Chyluria has become a rare clinical presentation in Sri Lanka, which may have a direct correlation with the low prevalence of lymphatic filariasis following the use of diethylcarbamazine and albendazole mass drug administration (MDA) for five rounds between 2002 and 2006. Here we report a 50-year-old male who presented with milky urine and progressive weight loss, diagnosed as having nonparasitic chyluria. The patient was initially managed with a trail of diethylcarbamazine (DEC) 6 mg/kg/day for 21 days and a low-fat diet with an unsatisfactory response. Subsequent management with endoscopic instillation of 0.5% silver nitrate brought in him a quick response, which was maintained for a year. Endoscopic sclerotherapy is considered a safer, effective and a minimally invasive treatment option for symptomatic patients.

Abstract Background It is very important for clinicians and dieticians to explore reasonable weight management strategies for obese people that address both short-term weight loss and subsequent weight maintenance. We hypothesized that resistance training combined with a high-protein diet would result in similar short-term weight loss but better long-term weight maintenance than either a conventional low-fat diet control or a high-protein diet alone. Methods/design This is an 8-week randomized parallel controlled trial followed by a 24-week observational follow-up study. A 48-week supplementary follow-up study will be carried out if necessary. The study will be conducted between June 2019 and October 2020. The 90 overweight or obese participants will be randomly assigned to the conventional low-fat diet group, the high-protein diet group and the high-protein diet and resistance training combination group. Primary outcomes are body weight change at week 8 and week 24 compared with the baseline level. Discussion High-quality research on the effect of a high-protein diet combined resistance training on weight loss and weight maintenance is limited in the Chinese population. Our study will provide a basis for obesity management in China and will promote the development of exercise- and diet-related studies. Paratuberculosis is an incurable chronic disease of ruminants, which is manifested with clinical signs of diarrhea and progressive weight loss. Causative agent is Mycobacterium avium subsp. paratuberculosis, and the disiase occurs in cattle, sheep and goats. The outspread of paratuberculosis is huge in the whole world, especially in the countries where animal breeding is a developed branch of economy. Animals get infected with food and water contaminated with feces of infected animals. The infection spreads within infected pastures and also when trading animals with latent infection. Clinical manifestation of the disease can firstly be noticed in young categories of sheep, which is different than in cattle. Primary clinical symptom is chronic weight loss. It is rather hard to determine the level of prevalence of the disease, in a certain region, because the diagnostic procedure is complex and not always reliable enough. Detection of antibodies with ELISA test is considered a method of choice for diagnostic of paratuberculosis, because of fast gaining of the results and relatively low price of the test. By applying the ELISA test, we have analysed 2000 sheep blood samples. Positive results for paratuberculosis were found in 66 ( 3,30%) animals in total. In sheep samples from southern Backa region, positive findings were gained from 42 (3,89%) samples, and in Srem region, positive finding were found in samples from 24 sheep (2,60%). Control programs are based on the removal of all infected animals, application of general preventive measures and vaccination. Application of vaccination aims for elimination of clinical form of the disease. An effective treatment has not been determined so far.

Objective To characterize the nutritional variables of patients referred to an outpatient oncology nutrition service. Methods The electronic medical records of patients diagnosed with malignant neoplasms were analyzed for several parameters: weight loss, body mass index (BMI), hand-grip strength (HGS), fat-free mass index (FFMI) and serum C-reactive protein (CRP). Statistical methods included Pearson’s correlation (significance was set at P ≤ 0.05). Results There was a high prevalence of malnutrition (44%, n = 28) and sarcopenia (61%, n = 23) and a relatively low prevalence of cachexia (13%, n = 6). Moderate positive correlations were observed between the tumor site and CRP level, correlation r = 0.328 (P = 0.026); the tumor site and BMI (r = 0, P = 0.001); FFMI and BMI (r = 0.606, P = 4.334056e-005) and CRP and weight loss (r = 0.355, P = 0.024). There were negative correlations between the tumor staging and BMI r = -0.409, P = 0.001) and weight loss and FFMI (r = -0.467, P = 3,228325e-004). Conclusion These preliminary findings may help institutions implement nutritional approaches, such as the adoption of a line-ofcare focused on nutrition in the department of oncology.

Tuberculosis (TB), a significant cause of morbidity and mortality worldwide, is particularly relevant in low/middle-income countries like India, where the disease is endemic. The female reproductive system is very vulnerable to this infection with, the clinical presentation being utterly silent in most patients. Symptoms of TB in pregnancy may initially be attributed to the gravidity itself besides temporary concealment of associated weight loss by the normally occurring weight gain during the pregnancy. Untreated TB may cause pregnancy loss by either placental damage or direct harm to both the mother and child. We report a case of latent disseminated TB in a young immunocompetent female that was revealed in the postpartum state (after full-term stillbirth delivery at home) as 20 ileal perforations secondary to intestinal TB. Due to ongoing sepsis and delayed presentation to the hospital, the patient could not be salvaged despite the best possible efforts.

Introduction: The incidence of obesity and its comorbidities is increasing at an alarming rate in US and around the globe. Our previous studies showed that the receptor for advanced glycation end products (RAGE) and its ligands contribute to the pathogenesis of obesity and insulin resistance (IR), as global Ager (gene encoding RAGE) and adipocyte-specific Ager- deleted mice fed a high fat diet (HFD) showed protection from weight gain and IR. However, the role of Ager deletion in mice with established obesity, switched to low fat diet has not been tested. We hypothesize that temporal adipocyte-specific deletion of Ager in obese mice could enhance weight loss and improves glucose homeostasis. Methods: Mice with conditional adipocyte-specific Ager deletion were generated by breeding Ager flox/flox mice with AdipoQ ERT2 Cre recombinase mice resulting in Ager flox/flox / AdipoQ ERT2 Cre (+) and Cre (-) animals. Mice were fed HFD (60% kcal/fat) for 20 weeks starting at 8 weeks of age to establish obesity and were then treated with tamoxifen (TAM) (75 mg/kg per day x 3 alternative days) to induce deletion of Ager . After 4 weeks of TAM treatment, mice were switched to standard chow for 7 weeks and body weight was monitored regularly. Fasting glucose, insulin and glucose tolerance was measured. Results: After 7 weeks of switching to standard chow following TAM, Cre (+) lost significantly more body weight whereas Cre (-) mice showed no significant weight loss over 7 weeks. Furthermore, Cre (+) mice exhibited significantly higher food intake, lower fasting glucose, lower epididymal and inguinal white adipose tissue weights, and improved glucose and insulin tolerance compared to Cre (-) mice. Conclusions: Temporal adipocyte-specific deletion of Ager in mice with established obesity promotes weight loss and improves glucose homeostasis. RAGE may act as a novel therapeutic target in obesity.

ABSTRACTGiardia is caused by protozoa, Giardia lamblia. It is one of the most common causes of water born or food born diarrhea and it has a worldwide distribution. The prevalence is approximately 20-30% in developing countries. Giardia duodenalis is the most commonly reported intestinal protozoan in humans mainly affecting children. The infection is usually associated with malabsorption, weight loss and growth retardation.We report a case of four years old boy who presented with diarrhea and malabsorption for last 2 years. Its clinical presentation and histopathological diagnosis on duodenal biopsy is discussed.
