Evaluating the Effect of a Community-Based Lay Health Advisor Training Curriculum to Address Immigrant Children's Caries Disparities

Previous programs had not designed the culturally adequate Lay Health Advisor (LHA) oral health training curriculum for medically underserved population. We evaluated the effects of LHA training curriculum for addressing immigrant children's caries disparities in their access to dental care. We used a pre/post-test study design. Immigrant women were recruited from churches, schools, and immigrant centers in an urban area. Four training classes were held. Each training cycle lasted 15 consecutive weeks, consisting of 1 weekly 2-h training session for 12 weeks followed by a 3-week practicum. The curriculum included training in caries-related knowledge, oral hygiene demonstrations, teaching techniques, communication skills, and hands-on practice sessions. Thirty-seven LHA trainees completed the course and passed the post-training exam. The data were collected using self-report questionnaires. The level of oral health knowledge, self-efficacy and attitudes toward oral hygiene were significantly increased after LHA training. There was a significant and over twofold increase in trainees' oral hygiene behaviors. An increase of >20% in LHA and their children's dental checkup was observed following training. After training, LHAs were more likely to have 3+ times of brushing teeth [Odds Ratio (OR) = 13.14], brushing teeth 3+ minutes (OR = 3.47), modified bass method use (OR = 30.60), dental flossing (OR = 4.56), fluoride toothpaste use (OR = 5.63) and child's dental visit (OR = 3.57). The cross-cultural training curriculum designed for immigrant women serving as LHAs was effective in improvement of oral hygiene behaviors and access to dental care.

Aim: The study aimed to assess oral health knowledge, dental behaviors, and use of dental services in individuals with vision impairment in Jordan and identify barriers that affect their access to dental care in comparison with individuals without vision impairment. Materials and Methods: A case-control study was carried out among 399 parents/caregivers of individuals with vision impairment and individuals without vision impairment, which involved the completion of a self-designed questionnaire. A closed-ended, validated self-designed questionnaire was distributed. The questionnaire included questions addressing participants’ oral health knowledge, oral health behaviour, dental service use and barriers to accessing dental care. Data were analyzed using SPSS® software Version 22 with a 0.05 level of significance. A Chi-square test and contingency-table analysis were performed on the data. Results: Individuals with vision impairment in Jordan were significantly less knowledgeable about different oral health aspects than sighted counterparts. In addition, individuals with vision impairment used toothbrush, dental floss, and mouth rinse less frequently. They also visited dentists less regularly and mostly when only feeling pain. Limited access to dental services was observed among individuals with vision impairment compared to sighted individuals. Barriers, including embarrassment (22%), lack of knowledge of how to treat people with disabilities among dentists (12.5%), and inadequate facilities (13.7%), were significantly (P<0.05) more likely to be reported by visually impaired participants than the controls. Conclusion: Visually impaired individuals and their families should be given appropriate education about oral health care by oral health professionals to reduce the risk of having dental problems and oral disease and enhance their quality of life. Furthermore, recognizing the challenges in accessing dental care for this population could help oral health professionals to minimize these difficulties.

The WHO 2010 reported connection between oral wellbeing and general wellbeing, with common risk factors including poor diet, tobacco use and alcohol consumption. People who suffer from general health problems (diabetes, HIV) are more likelihood to have oral diseases (periodontal diseases) that, in turn, further complicate their overall health.Thus, oral health is an integral part of general wellbeing . One of the many barriers to quality oral healthcare includes a lack of attention to oral health by non-dental health care professionals (e.g., nurses, pharmacists). Nurses, being the primary health care provider to the patients, play an important role in health sector. Aside from dental professional, nurses are an integral part in health promotion and preventive information dissemination, therefore it is important that oral health knowledge should be good to decrease poor oral health outcomes. The aim of this study was to assess oral health knowledge, attitude and oral hygiene practices among nursing student of Apollo School of Nursing , Mizoram.A Questionnaire study was conducted among 150 students of Apollo School of Nursing Mizoram. A self administered structured questionnaire containing oral hygiene knowledge and awareness, attitude and oral hygiene practices questions was distributed . Their response were analyzed and their knowledge and awareness, attitude and oral hygiene practice level(KAP) were evaluated. Statistical Analysis was done by calculating the mean of the KAP level (%) . The present study showed good oral hygiene practices and fair knowledge of oral health among nursing students of Apollo School of Nursing Mizoram

