Our aim was to evaluate clinical, biochemical and microbiological markers related to dental caries in adults. A sample that consisted of 75 volunteers was utilized. The presence of caries and the presence of plaque and gingival indices were determined. Unstimulated salivary flow, pH, lactate, Streptococcus mutans and Streptococcus dentisani were measured in the participants’ plaque and saliva samples before and after rinsing with a sugar solution. Lactate in plaque was found to be significantly related to age, gender, tooth-brushing frequency, the presence of cavitated caries lesions and plaque and gingival indices (p < 0.05). The levels of S. dentisani in plaque increased significantly with tooth-brushing frequency (p = 0.03). Normalized plaque S. dentisani values and the percentage of S. dentisani were slightly higher in patients with basal lactic acid levels ≤ 50 mg/L. After rinsing with a sugary solution, the percentage of S. mutans levels in plaque were higher in patients with lactic acid levels > 350 mg/L (p = 0.03). Tooth-brushing frequency was the factor which was most associated with oral health. Women reflected better clinical and biochemical parameters than men. Low pH and high lactic acid levels tended to be associated with high caries rates. No association was found between bacteria levels and caries indices.

Abstract: Human immunodeficiency virus (HIV) is a virus that attacks the human immune system, especially white blood cells called CD4 cells. Meanwhile, acquired immune deficiency syndrome (AIDS) is a syndrome that arises due to the decline in the human immune system caused by HIV infection. Several studies showed that people living with HIV/AIDS had a higher risk of developing dental caries compared to those without HIV/AIDS. Maintenance of oral hygiene, consumption of antiretroviral (ARV) drugs, and low salivary flow play a role in increasing the risk of caries in people living with HIV/AIDS. This study was aimed to determine the status of dental caries in people living with HIV/AIDS. This was a literature review using the databases of Google Scholar, PubMed, and Clinical Key. The results obtained five journals that were relevant to the topic of discussion. There was a high prevalence of caries in people with HIV/AIDS (56.78%-78.7%) and a higher average caries status (12.83±9.6, 15.14±6.09, and 11.87±8.08) compared to those without HIV/AIDS. The high prevalence of caries in people with HIV/AIDS was influenced by decreased salivary flow, use of ARVs, consumption of sweet foods, and lack of oral hygiene. In conclusion, the prevalence of caries in people living with HIV/AIDS was high.Keywords: dental caries, HIV/AIDS  Abstrak: Human immunodeficiency virus (HIV) adalah virus yang menyerang sistem kekebalan tubuh manusia kususnya sel darah putih yang disebut sel CD4 sedangkan acquired immune deficiency syndrome (AIDS) merupakan sindrom yang muncul akibat menurunnya sistem kekebalan tubuh manusia yang diakibatkan infeksi HIV. Beberapa penelitian menunjukkan bahwa pengidap HIV/AIDS berisiko lebih tinggi mengalami karies gigi dibandingkan dengan orang tanpa HIV/AIDS. Pemeliharaan kebersihan gigi mulut, konsumsi obat antiretroviral (ARV), dan aliran saliva yang rendah berperan dalam peningkatan risiko karies gigi pada pengidap HIV/AIDS. Penelitian ini bertujuan untuk mengetahui status karies gigi pada pengidap HIV/AIDS. Jenis penelitian ialah suatu literature review. Database yang digunakan untuk pencarian literatur ialah Google Scholar, PubMed, dan Clinical Key. Hasil penelitian mendapatkan prevalensi karies yang tinggi pada pengidap HIV/AIDS (56,78%-78,7%) dan rerata status karies lebih tinggi (12,83±9,6, 15,14±6,09, dan 11,87±8,08) dibandingkan dengan yang tanpa HIV/AIDS. Tingginya prevalensi karies pada pengidap HIV/AIDS dipengaruhi oleh penurunan laju aliran saliva, penggunaan ARV, konsumsi makanan manis, dan kurangnya menjaga kebersihan gigi mulut. Simpulan penelitian ini ialah prevalensi karies pada pengidap HIV/AIDS tergolong tinggi.Kata kunci: karies gigi, HIV/AIDS

Insulin dependent diabetes mellitus is a severe disease that raises blood glucose levels because of hyperglycemia and insulinopenia. Fluctuations in water and electrolyte levels may result in xerostomia and other changes in the salivary composition. Since diabetes has an influence on oral health, it is important for the dentist to be aware of newer advances in the field of diabetes and to recognize specific oral problems related to diabetes. Thus, the dentist becomes an important part of the health care team for the patients with diabetes. Aim: The present study correlated salivary flow rate, salivary pH and total salivary antioxidant levels and dental caries in type I diabetic patients. Method: A total of 200 children that included 100 known diabetic children (study group) and 100 healthy children (controls) of both the sexes and from similar socioeconomic backgrounds formed the part of this study. Dental caries was assessed using DMFT index. The salivary total anti-oxidant level was estimated using phospho molybdic acid using spectrophotometric method. The salivary flow rate was recorded using the Zunt method and the salivary pH using the pH indicating paper. The results were statistically analyzed using t-test. Conclusions: The analyzed parameters showed increase in salivary anti-oxidant levels, reduced salivary flow rate, increase incidence of dental caries, salivary pH was decreased when compared to the control group.

