Atherosclerosis (AS) is a complicated arterial disease resulting from abnormal lipid deposition and inflammatory injury, which is attributed to Yin deficiency, accumulation of heat materials, and stasis of blood flow in Traditional Chinese Medicine (TCM) theory. Thus, according to TCM theory, the method of nourishing Yin (Yangyin), clearing away heat (Qingre), and promoting blood circulation (Huoxue) is a reasonable strategy, which has achieved remarkable clinical efficacy in the treatment of AS, but the mechanisms remain to be known. In this study, we evaluated the effects of Yangyin Qingre Huoxue Prescription (YQHP) on AS in ApoE-/- mice suffering from a high-fat diet and heat shock protein (HSP65) attack. YQHP regulated levels of blood lipids and inflammation-linked cytokines as well as Th17/Treg ratio in peripheral blood. Suppressed IL-6-p-STAT3 signaling and restored IL-2-p-STAT5 signaling in the presence of YQHP may partake in the regulation of Th17 and Treg differentiation. Moreover, YQHP modulated transcriptional levels of costimulator CD80 in aortas as well corresponding to the downregulation of GM-CSF in serum and CD3 expression in CD4+ T cells, which might indicate the potential of YQHP to regulate antigen presenting cells. All these effects eventually promoted the improvement of atherosclerotic lesions. In addition, YQHP promoted less monocyte infiltration in the liver and lower levels of AST, ALT, and AKP production than simvastatin. Conclusively, lipid-regulating and anti-inflammatory functions mediated by YQHP with lower hepatotoxicity than simvastatin hindered the progression of HSP65 aggravated AS in ApoE-/- mice, indicating the effectiveness of Yangyin Qingre Huoxue Method in the treatment of AS.

ErChen and YinChen decoction (ECYCD) is an effective traditional Chinese medicine and has been widely used in traditional Chinese medicine to treat nonalcoholic steatohepatitis (NASH), with good curative effects. However, the specific mechanisms underlying these effects are unclear. In this study, we determined the efficacy of ECYCD in a high-fat diet-induced NASH rat model, established by 8-week administration of a high-fat diet. ECYCD was administered daily for 4 weeks, after which the rats were euthanized. The results demonstrated that ECYCD ameliorated high-fat diet-induced NASH, as evidenced by decreased liver indexes, reduced hepatic lipid deposition and liver injury, lower serum biochemistry markers (including low-density lipoprotein), and reduced HOMA-IR scores. Moreover, levels of free fatty acids, tumor necrosis factor, and malondialdehyde were decreased, whereas glutathione was increased in the liver. Serum high-density lipoprotein was also increased in the liver, and ECYCD regulated the c-Jun N-terminal kinase 1 (JNK1) signaling pathway by decreasing the levels of JNK1 protein,JNK1mRNA, activator protein- (AP-) 1 protein,AP-1mRNA, and phospho-insulin receptor substrate- (IRS-)1ser307and increasing phopsho-PKBser473levels. These results suggested that ECYCD could ameliorate high-fat diet-induced NASH in rats through JNK1 signaling. ECYCD may be a safe therapeutic option for the treatment of NASH.

Qushi Huayu Decoction (QHD), an important clinically proved herbal formula, has been reported to be effective in treating fatty liver induced by high-fat diet in rats. However, the mechanism of action has not been clarified at the metabolic level. In this study, a urinary metabolomic method based on gas chromatography-mass spectrometry (GC-MS) coupled with pattern recognition analysis was performed in three groups (control, model, and QHD group), to explore the effect of QHD on fatty liver and its mechanism of action. There was obvious separation between the model group and control group, and the QHD group showed a tendency of recovering to the control group in metabolic profiles. Twelve candidate biomarkers were identified and used to explore the possible mechanism. Then, a pathway analysis was performed using MetaboAnalyst 3.0 to illustrate the pathways of therapeutic action of QHD. QHD reversed the urinary metabolite abnormalities (tryptophan, uridine, and phenylalanine, etc.). Fatty liver might be prevented by QHD through regulating the dysfunctions of phenylalanine, tyrosine, and tryptophan biosynthesis, phenylalanine metabolism, and tryptophan metabolism. This work demonstrated that metabolomics might be helpful for understanding the mechanism of action of traditional Chinese medicine for future clinical evaluation.

