Introduction. Nonunion patella with quadriceps contracture is an unusual orthopaedic finding. Very few cases have been recorded in the past with this complication. We present a case of a 40-year-old male with nonunion patella with quadriceps contracture secondary to trauma. Case Report. A 40-year-old male with posttraumatic nonunion patella with quadriceps contracture since 6 months presented with complaints of defect in the left knee with restriction of movements. X-ray of the left knee confirmed our findings. He underwent quadricepsplasty with double-tension band wiring for the patella followed by rigorous physiotherapy to achieve the current level of the knee flexion of 110 degrees. Conclusion. We conclude that quadricepsplasty with tension band wiring and neutralization wire is one of the good modalities of treatment for a nonunion patella associated with quadriceps contracture.

<p class="abstract">Patella fracture consists of approximately 1% of all skeletal fractures. However bilateral patella fracture is even rarer (2-3% of all patella fractures) and is seen mostly in either dash board injuries or in different pathological conditions. A 21 year old male presented with bilateral patella fracture, where right patella was fractured due to RTA and the left one got fractured due to a fall while he was being helped for an X-ray for the first one. Bilateral tension band wiring was done and patient was being regularly followed up with good range of movement recovery. But again he had a fall in bathroom 3 month post-operative and on right side suffered a re-fracture. Modified tension band wiring was done. Patient gained full range of movement on both sides and returned to his normal activities at 9 months post op of index surgery. A case of bilateral patella fracture where mechanism of fracture for each patella fracture are different is unheard of. A case of bilateral patella fracture with re-fracture has not been reported yet in literature so far we know/ searched. Here we report and discuss the challenges in management of such a case and protocol we followed.</p>

Background.Tuberculosis (TB) is a disease that affects hundreds of millions of people across the world. However, the incidence in developed countries has decreased over the past decades causing physicians to become unfamiliar with its unspecific symptoms. Pregnant individuals are especially difficult because many symptoms of active TB can mimic normal physiological changes of pregnancy. We present a case report of a 26-year-old multiparous woman, G4P3003, at 38-week gestation with a history of positive PPD who emigrated from Ghana 6 years ago. She came to the hospital with an initial complaint of suprapubic pain, pressure, and possible leakage of amniotic fluid for the past week. Patient also complained of a productive cough for the past 3 to 4 months with a decrease in vision occurring with the start of pregnancy. Visual acuity was worse than 20/200 in both eyes. Definitive diagnosis of active TB was delayed due to patient refusal of chest X-ray. Fortunately, delay in diagnosis was minimized since patient delivered within 24 hours of admission. Active TB was confirmed with intraocular dissemination. Patient had optic atrophy OS (left eye) and papillitis, choroiditis, and uveitis OD (right eye) due to TB infiltration. Fetus was asymptomatic and anti-TB therapy was started for both patients.

Background: Nonunion and pseudoarthrosis formation following scapular spine fracture is a rare condition and is limited only to case reports. Some authors reported its association with rotator cuff impingement. Standard treatment is not defined well as the condition is rare. Case Report: Our patient was a 61-year-old man with painful nonunion in addition to cuff impingement. Surgical treatment by open reduction, internal fixation with reconstruction plate and tension band wiring, along with bone grafting led to fracture union, as well as complete resolution of the rotator cuff tendinopathy. Conclusion: Surgical fixation seems to be the best choice in treating scapular spine pseudoarthrosis. The first attempt should be the best, so we took an aggressive approach by the use of plating, tension band wiring, and bone grafting altogether.

An osteotomy with interposition of iliac crest bone graft and lengthening of the proximal ulna can be used to restore ulnohumeral congruency after a malunited comminuted olecranon fracture treated with figure-of-eight tension band wiring.
