Elderly care services are important to provide in response to the rapid growth of the elderly population. In developing countries like Indonesia, the speed of growth of the elderly population does not simultaneously occur, so the needs for care services vary. This study discusses the emergence of home care services in response to the increase in elderly population. By taking the case of community home care services in Sleman, this study found the pattern and process of the emergence of local initiatives in home care services. This study also revealed an important factor affecting the implementation of community home care services, that is, leadership.

The determination of nursing students’ attitudes toward elderly discrimination and their opinions about home care is important in that it will affect the quality of care provided to elderly individuals in their homes by the students in the future. For this reason, the aim of this study was to determine relationship between nursing students’ attitudes toward elderly discrimination and their opinions about home care services. This descriptive study was conducted with a total of 318 students from a university nursing faculty during the fall semester of the 2016-2017 academic year. Data for the study were collected using the Home Care Services Evaluation Questionnaire and the Ageism Attitude Scale. It was found that there was a positively significant but weak relationship between the students’ age discrimination scale total score and the positive discrimination subdimension score, and their opinions about home care services ( p < .001). Students’ attitudes toward elderly discrimination were found to make a statistically significant contribution to their opinions about home care services. It was ascertained that students’ attitudes toward elderly discrimination affected their opinions about home care services. It is recommended that geriatric nursing and home care nursing lectures be included in the nursing education curriculum, awareness of the importance of the concept of old age and elderly care be increased by establishing internship fields for students in institutions that provide home care for the elderly, and positive attitudes toward the elderly be promoted.

With the increase of the aging of the population in china, the demand of the elderly continues to increase, which has brought great challenges to the elderly care service industry. As a new type of elderly care service, home-based care services directly affect the quality of the elderly&rsquo;s health and daily life in their later years. Therefore, improving the product and quality of elderly care services has become the goal and purpose of the development of home-based care service for aged. This article uses Cite Space software to perform visual analysis of keyword clustering on existing literature, and concludes that scholars focus on the integration of medical care, social elderly care service system, service quality, and influencing factors, but the impact of service quality models and indicator systems is low. Then through the integration and induction of scholars&rsquo; research progress on home care services, existing problems in service quality, and service quality evaluation index systems, in order to continue to innovate on the basis of existing knowledge, improve the service quality index system, and focus on combing process indicators with outcome indicators effectively, thereby improving the quality of community home care services.

O Atendimento Domiciliar é uma tendência mundial que vem sendo implementada como estratégia através do processo de desospitalização. Com o objetivo conhecer as potencialidades e desafios no Atendimento Domiciliar da população idosa, foi feita uma revisão integrativa sobre a temática nas bases de dados LILACS e SCIELO. Os resultados mostram que a população idosa atendida em domicílio é predominantemente do sexo feminino, cujos principais problemas são: Hipertensão Arterial, Acidente Vascular Cerebral e Demência. Como potencialidades, destacam-se a desospitalização, redução de infecções e aumento da autonomia. As estratégias estão voltadas à capacitação dos profissionais, utilização de tecnologias e integração com redes de atenção à saúde. Conclui-se que apesar dos desafios relacionados aos serviços de atendimentos domiciliares, esta modalidade tem muitas potencialidades, como a redução de custos em saúde e infecções, além do cuidado mais centrado no paciente e familiares.Descritores: Atendimento Domiciliar, Assistência Domiciliar aos Idosos, Serviços de Cuidados Domiciliares. Home care of the elderly population: potentialities and challenges of this modalityAbstract: Home care is a global trend that has been implemented as a strategy through the process of dehospitalization. With the objective of knowing the potentialities and challenges in the home care of the elderly population, an integrative review was made on the theme in the LILACS and SCIELO databases. The results show that the elderly population attended at home is predominantly female, whose main problems are: Arterial hypertension, cerebrovascular accident and dementia. As potentialities, we highlight the dehospitalization, reduction of infections and increased autonomy. The strategies are focused on training professionals, using technologies and integrating with health care networks. Como potencialidades, se destacan la deshospitalización, la reducción de infecciones y el aumento de la autonomía. Las estrategias se centran en la formación de profesionales, el uso de tecnologías y la integración con redes de atención médica. Technological advances in elderly care have been rapid, and the introduction of robots in care will be a topical issue in the near future. There has been little research into the possibility of influencing care workers’ attitudes towards robots by project activities, and how to make the change easier for work communities. This study focuses on a robotics project that took place in elderly and home care services in one municipality in Finland (total of 45 care workers). During the project, four robotics workshops and one extended pilot session were implemented. The study follows quasi-experimental settings, and it included two measurements (before and after project activities) and a control group, but no randomization. The data were collected by questionnaires and were analyzed statistically. The project under study brought about minor positive changes in home care workers’ attitudes towards the usefulness of care robots. In the final measurement, the difference between the test group and the control group was significant in the two dimensions of positive attitudes. The research supports the hypothesis that project activities can be used to influence home care workers’ attitudes towards robots. This can also facilitate the introduction of care robots in home care services. However, the construction of a technology-positive care culture is a long-term process, which requires training and development, technological development and strong strategic management at various levels.

