Knee osteoarthritis (KOA) has become one of the leading causes of workforce loss in the middle-aged and elderly population and a global public health problem second only to cardiovascular disease, so we need to find more effective treatments for this disease. In this study, we selected 120 patients with KOA admitted to our hospital from June 2018 to December 2020 and divided them into treatment group 1, treatment group 2, and joint group according to the random number table method, with 40 patients in each group. Treatment group 1 was treated with Tongluozhitong prescription dip-soaking therapy, treatment 2 group was treated with intra-articular injection of sodium hyaluronate, and the joint group was treated with a combination of both modalities for 4 weeks in all three groups. Clinical efficacy, visual analogue scale (VAS), Lysholm knee score (LKS), activity of daily living score (ADL), the levels of bone metabolic markers such as cartilage oligomeric matrix protein (COMP), type II collagen degradation maker (CTX-II), and matrix metalloproteinase-3 (MMP-3), and the levels of inflammatory mediators such as interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), and hypersensitive C-reactive protein (hs-CRP) were used as observations to compare and analyze the therapeutic effects of the three treatment regimens in KOA patients. The results showed that the clinical excellence rates of the joint group, treatment group 1, and treatment group 2 were 72.50%, 50.00%, and 90.00%, respectively, with statistically significant differences between any two comparisons. After treatment, VAS scores, serum COMP, CTX-II, MMP-3, IL-1β, TNF-α, and hs-CRP levels decreased in all three groups, and the levels of each index were as follows: joint group < treatment group 1 < treatment group 2, and the difference between any two comparisons was statistically significant. The LKS score and ADL score increased in all three groups, and the levels of each index were as follows: joint group > treatment group 1 > treatment group 2, with statistically significant differences in any two groups compared. None of the patients in the three groups experienced any significant adverse effects during treatment. This suggests that the dip-soaking therapy of Tongluozhitong prescription is more advantageous than intra-articular sodium hyaluronate injection treatment in suppressing the level of serum bone metabolic markers and inflammatory mediators, reducing pathological joint damage, relieving symptoms of pain, alleviating degenerative joint symptoms, and improving knee function in KOA patients. The combination of the two in KOA patients can significantly improve the efficacy and has a good safety profile.

The state of low oxygen levels known as hypoxia in humans is considered a dangerous condition is apparently a normal physiological condition and required by the stem cells as they are in the body. Mesencyimal Stem Cells (MSCs) require physiologically optimal conditions of low O2 tension of 1-3% in the bone marrow. The purpose of this study was to reveal the difference between in vitro culture of MSC in normoxia condition (20% O 2 concentration) with hypoxia condition (1% O2 content) especially in terms of viability, pluripotent properties, and MSC proliferation ability of the culture it produces. This research is an explorative laboratory research invitro on Bone Marrow Mesenchymal Stem Cells (BMSCs) culture using hypoxia condition. The study design used Randomized Control Group Post-Test Only Design. This research was conducted for 2 months. There was a significant difference in mean slow proliferation based on the number of Least-like CFU-Cs between the control group, treatment group 1 and treatment group 2, the mean percentage of the number of cells expressing the OCT4 coding gene on immunofluorosense examination between the control group, 1 and the treatment group 2 and the mean percentage of cell numbers expressing the OCT4 coding gene on the immunofluorosense examination between the control group, the treatment group 1 and the treatment group 2 showing p <0.01. There was a significant difference of percentage of non-absorbing color cell number of trypsin blue (viable cells) between control group, treatment group 1, and treatment group 2 showing p value <0.05. This suggests that the precondition of culture with normoxia provides an opportunity for cells to adapt and proliferate before being conditioned in hypoxic cultures. Cultures with hypoxic conditions and preconditions of normoxia are the best culture conditions because they produce cells that are capable of maintaining pluripotency properties while still having better proliferation and viability capability compared with direct hypoxia conditions.Keywords: Hypoxia, normoxia, bone marrow, mesenchymal stem cells.

