High-intensity eccentric exercise can cause a delayed onset of muscle soreness (DOMS), a short-term condition characterized by muscle damage and tenderness that might hold up recovery and jeopardize exercise routine. Previous studies indicated that hydrogen-rich water (HRW) might be a helpful topical intervention to boost recovery in musculoskeletal medicine, yet no data are available concerning the effectiveness and safety of whole-body bathing with supersaturated HRW after DOMS-inducing exercise. This study evaluates the effects of a single-session bathing with HRW on biochemical markers of muscular damage in healthy young men. The six volunteers who were exposed to DOMS-inducing eccentric exercise were assigned to either supersaturated HRW or control whole-body bathing in a double-blind crossover design. Immediately after an exercise session, the participants were immersed up to the neck into a 200 L bathtub with supersaturated HRW (8 mg of H2 per L) or control water (no hydrogen) for 30 min. Blood biomarkers of inflammation and muscular damage and Visual Analogue Scale (VAS) scores for muscle soreness were assessed at baseline (before exercise) and at 24-hour follow-up. Two-way ANOVA revealed a significant difference between two groups in serum creatine kinase (CK) response over the period of intervention ( P = 0.04 ). A single-session bathing in HRW prevented a rise in circulating biomarkers of muscular damage induced by exercise at 24-hour follow-up, retaining the levels of all biomarkers similar to the baseline values ( P > 0.05 ). On the other hand, serum CK, aldolase, and aspartate transaminase were significantly elevated at 24-hour follow-up as compared to the baseline levels after the control bath (342 ± 309 U/L vs. 465 ± 295 U/L; P > 0.05 ). HRW bath also induced a significant drop in VAS scores for muscle soreness in comparison with control water, both immediately after an intervention (32.7 ± 8.6% vs. 20.0 ± 12.8%; P = 0.02 ) and at 24-hour follow-up (31.6 ± 24.3% vs. 22.4 ± 27.5%; P = 0.03 ), respectively. No participants reported any major side effects during the trial. This pilot study suggests that the whole-body bathing in supersaturated HRW is a safe procedure that attenuates muscular damage and can ease sore muscles after high-intensity eccentric exercise.

This study aimed to investigate the impact of percent body fat (%BF) on muscle damage after high-intensity eccentric exercise. Thirty healthy male undergraduates (mean age: 22.0 ± 2 years, height: 176.9 ± 5 cm, weight: 75.8 ± 11.6 kg) participated in this study, and they were classified according to their %BF into a high %fat group (HFG, ≥20%, n = 15) and a low %fat group (LFG, ≤15%, n = 15). For eccentric exercise, two sets of 25 reps were performed on a modified preacher curl machine using the elbow flexor muscle. Maximal isometric strength, muscle soreness (passive and active), creatine kinase (CK), and myoglobin (Mb) were measured as indices of muscle damage. The data were analyzed with repeated measures ANOVA. The results show that there is a significant group–time interaction for both CK and Mb after eccentric exercise (p = 0.007, p = 0.015, respectively), with a greater increase in the HFG than in the LFG. However, there was no significant group–time interaction for maximal isometric strength and muscle soreness (passive and active) (p > 0.05). These results suggest that %BF is a factor that alters the muscle damage indices CK and Mb, which indicate membrane disruption, after eccentric exercise.

This study aimed to investigate if old men were more susceptible than young men to muscle damage induced by exercise consisting of repeated-lengthening muscle actions. The responses to a bout of eccentric exercise were compared between 10 young (mean age ± SEM = 19.4 ± 0.4 y) and 10 old (70.5 ± 1.5 y) men. All subjects performed 6 sets of 5 lengthening actions of the left elbow flexors at a range of 90° from an elbow flexed (90°) to an extended (180°) position in 5 s using a dumbbell massed at 40% maximal isometric strength (MVC) at an elbow joint angle of 90°. Changes in MVC, range of motion (ROM), upper arm circumference (CIR), muscle soreness (DOMS), plasma creatine kinase activity (CK), and myoglobin (Mb) concentration over 7-10 d following exercise were compared between groups by 2-way repeated measures analysis of variance (ANOVA). Significant differences between groups were evident at baseline for ROM (significantly smaller for the older group) and CIR (significantly larger for the older group), but not for MVC and other measures. Contrary to the hypothesis, the young group showed significantly larger decreases in MVC and ROM and larger increases in circumference, DOMS, CK activity, and Mb con centration than those of the old group. These results suggest that muscle damage is not necessarily greater in old versus young men following voluntary eccentric exercise. It may be that physiological changes that occur with ageing, including a decrease in ROM, reduce damaging stress to muscles during lengthening muscle actions.Key words: ageing, maximal isometric strength, muscle soreness, creatine kinase, myoglobin.

