The evaluation and treatment of the patient with idiopathic chronic pain traditionally has involved a sequence of studies first by the internist, then the neurologist, and finally the psychiatrist. This has resulted in an overutilization of costly health care services and may paradoxically have helped to promote symptom chronicity. In keeping with recent developments in the field of psychosomatic medicine, a coordinated biopsychosocial approach is advocated with the identification and amelioration of the multiple determinants of symptom formation in each of these interrelated sub-systems. A case is presented in which the application of this holistic approach appeared to help curtail the overuse of health care services and at the same time helped to diminish psychosocial reinforcers in the form of secondary gain.

Current Psychiatry is in crisis. Decades of neuroscientific research have not yet delivered adequate explanations or treatments. One reason for this failure may be the wrongness of its central assumption, namely that mental symptoms and disorders are natural kinds. The Cambridge School has proposed that a new Epistemology must be constructed for Psychiatry, and that this should start with the development of a new model of mental symptom-formation. ‘Mental symptoms’ should be considered as hermeneutic co-constructions occurring in a intersubjective space created by the dialogue between sufferer and healer. Subjective experiences (caused either by neurobiological or psychosocial upheaval) penetrate the awareness of sufferers causing perplexity and/or distress. To understand, handle and communicate these experiences, sufferers proceed to configure them by means of templates borrowed from their own culture. Importantly, however, the same neurobiological information can be configured into different symptoms; and different neurobiological information into the same symptom. Therefore, ‘mental symptoms’ are dissimilar hybrid combinations of neurobiological and cultural information. To be ethical, therapeutic interventions must take into account such dissimilarities. Blind manipulation of the brain in all cases should be considered as counterproductive.

The paper raises the issue of exacerbation of psychological problems caused by the political and economic environment, which creates a general social tension and becomes a background that exacerbates the existential problems of the individual. That's why author emphasizes on a particular need for the development of therapeutic direction and a detailed and well-grounded development of therapeutic concepts. It creates ways of an effective solve of current problems of psychological health and well-being of the person. The article explained the conceptual provisions of the establishment, operation and therapeutic work with the symptom. The main characteristics, structure and overall symptom phenomenological expanse from the standpoint of the existential approach of the theory of system solutions and system modeling. Psychological functions of a symptom are considered. Issues of operationalization of a symptom and definition of conceptual provisions of its formation are solved . Basic rules of interpretation of a symptom and interpretation of the concept are described in terms of several approaches. The conceptual provisions of symptom formation that allow to include it in psycho-corrective methods are considered in detail. The methods of the existential-systemic technological approach for providing psychological assistance to the client are analyzed and empirically tested, providing dynamic changes in the content of the "picture of the world" of clients in the cognitive and ontological plans. According to the empirical study, psychosomatic, emotional and behavioral symptoms were presented. It has been shown that symptoms with different specificities have unchanging constituents and functions. It also has been shown that applying an existential approach helps the symptomatic client to produce a new, better reality that will take into account the actual needs and aspirations of the subject. Thus, the main task, which is directed and work with the symptom – is the person's approach to himself/herself, greater sensitivity to his own real existential aspirations.

Case material is used to illustrate specific clinical application of the concept of primary femininity. Some contemporary contributions to the psychoanalytic theory of female psychosexual development are presented as complementary with, rather than contradictory to, more familiar, longstanding formulations that emphasize phallic strivings in women. In the clinical example reported, a fantasy of genital damage underlay a woman's premenstrual distress. Aspirations and concerns related to both aspects of her fundamental bisexuality participated in symptom formation and had to be investigated in order to achieve symptom relief. As the analytic work unfolded, it could be seen that the patient's awareness of her feminine aims served to keep her masculine aims out of awareness, and vice versa.