Two-hundred and forty individuals were studied, divided into five groups as follows: caries-free children, children with caries, children with rampant caries, young adults with and without caries. Whole stimulated saliva was collected and all individuals were investigated for DMFT/dmft according to the WHO criteria and the simplified oral hygiene index (OHI-S). Quantitative analysis of the total aerobic flora and mutans streptococci in saliva was performed. Also, the level of salivary anti-S. mutans IgA was determined by ELISA. Children with rampant caries showed the highest OHI-S value. The highest total counts of microorganisms were found in the group of children with caries. No statistically significant differences were observed for salivary flow, OHI-S and microorganism counts between the groups of young adults. No correlation between mutans streptococci counts and anti-Streptococcus mutans IgA levels was observed in the studied groups. A correlation between increased anti-Streptococcus mutans IgA levels and caries-free status was observed among young adults but not among children.

Background. Human immunodeficiency viruses (HIV) / Acquired immunodeficiency syndrome (AIDS) is one of the most significant public health challenges in Surabaya, Indonesia where the greatest number of people living with HIV/AIDS (PLWHA) among key populations is in areas served by Sememi Public Health Center. HIV-infected persons have a greater risk for developing dental caries, such as salivary gland enlargement, and decreased salivary glands function. Given the fact that PLWHA are at high risk of dental caries, utilization of dental health service among PLWHA are still low. Objective. This study aims to know the factors influencing dental caries in HIV/AIDS patients.Methods. This is a descriptive, cross-sectional study conducted on 16 HIV-seropositive individuals. They were asked to complete a WHO questionnaire concerning basic oral health and quality of life. Dental caries was assessed using the Decayed, Missing, and Filled Teeth (DMFT) index. Whole stimulated saliva samples were also collected.Results. Nine out of 16 respondents had low salivary flow rate (56.3%). One patient had low DMF-T score (6.3%) and eight had high DMFT score (50%). Among seven respondents who had normal salivary flow rate (43.8%), two of whom had low DMFT score (12.5%) and five of whom had high DMFT score (31.3%).Conclusion. People living with HIV/AIDS have high DMFT and low salivary flow rate.

ABSTRACT BACKGROUND: High prevalence of dental caries in ulcerative colitis (UC) has been attributed to diet and changes in salivary environment. OBJECTIVE: We aimed to characterize the prevalence of dental caries, salivary flow rates, salivary buffering capacity and cariogenic bacteria counts of Mutans streptococci and Lactobacillus spp and to evaluate their relationship with drug therapy, disease activity and duration. METHODS: A cross-sectional study was performed with UC patients followed in a tertiary center. Participants were submitted to a questionnaire (including demographic data, oral hygiene, eating habits) and a clinical observation with assessment of plaque index and Decayed, Missing and Filled Teeth index. Unstimulated/stimulated saliva was collected. Medical records, disease activity (Partial Mayo Score) and disease duration were collected. Laboratory data included salivary flow rates, salivary buffering capacity (CRT® buffer) and cariogenic bacteria count (Mutans streptococci and Lactobacillus spp) in saliva using the CRT® bacteria test (results: high or low counts). RESULTS: Thirty UC patients were recruited. Oral hygiene routines were daily teeth brushing once or more (96.7%) and fluoride toothpaste (73.3%). Decayed, Missing and Filled Teeth index (mean 16.17±6.428) was not affected by the frequency of soft drinks, cakes, sweets and sugars between meals (P>0.2). Long-term disease showed a trend towards higher prevalence of caries (P=0.06). Most presented normal salivary flow rates, unstimulated (73.3%) and stimulated (60.0%), and high salivary buffering capacity (66.7%). Any association was found with age, gender, disease activity, disease duration and drug therapy. High Mutans streptococci and low Lactobacillus spp count were observed in 73.3% and 60% of patients, respectively. Patients with active disease (100%) and longer duration (88.9%) displayed higher Mutans streptococci count. CONCLUSION: The prevalence of dental caries observed in UC patients was significant and did not seem to be influenced by their eating habits. The high prevalence of Mutans streptococci count may be a major risk factor for dental caries and may be looked as part of the UC dysbiosis. Dental care of UC patients should be planned according with this microbiota variation.
