The Pneumatic Transfer System (PTS) is an auxiliary system of Ghana Research Reactor-1 (GHARR-1) used to transfer the sample capsule in and out of the reactor irradiation sites. The PTS′ controller unit design and construction was carried out because the original transfer system was not designed to operate in Cyclic Neutron Activation Analysis (CNAA). To address these situations, a Programmable Logic Controller (PLC) has been used to design and construct a control unit to facilitate a CNAA application for GHARR-1. The design has been simulated successfully using LOGO! Soft Comfort software, version 8. A Failure Mode and Effect Analysis (FMEA) was conducted on the PTS Control Unit (PTSCU) to evaluate and document, by item failure mode analysis, the potential impact of each functional or hardware failure of the control unit, personnel and system safety, system performance, maintainability, and maintenance requirements. Each potential failure is ranked by the severity of its effect so that appropriate corrective actions can be taken to eliminate or control the high-risk items. The result obtained upon the analysis shows that the likelihood of occurrence of failures, detection, and severity on the control unit is low per the risk priority number. The paper outlines the severity classification and description used in FMEA, the likelihood of detecting various failures of components, and failure causes and effect.

Abstract The characterization of failure modes of DRAM is often straight forward if array related hard failures with specific addresses for localization are concerned. The paper presents a case study of a bitline oriented failure mode connected to a redundancy evaluation in the DRAM periphery. The failure mode analysis and fault modeling focus both on the root-cause and on the test aspects of the problem.

Purpose – The purpose of this paper is to evolve a guideline for scientists and development engineers to the failure behavior of electro-optical target tracker system (EOTTS) using fuzzy methodology leading to success of short-range homing guided missile (SRHGM) in which this critical subsystems is exploited. Design/methodology/approach – Technology index (TI) and fuzzy failure mode effect analysis (FMEA) are used to build an integrated framework to facilitate the system technology assessment and failure modes. Failure mode analysis is carried out for the system using data gathered from technical experts involved in design and realization of the EOTTS. In order to circumvent the limitations of the traditional failure mode effects and criticality analysis (FMECA), fuzzy FMCEA is adopted for the prioritization of the risks. FMEA parameters – severity, occurrence and detection are fuzzifed with suitable membership functions. These membership functions are used to define failure modes. Open source linear programming solver is used to solve linear equations. Findings – It is found that EOTTS has the highest TI among the major technologies used in the SRHGM. Fuzzy risk priority numbers (FRPN) for all important failure modes of the EOTTS are calculated and the failure modes are ranked to arrive at important monitoring points during design and development of the weapon system. Originality/value – This paper integrates the use of TI, fuzzy logic and experts’ database with FMEA toward assisting the scientists and engineers while conducting failure mode and effect analysis to prioritize failures toward taking corrective measure during the design and development of EOTTS.

The aims of this study were to evaluate the shear bond strength (SBS) of four adhesive systems applied to primary dentin and enamel and verify, after SBS testing, the failure mode of the adhesive interface. Sixty extracted sound primary molars were selected and crowns were sectioned in a mesial-distal direction. Specimens were randomly assigned into two groups (adhesion to enamel and adhesion to dentin) and then subdivided into four subgroups according to the adhesive system (n=15): Scotchbond Multi-Purpose (SMP) – Single Bond (SB) – Clearfil SE Bond (and Adper Prompt LPop (APL) – SBS tests were performed and the obtained values were statistically analyzed using ANOVA and Tukey tests (p<0.05). The failure mode analysis was performed with a Scanning Electron Microscope (XL-30, Philips). SBS mean values on enamel were [MPa (SD)]: SMP – 27.89 (7.49); SB – 23.92 (8.8); CSB – 24.36 (6.69); APL – 25.96 (4.08); and on dentin: SMP – 17.29 (4.25); SB – 18.2 (8.74); CSB – 16.13 (7.14); APL – 6.04 (3.35). The predominant failure mode was cohesive (primarily of the bonding agent). On enamel SBS was statistically similar for all four adhesives. On dentin SBS of APL was lower than the other tested adhesives.
