Bacteria have been implicated in recurrent choledocholithiasis associated with endoscopic sphincterotomy (EST). This study was designed to clarify whether bacterial examination of bile provides information useful in predicting the risk of recurrence of choledocholithiasis in patients undergoing EST. Bacteria in bile collected via a duodenoscope before cholangiography were cultured. We compared bacterial isolates and quantity among 41 patients with choledocholithiasis (7 with and 34 without a history of recurrent choledocholithiasis) who had undergone EST more than 3 months previously and 13 control patients with no evidence of pancreatobiliary disease. The bile samples were cultured under aerobic and anaerobic conditions. The bacterial quantity was expressed as the mean logarithm of the number of colony forming units (CFU)/ml. Furthermore, cholescintigraphic studies of bile flow were performed with the use of 99 mTC-HIDA to study the clinical implication of these variables. No bacteria were detected in 10 of the 13 patients in the control group. In the other three control patients the bacterial count was 2.2 log CFU/ml or less. The mean bacterial count was significantly higher in patients with recurrence than in those without recurrence. Cholescintigraphy revealed a trend toward a higher number of isolates and a higher bacterial count in bile in patients with delayed bile passage than in those with good passage. The results suggest that an increased number of biliary isolates and an increased bacterial count indicate decreased bile flow in patients with choledocholithiasis who are being followed up after EST. These variables may potentially serve as indicators of the risk of stone recurrence. Especially when the bacterial count is higher than 7.0 log CFU/ml, the risk of a decrease in bile flow and an increased stone recurrence would be possibly found.

Aim-Objectives: Vitiligo is an acquired idiopathic disease which progresses with melanocyte destruction and is clinically characterized by depigmented lesions of unknown etiology. Vitiligo may be coexistence with a autoimmune and endocrine disorders. This study examined the sT3, fT4, TSH, Anti-TPO, Anti-Tg, Vitamin B12 and fasting blood glucose (FBG) values, and thrombocyte-to-lymphocyte ratio(TLR), neutrophil-to-lymphocyte ratio(NLR), the mean platelet volume(MPV) the correlation of depigmented lesions with the extent of body involvement sites(IS). Materials and Method: The study enrolled 67 patients aged 0‒65 who were diagnosed with non-segmental generalized vitiligo and in whom an increase in lesions had been observed in the last six months. The IS of the lesions in the patients were evaluated as IS ˂10%(1st group), 10%‒20% (2nd group), 20% - 30%(3rd group), 30% - 40%(4th group), 40% ‒50 %(5th group), and ˃50%(6th group). The control group consisted of patients who had presented to the outpatient clinic having had no history of vitiligo detected in themselves nor in their families. Results: No significant correlation was found between IS and sT3, fT4, TSH, Anti-TPO, Anti-Tg, Vit. B12, PBG or MPV. A moderately positive correlation was found between IS and duration (p <0.05) and a mildly positive correlation between IS and NLR and TLR (p <0.05). Conclusion: This study show that every patient diagnosed with vitiligo, independent of the IS, should undergo examination for autoimmune disease. A mild positive correlation between VTA and NLO-TLO was found to be an indicator of increased inflammation in vitiligo patients as the extent of lesions increased.

In this comparative study with a control group of prisoners, psychiatric morbidity was measured in two groups of sentenced prisoners, each group completing the GHQ-30 and 21-item Beck Depression Inventory (BDI). Group 1 consisted of 40 segregated HIV-positive prisoners and group 2 a matched control group in the main prison who had no history of HIV seropositivity. All members of group 1 had a history of intravenous drug abuse. The mean GHQ-30 and BDI scores were significantly higher in group 1, and 90% of group 1 were psychiatric ‘cases’ compared with just over 42% of group 2. Levels of psychiatric morbidity present in a third group, consisting of HIV-positive prisoners who had not been segregated (prison authorities were unaware of their seropositivity) are an interesting pointer for further research.

