Background. Esthetic appearance of primary anterior teeth is one of the major demands in the dental field. Destructed anterior primary teeth due to caries is considered a major issue due to the difficulty in regaining the esthetic crowns and attaching them until the normal exfoliation time. There have been many attempts and tools used to attach the composite crowns to the treated canals of primary anterior teeth. The study evaluates endodontic treatment for destructed primary maxillary incisors with a glass fiber-reinforced post as a retentive tool to hold the esthetic composite crowns until the normal exfoliation time of primary incisors. Case Presentation. A four-year-old child attending a dental clinic complained of pain of maxillary incisors. Dental examination showed irreversible pulpitis of four maxillary incisors indicated for root canal treatment and crown placement. Endodontic treatment was carried out, and a glass fiber-reinforced post was used to get successful retention for the composite crowns. Follow-up was carried out for 3 years. The 3 crowns were retained successfully until replaced by permanent incisors. One crown fell during the treatment course. Conclusion. Retention of primary teeth is one of the challenges in pediatric dentistry. Restoration of primary decayed incisors is important for child medical, physical, and psychological conditions. A glass fiber-reinforced post and core is a strong retentive tool for composite crown retention for primary incisors. This procedure opens the door for a strong tool to retain composite crowns for a long time. The glass fiber-reinforced post and core is a strong retentive tool for composite crown retention for primary incisors.

ABSTRACT Background Early childhood caries discloses a distinct clinical pattern, and the teeth most often involved are the maxillary central incisors, lateral incisors, and the maxillary and mandibular first molars. The maxillary incisors are most severely affected, with deep carious lesions usually involving the pulp. Teeth that have been endodontically treated often have little coronal tooth tissue remaining and as such require a post to retain the core and restoration .This study evaluated and compared the efficacy of EverStick glass fiber-reinforced composite post with ParaPost Taper Lux in primary maxillary anterior teeth. Aim An in vivo study was conducted to evaluate and compare the longevity and failures of two fiber post systems in primary maxillary anterior teeth. Materials and methods A total of 60 severely mutilated primary maxillary anterior teeth from children aged 3 to 5 years were selected according to the inclusion criteria. These teeth were treated endodontically and were randomly assigned into two groups with 30 samples in each group, group I: EverStick glass fiber-reinforced composite post, group II: ParaPost Taper Lux post. The evaluation of dislodgment of posts, secondary caries, root fracture, and post fracture was carried out clinically and radiographically during every follow-up at 3, 6, 9, and 12 months interval. Results Statistical tests (Chi-square test, Fisher's exact probability test) suggested that dislodgment of the posts was significant between the two groups at 6, 9, and 12 months follow-ups. But within the group during subsequent follow-up intervals, dislodgment of posts as a mode of failure was not statistically significant. However, clinically failures were seen in both the study groups. Conclusion Fiber post system has proved to be successful clinically in both primary and permanent teeth due to the monoblock effect with luting agent, post system, core material, and bonding to dentin. Thus, today the EverStick glass fiber post system provides a novel way of fabricating cost-effective and less time-consuming custom-made post in treating mutilated maxillary anteriors. Objectives. The main purpose of the study was to assess the material wear, antagonistic natural primary teeth wear, and microhardness of zirconia (ZR), a recently launched novel glass-fiber-reinforced composite crown (GFRC). The research question was, are these aesthetic crowns resulting in antagonistic natural primary tooth wear and the crown material itself? Methods. Forty-five primary canines were divided into three groups (15 per group) and mounted against Zr (Group A), GFRC (Group B), and natural teeth as control (Group C) in the wear test machine. All samples were assessed for surface wear with pre- and post-3-dimensional scanning. In addition, microhardness was assessed for all three groups. Results. The mean microhardness value for the Zr disc was 1157 ± 7 HV; for the GFRC disc, it was 29.35 ± 2 HV; while with natural teeth, it was 105 ± 4 HV. There was a statistically significant difference in teeth wear in the prescan and postscan in the natural tooth ( p < 0.05 ) group, highly significant difference ( p < 0.001 ) in the ZR group, and no significant difference in the GFRC group. Conclusion. There is more significant wear loss of glass-fiber-reinforced composite discs as compared to zirconia. In addition, the wear of the antagonistic tooth with zirconia and natural teeth is more remarkable than with GFRC. There is a vast difference of microhardness between natural teeth and zirconia (almost 10 times higher) which suggests further scope of study. Clinical Relevance. Pediatric dentistry deals with the transition of dentition from primary to permanent through mixed dentition. Selection of restorative material needs to be done cautiously when we are dealing with primary teeth and young permanent teeth as antagonistic teeth. Wear of the crown material itself and opposing natural teeth are essential factors that should be considered in selecting crowns in clinical practice. The present study results can be extrapolated to clinical practice, and the practitioner can consider various factors in selecting full-coverage crowns for primary teeth. The vast difference in aesthetic crowns and natural teeth microhardness indicates a further need for research. Additionally, there is no literature published for the recently launched GFRCs.

Background: Extensive restorative treatments of anterior primary teeth are challenging in pediatric dentistry. Objective: This study aimed to compare the fracture resistance of three post types for restoring the anterior primary teeth. Methods: This in-vitro study was carried out on 90 extracted maxillary anterior primary teeth with intact roots. They were randomly allocated into six groups to be restored with conventional composite resin post, X-tra fill composite resin post, Tetric N Ceram composite resin post, prefabricated glass fiber post with conventional composite build-up, prefabricated glass fiber post with X-tra fill composite build-up, and prefabricated glass fiber post with Tetric N Ceram composite build up. The samples were polished and placed in acrylic resin blocks with 1 mm of part of cervical root being out, thermocycled (×5000) and tested for fracture resistance. Intra-class correlation test, Kruskal-Wallis test and one-way ANOVA were used for statistical analyses (α=0.05). Results: The mean fracture resistance was significantly different among the six groups. It was the highest in prefabricated glass fiber post with conventional composite build-up (418.64 N), prefabricated glass fiber post with X-tra fill composite build-up (403.63 N) and prefabricated glass fiber post with Tetric N Ceram composite build up (361.63 N); and the lowest in Tetric N Ceram group (280.65 N). The groups were significantly different concerning the fracture strength and fracture state. Conclusion: Since the anterior teeth restored with prefabricated glass fiber posts were far more fracture resistant, and prefabricated glass fiber posts can be promisingly used for the restoration of anterior primary teeth.

ABSTRACT Introduction and aim Premature exfoliation or extraction of primary tooth may lead to loss of space due to drifting of the adjacent teeth. This may also lead to undesirable effects, such as impaction of the succedaneous tooth, overeruption of the opposing tooth, and a shift of the midline, with consequent functional impairment. It is prudent to consider space maintenance when primary teeth are lost prematurely. The current study aims to clinically evaluate the effectiveness of space maintainers comparing conventional band and loop space maintainer with glass fiber-reinforced space maintainer following loss of primary teeth in mixed dentition. Materials and methods A total of 30 children aged 4 to 8 years were randomly assigned into two groups – band-and-loop space maintainer (group I) and glass fiber-reinforced composite (GFRC) space maintainer (group II). Distortion/dislodgment of the loop or the fiber frame, fracture of loop or fiber frame, gingival health, and space loss were evaluated clinically and radiographically at 1, 3, 6, and 12 months interval. Gingival health was assessed by gingival index given by Loe and Silness. Results Based on the statistical analysis, overall success rate of group I was found to be 69.2% and of group II was 95.7% (p = 0.026), which was statistically significant.
