Fish bone and/or spine puncture injuries can result in infection of the upper extremities with aquatic bacterial pathogens. Additionally, in such injuries, the inoculation of foreign organic material is frequent and may further complicate the clinical presentation and course of the resulting infection. We describe the case of a 45-year-old female patient with a minimal fish rostrum puncture trauma acquired during preparation of fresh fish meal, which resulted in a galloping hand cellulitis. The alarming clinical presentation and the prompt response of the skin infection to clindamycin obscured the presence of inoculated fish rostrum remnants in the tissue that, three weeks later, gave rise to a foreign body granuloma, from which Aeromonas hydrophila was isolated. Final resolution was achieved with an additional two-week doxycycline treatment. In conclusion, the reported case highlights the potential of the accidentally implanted organic material, as are fish bones, not only to transfer uncommon pathogens but also to offer a sanctuary that favors microbial survival despite antibiotic therapy thus enabling latent or recurrent infections.

Eighty samples of fresh fish were collected in Tunisia and analyzed for microbial load. Quality and hygienic safety of the meat and intestines of wild and aquacultured fresh fish were determined. The mesophilic aerobic plate count and populations of psychrotrophic lactic acid bacteria (LAB) and other psychrotrophic bacteria ranged from 5.67 to 7.29, 4.51 to 6, and 5.07 to 6.21 log CFU/g, respectively. For all microbiological determinations, bacterial counts were lower in meat than in the intestines of fresh fish. For all samples lower microbial populations were found in most of the wild fish than in the aquacultured fish. No isolates of the pathogenic genera Salmonella and Listeria were detected in any sample. Among the 160 strains of biopreservative psychrotrophic LAB and the 150 strains of spoilage psychrotrophic gram-negative bacteria identified by biochemical and molecular methods, Lactobacillus (six species) and Pseudomonas (six species) predominated. Lactococcus, Leuconostoc, Carnobacterium (C. piscicola and C. divergens), Aeromonas, and Photobacterium were the most common genera, and Lactococcus lactis, Lactobacillus plantarum, Pseudomonas fluorescens, and Aeromonas hydrophila were the most common species. These findings indicate that the microbiological quality of fresh fish in Tunisia can be preserved by controlling pathogenic and psychrotrophic bacteria.

We report the case of a Spanish 60-year-old female who presented in 1997 with onychomycosis of the left thumbnail following an injury caused by a fresh fish bone. Microsporum racemosum was repeatedly cultured from nail scrapings, and its identity was confirmed by sequencing the isolate’s ITS1/ITS2 and 5.8S rRNA regions. The patient was successfully treated with itraconazole, which was administered for 12 weeks. This represents the first case of onychomycosis due to M. racemosum and the first time that this species has been isolated from a human in Europe.

Tujuan tulisan ini memaparkan hasil identifikasi dan potensi pemanfaatan limbah perikanan di Banda Aceh. Hasil kajian menunjukkan bahwa limbah yang dihasilkan dari usaha perikanan (penangkapan, budidaya, pengolahan) dan usaha non-perikanan sebagian besar belum dimanfaatkan secara optimal. Limbah yang dihasilkan dari usaha penangkapan adalah limbah padat berupa sisa ikan hasil pembongkaran dengan status sudah dimanfaatkan untuk pakan unggas. Limbah dari usaha pengolahan antara lain limbah padat berupa sisa ikan bagian kepala, isi perut dan tulang dengan status belum dimanfaatkan karena terbatasnya pengetahuan dan keterampilan dalam mengadopsi teknologi. Limbah dari usaha budidaya berupa padatan yang sudah dimanfaatkan sebagai pupuk kompos, sedangkan limbah dari budidaya kepiting soka belum dimanfaatkan. Jenis usaha yang berpotensi untuk dikembangkan dengan memanfaatkan limbah tersebut adalah: 1) usaha tepung ikan; 2) usaha pembuatan pakan ikan dan unggas; 3) usaha olahan makanan ringan (snack) tulang ikan; 4) usaha kerajinan aksesoris berupa tas atau dompet; 5) usaha pembuatan tepung bahan baku citosan.Title: Potential Use of Fisheries Waste in Banda AcehThis paper aimed to describe the identification of fisheries waste management in Banda Aceh. Results showed that waste from fisheries (catching, aquaculture, fish processing) and non-fisheries activities largely untapped optimally. Solid waste from marine captured fisheries is demolition of the remaining fish with status already used for poultry feed. Waste from processing businesses include solid waste such as leftover fish head, entrails and bone status untapped due to limited knowledge and skills in adopting technology. Waste from aguaculture  in the form of solids that have been used as compost, while soft-shelled crab waste from aquaculture untapped. Type of business which have potential to be developed by utilizing the waste are: 1) business of fish meal; 2) business of making fish feed and poultry; 3) business of processed snack fish bone; 4) craft business accessories such as handbags or wallets; 5) business of making starch feedstock citosan.

Mycobacterium marinum infection could have various presentations, from superficial skin infection to deep structure destruction. The prognosis is relatively poor when deep structure is involved as it is more destructive. The prognosis is even worse when operation is required. In the retrospective study of 136 patients who suffered this disease with deep structure involvement, their clinical presentations could be classified into benign and aggressive type. It was found that both types of presentation could be treated conservatively by medication alone. Benign presentations could be treated successfully with chemotherapy alone without complications. Patients with aggressive presentation were usually associated with worse prognosis as there were more complications regardless of the management option. Therefore, the clinical presentation not only had prognostic value but could also guide the treatment plan.

Although foreign body ingestion is a common occurrence, perforation and penetration of the gastrointestinal tract is unusual and the development of a hepatic abscess is even more rare. The authors describe two cases of fish bone perforation of the gastrointestinal tract with hepatic perforation and abscess formation, from distinctive age groups and varying presentation, although both developed septic shock. The lack of history of ingestion of foreign bodies, non-specificity of both clinical presentation and complementary examinations all play a role in delaying the diagnosis and therefore in the prognosis itself.

This prospective analysis is based on clinical forensic examinations and clinical case records of the victims who sustained burns and were admitted during a one-year period since 2017. Of the 90 patients (34 children and 56 adults), 54 % were male with ages ranging from 1 month to 80 years. Males below the age of 20 years (48 %) were highly vulnerable. Education status revealed that most of the patients have an education level below O/L representing 52 % (Ordinary Level/ O/L is similar to the General Certificate of Secondary Education/ GCSE in Cambridge Education System in United Kingdom) and the majority were married (52%). Scalds were seen in 52 %, while flame burns in 28 % cases. Most of the incidents had taken place at home (92%). Burn injuries were most frequently observed on upper extremities (47 %) and the majority were of first degree in nature. Furthermore, this study revealed that 57 % recovered without any complications, while 34 % resulted in scarring or disfigurement. It was highlighted that children are the most vulnerable to in sustaining burns, especially with hot water in domestic settings. The study recommends increasing awareness among parents/guardians regarding safe handling of hot water to minimize such incidents.
