Notch3 and pS6 play important roles in tumor angiogenesis. To assess the expression of Notch3 and pS6 in Chinese ovarian epithelial cancer patients, a ten-year follow-up study was performed in ovarian epithelial cancer tissues from 120 specimens of human ovarian epithelial cancer, 30 specimens from benign ovarian tumors, and 30 samples from healthy ovaries by immunohistochemistry. The results indicate that the expression of Notch3 and pS6 was higher in ovarian epithelial cancer than in normal ovary tissues and in benign ovarian tumor tissues (p<0.01). In tumor tissues, Notch3 expression and pS6 expression were negatively associated with age (p>0.05) but positively associated with clinical stage, pathological grading, histologic type, lymph node metastasis, and ascites (p<0.05orp<0.01). A follow-up survey of 64 patients with ovarian epithelial cancer showed that patients with high Notch3 and pS6 expression had a shorter survival time (p<0.01), in which the clinical stage (p<0.05) and Notch3 expression (p<0.01) played important roles. In conclusion, Notch3 and pS6 are significantly related to ovarian epithelial cancer development and prognosis, and their combination represents a potential biomarker and therapeutic target in ovarian tumor angiogenesis.

We aimed to develop a diagnostic model identifying ovarian cancer (OC) from benign ovarian tumors using metagenomic data from serum microbe-derived extracellular vesicles (EVs). We obtained serum samples from 166 patients with pathologically confirmed OC and 76 patients with benign ovarian tumors. For model construction and validation, samples were randomly divided into training and test sets in the ratio 2:1. Isolation of microbial EVs from serum samples of the patients and 16S rDNA amplicon sequencing were carried out. Metagenomic and clinicopathologic data-based OC diagnostic models were constructed in the training set and then validated in the test set. There were significant differences in the metagenomic profiles between the OC and benign ovarian tumor groups; specifically, genus Acinetobacter was significantly more abundant in the OC group. More importantly, Acinetobacter was the only common genus identified by seven different statistical analysis methods. Among the various metagenomic and clinicopathologic data-based OC diagnostic models, the model consisting of age, serum CA-125 levels, and relative abundance of Acinetobacter showed the best diagnostic performance with the area under the receiver operating characteristic curve of 0.898 and 0.846 in the training and test sets, respectively. Thus, our findings establish a metagenomic analysis of serum microbe-derived EVs as a potential tool for the diagnosis of OC.

We studied the epoophoron in women with benign ovarian tumor using morphological methods. It was established that the structure epoophoron in women with benign ovarian tumors, there are significant differences compared with controls. This manifests itself in reducing the number of tubules, increase in their specific area, outer and inner diameter, the height of the epithelium, the presence of secretions in the lumen.