ABSTRACTBackground: Collagen has vital role on healing process of burn injuries, mainly in connective tissue.Collagen triggers fibroblast proliferation, support to form new granulation tissue and epithelium aroundwound. Wound treatment through collagen extract of Sangkuriang catfish skin can increase healing process.It stimulates humidity on level II burn injuries and encourage re-epithelization, proliferation and cellmigration also increases growth. Methods: This is an experimental studies using 4 groups of RattusNorvegicus (Wistar strain). All of the group get a burn injuries in their back skin. Control group 1 (K1) wastreated by lidocaine for three days and control group 2 (K2) for ten days. Whereas treatment group 1 (P1) getlidocaine and collagen extract from Sangkuriang cat fish for 3 days and treatment group 2 (P2) for 10 days.Result: Number of fibroblast/field of view on treatment group showed significantly increased comparedcontrol groups on the 3rd days (P=0,046) and the 10th (P=0,004). Percentage of TNF-α on level II-burninjuries in wistar rats showed significant difference. Percentage of TNF-α lower than treatment group andshowed significant lowering level compared control groups on the10th (P=0,022). Conclusion: Increasing offiroblast number and lowering level of TNF-α significantly showed collagen extract of Sangkuriang cat fishskin accelerate wound healing process.

Inflammation of the periodontal tissue (periodontitis) is the highest problem of oral health in Indonesia after caries. Photoacoustic imaging (PAI) is a new imaging technique that can be simply constructed using a diode laser combined with a condenser microphone. This study aims to determine that a simple PAI system was able to image periodontal disease in animal model. Samples of the study were normal periodontal and periodontitis tissue, obtained from Sprague-Dawley rats that were divided into four groups, i.e. the control group, treatment group 1 (7 days periodontitis induction), treatment group 2 (11 days periodontitis induction), and treatment group 3 (14 days periodontitis induction). The PAI system was controlled by Labview and Arduino IDE software from a personal computer. Results of the study reveal that the optimal frequency of laser modulation for periodontal tissue imaging was 19 kHz with duty cycle of 50%. Photoacoustic (PA) intensity was obtained from higher to lower of -68,71 dB (treatment group 3), -70,69 dB (treatment group 2), -71,69 dB (treatment group 1), and -73,07 dB (control group) respectively. The photoacoustic images were analyzed to define the contrast between sample and media. The PA intensity of the samples were higher than media. Therefore, this study demonstrate the feasibility of simple PAI system to differentiate normal periodontal tissue and periodontitis.

Background: We aimed prospectively investigate the laboratory and electrocardiographic parameters (hearth rate, QRS, QT, QTc, Tpe, Tpe/QTc, arrhythmia prevalance) in patients with graves disease before and after antithyroid therapy. Methods: 71 patients (48 female, 23 male), age between 18-50 (mean±SD: 36.48±12.20 ) with GD were included into the study. Patients treated with antithyroid therapy (thionamids and/or surgical therapy) to maintain euthyroid status. Patients were examined in terms of electrocardiographic parameters before and after the treatment. Results: Mean TSH, free thyroxin (fT4) and tri-iodothyrionine (fT3) levels of all patients were 0.005±0.21, 3.27± 1.81, 11.42±7.44, respectively. While 9 patients (group 2) underwent surgical therapy, had suspicious of malignant nodule or large goiter and unresponsiveness to medical treatment; the other patients (n=62, group 1) were treated with medical therapy. Patients with surgical therapy had more increased serum fT4 (p=0.045), anti-thyroglobulin value (p=0.018) and more severe graves orbitopathy (n=0.051) before treatment when compared to medical therapy group. Baseline Tpe duration and baseline Tpe/QTc ratio and frequency of supraventricular ectopic beats were found to be significantly higher in group 2 when compared to group 1 (p=0.00, p=0.005). Otherwise baseline mean heart rate, QRS duration, QTc values of both groups were similar. Although the patients became their euthyroid status, group 2 patients had still suffered from more sustained supraventricular ectopics beats than group 1. Conclusion: Distinct from medical treatment group, surgical treatment group with euthyroidism at least 3 months had still suffered from an arrhythmia (Tpe, Tpe/QTc, supraventricular and ventricular ectopic beats).