A case-control study conducted in the Department of Dermatology and Venereology of Bangabandhu Sheikh Mujib Medical University, Dhaka from July 2008- December 2010. A total 60 female patients of telogen effluvium attending in outpatient department (OPD) were enrolled in the study with 30 having hair loss considered as case (Group-A) and another 30 females had no history of hair loss were considered as control (Group-B). The study included 60 patients and the mean age were 25.4±7 years ranging from 18 to 42 years in group A and 24.8±5.6 years ranging from 17 to 36 years in group B. Maximum number was found in the age group of 21- 30 years in both groups and nearly three fourth (73.4%) patients were unmarried in group A and 18(60.0%) in group B. Most of the patients were student in both groups, which were 21(70.0%) and 17(56.7%) in group A and group B respectively and majority of the the patients were HSC level in both groups, 15(50.0%) and 14(46.7%) in Group A and Group B respectively. Most of the patients came from middle class, which were 20(66.7%) and 16(53.3%) in group A and group B respectively. The mean serum ferritin were 18.8±8.1 g/L ranging from 4.5 to 36.54 g/L and 36.6±9.9 mg/L ranging from 18.46 to 56.3 mg/ L in group A and group B respectively. The mean Hb level was 11.5±1.4 gm/dl ranging from 8.5 to 14.0 gm/dl in group A and 12.8±1.1 gm/dl ranging from 10.5 to 14.5 gm/dl in group B. The mean difference of Hb level was statistically significant (p<0.05) between two groups in unpaired t-test. It can be concluded that low iron level is associated with telogen effluvium in women. Abstract INTRODUCTION: Recent studies in male subjects with haemophilia have described a prevalence of cardiovascular risk factors (CVRF) and cardiovascular events (CVE) similar to the general population. This finding has not been tested in severe haemophilia A carriers so far. We have little information about whether there is a particular bleeding profile in this group and if this tendency is able to modify cardiovascular risk in these women. OBJECTIVES: To evaluate bleeding profile from laboratory and clinical point of view in haemophilia A carriers and working population controls; to define and to calculate CVRF, CVE and cardiovascular risk scores in severe haemophilia A carriers and controls; to analyse if there is any difference in cardiovascular risk between symptomatic severe haemophilia A carriers and general population. PATIENTS AND METHODS: This is a descriptive, cross-sectional, non interventional, single center study. Ethics Committee evaluation and written informed consent are requested to be included for carriers and controls. The target population are severe haemophilia A carriers from our area aged between 18 and 70 years old. The control group are women from regular health laboral checkings. We evaluate bleeding, ischemic and thrombotic personal and familiar history, bleeding profile (ISTH/SSC bleeding assessment tool, ISTH BAT), factor VIII (FVIII) genetic study, complete blood count, basic biochemistry, haemostasis (aPTT, PT, fibrinogen, platelet function tests, FVIIIc, FvWAg and FvWRCo, FXIII, homocysteine, resistence to APC, antithrombin, protein C and S, 20210A prothrombin mutation), cardiovascular risk (Framingham score and Systematic Coronary Risk Evaluation Project, SCORE). The controls have been studied in the same way with the exception of laboratory studies of hemostasis. Only in controls with pathologic ISTH BAT (greater than 3), basic and primary hemostasis have been studied. To describe continuous variables we will use mean, median, standard deviation, maximum and minimum. For categorical variables will be used the percentage of every category. RESULTS: Out of a total of 81 carriers have been identified between August 2012 to December 2013. We have evaluated 69 carriers. To achieve a confidence level of 95% with 50% heterogeneity we have recruited 138 controls. The mean age of carriers and controls was 43.7+/-15 and 41.5 +/-11.7 years old (p 0.308). In the group of carriers, the mean and standard deviation (SD) of FVIII levels were 87.2+/-35.7%, FvW:RCo 75.6 +/-30% and vWF:Ag 75.6 +/-30, 1%. We found no relationship between levels of FVIII:c and haemophilia genetic defect (34.8% substitutions, 34.8% intron 22, 27.5% mutations). 20.3% of carriers and 2.2% of controls present a pathologic ISTH BAT score (p 0.001). We found two cases of thrombophilia. They are two women from the same family with high homocysteine levels and family history of heart attack and stroke in haemophiliacs men. Most of family history of ischemia in carriers group comes from haemophiliac male relatives. Among controls only one patient has experienced heart attack and other a deep vein thrombosis. They both were older controls with CVRF. We have analysed separately the 14 symptomatic carriers (pathological ISTH BAT). This particular group has a similar Framingham score to general population but remains in a higher risk of death from vascular event (SCORE) compared to general population. CONCLUSIONS: Low levels of FVIII do not prevent from developing vascular risk factors in syntomatic carriers of severe haemophilia A. In our media, we describe a higher prevalence of HBP, sedentariness, obesity and overweight in the group of carriers than in controls. The risk of suffering a cardiovascular event and the risk of death because of a cardiovascular events is higher in the group of severe hemophilia A carriers than in the working control population, even in symptomatic carriers. Introduction. The relevance of carbohydrate metabolism studying in patients with Cushing disease can be explained by frequent occurrence of glucose metabolism disturbances on the one hand, and difficulties in glucose-lowering therapy in these patients on the other. The effectiveness of hyperglycaemia treatment may be reduced due to difficulties in remission / cure of the underlying disease, as well as to the use of specific drug-therapy, leading to the hyperglycaemia. There is a growing interest in research aimed at studying the role of incretin system in the pathogenesis of secondary hyperglycemia associated with neuroendocrine diseases recently.Methods. A total of 20 patients with Cushing disease were included, (19 female and 1 male), the mean age was 37.5 years (18-69). All of the patients were diagnosed with Cushing disease for the first time (using urinary free cortisol levels and MRI-data); none of them had a history of previous drug therapy, radiotherapy or pituitary surgery. The mean HbA1c level was 5,8% (5,3-6,2). All patients underwent OGTT, during which glucose, glucagon, GLP1, GLP2, GIP, ghrelin were measured at 0, 30 and 120 min respectively. The control group included 21 patients without previous history of carbohydrate metabolism disturbances. After OGTT 57% were presented without any carbohydrate metabolism disturbances, 28,57% presented with prediabetes and 14,43% were diagnosed with diabetes.Results. After glucose levels analyzing 40% of patients were diagnosed with early carbohydrate metabolism disturbances ,15% were diagnosed with diabetes. After glucose intake a slight inrease in glucagon levels with a peak by 30’ (p=0,001) compared to gradually decreasing levels in controls was observed . The levels of GIP during OGTT were not significantly different compared to control group. GLP-1 and GLP-2 levels were significantly higher compared to controls (p=0,017 and p<0,001 respectively) with peak levels at 30’. Ghrelin levels were also significantly higher compared to controls (p=0,013)Conclusion. Incretins levels can be possible markers of specific carbohydrate metabolism disturbances in patients with Cushing disease and presumably will help to differentiate steroid diabetes from T2DM. Further investigations needed to prove these speculations.

