Objectives. To investigate whether the incidence of acute appendicitis increases in summer and whether complicated cases present more in summer. Methods. A single-center cross-sectional, retrospective study on 697 cases of appendicitis admitted in the year 2018. Inclusion criteria: patients admitted with acute appendicitis who underwent appendectomy of all ages. Exclusion criteria: conservative management. Analysis was performed using Microsoft Excel. Pearson correlation coefficient was calculated to assess the correlation between monthly incidence of appendicitis and mean temperature in that month. Results. Fifty-one patients who were managed conservatively were excluded. Accordingly, 646 patients were included. Ages ranged from three to 77 years. Males comprised the majority (500, 77.4%). Gangrenous, perforated, and purulent appendices were regarded as complicated appendicitis. The highest number of cases were admitted in summer (234), comprising 36.2% of cases. Complicated cases were equal to 65, of which 23 (35.4%) were admitted in summer and 30 (46.2%) in winter. The highest number of cases was during the month of July (68), while the lowest (40) was during February. This corresponded to the highest recorded mean temperature (36.2°C) and second lowest (19.8°C), respectively. Moderate positive correlation (Pearson’s R 0.5183) between the monthly incidence of appendicitis and the mean temperature is noted. Conclusion. More cases of appendicitis were noted during summer. Monthly incidence correlated positively with the temperature. Larger numbers over several years are needed to draw better conclusions and reach the possible causes behind such variation.

At the start of preelampsia there is a failure of cytotrophoblst invasion into the maternal spiral arteries that will lead to decreased uteroplacetal perfusion which will be followed by the failure of the unit fetoplacenter to get enough oxygen from the room intervillous that ultimately lead to a state of hypoxia in placenta. This will cause the expenditure of TNF-α dan IL-1β from placenta and a factors called hypoxia-inducible transcription factors that will spur the trophoblast to produce activin A lot more. This research was conducted by cross sectional method in maternal room of obstetrics and gynecology department of Central General Hospital of Dr. M. Djamil Padang from August 2015 until February 2016 with 20 patients of severe preeclampsia and 20 patients not severe preeclampsia, who met inclusion criteria and there is no exclusion criteria. Then performed statistical analysis using Mann-Whitney test to determine difference in mean maternal activin A serum levels of severe preeclampsia and not severe preeclampsia. The mean maternal serum levels of activin A in severe preeclampsia is 32,55 ± 1,84 ng/ml and in pregnancy with no severe preeclampsia is 8,59 ± 0,59 ng/ ml. Difference in mean maternal serum level of activin A in the two groups was statistically significant (p=0,001). Ma-ternal serum activin A levels is significantly higher in severe preeclampsia than pregnancy with no severe preeclampsia.Keywords: Activin A, severe preeclampsia, not severe preeclampsia

Objective : To find the difference in mean hemostatic factors in severe preeclampsia and eclampsia.Method : The study was analytic descriptive using a cross sectional study design by looking at the subject's Medical Record according to the time and place of research. The sample is the entire medical record of pregnant women who suffer from preeclampsia and eclampsia in the obstetric and gynecology departments of Dr. M. Djamil Padang Hospital in the period 15 January 2016 to 31 December 2017. The assessment was in the form of assessed platelet levels, PT, APTT and D-Dimer. Samples were taken from populations that met the inclusion criteria and did not have exclusion criteria. Sampling using consecutive sampling techniques taken from the Medical Record Dr. M.Djamil Padang. Statistical analysis to assess significance using the T-Test.Result : The severity the condition of pregnancy the lower the platelet mean and PT. Significant differences were obtained between platelets in PEB and eclampsia (p> 0.05). The mean PT, APTT and D-Dimer showed no significant differences in pregnancy conditions. Statistical test with T-Test did not show significant differences in mean PT, APTT, and D-dimer between eclampsia and PEB (p> 0.05).Conclusion : There were significant differences in mean in platelets, whereas in PT, APTT, and D-dimers in PEB and eclampsia there were no significant differences.Keywords: Pregnancy, severe preeclampsia, eclampsia, platelets, PT, APTT, D-Dimer.

