A 41-year-old man with a history of gout presented with an enlarging eyelid growth. Clinical examination revealed a mildly indurated nodule at the lateral canthus. Following resection, histopathological examination revealed needle-shaped, crystalline material surrounded by multinucleated giant cells, findings consistent with gouty tophus. This represents just the sixth case of gouty tophus of the eyelid reported in the literature.

Lupus vulgaris is an extremely chronic, progressive form of cutaneous tuberculosis. The earliest description of lupus vulgaris was given by Erasmus Wilson in 1865. It usually occurs through contagious extension of the disease from underlying affected tissue or hematogenous or lymphatic spread. A 55 years male, non diabetic, non hypertensive, non smoker, got himself admitted into Dermatology and Venereology Department of DMCH with the complaints of multiple ulcerated lesions over the left lower thigh and upper leg including knee for 8 years. Histological sections of skin revealed multiple epithelioid granuloma, multinucleated giant cells and infiltration of lymphocytes with areas of fibrosis and ESR was 80 in 1st hour. A 3 year old girl presented with a two- day history of multiple fluid filled lesions on buttocks and back of left thigh. As informed by the child's mother, she had fever 4 days back and was irritable with pain and discomfort on back of the left thigh. The pain was episodic and radiated from buttocks to the leg. On examination, multiple fluid filled lesions of various sizes with surrounding erythema were present involving the S1, S2, S3 dermatomes extending from buttock to the sole sparing few areas in between the vesicles. On further examination of whole body, there were multiple discrete fluid filled lesions on erythematous base ranging from 0.1 to 0.3 mm diameter in size on chest, abdomen and back. Multinucleated giant cells with acantholytic cells were found in Tzank smears. The authors report a rare case of primary pleomorphic carcinoma of the gallbladder in a 70-year-old woman. A polypoid tumor protruded into the lumen from the fundus of the gallbladder. Characteristic histologic findings Included a general lack of architectural cohesiveness, marked pleomorphism, presence of mononucleated and multinucleated giant cells, extensive necrosis, leukocyte-tumor cell phagocytosis or cannibalism. Immunoreactivity for cytokeratin, carclnoembryonic antigen and epithelial membrane antigen as well as histochemical positivity for mucins demonstrated the epithelial nature of the tumor. The neoplasm behaved aggressively; the patient died of metastases 9 months after the operation.

Background: Cryptococcosis is an important zoonotic disease that occurs worldwide. The disease is caused by a soilborne opportunistic fungus of the genus Cryptococcus, which can also be found in the feces of birds, especially pigeons. In Brazil, the geographical distribution of the agent is fairly defined, with the species C. gattii predominantly found in thenorth-eastern states. Diagnosis is based on the clinical history, physical examination findings, and results of complementary testing, such as cytopathological and histopathological examinations. This report aims to describe the clinical and anatomopathological findings in a case of systemic cryptococcosis in a dog.Case: A 4-year-old female Miniature Schnauzer was referred for necropsy. For 3 months prior to death, the dog had displayed generalized alopecia, pruritus, and severe weight loss, Initially, a private veterinarian had suspected leishmaniasis. Gross examination was performed and samples for cytopathology, histopathology, and fungal culture were obtained. Themacroscopic examination revealed generalized alopecia, congested mucosa, cachexia, hyperemia of the brain, and multiple white nodules measuring 0.5 cm to 4 cm in the lungs and the kidneys. None of the other organs showed significant lesions. Cytopathological examination of the nodules revealed a highly cellular sample, with a severe chronic inflammatory response, characterized by multinucleated giant cells and round-to-elliptical, yeast-shaped structures (5-10 μm), suggestive ofCryptococcus organisms. Histopathological examination of the lungs, kidneys, and brain revealed a severe diffuse chronic inflammatory process, with lymphocytic infiltration and multinucleated giant cells; countless yeast-shaped, round-to-ovoid structures (similar in appearance to “soap bubbles”) that were negative in hematoxylin-eosin stain were also present.Grocott’s methenamine silver stain was then applied, which positively stained the organism capsules black, confirming Cryptococcus. For mycological diagnosis, samples from the lungs nodules were cultured on Sabouraud’s dextrose agar supplemented with chloramphenicol 0.4% and incubated for 7 days at 25-30ºC. Abundant small, smooth, irregular-sized,cream-colored colonies were obtained, and a mycological smear, stained by India ink, was performed.Discussion: Considered an uncommon disease, canine cryptococcosis can be misdiagnosed. Clinical findings, such as alopecia, lymphadenopathy, and cutaneous nodules that may be ulcerated, are commonly seen in these patients due to the hematogenous spread of the organism. In the case described, the dog’s owner was contacted and asked to provide epidemiological information. The owner reported that the dog had lived in an apartment, with little access to the street; however, pigeons were frequently observed on the balcony, along with pigeon feces in the dog feeder. The presence of pigeon feces is a risk factor for the development and spread of the fungus, which may be problematic to public health. Cytopathological examination is a simple, quick diagnostic tool with good sensitivity for some infectious agents. Mycological culture can provide results due to the characteristic of the Cryptococcus sp., since the encapsulated yeast produces a mucous-like colony. Cryptococcosis has a relevant role in public health, so a precise diagnosis is imperative. Its diagnosis is based upon culture, cytology and serological tools. When the animal does not survive to the ilness, the association between necropsyfindings and histopathological examinations is essencial.Keywords: canine, diagnostic, yeast, anatomopathology.