We recorded the far-field EEG potential evoked by amplitude modulation of acoustic stimuli (the amplitude modulation following response, AMFR) in adults with developmental dyslexia and in a matched control group of adults with no history of reading problems. The mean AMFR recorded from participants with dyslexia was significantly smaller than that recorded from members of the control group. In contrast, the amplitude of the click-evoked auditory brainstem response ABR) was not significantly different between participant groups. Also, there was no difference between participant groups in the latency of the AMFR or ABR. The reduced AMFR in listeners with dyslexia may reflect impaired ability of the auditory system to follow rapid changes in stimulus energy, a cue believed to be important in the perception of speech.

Autoclaving (a process of sterilization) of instruments can lead to rust formation and corrosion especially the cutting instruments because they have a carbide steel tip. The aim of this study was to check the microbial load of instruments after treatment with any of these agents viz. sodium hypochlorite, vinegar, lemon and coconut oil followed by autoclaving and to assess the instruments for corrosion treated by these above mentioned agents followed by autoclaving. A sample of twenty five orthodontic pliers and cutters were selected for evaluation. They were divided into five groups depending on the agent used. After ultrasonic cleaning of these pliers, they were immersed in the respective solutions for 10 minutes, dried and then subjected to autoclaving. Microbial swabs were collected at two stages: before treating the instruments(T0) with anti rusting agents and after autoclaving(T1). Quantitative culture was performed using standard technique. The working edge of each instrument was examined visually on the amount of corrosion seen. The mean difference in colony forming units of each group were sodium hypochlorite-48.6, vinegar-32.2, coconut oil-31.4, lemon-7.4 and control group-16.8. In this study, use of additive agents like sodium hypochlorite, vinegar, lemon and coconut oil did not alter the process of sterilization, wherein, the CFU’s of all the four experimental groups were similar to the control group. Visual examination reveals that the anti-rusting property of coconut oil is far superior compared to all other agents used.