Objectives: The objective of this study was to compare the efficacy of ClinicalEvaluation and modified Alvarado scoring system in diagnosing acute appendicitis. StudyDesign: Cross sectional study. Place and Duration of Study: This study was conducted atSurgical Unit III of Civil Hospital Karachi from May 2010 to October 2010. Methodology: Thisstudy consisted of eighty patients. Patients were divided in two groups. Group A for completeclinical evaluation comprising of 40 patients and Group B for modified Alvarado scoring systemcomprising of 40 patients. Inclusion criteria were all patients presenting with RIF pain, nausea,vomiting, fever and/or anorexia, diagnosed as having acute appendicitis preoperatively andundergoing emergency appendectomy during this period, age >12 years and both gender.Exclusion criteria included not willing for surgery, General anesthesia problem, pregnant femalepatients and those who did not give written consent. Results: A total of 80 patients were includedin the study, placed alternatively into two groups of 40 patients each with majority being male(n = 61, 76.3%). The mean age was 22.46 years. The positive predictive value for patients ofGroup A was 92.5% while for Group B was 77.5%. When diagnostic accuracy was compared onthe basis of Gender for the two groups, the positive predictive value for male patients of GroupA and B was 90.09% and 89.28% respectively, but for females the positive predictive valueof Group A and B was 100% and 50% respectively. Conclusion: We conclude that modifiedAlvarado score can be used safely and effectively in diagnosing acute appendicitis in adultmales especially as the score increases from seven to nine.

Pleural effusion is present when there is >15ml of fluid is accumulated in the pleural space. It can be divided into two types; exudative and transudative pleural effusion. Tuberculosis and parapneumonic effusion are the common cause of exudative pleural effusion whereas heart failure accounts for most of the cases of transudative pleural effusion. This study was a hospital based cross sectional study performed at Nepal Medical College during the period of January 2016-December 2016. A total of 50 patients who fulfilled the inclusion criteria were enrolled. Pleural effusion was confirmed by clinical examination and radiology. After confirmation of pleural effusion, pleural fluid was aspirated and was analysed for protein, LDH, cholesterol. The Heffner criteria was compared with Light criteria to classify exudative or transudative pleural effusion. Among 50 patients, 30 were male and 20 were female. The mean age of patient was 45.4±21.85 years. The sensitivity and specificity of using Light criteria to detect the two type of pleural effusion was 100% and 90.9%, whereas using Heffner criteria was 94.87%, 100% respectively(P<0.01). There are variety of causes for development of pleural effusion and no one criteria is definite to differentiate between exudative or transudative effusion. In this study Light criteria was more sensitive whereas Heffner criteria was more specific to classify exudative pleural effusion. Hence a combination of criteria might be useful in case where there is difficulty to identify the cause of pleural effusion.

OBJECTIVES: The objective of this study was to compare working length calculated with conventional radiographs and an electronic apex locator (IPEX II) during the root canal treatment of mandibular anterior teeth. METHODOLOGY: A cross-sectional study was done in the Department of Operative Dentistry, Sardar Begum Dental Hospital, Peshawar during February and March 2018. A consecutive sampling technique was used for sampling. Only 30 patients fulfilled the inclusion criteria of our study. Detailed medical and dental history was taken. Only patient fulfilling inclusion criteria were enrolled in the study. Data were analyzed using SPSS version 20. RESULTS: The mean age for patients was 45.33±5.16. 33% out of 30 patients (10) were male and 20 were females. The mean working length calculated from radiographs was 22.25±1.29 (min 20.09-max 24.10). The mean working length calculated by the electronic apex locator (IPEX II) was 22.17±1.28 (min 20.00-max 24.07). The mean difference between working length calculated by radiograph and electronic apex locator was -0.084mm, which means the working length determined by radiographs and by electronic apex locator has no difference in mandibular anterior teeth with single canals. CONCLUSION: Both the methods can be used effectively in endodontics for single-rooted mandibular teeth, but if both are used in combinations can lead to an improvement in the working length accuracy, which may significantly reduce the number of radiographs exposure, and increase the success and comfort for endodontic patients. Objective: Marital satisfaction is considered as satisfaction with a marital relationship on which the presence of a child has different effects. Concerns about a childfree life and its effect on marital satisfaction in infertile couples are very critical. Therefore, this study was intended to characterize and compare concerns about a childfree lifestyle and the need for parenthood and their relationship with marital satisfaction in infertile couples. Method: A total of 200 men and 200 women who referred to fertility centers in Tehran participated in this cross-sectional study. Convenience sampling method was used for sampling. Demographic survey, ENRICH Marital Satisfaction Scale, and Fertility Problem Inventory were used for data collection. The resulting data were analyzed using descriptive and analytical statistical tests (Pearson Correlation Coefficient and Stepwise Regression). Results: The mean scores for concern about a childfree lifestyle and the need for parenthood in women were significantly higher than in men. The variables rejection of a childfree lifestyle and the need for parenthood were respectively predictors of marital satisfaction in women and men. Conclusion: Since marital satisfaction in infertile couples is affected by their feelings about having a child and becoming a parent, it is therefore suggested that appropriate counseling be provided in supportive healthcare programs for infertile couples to promote their marital satisfaction.