Purpose: To determine the therapeutic effect of Xuezhitong capsule in patients with microvascular angina (MVA), and its impact on vascular endothelial function.Methods: In total, 172 MVA patients treated in Beijing City Fengtai District Nanyuan Hospital from September 2017 to September 2019 were selected and randomized into control group which received conventional treatment, and treatment group which received Xuezhitong capsules plus. There were 86 patients in each group. Therapeutic effect, levels of inflammatory factors, i.e., high-sensitivity C- reactive protein (hs-CRP), interleukin-(IL-6), tumor necrosis factor-α (TNF-α), and vascular endothelial factors such as nitric oxide (NO), thromboxane B2 (TXB2) and endothelin (ET), were determined.Results: Markedly higher total treatment effectiveness was observed in the treatment group than in the control group (89.53 % vs. 72.94 %; p < 0.05). In both groups, treatment reduced the levels of hs-CRP, IL-6, TNF-α, TXB2 and ET, but elevated NO, with better results for treatment group than the control group (p < 0.05). Better optimizations of total cholesterol (TC), triglycerides (TGs), low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein (HDL) were observed in the treatment group, relative to the control group (p < 0.05). Patients in the treatment group experienced fewer (8.14%) adverse reactions than those in control group (21.18 %, p < 0.05).Conclusion: Xuezhitong capsule, when combined with conventional treatment, exerts high therapeutic effectiveness and safety in MVA patients by inhibiting inflammatory reactions, optimizing endothelialfunction, reducing blood lipid levels, and decreasing the incidence of cardiovascular adverse events. Thus, the combination therapy is a potentially superior therapeutic strategy to the conventional approach for the management of MVA patients.

Introduction. T helper cells (Th)-1 and -2 cytokines homeostasis control or predict clinical outcome of infected persons, especially those with HIV/AIDS. This case-control study evaluated the leucocyte differentials, TNF-α, IL-2 and -10 levels, among HIV-infected persons with serological evidence of leishmaniasis attending the University of Abuja Teaching Hospital, Nigeria. Material and Methods. This study involved blood samples from 28 HIV-infectedwith Leishmania donovani rK39 and IgG positive (group 1), 30 age- and sex-matched HIV-infected individuals without Leishmania antibodies (group 2) and 30 apparently healthy persons without HIV and Leishmania antibodies (group 3). Full blood counts, TNF-α, IL-2 and IL-10 levels were analysed using an automated haematology analyser and ELISA, respectively. Structured questionnaires were used to collate biochemical and clinical data from participants. Results. Ten (35.7%) participants in group 1 were on ART, 15 (50%) in group 2 were on ART, while group 3 were ART naïve. There were significantly higher values in basophil (4.4 ± 2.5%) and eosinophil counts (12.9 ± 3.8%) in HIV/Leishmania coinfected persons (p˂0.005), whereas other white cell subpopulations were significantly lower in the HIV/Leishmania coinfected participants (p˂0.05). There were significantly reduced CD4+ T cell counts (119 ± 26 vs 348 ± 63 vs 605 ± 116 cells/mm3), TNF-α (36.82 ± 8.21 vs 64.67 ± 12.54 vs 254.98 ± 65.59 pg/mL) and IL-2 levels (142.14 ± 20.91 vs 507.6 ± 84.42 vs 486.62 ± 167.87 pg/mL) among HIV/Leishmania coinfected participants compared to group 2 and group 3 participants, respectively. However, higher IL-10 levels (80.35 ± 14.57 pg/mL) were detected in HIV/Leishmania coinfected participants compared to the HIV mono-infected (62.2 ± 10.43 pg/mL) and apparently healthy persons (23.97 ± 4.88 pg/mL; p˂0.001). Conclusion. Eosinophil and basophil counts, and serum IL-10 level were high in HIV/Leishmania coinfected patients demonstrating parasite-induced hypersensitivity and immunosuppression.

