Successful endodontic treatment depends on cleaning and disinfecting the root canals, in order to provide conditions for three-dimensional filling, which should prevent root canal reinfection. However, anatomical complexities pose a challenge during endodontic treatment. The present study was a literature review carried out in the following databases: PubMed, SciELO, and MEDLINE, which were searched for articles published from 2017 to 2021. Micro-CT studies published in English, which analysed the capacity for preparation of oval and long-oval root canals, were included. The following keywords were used: “oval-shaped canals,” “long-oval-shaped canals,” “endodontics,” and “micro-CT.” The aim of this study was to carry out a literature review of micro-CT studies on the scope of the capacity for preparation performed in oval and long-oval root canals with rotary and reciprocating instruments.

ABSTRACT Introduction The study aimed to compare the shaping and preservation of the original curvature of simulated curved root canals using the following instruments: Reciproc (Rcp), WaveOne (Wo), and the ProTaper Next system (Ptn). Materials and methods A total of 45 resin blocks with simulated curved root canals were divided into three groups (n = 15), prepared using the Rcp (R25), Wo (25/0.8), and Ptn (X2) instruments. Standardized photographs were taken before and after canal instrumentation. After the superimposition of the images, the amount of resin removed from the curvature's inner and outer walls was measured at six apical levels, at intervals of 1 mm. The canals’ angles of curvature before and after instrumentation were subtracted. Results There were no significant differences between the instruments in terms of the total amount of resin removed of the inner or outer walls of the apical curvature (p > 0.05). The Rcp instruments provided the best resin removed ratios between the walls. The means of the change in angle were as follows: Wo = 2.15°, Ptn = 0.92°, and Rcp = 0.21°. WaveOne caused significantly higher deviations than Rcp. Conclusion All of the instruments demonstrated a tendency to straighten the simulated root canal. Instruments that use rotary movement achieved an effect similar to that of the reciprocating instruments in relation to change in angle. Clinical significance Deviations from the original shape of the root canal could have a negative impact on the quality of a filling and consequently on the success of the endodontic treatment. Por meio deste estudo é possível analisar que a instrumentação dos canais radiculares após o uso do Glide Path traz inúmeros benefícios, incluindo o melhor deslizamento dos instrumentos, redução do número de fraturas e menor intercorrências intrarradiculares. Além disso, essa preparação inicial do canal radicular com o uso do Glide Path realiza uma desobstrução, criando um “caminho” livre para o melhor deslize dos instrumentos no canal radicular. Os instrumentos possuem características e particularidades que levam ao mesmo objetivo de preparar o conduto para uma segura instrumentação, destaca-se aqui o WaveOne Gold, R-Pilot e o PathFile. É uma técnica que vem sendo difundida e utilizada na endodontia contemporânea com o intuito de realizar um procedimento eficaz.---Abstract: Analyze the use of instruments to prepare the Glide Path and understand how this endodontic instrument brought advantages in instrumentation techniques. This is a literature review with selected articles from the last 10 years, in order to achieve the objectives proposed by the theme. Articles in English and Portuguese were used, with all information pertinent to the topic extracted. Through this study, it is possible to analyze that the instrumentation of root canals after the use of the Glide Path brings numerous benefits, including better sliding of the instruments, reduction in the number of fractures and less intra-radicular complications. In addition, this initial preparation of the root canal using the Glide Path performs clearance, creating a clear “path” for the best glide of the instruments in the root canal. The instruments have characteristics and particularities that lead to the same objective of preparing the conduit for safe instrumentation, the Wave One Gold, R-Pilot and PathFile stand out here. It is a technique that has been disseminated and used in contemporary endodontics with the aim of performing an effective procedure. Through this bibliographical study, it is possible to consider that the use of the Glide Path favors the preparation of the root canal and proves to be effective and safe when used before instrumentation, thus, with the creation of a more favorable environment for instrumentation, the chances of success in endodontic treatment.

