Background.The widespread uses of antibiotics, together with the length of time over which they have been available, have led to the emergence of resistant bacterial pathogens contributing to morbidity and mortality. This study was aimed to assess bacterial isolates and their drug susceptibility patterns from inpatients and outpatients with pus and/or wound discharge.Methods.A cross-sectional study was conducted at the University of Gondar Referral Hospital from March to May, 2014. Wound swab samples were collected from each study participant and inoculated into appropriate media. The bacterial pathogens were identified using standard microbiological methods. Antimicrobial susceptibility tests were performed using disk diffusion technique following Kirby-Bauer method.Results.A total of 137 study subjects were included in the study with bacterial isolation rate of 115 (83.9%). Of all, 81 (59.1%) were males. Seventy-seven (57%) of the isolates were Gram-negative and 59 (43%) were Gram-positive. From the total isolates,Staphylococcus aureuswas the most predominant isolate 39/115 (34%) followed by Klebsiella species (13%), coagulase negative staphylococci spp. (12%) andPseudomonas aeruginosa. Gram-positive isolates were resistant to ampicillin (86.4%), amoxicillin (83%), penicillin (81.3%), oxacillin (74.6%), and tetracycline (59.4%), while Gram-negative isolates were resistant to amoxicillin (97.4%), ampicillin (94.8%), tetracycline (72.7%), trimethoprim/sulfamethoxazole (66%), and chloramphenicol (54.5%).Conclusion. High prevalence of bacterial isolates was found,Staphylococcus aureusbeing the most dominant. High rates of multiple drug resistance pathogens to the commonly used antimicrobial agents were isolated. Therefore, concerned bodies should properly monitor the choice of antibiotics to be used as prophylaxis and empiric treatment in the study area.

Background: First six to twelve months after initial urinary tract infection, most infections are caused by Escherichiacoli, although in the first year of life Klebsiella pneumoniae, Pseudomonas, Enterobacter spp andEnterococcus spp, are more frequent than later in life, and there is a higher risk of urosepsis compared with adulthood Objectives: To determine the prevalence of bacterial isolates from Urinary Tract Infections of children at a children hospital in Baghdad and their antimicrobial susceptibility patterns. Type of the study: Cross-sectional study. Methods: During six months of study (1 June to 31 December, 2016), 117urine specimens were collected from a children hospital in Baghdad. Results: Out 38 isolates of Gram positive and Gram negative bacteria were obtained from urine specimens in a percentage of 5(13.2%) and 33(86.8%),respectively. The lowest incidence was among the 11-15 years old age group (11.9%) whereas the highest incidence was among the 1-5 years old age group (43.6%)(P≤0.05).Also out of 38positive cultures, the incidence was higher in females 31(81.6%)than that of males 7(18.4%). Out of 38 positive cultures, the isolation rate was 5(13.2%) for Gram positive isolates and 33(86.8%)for Gram negative isolates. The most frequently isolated bacteria was Escherichia coli19(50%)(P-value ≤ 0.05), while the only isolated Gram positive was Staphylococcus aureus5(42.86%) isolates.The most effective antibiotics for Staphylococcus aureus isolates were Vancomycin and Amikacinfor (100%) and for Gram negative isolates wasimipenem (100%). Ampicillin showed the highest resistance rate for both Gram positive and Gram negative isolates(100%) and Pesudomonas aeruginsa isolates showed the highest resistant rates to most antibiotics under study. Conclusions: The most frequently isolated bacteria was E. coli from all urine cultures and the most effective antibiotics for Staphylococcus aureus isolates were Vancomycin and Amikacinfor

