Background. The effects of vitamin D on sexual function are very unclear. Therefore, we aimed at evaluating the possible association between vitamin D and sexual function and at assessing the influence of vitamin D administration on sexual function. Methods. We retrospectively studied 114 men by evaluating clinical, biochemical, and sexual parameters. A subsample (n=41) was also studied longitudinally before and after vitamin D replacement therapy. Results. In the whole sample, after performing logistic regression models, higher levels of 25(OH) vitamin D were significantly associated with high values of total testosterone and of all the International Index of Erectile Function (IIEF) questionnaire parameters. On the other hand, higher levels of total testosterone were positively and significantly associated with high levels of erectile function and IIEF total score. After vitamin D replacement therapy, total and free testosterone increased and erectile function improved, whereas other sexual parameters did not change significantly. At logistic regression analysis, higher levels of vitamin D increase (Δ-) were significantly associated with high values of Δ-erectile function after adjustment for Δ-testosterone. Conclusions. Vitamin D is important for the wellness of male sexual function, and vitamin D administration improves sexual function.

To examine the adaptations of the endocrine system to heavy-resistance training in younger vs. older men, two groups of men (30 and 62 yr old) participated in a 10-wk periodized strength-power training program. Blood was obtained before, immediately after, and 5, 15, and 30 min after exercise at rest before and after training and at rest at −3, 0, 6, and 10 wk for analysis of total testosterone, free testosterone, cortisol, growth hormone, lactate, and ACTH analysis. Resting values for insulin-like growth factor (IGF)-I and IGF-binding protein-3 were determined before and after training. A heavy-resistance exercise test was used to evaluate the exercise-induced responses (4 sets of 10-repetition maximum squats with 90 s of rest between sets). Squat strength and thigh muscle cross-sectional area increased for both groups. The younger group demonstrated higher total and free testosterone and IGF-I than the older men, training-induced increases in free testosterone at rest and with exercise, and increases in resting IGF-binding protein-3. With training the older group demonstrated a significant increase in total testosterone in response to exercise stress along with significant decreases in resting cortisol. These data indicate that older men do respond with an enhanced hormonal profile in the early phase of a resistance training program, but the response is different from that of younger men.

IntroductionErectile dysfunction (ED) is defined as the inability to achieve or maintain an erection sufficient for sexual activity.ObjectivesED is a common condition in psychiatric patients, which can modify their quality of life.AimsThe aim of this study is to assess the prevalence and the severity of ED in psychiatric patients.MethodsThis naturalistic, observational study was conducted during a six months period. The International Index of Erectile Function (IIEF) 15-item questionnaire was used to assess 144 male inpatients with different psychiatric disorders. IIEF is a brief, cross-culturally valid, self-administered scale for detecting treatment-related changes in patients with erectile dysfunction and addresses the relevant domains of male sexual function: erectile function, orgasmic function, sexual desire, intercourse satisfaction, and overall satisfaction. The patients were analyzed by age, psychiatric diagnosis, medication, IIEF scores at admission.ResultsThen mean age was 36.4 years. The lot included 44 patients with Psychotic Disorders, 68 with Mood Disorders, 21 with Alcohol Dependence, 11 with Personality Disorders. Severe ED was registered in 38% of investigated patients, which determined abandon of sexual attempts in 57% of cases; 12% had moderate ED, 15% had mild to moderate ED, 18% had mild ED and 17% had normal sexual function. No orgasm was reported in 32% of patients and 37% had no sexual desire. Under treatment with antipsychotic medication 78% had severe or moderate ED.ConclusionsSevere ED was correlated with age, also being seen in young patients. ED was correlated with depression, schizophrenia and antipsychotic medication.

Second generation antipsychotics have less influence on prolactin levels than conventional antipsychotics (CA), which are commonly associated with sexual dysfunction and hyperprolactinaemia. However, only a few studies have been conducted assessing these newer antipsychotics and sexual function/dysfunction. The aim of this study is to evaluate the sexual function and hormonal profile of male schizophrenic patients taking olanzapine or CA. Sixty-three inpatients with acute episodes of schizophrenia were randomly assigned to take either olanzapine, or go on conventional antipsychotic treatment. The Dickson-Glazer sexual functioning questionnaire was used to assess sexual functioning where serum prolactin, luteinizing hormone, follicle-stimulating hormone, total testosterone, free testosterone, and sex hormone-binding globulin concentrations were measured. All measurements were taken on discharge from the inpatient unit (baseline), and again at 3 and 9 months after discharge. Prolactin levels in the olanzapine group decreased more rapidly and were significantly lower than in the CA group after 3 months (12.1±6.3µg/l, p=0.01; 18.1±11.2µg/l, p=0.564, respectively). After nine months, there was a tendency toward normal levels in both groups, and the frequency of sexual complaints did not differ between the groups. This study showed no difference between olanzapine and conventional antipsychotics regarding sexual complaints in the treatment of schizophrenia, but did show a difference in the hormone level normalization rate.