Introduction: The Community Referral by Emergency Medical Services (CREMS) program was implemented in January 2015 in Southwestern Ontario. The program allows Paramedics interacting with a patient to directly refer those in need of home care support to their local Community Care Access Centre (CCAC) for needs assessment. If indicated, subsequent referrals are made to specific services (e.g. nursing, physiotherapy and geriatrics) by CCAC. Ideally, CREMS connects patients with appropriate, timely care, supporting individual needs. Previous literature has indicated CREMS results in an increase of home care services provided to patients. Methods: The primary objective of this project is to evaluate the impact of the CREMS program on Emergency Department utilization. Data for all CCAC referrals from London-Middlesex EMS was collected for a thirteen month period (February 2015-February 2016). For all patients receiving a new or increased service from CCAC the number of Emergency Department visits 2 years before referral and 2 years after referral were calculated. A related samples Wilcoxon Signed Rank Test was performed to examine the difference in ED visits pre and post referral to CCAC. Results: There were 213 individuals who received a new or increased service during the study timeframe. Median [IQR] patient age was 77 [70-85.5]. 113/213 (53%) of patients were female. The majority of patients 135/213 (63.4%) were a new referral to CCAC. The median [IQR] number of hospital visits before referral was 3 [1-5] and after referral was 2 [0-4]. There was no significant difference in the overall number of ED visits before versus after referral (955 vs 756 visits, p = 0.051). Conclusion: Community based care can improve patient experience and health outcomes. Paramedics are in a unique position to assess patients in their home to determine who might benefit from home care services. CREMS referrals for this patient group showed a trend towards decreased ED visits after referral but the trend was not statistically significant.

RÉSUMÉBut: Comparer au moyen d'un plan d'étude transversale les caractéristiques démographiques et fonctionnelles des utilisateurs âgés et des non-utilisateurs de services de soins à domicile dans les centres urbains de la Nouvelle-Écosse (n = 533) et de la Terre-Neuve (n = 449).Résultats: Plus de Néo-Écossais et Néo-Écossaises (24%) que de Terre-Neuviens et Terre-Neuviennes (11%) reçoivent des services de soins à domicile. Dans les deux provinces, la plupart des gens dépendants par rapport aux activités de la vie quotidienne (AVQ) ne reçoivent pas des services de soins à domicile, et la plupart des utilisateurs de soins à domicile n'ont pas de dépendence par rapport aux AVQ. À Terre-Neuve, la dépendence et la démence sont plus fréquentes chez les utilisateurs de soins à domicile, contrairement aux utilisateurs en Nouvelle-Écosse ou le fait d'avoir 85 ans ou plus, de ne pas avoir de soutien dans la même habitation, de ne pas être marié et d'être limité par rapport aux activitiés instrumentales de la vie quotidienne (ATVQ) a pour conséquence une augmentation importante de la probabilité d'utilisation de soins à domicile. À Terre-Neuve, la tendance à recourir à des soins à domicile est plus forte chez les gens ayant une dépendence par rapport aux AIVQ.Conclusions: Si les soins à domicile doivent se substituer à des soins en établissement, il faut être prêt à adapter à un nouveau type d'utilisateur les programmes existants de soins à domicile.
