing breast reduction mammoplasty and reviewed its oncologic outcomes and complications. Methods: This is mixed method study design using retrospective chart review and prospective cohort study. Patient demographics were reviewed. Outcome measures included clinicopathologic characteristics, complications, margin status, local recurrence, tumor histopathologies, duration of follow-up, patient satisfaction, self-esteem, event-related stress, and quality of life. Results: A total of 48 patients were included in this study. Complete excision with negative margins was obtained in 42 (87.5%) patients, positive margins in 6 (12.5%) patients, all who had re-excision with repeat lumpectomy. Thirteen patients developed minor complications, defined as being managed as an outpatient. No patients developed major complications requiring inpatient admission. These complications did not delay commencement of chemotherapy or radiotherapy. Postsurgery BREAST-QTM26 scores demonstrated no statistical difference in satisfaction with breasts, nipples, and sexual well-being. There was high satisfaction with overall outcome with average score of 80.8%. For the Rosenberg self-esteem scale, the results were similar for 3- and 12-month post-operative indicating maintenance of normal self-esteem post-operatively. The Impact of Events Scale showed statistically significant difference at 12-month post-operative (25.1) when compared with preoperative scores indicating that patients had lower event-related stress. There was no significant change in Hospital Anxiety and Depression Scale. Conclusion: Our study has shown that the patient who undergo OBCS have high patient-reported outcomes with acceptable oncologic outcomes and complication rates.

The purpose of this study was to examine actor and partner effects among infertile couples in determining whether self-esteem affects the degree of infertility-related stress via perceived spousal support. The sample comprised 219 couples who experienced infertility, each of whom completed an online survey. To analyze the data, descriptive statistics, t-test, correlation analysis and APIM (Actor-Partner Interdependence Model) were performed using SPSS 25.0 and Mplus 7.3 program. The main results were as follows. First, the actor effect of spousal support on self-esteem was significant in both husbands and wives. Second, the actor effects of spousal support on infertility-related stress and self-esteem on infertility stress were significant only in husbands. Third, in the association between husbands’ and wives’ spousal support and infertility-related stress, three mediating pathways via husband’s self-esteem were found to be significant. Based on these results, the necessity for a couple-level analysis in infertility research, psycho-emotional interventions for infertile couples, and implications for follow-up studies were discussed.