Objective: To determine the prevalence of metastasis to appendix from primary surgery of ovarian epithelial cancer at National General Hospital Dr. Cipto Mangunkusumo (RSCM), Indonesia.Methods: A cross sectional study was done using ovarian epithelial cancer patient medical record whose primary ovarian cancer and appendectomy surgery were conducted on July to December 2019 at RSCM. Patients without appendix histopatology result and previous chemotherapy were excluded in this study. Consecutive method and random sampling were used in this study.Results: : A total of 80 subjects were included in this study. Subjects have average age of 48 years old. Out of all samples, 43 samples (53.8%) were defined as stage I patient, 7 subjects (8.8%) as stage II, 30 subjects (37.5%) as stage III, and none as stage IV. Appendectomy were done and eight subjects (10%) experienced metastasis to appendix. A total of 19 subjects (23.8%) had chronic appendicitis and 53 subjects (66.3%) did not have metastasis to the appendix. Among eight subjects having appendix involvement, 4 had mucinous histology, 2 serous, and 2 endometrioid. Six out of eight were diagnosed at clinical stage III and two were diagnosed at stage I.Conclusion: The prevalence of appendix metastases from primary surgery in ovarian epithelial cancer at RSCM was 10%. Based on this research, appendectomy can be considered on ovarian cancer surgeryKeywords:    appendix, metastasis, ovarian cancer.   Abstrak Tujuan: Mengetahui prevalensi metastasis kanker epitelial ovarium ke apendiks pada pembedahan primer kanker epitelial ovarium di Rumah Sakit Umum Pusat Nasional Dr. Cipto Mangunkusumo (RSCM), Indonesia. Metode: Penelitian ini merupakan studi potong lintang menggunakan data rekam medis pasien kanker ovarium epitelial yang menjalani pembedahan primer dan apendiktomi pada bulan Juli hingga Desember 2019 di RSCM Pasien tanpa histopatologi apendiks atau pernah dilakukan kemoterapi sebelumnya dieksklusi dari penelitian. Digunakan metode pengambilan sampel secara acak. Hasil: Didapatkan 80 subjek penelitian yang diikutsertakan dalam penelitian. Dari 80 subjek penelitian, didapatkan rerata usia 48 tahun. Sebanyak 43 subjek (53,8%) didiagnosis dengan stadium I, 7 subjek (8,8%) sebagai stadium II, 30 subjek (37,5%) stadium III. Dari 80 subjek yang menjalani apendiktomi, didapatkan 8 subjek (10%) anak sebar ke apendiks, 19 subjek (23,8 %) apendisitis kronis, 53 subjek (66,3%) tidak terdapat anak sebar. Dari 8 subjek yang terdapat anak sebar ke apendiks dengan temuan histologi 4 musinosum, 2 serosum, 2 endometroid. Sebanyak enam dari delapan subjek terdiagnosis pada stadium klinis stadium III dan dua lainnya pada stadium klinis satu. Kesimpulan: Prevalensi metastasis apendiks pada operasi primer kanker ovarium epitelial di RSCM adalah sebesar 10%. Berdasarkan hasil penelitian ini, apendektomi dapat dipertimbangkan dilakukan pada pembedahan baik stadium awal maupun stadium lanjut.Kata kunci: , apendiks , kanker ovarium, metastatis

(1) Background: The aim of this study is to compare the IOTA-ADNEX (international ovarian tumor analysis–assessment of different neoplasias in the adnexa) model with gynecologic experts in differentiating ovarian diseases. (2) Methods: All participants in this prospective study underwent ultrasonography (US) equipped with the IOTA-ADNEXTM model and subjective assessment by a sonographic expert. Receiver operating characteristic (ROC) curves were also generated to compare overall accuracies. The optimal cut-off value of the ADNEX model for excluding benign diseases was calculated. (3) Results: Fifty-nine participants were eligible: 54 and 5 underwent surgery and follow-up computed tomography (CT), respectively. Benign and malignant diseases were confirmed in 49 (83.1%) and 10 (16.9%) participants, respectively. The specificity of the ADNEX model was 0.816 (95% confidence interval (CI): 0.680–0.912) in all participants and 0.795 (95% CI, 0.647–0.902) in the surgical group. The area under the ROC curve of the ADNEX model (0.924) was not significantly different from that of subjective assessment (0.953 in all participants, 0.951 in the surgical group; p = 0.391 in all participants, p = 0.407 in the surgical group). The optimal cut-off point using the ADNEX model was 47.3%, with a specificity of 0.977 (95% CI: 0.880–0.999). (4) Conclusions: The IOTA-ADNEX model is equal to gynecologic US experts in excluding benign ovarian tumors. Subsequently, being familiar with this US software may help gynecologic beginners to reduce unnecessary surgery.

