Insecticides are still largely applied in public health to control disease vectors. In Brazil, organophosphates (OP) and pyrethroids (PY) are used againstAedes aegyptifor years. Since 2009 Insect Growth Regulators (IGR) are also employed in the control of larvae. We quantified resistance to temephos (OP), deltamethrin (PY), and diflubenzuron (IGR) ofA. aegyptisamples from 12 municipalities distributed throughout the country, collected between 2010 and 2012. High levels of resistance to neurotoxic insecticides were detected in almost all populations: RR95to temephos varied between 4.0 and 27.1; the lowest RR95to deltamethrin was 13.1, and values higher than 70.0 were found. In contrast, all samples were susceptible to diflubenzuron (RR95< 2.3). Biochemical tests performed with larvae and adults discarded the participation of acetylcholinesterase, the OP target, and confirmed involvement of the detoxifying enzymes esterases, mixed function oxidases, and glutathione-S-transferases. The results obtained were discussed taking into account the public chemical control component and the increase in the domestic use of insecticides during dengue epidemic seasons in the evaluated municipalities.

The main content of the work consists of certain systematization and addition of longexisting, but eventually deformed and partly lost qualitative ideas about the role of thermal and wind factors that determine the physical mechanism of the World Ocean’s General Circulation System (OGCS). It is noted that the conceptual foundations of the theory of the OGCS in one form or another are contained in the works of many well-known hydrophysicists of the last century, but the aggregate, logically coherent description of the key factors determining the physical model of the OGCS in the public literature is not so easy to find. An attempt is made to clarify and concretize some general ideas about the two key blocks that form the basis of an adequate physical model of the system of oceanic water masses motion in a climatic scale. Attention is drawn to the fact that when analyzing the OGCS it is necessary to take into account not only immediate but also indirect effects of thermal and wind factors on the ocean surface. In conclusion, it is noted that, in the end, by the uneven flow of heat to the surface of the ocean can be explained the nature of both external and almost all internal factors, in one way or another contributing to the excitation of the general, or climatic, ocean circulation.

IntroductionThe COVID-19 pandemic has dramatically affected the Italian health systems and drastically impacted healthcare workers’ daily routine and training. Simulation is an efficient tool to provide medical education, especially in the case of incoming public health emergencies. This study investigated the role and activities of Italian simulation centres (SCs) during the acute phase of the COVID-19 pandemic.MethodsThe population was identified through a web search. The directors of Italian SCs were contacted via email and then enrolled. A structured interview was created, internally validated and administrated by phone to participants.ResultsFollowing the government’s ordinance, 37 (88.37%) SCs had to be closed to the public. Twenty (46.51%) SCs organised in situ simulation while 7 (16.28%) of them organised simulation inside the centre. Twenty-three (53.49%) SCs resorted to telematic modalities to provide training about COVID-19 and 21 (48.84%) of them for other training. Up to date, 13 SCs are still closed to the public.ConclusionsItaly has been severely hit by COVID-19, with differences between the regions. Almost all the SCs were closed, with only a few delivering training. The SCs took advantage of emergent technologies to create new ways to train people safely. Unfortunately, nearly one-fourth of Italian SCs have not reopened yet. The evolution of the COVID-19 epidemic calls for reconsideration about training activities including adequate safety measures implemented for all individuals involved.

When the hashtag #metoo began to circulate in digital and social media, it challenged a familiar interpretation of those who are raped or sexually harassed as victims, positioning women as embodied agents. Yet, almost exactly a year after the #metoo movement shot to visible prominence, a different, though eerily similar, story began to circulate on the same multi-media platforms as #metoo: a story about white male victimhood. Powerful men in positions of privilege (almost always white) began to take up the mantle of victimhood as their own, often claiming to be victims of false accusations of sexual harassment and assault by women. Through the analysis of five public statements by highly visible, powerful men who have been accused of sexual violence, I argue that the discourse of victimhood is appropriated not by those who have historically suffered but by those in positions of patriarchal power. Almost all of the statements contain some sentiment about how the accusation (occasionally acknowledging the actual violence) ‘ruined their life’, and all of the statements analyzed here center the author, the accused white man, as the key subject in peril and the authors position themselves as truth-tellers about the incidents. These statements underscore certain shifts in the public perception of sexual violence; the very success of the #metoo movement in shifting the narrative has meant that men have had to defend themselves more explicitly in public. In order to wrestle back a hegemonic gender stability, these men take on the mantle of victimhood themselves.

AbstractConsumption of illicit drugs poses health risks to the public and environment. Knowledge on their usage helps better implementations of intervention strategies to reduce drug-related harms in the society and also policies to limit their releases as emerging contaminants to recipient environments. This study aimed to investigate from the daily consumption to treatment efficiency and subsequent discharge of illicit drugs by the Swedish urban populations based on simultaneous collection and analysis of influent and effluent wastewater. Two different weekly monitoring campaigns showed similar drug prevalence in Stockholm and Uppsala, with amphetamine as the most popular drug. Almost all target drug residues were still measurable in effluent wastewater. High removal efficiencies (> 94%) were observed for amphetamine, cocaine and benzoylecgonine, whereas ketamine, 3,4-methylenedioxymethamphetamine (MDMA), mephedrone and methamphetamine were the least removed substances (< 64%), with the highest discharge observed for MDMA in both catchments (~ 3.0 g/day in Uppsala; ~ 18 g/day in Stockholm). Our study provides new insights into short-term changes in the use and related discharge of illicit drugs by urban populations. Such wastewater monitoring can provide useful information to public health, forensic and environmental authorities in planning future intervention and regulation policies.

