Abstract Collagenous sprue is a severe malabsorptive disorder, histologically characterized by small intestinal villous and crypt atrophy, and a subepithelial collagen deposit, thicker than 12 µm, that entraps lamina propria cellular elements. Collagenous sprue is a rare disease entity, with only about 60 sporadic cases reported worldwide since it was first described in 1947. Its exact etiology is still under investigation, and its relationship with classic celiac disease and other refractory, spruelike intestinal disorders remains controversial. Two larger-scale studies, in 2009, brought new insights into this elusive, yet emerging, topic. Here, we present a review of the literature on the possible etiology of collagenous sprue, its proposed links to classic celiac disease and to refractory sprue, and its clinical, biochemical, histologic, and molecular features. To our knowledge, all case reports on collagenous sprue in the medical literature to date are summarized.

A refrakter sprue-ra a coeliakiára jellemző morfológiai eltérések mellett a klonális intraepithelialis lymphocyták megjelenése jellemző. Célkitűzés: A közleményben szereplő esettel a szerzők a refrakter sprue diagnosztikájára hívják fel a figyelmet. Módszer: A bemutatott beteg biopsziás mintáit – a kórszövettani feldolgozás mellett – immunmorfológiai vizsgálatnak és a T-sejtes klonalitás kimutatása érdekében, molekuláris genetikai vizsgálatoknak vetették alá. Eredmények: A súlyosan lesoványodott beteg malabsorptiós szindrómájának hátterében álló coeliakiára a duodenalis biopszia vizsgálata derített fényt, míg az intraepithelialis T-lymphocyták részéről már ekkor is észlelt CD8-negativitás a refrakter sprue diagnózisát vetítette előre. Mivel a jejunalis táplálás és a gluténmentes diéta nem vezetett eredményre, azathioprine-t és glükokortikoidot kapott a beteg, ami klinikailag teljes, szövettanilag részleges remissziót eredményezett. Az első klinikai észlelés kezdetétől számított két éven belül manifeszt intestinalis T-sejtes lymphoma alakult ki az addig többször vizsgált ileocaecalis régióban. Következtetések: A gluténmentes diétára refrakter sprue és az enteropathia típusú T-sejtes lymphoma folyamatos betegségspektrumot képez. Az eset arra hívja fel a figyelmet, hogy a refrakter sprue viszonylag egyszerű vizsgálattal kideríthető.

Nongranulomatous ulcerative jejunoileitis (NGUJI) is a rare, often fatal disorder that produces multiple nonmalignant small bowel ulcerations. A 55-year-old woman with presumed celiac disease presented with steroid-refractory diarrhea, weight loss and abdominal pain. A laparotomy was performed to exclude the possibility of a lymphomatous disorder, and multiple nonmalignant small bowel ulcerations were discovered. Despite a combination of treatment with total parenteral nutrition (TPN) and prednisone 30 mg/day she continued to deteriorate. The addition of azathioprine to her treatment regimen resulted in marked clinical and biochemical improvement. Her enteroscopy normalized, and she was able to discontinue TPN and reduce her steroid requirements. Although azathioprine has been used occasionally to treat refractory sprue, there have been no reports of its use in NGUJI. In this case, azathioprine played a key role in the management of NGUJI and should be considered a treatment option for patients with this disorder.