Objective: To compare the expression of telomerase in benign ovarian tumors and borderline ovarian tumors. Methods: Thirteen samples of paraffin blocks of ovarian borderline tumors and benign ovarian tumors were taken from patients who underwent surgery from January 2006 to December 2011. In all samples, we performed immunohistochemical staining to the paraffin blocks. Semi quantitative determination of the expression of telomerase is done by an Anatomic Pathology specialist and Gynecologist, which already had the same perception about assessing the standardization sample. We used Fisher’s test to analyze the data. Results: There were significant relationship in the moderate expression of telomerase in the nucleus and cytoplasm between benign and borderline ovarian tumors, with an odds ratio of moderate telomerase expression in the cytoplasm and nucleus of 19.3 (95% CI: 1.4 - 943) and 26 (95% CI : 2.3 - 1211). This means that the risk of borderline ovarian tumors in the expression of moderate telomerase in the cytoplasm was 3.19 times compared to the negative expression, whereas in the nucleus it became 26-fold. There is no significant relationship among menopause, age, and telomerase expression in the nucleus and cytoplasm. Conclusion: There are significant differences in the expression of moderate strength telomerase in nucleus and cytoplasm between benign and borderline ovarian tumors. BackgroundFluorodeoxyglucose positron emission tomography/computed tomography (FDG-PET/CT) is useful for diagnosing malignant tumors. Intracellular FDG uptake is measured as the standardized uptake value (SUV), which differs depending on tumor characteristics. This study investigated differences in maximum SUV (SUVmax) according to histologic type in ovarian epithelial cancer and the relationship of SUVmax with prognosis.MethodsThis study included 80 patients with ovarian epithelial cancer based on histopathologic findings at surgery and who had undergone PET/CT before treatment. Maximum SUV on PET/CT of primary lesions and histopathology were compared based on histologic type, and the prognosis associated with different SUVmax was evaluated.ResultsClinical tumor stage was I in 35 patients, II in 8, III in 25, and IV in 12. Histologic type was serous adenocarcinoma (AC) in 33 patients, clear cell AC in 27, endometrioid AC in 15, and mucinous AC in 5. Median SUVmax was lower in mucinous AC (2.76) and clear cell AC (4.9) than in serous AC (11.4) or endometrioid AC (11.4). Overall, median SUVmax was lower in clinical stage I (5.37) than in clinical stage ≥II (10.3). However, in both clear cell AC and endometrioid AC, when histologic evaluation was possible, no difference was seen between stage I and stage ≥II. Moreover, in clear cell AC, the 5-year survival rate was significantly higher in the low-SUVmax group (100%) than in the high-SUVmax group (43.0%, P = 0.009).ConclusionsMaximum SUV on preoperative FDG-PET/CT in ovarian epithelial cancer differs according to histologic type. In clear cell AC, SUVmax may represent a prognostic factor.

Background: We investigated the long-term clinical results of percutaneous transluminal angioplasty (PTA) in patients with peripheral arterial occlusive disease (PAOD) and the influence of different parameters on the primary success rate, the rate of complications and the long-term outcome. Patients and methods: We reviewed clinical and hemodynamic follow-up data of 166 consecutive patients treated with PTA in 1987 in our department. Results: PTA improved the clinical situation in 79.4% of patients with iliac lesions and in 88.3% of patients with femoro-popliteal lesions. The clinical stage and ankle brachial index (ABI) post-interventional could be improved significantly (each P < 0,001), the same results were observed at the end of follow-up (each P < 0,001). Major complications occurred in 11 patients (6.6%). The rate of primary clinical long-term success for suprainguinal lesions was 55% and 38% after 5 and 10 years (femoro-popliteal 44% and 33%), respectively, the corresponding data for secondary clinical long-term success were 63% and 56% (60% and 55%). Older age (P = 0,017) and lower ABI pre-interventional (P = 0,019) significantly deteriorated primary clinical long-term success for suprainguinal lesions, while no factor could be identified influencing the outcome of femoro-popliteal lesions significantly. Conclusion: Besides an acceptable success rate with a low rate of severe complications, our results demonstrate favourable long-term clinical results of PTA in patients with PAOD.