Background: PVNS is a rare, benign & aggressive disorder arising from either synovial joints or tendon sheaths; it may erode articular structures and bones. We present a case with unique features of PVNS being extra-synovial and by this report we open a gate for more researches in this field. Case Presentation: This case report concerns a 35-year-old female with a history of right knee pain for 6-month duration proceeded by gradual swelling over posterior aspect of the knee, she denies any history of trauma, clinical examination was unremarkable but apart from tenderness over the infrapatellar region with full flexion. MRI shows a heterogenous signal extra-articular and extra-synovial lesion in posterior aspect of the knee suggesting Pigmented Villonodular Synovitis, FNA revealed a hemosiderin-laden macrophages and multinucleated giant cells, Tru-cut biopsy result was suggesting PVNS as synovial cells were seen admixed with hemosiderin-laden macrophages with fibroblastic elements. Through posterior approach; the lesion was surgically excised and histopathological examination confirmed the diagnosis, the lesion was recurrent after 1-year and MRI revealed the same features, the lesion was excised by arthroscopic intervention. Conclusion: We concluded that PVNS cannot be excluded when extra-synovial lesion is assessed, and further researches on this topic will expand our understanding of the etiological and pathological aspects of this tumor.

The separation of the follicle from around the crown of an unerupted tooth causes formation of dentigerous cyst. It may undergo metaplasia in several cases. In this case a 30 year old female patient reported with a history of dentigerous cyst with respect to impacted 48. The swelling started growing gradually causing facial asymmetry. On intraoral examination the swelling seem to extend from 48 to 46 region and cortical plates were expanded. The histopathological examination revealed the sample to be cystic ameloblastoma (mural variety). This paper sends a message that the potential of a cyst should never be understated, instead it must be kept in an observation or understanding the biological behavior over a course of time.

we report a case of 51 year old lady, presented with complaints of hard lump in the right breast.On clinical examination lump of size 4x3cms in outer lower quadrant was noted. Ultrasonographic imaging showed evidence of 3x2cms ill dened hypoechoic lesion with spiculated margins and microcalcications. Trucut biopsy was done and histopathological report was given as Suspicious for malignancy which was followed by modied radical mastectomy with axillary clearance. Histopathological examination and Immunohistochemistry(IHC) was done.

AbstractPhaeohyphomycosis is a collective term used for fungal infections caused by moulds and yeasts that have brown pigmented cell walls due to the presence of melanin. These are also known as dematiaceous fungi. We report this patient who presented with headache, right hemiparesis, slurred speech, and altered sensorium. Patient was a known case of rheumatic valvular heart disease (RVHD) and had undergone balloon valvotomy for mitral stenosis 1 year back. Radiological features were suggestive of high grade glioma. Left fronto-parietal decompressive craniectomy with complete excision of mass lesion was performed. Histopathological examination of the surgical specimen revealed multiple granulomas with giant cells. These giant cells contained branched septate pigmented fungal hyphae in their cytoplasm. After the histopathology report, patient was started on intravenous amphotericin and was discharged on oral itraconazole 200 mg twice daily. Unfortunately, the patient was non-compliant and stopped taking oral itraconazole after 1 month. He landed up in fulminant fungal meningo-encephalitis and died 10 weeks after the initial diagnosis. We report a rare case of cerebral phaeohyphomycosis in a patient of RVHD which, to our knowledge, is nowhere mentioned in the literature.

Abstract OBJECTIVE AND IMPORTANCE: Intramedullary spinal tuberculoma is a rare form of central nervous system tuberculosis. This article describes an affected patient who presented with left leg paresis. CLINICAL PRESENTATION: A 46-year-old man presented with a 7-day history of left leg weakness. The patient's medical history included infection with pulmonary tuberculosis 15 years previously, at which time he had been treated with antituberculosis therapy. The neurological examination performed at admission revealed left leg paresis with Grade 2/5 power in all muscle groups. The patient reported no urinary or bowel problems. INTERVENTION: Surgery was performed with the patient in the prone position. The procedure involved laminectomies at T11, T12, and L1, followed by a midline myelotomy. The mass was excised completely. Histopathological examination revealed a granulomatous lesion that contained Langhans' giant cells, inflammatory cells, and evidence of caseating necrosis. The patient was prescribed a 6-month course of antituberculosis therapy with pyrazinamide, isoniazid, and rifampin. CONCLUSION: The outcome was favorable. Recently, a number of authors have reported success with medical management of intraspinal tuberculoma. Intraspinal tuberculoma produces a mass effect that can jeopardize spinal cord function. The optimal treatment is a combination of microsurgical resection and antituberculosis chemotherapy.

Malignant melanoma is a neoplasm of melanocytes. Primary malignant melanoma of the oronasal region is rare. When it does occur, it is difficult to manage, and the prognosis is not good. The current case was a 48 years old male patient who presented with history of sore throat, pain in left tonsillar region, hemoptysis with difficulty in swallowing. Clinical examination revealed a blackish ulcerated polypoid mass in the left palatine tonsil. Left sided tonsilectomy was done. Histopathological examination revealed primary malignant melanoma of the tonsil.TAJ 2009; 22(1): 279-281