It is well described that the presence of microbial flora in the pulp space of the tooth, and the inability of the immune system to remove these pathogens, are the major sources of peri-apical and radicular inflam-mation.1 Eradication of these pathogens from the pulp and root canal space by means of cleaning, shaping, disinfecting and complete obturation is necessary to safeguard the health of the periodontal tissues from endodontic infection and subsequent breakdown.2,3 The basic objectives of cleaning and shaping of root canals include: (1) removal of all infected soft and hard tissues; (2) creating space for delivery of disinfectants and medicaments to the apical part of the canal; (3) facilitating three dimensional obturation and (4) preservation of radicular structures.4 Even modern endodontic file systems leave untouched areas on the root canal walls after preparation and show compaction of hard tissue debris.5 This debris consists of pulp tissue remnants, bacteria and dentine chips of which most is found in the apical part of the prepared root canal system.6 Aiming for a centred preparation that corresponds to the original canal anatomy accompanied by the lowest amount of canal transportation, especially in middle and apical parts of curved canals, will result in the most favourable post instrumented canal shape. The four optimal canal shaping objectives are: (1) to have a tapered funnel from orifice to apex, (2) maintenance of original anatomical canal pathway, (3) apical foramen position should remain constant and (4) leaving the apical opening as small as possible.2,7

Instrument fracture is a serious complication during endodontic treatment of teeth, having an adverse effect on the outcome of the nickel titanium (NiTi) treatment, especially if the fracture prevents apical access to the infected root canal. Despite the advent of NiTi files, the risk of fracture during the endodontic preparation of root canals, especially in severely curved canals, remains a serious concern. The fracture of NiTi files during preparation may result in a compromised prognosis for the tooth. In the presence of periapical lesions, instrument fracture may reduce the chances of successful healing.

Endodontic therapy seeks to prevent and when necessary to eliminate endodontic infections. The persistence of microorganisms to the mechanical chemical preparation or the intracanal medication is considered the main cause of the failures of the endodontic treatment. In these cases Enterococcus faecalis is the most commonly found microorganism. Photodynamic therapy (PDT) emerges as a promising adjuvant therapy to conventional endodontic treatment to eliminate these persistent microorganisms. This therapy consists in the association of a specific light source with a photosensitizer in the presence of oxygen, producing cytotoxic species to the microorganisms. Thus, the present article aims at a review of the literature on the use of photodynamic therapy in endodontics, a test that is shown to assist in the preparation of the root canal.

Background: One of the key factors in successful endodontic therapy is to adequately fill the root canals. Vertical condensation technique is regarded as a high probability of three-dimensional filling of root canal space which is required to achieve long-term success. Aim: To evaluate the endodontic treatment outcomes of vertical condensation technique using Touch’N Heat device. Subjects and methods: A prospective, descriptive study was conducted across two hospitals in Hue city from 11/2016 to 5/2017. All teeth were instrumented by manual Protaper files and obturated by the vertical condensation technique with Touch’N Heat device. The radiographic evaluation for initial assessment of obturation quality was performed. Evaluation of endodontic treatment outcomes in terms of clinical symptoms and radiographic assessment were recorded after 3-month follow-up. Results: A sample of 30 patients (with 39 incisors) from 13 to 73 years old was recruited. The principal reasons for visiting were pain (30.8%), decay (20.5%) or trauma (20.5%). Most patients seeked treatment at chronic stage of diseases with chronic pulpitis (38.4%) and chronic periapical periodontitis (30.8%). In radiographical quality of obturation, there were 27 (69.3%) fulllength obturated teeth, 8 (20.5%) overfilled teeth (sealer). The percentage of homogeneity of root canal filling was 82.1%. Over 82% patients (25 patients with 32 teeth) presented after there-months followup. Clinical assessment yeiled 30 (93.7%) “adequate” teeth, 2 (6.3%) “poor” teeth. Refer to radiographical assessment, the teeth were defined as “success”, “questionable” and “failure” (29 teeth (90.6%), 3 teeth (9.4%) and zero, respectively). Conclusions: The vertical condensation using Touch’N Heat device ensure the full-length obturation, high homogenetious root canal filling, and sucessful prognosis in clinical and radiographic outcomes. Abstract This study aimed to analyze the potential occurrence of dentinal defects after the removal of a root canal filling with two different sealers using Reciproc (RC) or Reciproc Blue (RB). The mesial roots of 60 mandibular molars with a Vertucci type IV configuration were selected. The samples were initially instrumented with Reciproc (R25) and then divided into the following four experimental groups according to the endodontic sealer and retreatment instrument (n = 15): BC Sealer/Reciproc (BCRC); BC Sealer/Reciproc Blue (BCRB); AH Plus/Reciproc (AHRC); and AH Plus/Reciproc Blue (AHRB). Then, the samples were scanned under micro-CT after obturation and removal of the filling material. Two analyses were conducted. First, an evaluation was performed on all the axial images, and another analysis evaluated each millimeter of the 10 mm from the apex. Dentinal defects were observed in all the samples. All of the identified defects in the images after filling material removal were present in the corresponding images after obturation. The use of AH Plus and EndoSequence BC Sealer, and filling material removal using RC and RB instruments did not induce dentinal defects.