Abstract Background: Urinary tract infection (UTI) is caused by colonization and growth of microorganisms within the urinary system. Diabetic patients are more prone to bacterial urinary tract infections due to impaired host defense and high glucose concentration in urine. Surveillance of urinary tract pathogens and their antibiogram is a key to patient management. The aim of this study was to determine the prevalence of bacterial UTI and antimicrobial susceptibility patterns (AST) among diabetes patients. Methods: Hospital-based cross-sectional study was conducted from May to July, 2018. Two hundred twenty-five mid-stream urine samples were collected for culture and identification based on the standard protocol. Antimicrobial susceptibility test was done for all positive urine cultures by Kirby Bauer's disk diffusion method based on CLSI guidelines. Data were entered into Epi-data version 3.2.1 and exported to the Statistical Package for the Social Science (SPSS) version 20 statistical software. Binary and multiple logistic regression test results were used.Results: The study result revealed 9.8% overall UTI prevalence. Five species of bacterial uropathogens were isolated. Among these, E. coli (63.6%) was the leading followed by K. pneumoniae (13.6%). In this study, significant bacteriuria was strongly associated with duration of diabetics, previous UTI and symptomatic UTI. Gram-negative bacterial isolates showed high level of sensitivity (100%) to nitrofurantoin and meropenem. On the contrary, high level of resistance (100 %) for ampicillin, doxycycline, cefuroxime, and (94.4 %) amoxacillin-clavulanate were observed. No resistance was observed among gram positive bacterial isolates except penicillin (100 % resistance). Over all prevalence of MDR was 100 % For Gram-negative bacteria.Conclusion: Presence of previous urinary tract infection and duration of diabetes were found as important factors that increase the prevalence of UTI among diabetes patients. This study also showed high prevalence of drug resistance to doxycycline, amoxacillin-clavulanate, cefuroxime and penicillin for both Gram- negative and Gram- positive bacteria. Therefore, since therapeutic selection for empirical treatment and management should be based on the knowledge of the local bacterial profile and antimicrobial response, we suggest physicians take this high resistance profile in to consideration when prescribing antimicrobials against the pathogens in question.

In Nepal, little is known about the microbiological profile of wound infections in children and their antimicrobial susceptibility patterns. Total of 450 pus/wound swab samples collected were cultured using standard microbiological techniques and the colonies grown were identified with the help of biochemical tests. The antimicrobial susceptibility testing was performed by Kirby-Bauer disc diffusion technique. Methicillin-resistantStaphylococcus aureusisolates were detected by using cefoxitin disc and confirmed by determining minimum inhibitory concentrations (MIC) of oxacillin. 264 (59%) samples were culture positive. The highest incidence of bacterial infections was noted in the age group of less than 1 year (76%). Out of 264 growth positive samples, Gram-positive bacteria were isolated from 162 (61%) samples and Gram-negative bacteria were found in 102 (39%) samples.Staphylococcus aureus(99%) was the predominant Gram-positive bacteria isolated andPseudomonas aeruginosa(44%) was predominant Gram-negative bacteria. About 19% ofS. aureusisolates were found to be methicillin-resistant MIC of oxacillin ranging from 4 μg/mL to 128 μg/mL. Among the children of Nepal, those of age less than 1 year were at higher risk of wound infections by bacteria.Staphylococcus aureusfollowed byPseudomonas aeruginosawere the most common bacteria causing wound infections in children.

Urinary tract infection (UTI) is one of the most common diseases encountered worldwide and is a major public health problem in terms of morbidity and financial costs. A cross-sectional study was conducted in Alka Hospital, Lalitpur, Nepal from February to July, 2014. A total of 353 midstream urine samples were collected from postmenopausal women visiting Alka Hospital, Lalitpur, Nepal. The samples were examined by microscopically and culture methods. The isolated organisms were identified by conventional microbiological methods. Antibiotic susceptibility test was performed by modified Kirby-Bauer disc diffusion method according to CLSI (2011) guidelines. Among 353 urine samples processed, 32 % (113) showed significant bacteriuria. Out of 113 bacterial isolates, the prevalence of gram negative bacteria was 97.3% (110) while that of gram positive was 2.7% (3). The most predominating organisms causing UTI were Escherichia coli (84.9%) followed by Klebsiella pneumoniae (5.3%), Enterococcus faecalis (1.7%), Providencia spp. (1.7%), Klebsiella oxytoca (1.7%), Proteus mirabilis (1.7%), Proteus vulgaris (0.9%), Citrobacter freundii (0.9%) and Staphylococcus aureus (0.9%). Antibiotic susceptibility tests of the isolates showed that, most of the gram negative bacterial isolates were sensitive to Piperacillin+Tazobactam followed by Amikacin, Imipenem and Nitrofurantoin, while they were resistant to Amoxicillin. All gram positive isolates were sensitive to Amoxicillin and resistant to Gentamycin. This study showed that higher aged postmenopausal women (>80 years) were at higher risk for UTI than those of lower aged ones (<80 years).

