Hepatitis C in Chronic Kidney Disease: Predialysis Patients Present More Severe Histological Liver Injury than Hemodialysis Patients?

Background: Approximately 5-60% of hemodialysis patients have comorbid of hepatitis C virus (HCV) infection. The survival rate of hemodialysis patients HCV is lower than those without HCV due to the risk of liver fibrosis and cardiovascular disease. The combination of Elbasvir and Grazoprevir is the drug of choice for these patients with HCV genotype 1 and 4 which mainly inherited in populations in Indonesia. However, a high cost is required for this genome testing.  Eradication of HCV might reduce liver fibrosis. One of the methods to evaluate liver fibrosis is by using APRI score. The aim of this study is to assess liver fibrosis using APRI score after  Elbasvir/Grazoprevir therapy completion.Method: This was a quasi-experimental study without a control group which conducted during April-November 2019 in Dr. Hasan Sadikin Hospital, Bandung. Patients with reactive anti HCV, detected HCV RNA without genotype test, and sustained virological response (SVR) 12 achieved were included. APRI score and HCV RNA tests were performed before and after 12 weeks end of therapy (SVR12). The data were analyzed using paired T-test at a 95% confidence level.Results: Approximately 30% of hemodialysis patients had HCV infection. A total of 37 HCV patients in the hemodialysis unit were given Elbasvir/Grazoprevir for 12 weeks. The APRI score before and after 12 weeks after therapy were 0.336 and 0.270 (p = 0.002) respectively.Conclusion: The combination of Elbasvir/Grazoprevir might reduce the degree of liver fibrosis based on APRI score from 0.336 to 0.270  in HCV patients with chronic kidney disease on hemodialysis

Abstract Background Hemodialysis is considered a major therapeutic method for patients with chronic kidney disease. Pruritus is a common complaint of hemodialysis patients. The 5-D pruritus scale is amongst the most common tools to evaluate several dimensions of itch. Psychometric properties of the 5-D scale have not been evaluated in Persian speaking population with hemodialysis; hence, the objective of this study was to assess reliability and validity of the Persian version of the scale. Methods Ninety hemodialysis patients (men: 50, women: 40, mean age: 54.4 years) participated in this cross-sectional study. The final Persian version of 5-D scale was given to the participants. Tests Compared: One-third of the participants completed the scale twice within 3–7 days apart to evaluate test- retest reliability. Other psychometric properties including internal consistency, absolute reliability, convergent, discriminative and construct validity, floor/ceiling effects were also evaluated. Results The Persian 5-D scale has strong test-retest reliability (ICC= 0.98) and internal consistency (Cronbach’s alpha= 0.99). Standard error of measurement and minimal detectable change were 0.33 and 0.91, respectively. Regarding convergent validity, the scale had moderate correlation with numeric rating scale (r =0.67) and quality of life questionnaire related to itch (r = 0.59). Exploratory factor analysis revealed two factors within the scale. No floor or ceiling effect was found for the scale. Conclusion The Persian version of 5-D the itching scale is a brief instrument with acceptable reliability and validity. Therefore, the scale could be used by experts, nurses, and other health service providers to evaluate pruritus among Persian speaking hemodialysis patients.

AbstractAssociations between hepatitis C virus (HCV) and chronic kidney disease (CKD) have been reported; however, differences of renal progression between general and CKD population remain to be elucidated in prospective studies. A total of 1179 participants, who have tested for anti-HCV antibody, were enrolled and prospectively followed for 3 years. The risks associated with HCV infection, in terms of incidence of CKD, annual estimated glomerular filtration rate (eGFR) changes and 50% decline of eGFR at 3-year from baseline, were compared between normal renal function subjects and CKD patients. Overall, 111 of 233 (47.6%) CKD patients and 167 of 946 (17.7%) non-CKD subjects had HCV infection. The crude incidence rates of CKD were 226.9 per 1000 person-years and 14.8 per 1000 person-years in in HCV and non-HCV infected patients, respectively. The adjusted hazard ratio of HCV infection for incident CKD was 7.9 (95% CI 5–12.7). The HCV-infected normal renal function subjects were independently associated with increased risks of eGFR decline in the 1-year, 2-year and 3-year, respectively. The risk associations remained significant in 50% decline of eGFR at 3 years models and in different subgroup analyses. The increases of risks of eGFR decline were also notorious among overall HCV-infected CKD patients. However, the risk associations were less prominent in subgroup analyses (elderly, women and diabetic patients). The findings highlighted the importance of viral diagnosis with not only prognostic but also public health implications for preserving kidney function.

An epidemic of chronic kidney disease (CKD) has been observed in Central America among workers in the sugarcane fields. One hypothesis is that the CKD may be caused by recurrent heat stress and dehydration, and potentially by hyperuricemia. Accordingly, we developed a murine model of kidney injury associated with recurrent heat stress. In the current experiment, we tested whether treatment with allopurinol (a xanthine oxidase inhibitor that reduces serum urate) provides renal protection against recurrent heat stress and dehydration. Eight-week-old male C57BL/6 mice were subjected to recurrent heat stress (39.5°C for 30 min, 7 times daily, for 5 wk) with or without allopurinol treatment and were compared with control animals with or without allopurinol treatment. Mice were allowed ad libitum access to normal laboratory chow (Harlan Teklad). Kidney histology, liver histology, and renal function were examined. Heat stress conferred both kidney and liver injury. Kidneys showed loss of proximal tubules, infiltration of monocyte/macrophages, and interstitial collagen deposition, while livers of heat-stressed mice displayed an increase in macrophages, collagen deposition, and myofibroblasts. Allopurinol provided significant protection and improved renal function in the heat-stressed mice. The renal protection was associated with reduction in intrarenal uric acid concentration and heat shock protein 70 expression. Heat stress-induced renal and liver injury can be protected with allopurinol treatment. We recommend a clinical trial of allopurinol for individuals developing renal injury in rural areas of Central America where the epidemic of chronic kidney disease is occurring.
