Zusammenfassung Einleitung Die primäre angeborene Hypothyreose ist die häufigste angeborene endokrine Erkrankung. Ziel unserer Studie war es, zu untersuchen, wie hoch die Qualität der medizinischen Behandlung von Kindern mit primärer angeborener Hypothyreose in Deutschland ist und inwiefern die kürzlich veröffentlichten Empfehlungen der Europäischen Gesellschaft für Kinderendokrinologie (European Society for Paediatric Endocrinology [ESPE]) in der Diagnostik und Therapie der Kinder mit einer primären angeborenen Hypothyreose bereits in der Vergangenheit von den behandelnden Ärzten in der alltäglichen Praxis umgesetzt worden sind. Methoden Hierzu wurden retrospektiv die Daten aller Patienten, die von 1998 bis 2008 ein positives Testergebnis für eine primäre angeborene Hypothyreose im Neugeborenscreening (NGS) im Screeningzentrum Leipzig aufwiesen, analysiert. Ergebnisse Das NGS wurde mehrheitlich im empfohlenen Zeitraum durchgeführt. Eine ätiologische Zuordnung der angeborenen Hypothyreose gelang in 71 % der Fälle. Die medikamentöse Therapie mit L-Thyroxin (LT4) wurde durchschnittlich mit 8,5 Lebenstagen begonnen. Eine Normalisierung der Schilddrüsenhormone wurde durchschnittlich innerhalb von 11,4 Tagen nach Behandlungsbeginn erreicht. Eine formale audiologische und neurokognitive Testung im Verlauf wurde zwar lediglich in 25 % und 5 % durchgeführt und dokumentiert. Betroffene Kinder erhielten dennoch die notwendigen Fördermaßnahmen auf Grundlage der klinischen Einschätzung der behandelnden Ärzte. Diskussion Unsere Studie zeigt, dass die Empfehlungen der kinderendokrinologischen Fachgesellschaft bereits mehrheitlich umgesetzt wurden. Die Diagnostik zur ätiologischen Zuordnung der primären angeborenen Hypothyreose, die Durchführung von audiologischen Untersuchungen sowie die Dokumentation und Nachverfolgung der Patienten bedürfen Verbesserung.

AbstractObjectivesNeurofibromatosis type 1 (NF1) is an autosomal dominant genetic disorder, caused by mutation in NF1. The condition is typified by the development of benign and malignant tumours in both the central nervous system and peripheral tissues. Isolated menarche is a sub-classification of incomplete isosexual precocious puberty typified by menarche in girls with no other features of pubertal development. The effects of NF1 on pubertal timing are poorly understood, we report two siblings with NF1 and apparent abnormal pubertal development.Case PresentationTwo siblings were referred to the tertiary paediatric endocrinology clinic at 6 and 7 years of age with recurrent, cyclical vaginal bleeding. There was a strong family history of NF1, the mother of the siblings and two brothers were also diagnosed at a young age. On examination both patients were prepubertal at presentation. Both siblings underwent a gonadotrophin releasing hormone test, which revealed a follicle-stimulating hormone dominant (prepubertal) response. The features were suggestive of isolated premature menarche as no other cause was identified. The elder sibling established menarche and developed signs of consonant pubertal development at 12 years of age. The younger sibling remains under regular follow-up.ConclusionsNF1 has previously been associated with alterations in pubertal timing. We report, for the first time, two siblings with NF1 who presented with isolated menarche.

<b><i>Background:</i></b> Independent peer review of healthcare services can complement existing internal-, institutional-, and national-level regulatory mechanisms aimed at improving quality of healthcare. However, this has not been reported for paediatric endocrinology services in the UK. We aimed to test feasibility and acceptability through a first cycle of a national peer review of paediatric endocrine services. <b><i>Methods:</i></b> Tertiary centres in paediatric endocrinology across the UK were assessed against 54 quality standards, developed by the British Society for Paediatric Endocrinology and Diabetes (BSPED) in five domains of healthcare by a team comprising paediatric endocrinologists and specialist nurses. The evaluation was supported by a self-assessment. A post-peer-review questionnaire was used as feedback. <b><i>Results:</i></b> All 22 centres in the UK underwent independent peer review between 2011 and 2017. Each served a median population of 2.6 million (range 1–8 million) and offered 1,872 (range 779–6,738) outpatient consultations annually. A total of 43 (range 30–49) standards were met in combined evaluation of all centres. Variance of adherence for essential standards ranged from 52 to 97% at individual centres with 90% adherence demonstrated by 32% of centres. Post-review feedback showed 20/22 (95%) validating the utility of the peer review. <b><i>Conclusions:</i></b> The BSPED peer review of all UK centres providing paediatric endocrine services is shown to be feasible and provides a quality benchmark for replication by national services.