Purpose: Purpose of this study is to evaluate the measuring consistency of central refraction between multispectral refraction topography (MRT) and autorefractometry.Methods: This was a descriptive cross-sectional study including subjects in Sun Yat-sen Memorial Hospital from September 1, 2020, to December 31, 2020, ages 20 to 35 years with a best corrected visual acuity of 20/20 or better. All patients underwent cycloplegia, and the refractive status was estimated with autorefractometer, experienced optometrist and MRT. We analyzed the central refraction of the autorefractometer and MRT. The repeatability and reproducibility of values measured using both devices were evaluated using intraclass correlation coefficients (ICCs).Results: A total of 145 subjects ages 20 to 35 (290 eyes) were enrolled. The mean central refraction of the autorefractometer was −4.69 ± 2.64 diopters (D) (range −9.50 to +4.75 D), while the mean central refraction of MRT was −4.49 ± 2.61 diopters (D) (range −8.79 to +5.02 D). Pearson correlation analysis revealed a high correlation between the two devices. The intraclass correlation coefficient (ICC) also showed high agreement. The intrarater and interrater ICC values of central refraction were more than 0.90 in both devices and conditions. At the same time, the mean central refraction of experienced optometrist was −4.74 ± 2.66 diopters (D) (range −9.50 to +4.75D). The intra-class correlation coefficient of central refraction measured by MRT and subjective refraction was 0.939.Conclusions: Results revealed that autorefractometry, experienced optometrist and MRT show high agreement in measuring central refraction. MRT could provide a potential objective method to assess peripheral refraction.

Exposure to lead-based paints is a major threat to the health of painters. This study aimed to evaluate the blood concentration of lead (Pb) in painters of buildings and cars. The present study was a cross-sectional study in which a semi-structured questionnaire was used to collect the socio-demographic information. Lead concentration in blood samples was determined using the atomic absorption spectrometry method. A total of 32 male painters were selected based on inclusion criteria. The mean blood lead level (BLL) in the painters was 8.1 ± 4.93 μg/dL. Pb levels in car and building painters were 9.42 ± 5.5 μg/dL and 6.7 ± 1.85 μg/dL, respectively. Pb concentration in none of the blood samples was more than 30 μg/dL. The prevalence of BLL ≥ 5 μg/dL and BLL ≥ 10 μg/dL was 97% and 19%, respectively. According to the findings, the rate of BLL among car painters was higher than building painters. Considering the presence of Pb in all blood samples, it seems necessary to increase the awareness of painters about the adverse effects of lead exposure even in low concentrations. However, the sample size in this study was small and more investigations are required in this regard.

Objective: To determine the mean placental birth weight ratio at term in primigravidae Study design: Cross-sectional study Place and Duration: Department of Obstetrics and Gynecology, Civil Hospital Karachi, duration was six months after the approval of synopsis from 1st January 2016 to 30th June 2016 Subjects and Methods: A total of pregnant women who fulfill the inclusion criteria were included in this study. After delivery, baby was weighed by using weight machine and weight of baby was also noted (as per operational definition). After expulsion of complete placenta, placental weight was measured by using weight machine. The placental-birth weight ratio (PBWR) were calculated as ratio of placental weight to neonatal weight multiplied by 100. Results: Mean ± SD of maternal age was 24.77±4.04 with C.I (24.11----25.42) years. Mean ± SD of placental weight was 505.84±99.97 with C.I (489.71----521.97) grams. Out of 150 neonatal babies 101 (67.3%) were male and 49 (32.7%) were female. Mean placental birth weight ratio was found to be 16.82±2.63 with C.I (16.39----17.24). Conclusion: It is to be concluded that placental weight increased according to the birth weight. The placental weight to birth weight ratio decreased slightly with advancing gestational age. ABSTRACT Objective: To evaluate the amount of buccal and lingual supporting bone tissue of 60 upper central incisors and the relationship with their inclination. Materials and Methods: Thirty healthy adult patients with no previous orthodontic treatment were evaluated using cone-beam computed tomography. Cross-sectional views were analyzed to check the amount of the bone tissue on the cervical (cervical buccal thickness/CBT; lingual/CLT), middle (middle buccal thickness/MBT; lingual/MLT), and apical regions (apical buccal thickness/ABT; lingual/ALT). The Pearson correlation, linear regression, and analysis of variance tests were used (P &lt; .05). Results: The values of ABT of both teeth (11, right upper central incisor; 21, left upper central incisor) were significantly increased with the increase in the angle between the axis of the upper central incisor and the palatal plane (1/PP) (tooth 11 P  =  .034; tooth 21 P  =  .009), yet without a strong linear correlation. At the buccal and lingual surfaces, the mean apical supporting bone tissue was significantly greater than the other areas, and the middle region significantly greater than the cervical (P &lt; .001). Conclusions: For both surfaces (buccal and lingual), the amount of bone tissue in the apical region was significantly higher than the middle and cervical regions, and the middle region was significantly higher than the cervical region. In relation to the upper central incisor's inclination, the higher the 1/PP the higher was ABT. However, the coefficient values for both teeth were low.