Abstract Introduction The merchant marine fleet is under inspection by several parties to ensure maritime regulation compliance. International Maritime Organization mainly regulates the industry, and the most effective defender is indeed Port StateControl run by the regional memorandum of understandings. Objective This article aims to analyze all detention remarks of Paris Memoranda of Understanding (MOU) from 2013 to 2019 for EU15 countries (except Luxemburg and Austria) to guide marine industry on detainable Port State Control remarks and country risk profile. Methods The data of the detained vessels taken from the public website of Paris MOU and each report considered as a professional judgment that causes detention. Analytical Hierarchy Process Approach has been utilized to indicate the ranking of basic maritime regulations from the perspective of the Port State Control, and Geographic Information System (GIS) helps us to demonstrate the regional dispersion amongst EU15. Results Through an approach based on Analytical Hierarchy Process and demonstrating the results on GIS has been shown that almost all the country’s top priorities for regulation are Safety of Life at Sea (SOLAS) and Fire Safety Systems (FSS). Moreover, a comparative demonstration of the detention percentage of each regulation to AHP results demonstrates a better understanding of EU15 countries' detention profile. Conclusion The results of the study can assist Port State Officers, ship crew, ship owners, and managers in presenting the facts of their inspection and able to improve themselves. The spatial analysis is also expected to guide ship owners and managers to focus their vessel’s deficiencies on preventing sub-standardization. Policymakers also utilize these reports to evaluate their inspection practices.

Abstract Coyotes are ubiquitous in habitats across North America, including in urban areas. Reviews of human–coyote encounters are limited in scope and analysis and predominantly document encounters that tend to be negative, such as human–wildlife conflict, rather than benign experiences. The objective of our study was to use citizen science reports of human–coyote interactions entered into iNaturalist to better understand the range of first person accounts of human–coyote encounters in Madison, WI. We report 398 citizen science accounts of human–coyote encounters in the Madison area between October 2015 and March 2018. Most human–coyote encounters occurred during coyote breeding season and half of all encounters occurred in moderate development land cover. Estimated level of coyote aggressiveness varied significantly, with 90% of citizen scientists scoring estimated coyote aggression as a 0 and 7% scoring estimated aggression as a 1 on a 0–5 scale (with 0 being calm and 5 being aggressive). Our best performing model explaining the estimated distance between the human observer and a coyote (our proxy for a human–coyote encounter) included the variables distance to nearest paved road, biological season of the year relative to coyote life history, and time of day/night. We demonstrate that human–coyote interactions are regularly more benign than negative, with almost all first-hand reported human–coyote encounters being benign. We encourage public outreach focusing on practices that can foster benign encounters when educating the public to facilitate human–coyote coexistence.

Smoking during pregnancy can endanger pregnancy and the fetus, especially the health of pregnant women and fetal development in the womb. One of the complications of pregnancy that causes fetal death is due to oxygenation disorders. In Indonesia, more than half of households have at least one smoker, and almost all smokers’ smokes at home. The cause of neonatal death is fetal death in utero, asphyxia or respiratory problems due to smoking and premature. This study aims to determine the knowledge of pregnant women about the dangers of smoking to pregnancy and the fetus in Gasaribu Village, Laguboti, Toba Samosir Regency. This research was conducted in September-October 2019 using adescriptive research design. The number of samples was in this study were 40 pregnant women. Sampling is done by using saturated sampling. The results showed that good knowledge of 16 people (40%) was enough 22 people (55%) and less 2 people (5%).  The results of this study are expected to be used as additional material in adding knowledge and information to increase real health education (real) to the public about the dangers of smoking to pregnancy and the fetus.

