BackgroundFetal Alcohol Spectrum Disorders (FASD) is an important global health problem in need of prevention. For FASD prevention it is important to understand why pregnant women engage or do not engage in drinking alcohol. It remains unknown which psychosocial determinants related to maternal alcohol consumption are most in need of prevention. The objective of this study was to identify these.MethodWe searched in PubMed, PsychINFO, PsychARTICLES, ERIC, CINAHL, EMBASE and MEDLINE databases up to May 2018 using an extensive query consisting of keywords related to pregnancy (e.g., maternal, prenatal), alcohol use (e.g., alcohol, drink) and determinants (e.g., attitude, norm). Studies were excluded when not published in English, were reviews, or involved non-human subjects. Substantial heterogeneity precluded aggregation or meta-analysis of the data. Instead, data were qualitatively inspected.ResultsA total of 23 studies including 150 identified items were eligible for data analysis. Studies covered over 15 psychosocial determinants (e.g., attitude, perceived social norm, risk perception). Studies differed in their operationalizations. As a majority of data was based on univariate analysis, little is known about the relationship with specific drinking behaviors. The majority of studies targeted perceived risk and motivation to comply with each social referents' approval or disapproval. A large proportion of studies focused on disadvantages and risks of maternal alcohol consumption. Results from these studies show that women do not continue to drink because the risks are unknown to them. Cautious interpretation is needed while the observed heterogeneity hindered firm conclusions. Conclusion We aimed to identify all relevant psychosocial determinants of maternal alcohol consumption behavior(s). The state of the literature precludes such conclusions. It remains unknown which determinants are most in need of intervention. It is recommended for future studies to (i) identify all possible psychosocial determinants of drinking during pregnancy using both quantitative and qualitative methods; (ii) include different target groups (e.g., women with unplanned pregnancies, pregnant women, women in childbearing age); (iii) identify key environmental agents; (iv) operationalize their measures based on theoretical models; (v) report specific variables such as the study method and association with behavior.

BACKGROUND E- and m-health interventions have gained momentum to change health behaviours such as physical activity (PA) and sedentary behaviour (SB). Although these interventions show promising results in terms of behaviour change, they still suffer from high attrition rates, resulting in a lower potential and reachability. In order to reduce attrition rates in the future, there is a need to investigate the reasons why individuals stop using e- and m-health interventions. Certain demographic variables have already been related to attrition in e- and m-health interventions, however the role of psychosocial determinants of behaviour change as predictors of attrition has not yet been fully explored. OBJECTIVE The aim of this study was to examine when, which and why users stop using an e- and m-health intervention. In particular, we aimed to investigate whether psychosocial determinants of behaviour change were predictors for attrition. METHODS The sample consisted of 473 healthy adults who participated in the e-and m-health intervention ‘MyPlan 2.0’ to promote PA or reduce SB. The intervention was developed using the Health Action Process Approach (HAPA) model, which describes psychosocial determinants that guide individuals in changing their behaviour. If participants stopped with the intervention, a questionnaire with eight question concerning attrition was sent by email. To analyze when users stopped using the intervention, descriptive statistics were used per part of the intervention (including pre- and post-test measurements, and 5 website sessions). To analyze which users stopped using the intervention, demographic variables, behavioural status and HAPA-based psychosocial determinants at pre-test measurement were investigated as potential predictors of attrition using logistic regression models. To analyze why users stopped using the intervention, descriptive statistics of answers to the attrition related questionnaire were used. RESULTS The study demonstrated that 227 of the 473 (47,9%) participants stopped using the intervention, and drop out occurred mainly in the beginning of the intervention. The results seem to indicate that gender and participants’ scores on the psychosocial determinants action planning, coping planning and self-monitoring were predictors of first session, third session and/or whole intervention completion. The most endorsed reasons to stop with the intervention were the time-consuming nature of questionnaires, not having time, dissatisfaction with the content of the intervention, technical problems, already meeting the guidelines for PA/SB, and to a lesser extent the experience of medical/emotional problems. CONCLUSIONS This study provides some directions for future studies. To decrease attrition, it will be important to personalise interventions on different levels, questionnaires (either for research purposes or tailoring) should be kept to a minimum especially in the beginning of interventions by for example using objective monitoring devices, and technical aspects of e-and m-health interventions should be thoroughly tested in advance.

Abstract Background: Diversity sensitive task perception and diversity sensitive behavioural intentions are strong predictors of diversity sensitive care. Nevertheless, it has been less clear which psychosocial determinants are affecting these concepts in health care providers. This article also explores whether there are differences in relationships between these concepts, and how these concepts are interrelated. The study focussed on the following psychosocial determinants: (a) ethnocentric attitudes and (b) intercultural capabilities. The latter concept is drawn from the four-dimensional construct of intercultural intelligence. Methods: A survey was conducted in a health care student population (N = 364). In addition to classic survey questions, behavioural intentions were measured through the use of eight vignettes comprising situations characterised by diversity in health care. Results: Regression analyses show that ethnocentric attitudes were negatively associated with diversity sensitive task perception and behavioural intentions. Interestingly, we found no significant relationship between cognitive intercultural capabilities (IC) and diversity sensitivity. Motivational- and behavioural intercultural capabilities were significantly associated with diversity sensitive task perception and behavioural intentions. Motivational IC had a stronger relationship with task perception. Behavioural IC had a stronger relationship with behavioural intentions. Adding diversity sensitive task perception in a supplementary regression model as an independent variable on behavioural intentions caused a shift in associations. The significant association between motivational IC and behavioural intentions disappeared, and a significant association between diversity sensitive task perception and diversity sensitive behavioural intention appeared. Conclusion: Diversity education should increasingly focus on diminishing ethnocentric attitudes and encouraging behavioural IC and motivational IC in health care students. The paper also suggests two theoretical hypothesises. Firstly, metacognitive IC is a concept strongly related to two IC: motivational IC and behavioural IC. Secondly, the relationship between motivational IC and behavioural intentions is fully mediated by diversity sensitive task perception. Further research that substantiate these theoretical hypothesises is recommended.