The aim of this study was to undertake a literature review on the use of XP-endo Shaper (FKG Dentaire SA, La Chaux-de-Fonds, Switzerland) for root canal preparation evaluated using microcomputed tomography (micro-CT) technology, with emphasis on the analysis of untouched canal walls. We searched the PubMed, Science Direct, SciELO, and MEDLINE databases for articles published from 2017 to 2019 using the following keywords: micro-CT, untouched walls, XP shaper, and XP endo. Articles without an abstract were excluded. Five papers were selected. Few studies on this topic are available. The studies published so far show that the XP-endo® Shaper system performs well in terms of canal preparation but leaves untouched walls.

Evidence indicates that root canal preparation is a critical step upon which the prognosis of the endodontic treatment depends. Vertical root fractures can furtherly develop as propagations from the previously formed dentin cracks. Therefore, root canal preparation should be carefully approached with the minimal crack formation and careful dentin removal. Different modalities have been proposed in the literature and rotary systems are most commonly used nowadays. Although the reported efficacy of these modalities is high, evidence shows that they are usually associated with a considerable risk of crack formation. In the present literature review, we have discussed the effects of rotary instrumentation and root canal preparation on dentinal crack formation based on evidence from the different studies in the literature. Our evidence showed that all of the investigated approaches to prepare root canals adequately can result in crack formation. The different rotary instrumentations also showed variable frequencies of developing dentinal cracks, and evidence from some comparative investigations shows that ProTaper modalities are associated with the least frequencies of the events. However, to make a solid conclusion for clinical practice, further research is still needed to validate our current evidence.

The objective of this study was to evaluate the efficacy of reciprocating instruments in the removal of bioceramic and epoxy resin-based sealers using micro-CT analysis. Root canals of 40 extracted human teeth were instrumented with a size R25 Reciproc instrument. Specimens were randomly divided into two groups (n = 20) according to the root canal filling material. In the first group, root canals were obturated with AH Plus sealer and Reciproc R25 gutta-percha. In the second group, a combination of bioceramic gutta-percha (TotalFill BC) and bioceramic sealer (TotalFill BC) was used. After one week, the retreatment of all specimens was performed using Reciproc instruments. To analyze the differences in the filling remnants, specimens were scanned in a micro-CT device after obturation and after the retreatment procedure. Statistical analysis was performed using the Mann–Whitney U test (p < 0.05). A combination of bioceramic sealer and bioceramic gutta-percha was more effectively removed from canals using a reciprocating instrument, with a filling remnants volume of 4.01 ± 3.13 mm3, in comparison to the combination of epoxy resin-based sealer and gutta-percha (6.96 ± 2.70 mm3) (p < 0.05). A reciprocating instrument was more effective in removing bioceramic sealers than epoxy resin-based sealers, although none of the root canal filling materials were completely removed from the root canals.