AbstractCOVID-19 is a serious problem that faced by almost all countries in the world. Since announced as a pandemic by WHO on March, the number of positive COVID-19 cases in Indonesia has reached 287,008 patients until the end of September 2020. COVID-19 cases dispersion depends on the local government policies and the awareness of the public to obey. About 60 percent of positive cases and 65 percent of death cases were in Java. Therefore, the aim of this study is to analyze the growth of the COVID-19 case in Java from March to September 2020 in relation to the local government policies. The results showed that six provinces in Java had the same pattern. The large-scale social restriction (PSBB), which was implemented since April 10 2020, seen to be able to contain the dispersion of cases because from April to June the positive cases did not increase significantly. A very high increase occurred in August and September, this was probably due to the easing of the PSBB become new-normal where various public places and facilities have been reopened. Meanwhile, death cases, the number is very high in mid-April and June and late July to September 2020. For cured cases, there was a significant increase in late August to September 2020. The importance of information about the growth of the COVID-19 cases can help government to formulate strategies and policies to prevent the dispersion of COVID-19.Keyword: COVID-19, Pandemic, Java, cases growth AbstrakCOVID-19 merupakan suatu masalah serius yang sedang dihadapi oleh hampir seluruh negara. Sejak diumumkan sebagai pandemi oleh WHO pada Maret lalu, jumlah kasus positif COVID-19 di Indonesia mencapai 287.008 pasien hingga akhir September. Penyebaran kasus COVID-19 sangat bergantung pada kebijakan yang ditetapkan oleh pemerintah serta kesadaran masyarakat untuk mematuhinya. Sebesar 60 persen kasus positif dan 65 persen kasus meninggal berada di Pulau Jawa. Oleh karena itu, tujuan penelitian ini adalah menganalisis perkembangan kasus COVID-19 di Pulau Jawa sejak Bulan Maret-September 2020 dikaitkan dengan kebijakan pemerintah setempat. Hasil penelitian menunjukkan bahwa keenam provinsi memiliki pola yang sama. Pembatasan Sosial Berskala Besar (PSBB) yang dilakukan sejak 10 April 2020 terlihat dapat menahan penyebaran kasus karena pada bulan April-Juni 2020 kasus positif tidak mengalami kenaikan yang signifikan. Kenaikan yang sangat tinggi terjadi pada bulan Agustus dan September 2020, hal ini mungkin disebabkan oleh dilonggarkannya kebijakan PSBB menjadi PSBB Transisi dimana berbagai tempat dan fasilitas umum telah dibuka kembali. Sementara untuk kasus meninggal, angka tertinggi terjadi pada pertengahan April dan Juni serta akhir Juli hingga September 2020. Untuk kasus sembuh, terjadi peningkatan yang signifikan pada akhir Agustus hingga September 2020. Pentingnya informasi tentang perkembangan kasus COVID-19 ini dapat membantu pemerintah daerah untuk membuat strategi dan kebijakan untuk mencegah penyebaran rantai virus COVID-19.Kata Kunci: COVID-19, Pandemi, Jawa, perkembangan kasus

Bureaucratic reformation is the fundamental structuring efforts are expected to havean impact on changing systems and structures. The system deals with the relationshipbetween the unsure or the element that influence each other and are associated to makea form totally. The change in one element can influence the other elements in the system.The structure relates with the order of who arrayed a regular basis and systematically.Structure changes is also included with the mechanism and procedure, human resources,facilities and infrastructure, organization and organization’s environment in terms of theachievement of the efficiency of government bureaucracy. These changes include allowingall of the aspects of the bureaucracy has sufficient capacity to carry out the duties and thebasic function. Bureaucratic failure in a term for service the public until now representpoor government both at central and local government level. The urgency of bureaucraticreform in Indonesia is driven by a number of important note. First, the increased apparatusexpenditure is caused by increased of apparatus recruitment without unmeasured control.Second, the ballooning cost of democracy (election) affected the floated of local governmentbudget have increased significantly. The magnitude of the election budget and the impact ongovernmental bureaucracy resulted not ready to close the budget deficit. More than thesebureaucracy has loyalty dilemma caused by dispersed of concentration in every election’sactivity. Third, increased of develop the bureaucratic organization without planning andanalysis of the measured trigger financing and recruitment of apparatus that not less. Asa result, the bureaucracy in most areas are overload, or even lack in outside of Java. Inother side, less of local incomes make dependence to central government, while the localgovernment expenditure is to far from efficiency, even tend to be less controlled due to thehigh cost of the organization. The purpose and objectives of the study is to identify the hormonal, biochemical and ultrasound features of the gestation course in women with beneficial tumours of the uterus. Results. We conducted a complete clinical examination and prospective observation of 182 pregnant women. Of which, 98 puerperas with a verified diagnosis of uterine fibroids and/or endometriosis, which developed before gestation, but did not prevent the onset of pregnancy, were included into the main group. The comparison group included 84 women with physiological pregnancy. The studies showed that 14 pregnant women in the main group had C677T (Ala222Val) T/T mutation, while only 2 women in the comparison group had a decrease in enzyme activity due to genetic mutation. Accordingly, the homocysteine level was almost 3 times lower in the comparison group than in the main group. The women with hyperhomocisteinemia (HHC) and uterine fibroid in the main group showed the lowest estriol level and hCG level, while women without HHC had higher estriol level and hCG levels. It was found that almost all patients with HHC had subclinical hypothyroidism. Ultrasound imaging and biochemical tests at the beginning of the 2nd trimester: no ultrasound markers of fetal anomalies were identified in women from both groups; however, signs of retrochorial hematoma were detected in 57 women from the main group, which was confirmed by clinical manifestations and previous ultrasound imaging in earlier gestation periods. Conclusion. Thus, placenta formation in women with HHC and uterine fibroids is accompanied by relative hormonal insufficiency, which is clinically manifested as a threat of miscarriage in the early stages, but carrying a child is possible due to appropriate management of a patient as part of the preserving therapy. However, the issue of preventing the development of fetoplacental insufficiency is a valid one for further investigation.