<p><strong>Objectives:</strong> There is not yet a consensus on the optimal surgical technique for cesarean section. This is the first study comparing two different (Cesarean Section) with respect to the following inflammatory reaction in means of changes in inflammatory marker levels.<br />To evaluate the differences in inflammatory reactions following two different (Cesarean Section) techniques, the modified Misgav-Ladach versus the Pfannenstiel-Kerr technique.</p><p><strong>Study Desıgn:</strong> The study population included 88 pregnant women who met the inclusion criteria. These women were randomized into two groups according to Consolidated Standards of Reporting Trials guidelines: Group 1 (Misgav-Ladach group) and Group 2 (Pfannenstiel Kerr group). To compare the inflammatory reactions following surgery, Interleukin-6 (IL-6) and Tumor Necrosis Factor-α (TNF-α) levels were measured in venous blood samples drawn from the patients just before (0 hour) and 24 hours (24th hour) after the surgery. In 5 women from Group 1 and 2 women from Group 2, the 24th hour blood samples could not be obtained or were lost. Thus, a total of 81 women, 39 women from Group 1 and 42 women from Group 2, comprised the population of study. The differences in inflammatory reactions between the 0 and 24th hours were analyzed by calculating the percent change in IL-6 and TNF-α levels, and these percentages were then compared between the groups.</p><p><strong>Results:</strong> There was a statistically significant difference between Group 1 and Group 2 regarding the serum IL-6 level change between 0 and 24th hour (530±653% and 196±168%, respectively, p=0.022. The difference in TNF-α was also higher in Group 2, but the difference was not statistically significant (229±306% vs. 571±824%, p=0.12). The mean operation time was significantly shorter in Group 1 (9.44 min. vs. 16.86 min, p=0.0001).</p><p><strong>Conclusions:</strong> The results of this study indicate that the modified Misgav-Ladach technique has a weaker inflammatory reaction, which indicates fewer short- and long-term surgical complications.</p>

Introduction: Kawasaki disease (KD), which is the most common multisystem vasculitis with unknown causes in childhood, causes coronary artery lesions (CALs). Treatment with a high dose of intravenous immunoglobulin (IVIG), plus steroids if needed, is the most effective therapy for the acute phase of KD. However, there are some very severe cases who need several times additional treatments and are at risk for CALs. In Japan, there are some scoring systems that initially predict IVIG-resistant patients. However, the problem is that these scoring systems fail in multiethnic populations. The aim of this study is to find universal biomarkers that predict treatment-resistant cases of KD. Methods: The subject was 276 KD patients, including Group 1 (n=214) who needed only 1 st line treatment, Group 2 (n=48) who needed 2 nd line treatment, Group 3 (n=14) who needed 3 rd line treatment or more. Tenascin C (TN-C), Pentraxin 3 (PTX3) and Procalcitonin (PCT) values, which were selected by systematic review, were measured before initial treatment in each group. Results: TN-C; 99.8±41.05 ng/ml in Group 1, 118.0±71.4 ng/ml in Group 2 and 183.0±25.0 ng/ml in Group 3. The TN-C level of Group 3 was significantly higher than that of all the others (p<0.01). The cutoff value for distinguishing Group 3 was 142.0 ng/ml (Area under the Curve (AUC)=0.81). PTX3; 16.2±9.0 ng/ml in Group 1, 31.4±19.7 ng/ml in Group 2 and 58.0±33.0 ng/ml in Group 3. The PTX3 level of Group 3 was significantly higher than that of all the others (p<0.01). The cutoff value for distinguishing Group 3 was 35.1 ng/ml (AUC=0.86). PCT; 0.79±0.77 ng/ml in Group 1, 2.55±3.01 ng/ml in Group 2 and 4.15±4.49 ng/ml in Group 3. The PCT level of Group 3 was significantly higher than that of all the others (p<0.01). The cutoff value for distinguishing Group 3 was 2.55 ng/ml (AUC=0.88). When those three biomarkers combined, Group 3 can be predicted with the sensitivity 79%, the specificity 96%, the positive predictive value 50% and the negative predictive value 99%. Conclusions: It may be possible to predict treatment-resistant KD cases with high sensitivity and specificity by combining the measurement from the universal biomarkers, TN-C, PTX3 and PCT, before initial treatment.