Introduction: This study aimed to explore the associations between constitutions of traditional Chinese medicine (TCM) and coronary artery disease (CAD).Methods: A large-scale, community-based, cross-sectional study was performed to investigate the associations. A total of 3,748 participants were available for analysis in this study. The assessment of the constitution of TCM was based on recommendations by the Association for Chinese Medicine in China. In this study, a diagnosis of CAD was based on self-reported medical history. The associations were analyzed using univariate and multivariable logistic regression (MLR).Results: Univariate analysis showed Phlegm–dampness, Qi-deficiency, Yang-deficiency and Yin-deficiency constitutions were significantly associated with CAD, respectively ([Formula: see text] for Phlegm–dampness, [Formula: see text] for Qi-deficiency, [Formula: see text] for Yang-deficiency and [Formula: see text] for Yin-deficiency). Furthermore, MLR demonstrated significant associations among the four constitutions and CAD, after controlling for potential confounding factors (Phlegm–dampness: [Formula: see text], [Formula: see text]; Qi-deficient: [Formula: see text], [Formula: see text], Yang-Deficient: [Formula: see text], [Formula: see text]; Yin-deficient: [Formula: see text], [Formula: see text]). As compared with Neutral participants, participants with the four constitutions of TCM had higher prevalence of CAD.Conclusion: Our findings provided evidence that the four constitutions of TCM including Phlegm-dampness, Qi-deficiency, Yan-deficiency and Yin-deficiency were significantly associated with CAD, respectively. Qushi Huayu Decoction (QSHY), clinically derived, consists of five crude drugs, commonly used in treating fatty liver in a clinical setting. However, little is known about its metabolomics study. Herein, the serum and liver tissue metabolomics approach, based on gas chromatography coupled to spectrometry (GC/MS), was employed to evaluate the efficacy and the mechanism underlying QSHY in a rat model of high-fat diet-induced fatty liver. With pattern recognition analysis of serum and liver tissue metabolite profile, a clear separation of model group and control group was acquired for serum and liver tissue samples, respectively. The QSHY group showed a predisposition towards recovery mimicking the control group, which was in agreement with the biochemical alterations and histological results. 23 candidate biomarkers were identified in the serum and liver tissue samples that were utilized for exploring the underlying mechanism. The present study suggests that QSHY has significant anti-fatty liver effects on high-fat diet-induced fatty liver in rats, which might be attributed to regulating the dysfunction of beta-alanine metabolism, alanine, aspartate, and glutamate metabolism, glycine, serine, and threonine metabolism, pyruvate metabolism, and citrate cycle. Thus, metabolomics is a useful tool in the evaluation of the efficacy and elucidation of the mechanism underlying the complex traditional Chinese medicine prescriptions.

Glossodynia, or tongue pain, is resistant to conventional therapies. Kampo medicines were evaluated in patients suffering from incurable glossodynia. Patients were diagnosed by traditional Chinese medicine (TCM) theory in order to determine the appropriate herbal prescriptions. Five Japanese females (50–76 years old) with glossodynia refractory to conventional therapy were enrolled in this study. Small portions of Rikkunshito, Jiinkokato, Hachimijiogan and Ryutanshakanto worked for a female diagnosed with "Spleen" and "Heart" Yin deficiency, "Kidney" Yang deficiency and "Liver" Qi stagnation producing heat syndrome. Seishoekkito and Bakumondoto were effective for a patient diagnosed with "Spleen Qi" deficiency and "Stomach" Yin deficiency producing heat syndrome. Rikkunshito, Kamikihito and Chikujountanto worked for a patient diagnosed with "Spleen Qi" and "Heart Yin" deficiency, stagnation of "Liver" Qi producing fire and "Gallbladder" Qi deficiency. Rokumijiogan, Kamishoyosan and Kambakutaisoto were effective for a patient with Yang rise based on Yin deficiency of "Kidney" and "Liver," and restless organ disorder based on Yin deficiency of 5 viscera. A patient diagnosed with "Spleen" Yang deficiency responded to a combination of Anchusan and Hangeshashinto. These patients with glossodynia had resolution of pain within 1 month of treatment. Herbal mixtures containing Ganoderam lucidum, not prescribed based on TCM theory, but effective for herpes virus infection, worked for a female suffering from glossodynia for 1 year after artificial teeth were placed, but required about 5 months to note improvement. Kampo medicines, properly prescribed based on TCM theory, quickly resolved the pain of refractory glossodynia.
