Background: The central venous catheters (CVC) has recently become a worldwide commonest technique for vascular access, however; prone to infection and associated with other complications more than other vascular access techniques, such as fistula or graft. Objective: To investigate the incidence, the types of micro-organism, the most sensitive and suitable antibiotics, the outcomes associated with CVC infection in chronic hemodialysis patients treated through tunneled CVC.in a single-center population. Methods: The records of hemodialysis patients from January 2012 to December 2016 were reviewed. Patients above 14 years of age, on hemodialysis via a permanent tunneled catheter at any time of the mentioned period, were included. The rate of CRBSI (Catheter-Related Blood Stream Infections) was calculated as density of incidence and reported per 1000 catheter days. Patient mortality was recorded. Results: In 5 years, a total of 73 episodes of CRBSI in 49 hemodialysis patients were recorded. 59.2% were male with a mean age of 53.57 years. 40.8% had end-stage kidney disease secondary to diabetic nephropathy. In all, 67.3% episodes grew Gram-positive isolates, among those staphylococci epidermidis (26.4%, n=19) and staphylococci aureus (25%, n=18) were the most common organism, whereas Klebsiella pneumonia (8.3%, n=6), Enterobacter cloacae (8.3%, n=6), pseudomonas aeruginosa (5.5%, n=4) and E.coli (5.5%, n=4) were the most common Gram-negative isolates. A total of 6 isolates were multidrug-resistant, which includes Klebsiella pneumonia, Citrobacter, and E. coli. Only 1 out of 18 staphylococci aureus cases were methicillin-resistant and 21% (n=6) of Gram-negative isolates were Extended-spectrum beta-lactamase (ESBL) producing organisms. The average hospital stay was 9 days (2-30 days). The hemodialysis catheter was removed for 19 (38.77%) patients and 4 (8.1%) patients expired due to septic shock despite catheter removal and being on appropriate antibiotics. Conclusion: The mainstay to reduce CRBSI would be ideal to avoid catheters or decrease the duration of catheters. This can be achieved through more aggressive counseling and fast-track protocols for AV Fistulae creation at the pre-dialysis stage.

Background: Daily management to shield chronic dialysis patients from SARS-CoV-2 contamination makes patient care cumbersome. There are no screening methods to date and a molecular biology platform is essential to perform RT-PCR for SARS-CoV-2; however, accessibility remains poor. Our goal was to assess whether the tools routinely used to monitor our hemodialysis patients could represent reliable and quickly accessible diagnostic indicators to improve the management of our hemodialysis patients in this pandemic environment. Methods: In this prospective observational diagnostic study, we recruited patients from La Conception hospital. Patients were eligible for inclusion if suspected of SARS-CoV-2 infection when arriving at our center for a dialysis session between March 12th and April 24th 2020. They were included if both RT-PCR result for SARS-CoV-2 and cell blood count on the day that infection was suspected were available. We calculated the area under the curve (AUC) of the receiver operating characteristic curve. Results: 37 patients were included in the final analysis, of which 16 (43.2%) were COVID-19 positive. For the day of suspected COVID-19, total leukocytes were significantly lower in the COVID-19 positive group (4.1 vs. 7.4 G/L, p = 0.0072) and were characterized by lower neutrophils (2.7 vs. 5.1 G/L, p = 0.021) and eosinophils (0.01 vs. 0.15 G/L, p = 0.0003). Eosinophil count below 0.045 G/L identified SARS-CoV-2 infection with AUC of 0.9 [95% CI 0.81—1] (p < 0.0001), sensitivity of 82%, specificity of 86%, a positive predictive value of 82%, a negative predictive value of 86% and a likelihood ratio of 6.04. Conclusions: Eosinophil count enables rapid routine screening of symptomatic chronic hemodialysis patients suspected of being COVID-19 within a range of low or high probability.

Abstract Background Hemodialysis patients with COVID-19 have been reported to be at higher risk for death than the general population. Several prognostic factors have been identified in the studies from Asian, European or American countries. This is the first national Lebanese study assessing the factors associated with SARS-CoV-2 mortality in hemodialysis patients. Methods This is an observational study that included all chronic hemodialysis patients in Lebanon who were tested positive for SARS-CoV-2 from 31st March to 1st November 2020. Data on demographics, comorbidities, admission to hospital and outcome were collected retrospectively from the patients’ medical records. A binary logistic regression analysis was performed to assess risk factors for mortality. Results A total of 231 patients were included. Mean age was 61.46 ± 13.99 years with a sex ratio of 128 males to 103 females. Around half of the patients were diabetics, 79.2% presented with fever. A total of 115 patients were admitted to the hospital, 59% of them within the first day of diagnosis. Hypoxia was the major reason for hospitalization. Death rate was 23.8% after a median duration of 6 (IQR, 2 to 10) days. Adjusted regression analysis showed a higher risk for death among older patients (odds ratio = 1.038; 95% confidence interval: 1.013, 1.065), patients with heart failure (odds ratio = 4.42; 95% confidence interval: 2.06, 9.49), coronary artery disease (odds ratio = 3.27; 95% confidence interval: 1.69, 6.30), multimorbidities (odds ratio = 1.593; 95% confidence interval: 1.247, 2.036), fever (odds ratio = 6.66; 95% confidence interval: 1.94, 27.81), CRP above 100 mg/L (odds ratio = 4.76; 95% confidence interval: 1.48, 15.30), and pneumonia (odds ratio = 19.18; 95% confidence interval: 6.47, 56.83). Conclusions This national study identified older age, coronary artery disease, heart failure, multimorbidities, fever and pneumonia as risk factors for death in patients with COVID-19 on chronic hemodialysis. The death rate was comparable to other countries and estimated at 23.8%.

A Prospective Study

Abstract Objective The evaluation of physical activity for chronic hemodialysis patients is a new approach for patient global care. The objective of this work is to evaluate the physical activity in chronic hemodialysis patients and identify the risk factors associated with reduced physical activity. This is a prospective study for 6 months including 150 chronic hemodialysis patients in the Guelmim-Oued Noun Regionin Moroccan Sahara. We use Baecke's survey, translated and validated in Arabic local language. The socio-demographic, clinical, and biological data were completed during the interrogation and from the medical records of the patients. Results The mean age of our patients was 54.6 ± 16.4 years, with male predominance (59%). Most patients have a low education level and 60% were illiterate. Hypertension was found in 54% of our patients, diabetes in 39%, and cardiovascular disease in 10% of patients. Low Physical activity was associated with gender (OR = 4.05), age (OR = 1.03) and high education level (OR = 0.2). Our work has met the various pre-established objectives, however other more specific studies must be conducted to better characterize the profile of physical activity in chronic hemodialysis patients.