Abstract Background Superresponders to cardiac resynchronization therapy (CRT) demonstrate significant reverse remodeling, improvement in cardiac function, decrease in inflammatory mediators and markers of cardiac fibrosis. It is not clear if superresponse (SR) can be early or late and if the time of SR to CRT is associated with different degree of biochemical improvement. Aim To assess structural and functional heart parameters, sympathetic activity, levels of biomarkers of myocardial fibrosis, inflammatory and neurohormonal mediators in patients with various time of SR to CRT. Methods The study enrolled 82 superresponders to CRT (decrease in left ventricular end-systolic volume (LVESV) >30%) (mean age 60.4±9.3 years; 80.5% men, 19.5% women; 54.9% with ischemic etiology of heart failure). Patients were divided into two groups: group 1 (n=19) – SR was achieved within 24 months (14.0 [8.0; 21.0] months); group 2 (n=63) - SR was achieved after 24 months (59 [43.0; 84.0] months). Echocardiographic parameters, plasma levels of epinephrine, norepinephrine, NT-proBNP, interleukin (IL) 1β, IL-6, IL-10, tumor necrosis factor alpha (TNF-α), metalloproteinase (MMP) 9, tissue inhibitors of metalloproteinase (TIMP) 1 and 4 were evaluated. Results At baseline there were no differences in demographic, clinical and echocardiographic characteristics between the groups. Levels of epinephrine (1.1 [0.1; 2.2] ng/ml vs 2.1 [0.7; 3.4] ng/ml; p=0.049) and IL-10 (1.8 [1.5; 3.5] pg/ml vs 3.9 [2.7; 5.1] pg/ml; p=0.019) were significantly higher in group 2. Both groups demonstrated significant improvement in echocardiographic parameters. On follow-up left ventricular (LV) end-systolic dimension (p=0.041), LV end-diastolic dimension (p=0.049), LVESV (p=0.014), LV end-diastolic volume (p=0.045) were lower in group 2. In group 1 IL-6 (p=0.047), TNF-α (p=0.047) decreased significantly and there was a tendency for IL-1β (p=0.064) and norepinephrine (p=0.069) levels to increase. In group 2 levels of IL-1β (p<0.001), IL-6 (p=0.030), IL-10 (p=0.003), TNF-α (p<0.001), TIMP-1 (p=0.010) and epinephrine (p=0.024) decreased significantly while MMP-9/TIMP-1 (p=0.023) increased as compared to baseline levels. Additionally there was a tendency for NT-proBNP level to decrease in group 2 (p=0.069). Follow-up level of norepinephrine (7.8 [2.9; 17.2] ng/ml vs 1.1 [0.2; 8.7] ng/ml; p=0.011 was lower and MMP-9/TIMP-4 level was higher (0.058 [0.044; 0.091] vs 0.092 [0.064; 0.111]; p=0.013) in group 2. Diverse trends were observed in IL-10 (0.4 [−0.6; 1.2] pg/ml in group 1 vs −2.3 [−3.4; −0.5] pg/ml in group 2; p=0.007) and norepinephrine (4.0 [−5.2; 14.3] ng/ml in the group 1 vs −1.2 [−11.6; 4.0] ng/ml in the group 2; p=0.015) between the groups. Conclusion CRT modulates sympathetic, neurohumoral, immune and fibrotic activity. Late SR to CRT is associated with decrease of sympathetic and inflammatory activity and more pronounced LV reverse remodeling.

The aim of this study was to clinically assess the results of treatment for functional posterior cross-bites by means of selective grinding in individuals at the deciduous dentition phase over a period of 12 months. From a total of 1,011 children examined in the 2-to-6-year age bracket, 26 with functional posterior cross-bites were selected. The sample was divided into 2 groups of 13 children each, group 1 receiving the treatment proposed and group 2 serving as the control group. Treatment was followed up by exercises designed to alter the children's muscular memory. The results showed that correction of functional posterior cross-bite was achieved for all the children treated, the correction remaining stable 12 months later. No self-correction of malocclusion occurred among the control group.