Bacterial pathogens were isolated from  pus, wound swab, urine, blood and throat swab. A total  of 300 samples were collected from Sir Salimullah Medical College & Mitford Hospital (SSMC & MH), BIRDEM and National Medical College (NMCH) and processed following standard microbiological methods. Antibiotic susceptibility testing were performed on pure culture isolates by employing Kirby-Bauer disc-diffusion method for the commonly used antibiotics. 326 (93.33%) bacterial pathogens were isolated from 300 patients. Single bacterial pathogen was present in 78% cases and mixed bacterial pathogens were in 15.40% cases.  Staphylococcus aureus was the predominant species (38.66%) followed by Escherichia coli (38%), Pseudomonas spp. (13.33%), Proteus spp. (8.33%), CoNS (7.66%), Serratia spp (2.85%), Klebsiella spp. (2.00%) and  Acinetobacter spp. (0.97%). Resistance rate towards amoxicillin, ciprofloxacin, co-trimoxazole and  ceftriaxone were high among both Gram-positive and Gram-negative isolates. However, both groups showed good susceptibility to gentamicin and levofloxacin. S. aureus and CoNS showed 100% sensitivity to vancomycin and all isolated Gram negative organisms showed 98-100% sensitivity to imipenem.These results indicate that gentamicin and levofloxacin may be convenient alternative antimicrobial agent for both Gram-positive and Gram-negative bacteria and vancomycin for Gram positive and imipenem for Gram negative bacteria as well.DOI: http://dx.doi.org/10.3329/bjmm.v7i2.19327 Bangladesh J Med Microbiol 2013; 07(02): 13-16

Abstract Introduction: Cancer and sepsis comorbidity is a major public health problem in most parts of the world including Zimbabwe. The microbial aetiologies of sepsis and their antibiograms vary with time and locations. Knowledge on local microbial aetiologies of sepsis and their susceptibility patterns is critical in guiding empirical antimicrobial treatment choices. Methods: This was a descriptive cross sectional study which determined the microbial aetiologies of sepsis from blood cultures of paediatric and adult cancer patients obtained between July 2016 and June 2017. The TDR-X120 blood culture system and TDR 300B auto identification machine were used for incubation of blood culture bottles and identification plus antimicrobial susceptibility testing, respectively. Clinical and laboratory standards institute (CLSI) standard breakpoints were used to interpret the antimicrobial susceptibility results. Results: A total of 142 participants were enrolled; 50 (35.2%) had positive blood cultures with 56.0% Gram positive, 42.0% Gram negative bacteria and 2.0% yeast isolates. Most common isolates were coagulase negative Staphylococcus spp. (CoNS) (22.0%), Escherichia coli (16.0%), Klebsiella pneumoniae (14.0%), Enterococcus faecalis (14.0%) and Staphylococcus aureus (8.0%) in all cancer patients. These isolates were similar in both haematological and solid cancers. Gram negative isolates exhibited high resistance to gentamicin (61.9%) and ceftriaxone (71.4%) which are the empiric antimicrobial agents used in our setting. Amikacin and meropenem showed 85.7% and 95.2% activity respectively against all Gram negative isolates while vancomycin and linezolid were effective against 96.2% and 100.0% of all Gram positive isolates respectively. Ten (66.7%) isolates of E. coli and K. pneumoniae were extended spectrum β-lactamase (ESBL) positive. Among Staphylococcus species it was also observed that 10/15 (66.7%) of the isolates were methicillin resistan t. Conclusions : The major microbial aetiologies of sepsis among patients with cancer in Zimbabwe were CoNS, E. coli , K. pneumoniae , E. faecalis and S. aureus . Most isolates were resistant to commonly used empirical antibiotics and there was high level of ESBL and methicillin resistance carriage. A nationwide survey on microbial aetiologies of sepsis and their susceptibility patterns would assist in the guidance of effective sepsis empiric antimicrobial treatment among patients with cancer.

