Vestibuloneurological Findings in Children with Severe or Profound Loss of Hearing

Objectives: To report a case of profound bilateral sensorineural hearing and vestibular loss from relapsing polychondritis and hearing outcomes after cochlear implantation. Methods: Case report and literature review. Results: A 43 year-old woman developed sudden loss of hearing and balance that progressed over several weeks to bilateral, profound hearing and vestibular loss. Steroid treatments were ineffective. She underwent vestibular physical therapy and left cochlear implantation. About 10 months after her initial presentation, she developed erythema, warmth, swelling, and pain of the left auricle sparing the lobule, flattening of the bridge of her nose, and right ankle swelling, warmth, and skin erythema. A biopsy of the left auricle revealed histopathologic findings consistent with relapsing polychondritis. She was treated with high dose prednisolone. The ear inflammation resolved, however, despite excellent auditory response to pure tone thresholds, the patient reported no improvement in speech perception after cochlear implantation. Conclusions: Relapsing polychondritis can present with rapidly progressive, profound loss of hearing and vestibular function. Hearing outcomes after cochlear implantation can include poor speech discrimination despite good pure tone detection thresholds.

ABSTRACT Pendred's syndrome is a condition which, in its complete form, is characterized by congenital deaf-mutism, goitre, and defective organic binding of iodide in the thyroid. However, there are less typical cases, e. g. without a goitre, with only limited hearing loss, or a normal perchlorate discharge test. A family was studied in which Pendred's syndrome was found in two generations. The complete form was present in two members; two other members were considered to show less typical forms of the same disorder. All were euthyroid. Three out of five sons were deaf-mute and goitrous. Perchlorate caused a discharge of radioiodine from the thyroid in two of them, and also in the father who had no goitre and a slight loss of hearing only detectable by audiometry. Peripheral deiodination of radioactive diiodotyrosine was normal in all patients, but had been abnormal in one son while he was hypothyroid.

Inability to speak or to talk is called muteness ormutism; often caused by speech disorder, loss of hearing or insome cases, surgery. The families of these people suffering fromsuch disease often learn sign languages like American SignLanguage (ASL) to communicate, but if a mute person meetssomeone who doesn’t know ASL then there will be acommunication barrier between them. So, to overcome thiscommunication barrier we require a system through which amute person can communicate with everyone, hence breakingdown a speech barrier. In this regard, we proposed a systembased on image processing to identify the sign and perform somecommon speech patterns using the speaker.

While there are many causes of hearing loss, many of which have nothing to do with exposure to loud noises, noise (or music)-induced hearing loss is theoretically preventable 100% of the time. Loss of hearing is typically a major problem if it happens to dancers and actors, but it is catastrophic for musicians. Thus, it is not surprising that this topic has been covered extensively in Medical Problems of Performing Artists, starting with “The Musician and Occupational Sound Hazards” by Hart et al. in volume 2, over 20 years ago. Without duplicating the content of those articles, this editorial will try to determine why this remains a somewhat controversial topic in the performing arts and what can be done to move us forward.

In 1994 Bradbury placed six stories, more than he had in a number of years, but his fiction continued to miss the major market magazines. Chapter 33 documents the film projects of the mid-1990s—a Fahrenheit 451 screenplay for Mel Gibson, and a script for The Wonderful Ice Cream Suit under consideration with Disney. A collaboration with NASA artist Robert McCall, astronaut Buzz Aldrin, and special effects master Douglas Trumbull centered on an IMAX Corporation film theater project titled The City in the Stars, but Bradbury’s storyboard narrative was never produced. The chapter also describes Ray and Maggie Bradbury’s 1994 and 1995 summer sojourns in Paris with American expatriate journalists, and Bradbury’s increasing loss of hearing from mastoiditis.

Paclitaxel and platinum combination is the standard chemotherapy regimen for patients with advanced epithelial ovarian cancer. The dose-limiting toxicity effects of this combination are myelosuppression and neuropathy. Herein, we report a case of a 71-year-old female with advanced epithelial ovarian cancer who developed bilateral total loss of hearing and acute renal failure related with paclitaxel- and carboplatin-based chemotherapy. Acute renal failure accompanied by complete loss of hearing in patients treated with carboplatin and paclitaxel combination has not been previously reported. This uncommon adverse effect of carboplatin and paclitaxel combination was discussed, and all the literature in English related with the toxicity of paclitaxel and carboplatin were reviewed.