Objective: The authors conducted an integrative literature review of recent studies that explored the impact of interventions implemented in the U.S. that focused on improving access to dental care for low-income and vulnerable populations. Methods: The authors conducted an integrative literature review of studies published between 2012-2018 that addressed six oral health policy spheres. 1) Community-based dental access programs; (2) Medicaid reimbursement and expansion; (3) Student loan support; (4) Oral health services in non-traditional settings and dental residency programs; (5) Programs to improve oral health literacy; and (6) Use of dental therapists. Results: The authors included 39 articles for qualitative synthesis. Numerous public health initiatives and programs exist in the US aimed at increasing access to quality oral health care. Medicaid expansion, increased Medicaid fee-for-service reimbursement rates, and state loan repayment programs have demonstrated some success in improving access among underserved populations. A diversified dental workforce, with community dental health workers and mid-level providers like dental therapists, as well as interprofessional training of nurses and primary-care physicians in oral health have also shown positive impacts in advancing health equity. Further studies are needed to understand how oral health literacy programs can affect access and utilization of dental services. Conclusions: Improvements to the oral health care safety net will require a holistic and multifaceted approach in order to reduce oral health disparities. Policy levers should work, not in isolation, but rather in complementary fashion to one another.

The pediatrician can assist the dental profession in preventing dental disease and maintaining the oral health of children. Pediatricians are urged to counsel parents in dietary and oral hygiene practices that will prevent dental caries and to refer patients for professional dental care when appropriate. Systemic fluoride supplements should be prescribed when needed but only after a careful review of requirements and daily consumption.

According to the current paradigm for promoting health, dental care should be a consideration from the first months of life, or even before birth. The aim of this paper is to evaluate mothers' knowledge of and attitude toward their babies' oral health after receiving guidance during the neonatal period. Forty-six mothers were contacted and asked about the advice they had received and how they felt about the information provided. The mothers recruited for the study were divided into two groups, A (n=25) and B (n=21), according to the time elapsed since their participation in the project, that is, less than or equal to three months and more than three months, respectively. A Wilcoxom rank sum test did not show any statistically significant difference between the two groups (p>0.05). Guidance on the baby's oral hygiene, breastfeeding the baby exclusively until the sixth month, as well as the restrictions imposed on sugar intake were what the mothers remembered most. Recommendations concerning good arch development and the use of bottles were what mothers remembered least. Regarding infant oral health, it would be advisable to schedule prenatal and neonatal visits, with the second post-natal consultation no later than four months after childbirth.

Inadequate oral health knowledge and awareness is more likely to cause oral diseases among all age groups, including children. Reports about the oral health awareness and oral hygiene practices of children in Bangladesh are insufficient. Therefore, the objective of this study was to evaluate the oral health awareness and practices of junior school children in Mathbaria upazila of Pirojpur District, Bangladesh. The study covered 150 children aged 5 to 12 years of age from three primary schools. The study reveals that the students have limited awareness about oral health and poor knowledge of oral hygiene habits. Oral health awareness and hygiene practices amongst the school going children was found to be very poor and create a much-needed niche for implementing school-based oral health awareness and education projects/programs.

Objective: To assess the oral health knowledge of mothers of children with cleft lip and/or palate, analyzing the practical application of this information on the basis of the oral hygiene of the children and caries prevalence of mothers and children. Design: Cross-sectional. Setting: Pediatric Dentistry sector of the Hospital for Rehabilitation of Craniofacial Anomalies. Patients: Mothers (n = 300) and their children with cleft lip and/or palate, ages 3 years to 5 years 6 months. Materials and Methods: An interview with questions related to dietary habits and oral health knowledge and habits was conducted. Children and their mothers also were evaluated by intraoral clinical examination. Results: Of the mothers, 47.3% displayed poor oral health status (Group A) and 52.7% had a satisfactory oral health status (Group B). Children in Group A presented a mean dmft of 6.0, whereas children in Group B had a mean dmft of 5.6. No significant differences were observed between groups. Mothers demonstrated a reasonable knowledge on prevention of dental caries and oral hygiene. Conclusion: Even though the mothers interviewed had some knowledge on the causes and prevention of dental caries, other factors should be regarded as relevant in the caries process.