Background. Urinary tract infection is a major health problem especially in developing countries. Information about bacterial pathogens isolated from urinary tract infection in diabetic patients and their antimicrobial susceptibility patterns is limited in Ethiopia. Therefore, this study aimed at isolating bacterial pathogens and their antimicrobial susceptibility patterns.Methods. A hospital based comparative cross-sectional study was conducted at Debre Tabor. Urine sample was inoculated onto cysteine lysine electrolyte deficient (CLED) medium. Bacterial pathogens were identified using standard bacteriological methods. The data were cleaned and entered into SPSS version 20.Pvalue less than 0.05 is considered statistically significant.Result. A total of 384 study participants were included in the study. Of them, 21 (10.9%) were from diabetics and 9 (4.7%) of them were from nondiabetics. Large proportion of gram positive bacteria at 18 (58.1%) were isolated compared to gram negatives at 13 (41.9%). Gram positive isolates were resistant to cotrimoxazole 10 (58.8%).Conclusion. The isolation rates of bacterial pathogens were higher in diabetic than nondiabetic patients. Bacteriuria was significantly associated with sex and type of diabetes. Multidrug resistance to two or more antibiotics was observed in 56.7% of bacterial isolates. Rational use of antimicrobial agent should be thought of to prevent the emergence of multidrug resistance.

Background. Healthcare workers (HCWs) may be using their mobile phones (MPs) to carry microbes that cause hospital-acquired and community infections in general. With antibiotic resistance problem emergence, these infections can be challenging to eradicate. Hence, this study aimed to determine the microbial contamination of HCW MPs and identify and classify bacterial isolates in Palestine. Methods. This was a 7-month comparative cross-sectional analysis of 200 HCW MPs from 2 hospitals and 100 MPs from university students (non-HCWs). Data collection was done using a self-administrated questionnaire, and a swab sample from both HCW and non-HCW MPs was obtained and transferred to An-Najah National University (NNU) microbiology lab for bacterial identification and antibiotic susceptibility. Data were analyzed using Social Sciences Statistical Package (SPSS) version 22.0. Result. Among HCWs, the microbial contamination was 87.5%. Coagulase-negative staphylococci (CoNS; 67.3%), methicillin-sensitive Staphylococcus aureus (MSSA; 17.5%), Gram-positive bacilli (4.1%), methicillin-resistant Staphylococcus aureus (MRSA; 1.6%), and Gram-negative species (1.6%) were the most predominant bacterial isolates. More than half of staphylococci isolates were resistant to penicillin and erythromycin. Male gender, using a mobile phone in the bathroom, and entry to the operating theatre were associated with mobile phone contamination and increased resistance against specific antibiotics. Among non-HCWs, the contamination was 86%. The most predominant bacterial isolates were CoNS, MSSA, and Gram-positive bacilli, with a contamination of 66.8%, 28.5%, and 2.6%, respectively. No MRSA or Gram-negative species were detected in this group. Antibiotic resistance percentage of staphylococci was nearly half of that yielded in the HCW group against each antibiotic. Conclusion. Significant numbers of bacteria have been isolated from HCW MPs. Working in a hospital environment frequently raises the probability of presence of antibiotic-resistant bacteria on a MP. Therefore, infection control teams should discuss methods to prevent the transmission of drug-resistant pathogens from HCW MPs.