Context: Vitamin D is associated with a host of reproductive outcomes, but there is little research investigating these relationships in healthy, regularly cycling, premenopausal women.Objective: Our objective was to assess the relationship between vitamin D and hormonal biomarkers, sporadic anovulation, menstrual cycle length, and premenstrual syndrome and its symptoms. We hypothesize that vitamin D will be inversely associated with aberrations in reproductive and gynecologic function.Methods: This was a prospective cohort of 1191 participants attempting to conceive, aged 18-40, with 1-2 prior pregnancy losses, no history of infertility, and enrolled in the EAGeR trial. Patients answered questionnaires regarding demographic information and gynecologic histories and serum samples were collected pre-randomization. Patients collected and froze daily first-void urine samples for up to two menstrual cycles. Patients were followed for risk of anovulation for two menstrual cycles and followed all together for up to six menstrual cycles.Results: Vitamin D was associated with free androgen index and sex hormone binding globulin concentration, but not total testosterone, free testosterone, or dehydroepiandrosterone sulfate. Vitamin D was negatively associated with estrone-1-glucoronide in urine but not with pregnanediol glucuronide. Vitamin D was not associated with menstrual cycle length or its phase components and was not associated with risk of sporadic anovulation. Vitamin D was associated with breast tenderness/fullness and generalized aches and pains during the premenstrual week but not with other symptoms or overall risk of PMS.Conclusions: Vitamin D may play a role in a host of reproductive and endocrine outcomes, including the bioavailability of androgens, concentrations of estrogens, and physical symptoms of PMS.

Abstract. Sexual dysfunction can adversely affect the quality of life, self-confidence, and interpersonal relationships. Some studies reported a relationship between vitamin supplementation and sexual health. This systematic review aimed to evaluate the effect of vitamins on female and male sexual function. All relevant English and Persian articles published in English databases (Medline, Scopus, Cochrane Library, Web of Sciences, Psycho-Info, and Proquest) and Farsi databases (SID and Magiran) until July 2020 were searched. The Cochrane Handbook for Systematic Reviews of Interventions was used for assessing the risks of bias. A total of 11 randomized controlled trials (RCTs) on 337 women, 472 men, and 35 couples were included. One study reported that vitamin D 300000 IU supplement administrated twice through intramuscular injection (at the beginning and after four weeks) had a significant effect on female sexual function (p<0.05), whereas another study showed that vitamin D 2000 IU supplement for six months did not have any significant effect on female sexual function (p>0.05). A study reported the significant effect of niacin (vitamin B3) 1500 mg daily for 12 weeks on erectile function (p=0.004). The results showed that vitamins A, C, and E had no significant effect on male and female sexual function. The conduction of trials with long-term interventions is recommended to reach a more definitive conclusion about the effect of vitamins on sexual function.

Abstract Circulating leptin shows a pulsatile secretory pattern along with a nocturnal rise. We have previously shown that circulating leptin concentrations are high in males with untreated idiopathic hypogonadotropic hypogonadism (IHH). However, circadian leptin secretion in IHH before and after gonadotropin treatment is not known. Thus, we studied 14 adult males with IHH who had no history of previous hormonal therapy, and 12 age- and body mass index-matched healthy men. Plasma leptin concentrations were measured with 1-h intervals for 24 h before and 6 months after gonadotropin treatment. The 24-h mean leptin concentration showed a significant decrease, from 11.78 ± 1.908 μg/liter at baseline to 10.85 ± 1.939 μg/liter after 6 months of therapy (z = 3.107; P = 0.002). Before and after treatment, 24-h mean leptin concentrations were also significantly higher in the patient group when compared with controls (4.275 ± 0.711 μg/liter) (z = 5.938; P = 0.0001). Hourly leptin levels demonstrated a diurnal pattern in hypogonadal patients, a surge in the midday, and a peak just after midnight, and this pattern did not differ before and after treatment. We observed a similar diurnal pattern in the control subjects too. Leptin levels were negatively and significantly correlated with free testosterone and total testosterone levels both before (r = −0.656, P = 0.011; and r = −0.639, P = 0.014, respectively) and after (r = −0.537, P = 0.048; and r = −0.563, P = 0.036, respectively) gonadotropin administration. Our observations suggest that the diurnal rhythm of leptin is intact in males with IHH, and short-term gonadotropin treatment does not effect its diurnal rhythm. Moreover, testosterone produced under the influence of the gonadotropin treatment led to decreases in the leptin levels.

Abstract. The serum concentrations of the different forms of circulating testosterone, total testosterone, free testosterone and non-sex-hormone binding globulin bound testosterone (albumin bound + free fractions) which is considered as the biovailable hormone, were measured in 15 hyperthyroid women before and after anti-thyroid drug therapy and in 15 age-matched healthy women. Sex-hormone binding globulin and albumin were quantified. Total testosterone was significantly higher in hyperthyroid women before treatment, whereas free testosterone and non sex-hormone binding globulin bound testosterone were significantly decreased. After recovery, all the parameters returned to the normal range. In hyperthyroid patients, the variations in the different fractions of testosterone can be related to the rise of sex-hormone binding globulin. These variations could be explained by the displacement of the equilibrium defined by the binding equation.