Abstract Purpose Previous studies have showed that age,sex and comorbidities might be associated with severity of patients infected with COVID-19 ,which endangers public health worldwide rapidly.The characteristics and length of negative conversion of Non-severe COVID-19 patients with or without hypertension is limited.This study aims to assess whether non-severe COVID-19 patients with hypertension undergone more longer period of negative conversion .Methods This single-center, retrospective study was performed in Xiantao first People’s Hospital Affiliated to Yangtze University(xiantao, hubei,China) by using medical records.Non-severe COVID-19 patients with a history of hypertension From January 23 to February15 were enrolled as group A. A Control group(group B) was matched (1:1) according to age,sex and the admission date.Data on clinical records, laboratory results, and radiological tests was collected. we conducted all analyses with SPSS software(22.0).Results We enrolled 24 and 24 Non-severe COVID-19 patients with and without hypertension, respectively. The most common symptoms were fever and cough in both groups.The frequency of fatigue is more in patients with hypertension.The levels of AST,ALT and CRP were higher in group A.The mean periods of negative conversion for COVID-19 virus were 17 days(SD:5.5) and 15 days(SD:3.6)for patients with and without hypertension(P=0.021).The mean hospitalization periods were 16.8 days (SD:5.6) and 13.7 days (SD:3.8) , respectively (p=0.083) .Conclusion Non-severe COVID-19 patients with hypertension undergone a longer negative conversion for COVID-19 virus and spent more time on clearing COVID-19 virus.

Abstract Background Lipocalin-2 (LCN2) is a novel adipokine with potential roles in obesity, insulin resistance, and inflammation. This study aims to assess the concentrations of LCN2 and vascular endothelial growth factor (VEGF) expressed in the vitreous humors of patients with proliferative diabetic retinopathy (PDR). Methods The concentrations of LCN2 and VEGF were measured from the vitreous of 67 patients undergoing vitrectomy (20 controls and 47PDR) via enzyme-linked immunosorbent assay (ELISA). Results The vitreous concentration of LCN2 was statistically significantly higher in the PDR group compared to the control group (63,522 ± 30,009 pg/ml versus 1663 ± 1191 pg/ml, respectively; P<0.001). VEGF level was also significantly higher in the PDR group than in the control group (1038 ± 1326 pg/ml versus 9 pg/ml, respectively; P<0.001). The mean vitreous LCN2 and VEGF levels in active PDR patients were significantly higher than that of the inactive PDR patients. The mean LCN2 concentration in vitreous humor was significantly lower in the 28 PDR patients with a history of complete PRP (37,304 ± 16,651 pg/mL) in comparison with 19 PDR patients without preperformed panretinal photocoagulation or with preperformed incomplete panretinal photocoagulation (79,796 ± 24,391 pg/mL). A significant correlation between the vitreous LCN2 level and VEGF level was found in patients with PDR (R=0.34; P=0.019). Conclusions This report shows a significant increase of LCN2 in the vitreous fluid of patients with PDR and present a significant correlation between LCN2 and VEGF, suggesting LCN2 might be involved in the pathogenesis of PDR.

Introduction: Noise Induced Hearing Loss (NIHL) is the first cause of acquired hearing loss. Dentists and dental prosthodontics technicians are exposed to different sounds produced by a variety of devices in their occupations. In this study, we studied the effect of dental prosthesis sounds on the auditory power of the respective staff. Methods: This case-control study was carried out among all technicians of dental prosthesis laboratories in Yazd in 2018 selected by census sampling method. A total of 23 technicians who were willing to cooperate and met the inclusion criteria were included in the experimental group. Furthermore,  23 people who referred to the audiology clinic, were matched with the experimental group in terms of age and gender, and were not working in noisy environment, and had no congenital or acquired deafness were examined as the control group.  Finally, the collected data were analyzed by SPSS version 19 using statistical tests. Result: The mean hearing threshold of the right ear at frequencies of 3000 and 4000 Hz and the mean hearing threshold of the left ear at frequencies of 3000, 4000, 6000, and 8000 Hz was significantly higher in the experimental than the control group. The mean hearing threshold of the right ear at the frequency of 4000 Hz and the hearing threshold of the left ear at frequencies of 3000, 4000, and 6000 Hz in participants with job history>4 years was significantly higher. Furthermore, the mean hearing threshold of the right ear in the frequency of 4000 Hz and the hearing threshold of the left ear at frequencies of 3000, 4000, and 6000 Hz years was significantly higher in people with job history >30 years. Conclusion: According to findings, the staffs working in dental prosthodontics laboratories suffer from NIHL at frequencies of 3000 to 8000 Hz. The hearing loss in the left ear was more severe  in  people with job history of >30 years and in people with job history of > 4 years.Therefore, the use of hearing impaired and hearing protectors is recommended in these people.