Abstract Background Delayed onset muscle soreness (DOMS) is a common non-structural muscle injury which can disrupt training and impair performance in elite athletes. Vibration therapy reduces inflammation and improves neuromuscular efficiency, leading to reductions in pain and stiffness, and may be effective for the prevention or treatment of DOMS. However, the effect of whole-body vibration (WBV) used after sport in elite athletes has not been reported. Methods A randomised, controlled trial was performed. Participants were elite (national or international level) hockey players and underwent an eccentric exercise protocol previously shown to produce clinical DOMS. After exercise, one group underwent static stretching with WBV therapy, and the other performed stretching only. Baseline and serial post-exercise pain scores and measurements of quadriceps tightness were obtained. Results Eleven participants were recruited into each study arm. There were no significant differences in baseline group characteristics. Participants receiving WBV had significant reductions in both pain (p = 0.04) and quadriceps tightness (p = 0.02) compared with stretching only. Conclusions Post-exercise WBV is effective in elite hockey players to reduce DOMS after eccentric exercise. Elite athletes in multi-sprint sports are at risk of DOMS during training and competition, and its reduction could contribute to reduced injury risk and improved performance. This treatment modality is favourable because it can be incorporated with minimal disruption into the recovery section of existing training regimes. These findings may also be extrapolated to other multi-sprint sports.

Background: It has been assumed that during and after BFR exercises, many blood factors are activated and angiogenesis response is stimulated in the arteries. Objectives: Therefore, the current study aimed to determine the effect of two eccentric resistance training methods with and without blood flow restriction on serum IL6 and MMP9 levels in active young men. Methods: In this quasi-experimental study, 16 healthy men with a mean age of 27.8 ± 2.85 (years), the weight of 79.4 ± 12.4 (kg), and a body mass index of 25.5 ± 3.7 (kg/m2) were randomly assigned to either low-intensity eccentric group with BFR or the high-intensity eccentric without BFR. The high-intensity (70% - 80% maximum voluntary contraction MVC) eccentric exercise without BFR included 3 - 5 cycles of eccentric contraction of the quadriceps muscles, up to exhaustion, whereas low-intensity eccentric exercise executed similar exercise modality at intensity of 20% - 30% MVC with blood flow restriction up to exhaustion. Blood samples were taken from antecubital both before and after the exercise to measure serum IL6 and MMP9 values using the ELISA method. Data were analyzed using paired t-test and analysis of variance with repeated measure test in SPSS version 22. A P value of < 0.05 was defined as statistically significant. Results: High-intensity resistance per se and low-intensity resistance exercise with BFR similarly resulted in a non-significant reduction of IL-6 and elevated levels of MMP-9 serum levels in active young men. Conclusions: Overall, the results indicated that a low-intensity resistance exercise session with restricted blood flow and a high-intensity resistance exercise without blood flow restriction equally did not affect IL6 and MMP9 serum of active young men. Further studies are needed to clarify the exact exercise modality that sufficiently stimulates angiogenesis.

The effects of eccentric exercise on whole body protein metabolism were compared in five young untrained [age 24 +/- 1 yr, maximal O2 uptake (VO2max) = 49 +/- 6 ml.kg-1.min-1] and five older untrained men (age 61 +/- 1 yr, VO2max = 34 +/- 2 ml.kg-1.min-1). They performed 45 min of eccentric exercise on a cycle ergometer at a power output equivalent to 80% VO2max (182 +/- 18 W). Beginning 5 days before exercise and continuing for at least 10 days after exercise, they consumed a eucaloric diet providing 1.5 g.kg-1.day-1 of protein. Leucine metabolism in the fed state was measured before, immediately after, and 10 days after exercise, with intravenous L-[1–13C]leucine as a tracer (0.115 mumol.kg-1.min-1). Leucine flux increased 9% immediately after exercise (P less than 0.011) and remained elevated 10 days later, with no effect of age. Leucine oxidation increased 19% immediately after exercise and remained 15% above baseline 10 days after exercise (P less than 0.0001), with no effect of age. In the young men, urinary excretion of 3-methylhistidine per gram of creatinine did not increase until 10 days postexercise (P less than 0.05), but in the older men, it increased 5 days after exercise and remained high through 10 days postexercise (P less than 0.05), averaging 37% higher than in the young men. These data suggest that eccentric exercise produces a similar increase in whole body protein breakdown in older and young men, but myofibrillar proteolysis may contribute more to whole body protein breakdown in the older group.
