Aim. The aim of this study is to analyze the etiological factors underlying the presence of maxillary midline diastema in a sample of orthodontic patients.Materials and Methods. One hundred patients who fulfill the inclusion criteria were selected from 1355 patients seeking orthodontic treatment. The pretreatment orthodontic records were analyzed. The width of the maxillary midline diastema was measured clinically with a digital caliper at two levels: the mesioincisal angles of the central incisors and five millimeters from the incisal edge. The two measurements were averaged, and patients with diastema of more than 0.5 millimeter in width were enrolled.Results. Diastema is a multifactorial clinical finding with more than one underlying etiological cause. The interrelationship between the familial pattern of midline diastema and the microdontia, macroglossia, labial frenum, and alveolar cleft conforms was clear. The effect of a mesiodens and the upper lateral incisor whether bilaterally missing, unerupted, or peg shaped was minimal.Conclusion. Etiological factors underlying maxillary midline diastema are interconnected. Using a checklist as a guide during handling maxillary midline diastema is important in the different stages of treatment.

ABSTRACT Objectives: To examine possible changes in the levels of salivary antioxidants, C-reactive protein (CRP), cortisol, pH, proteins, and blood in patients treated with fixed orthodontic appliances. Materials and Methods: Salivary samples from 21 orthodontic patients who met specific inclusion criteria were collected before the beginning of orthodontic treatment (T0; baseline), 1 hour after bonding (T1), and 4–6 weeks after bonding (T2). Oxidant-scavenging ability (OSA) was quantified using a luminol-dependent chemiluminescence assay. Cortisol and CRP levels were measured using immunoassay kits. pH levels and presence of proteins and blood in the samples were quantified using strip-based tests. Results: A significant decrease in salivary pH was observed after bonding (P = .013). An increase in oxidant-scavenging abilities during orthodontic treatment was detected, but the change was not statistically significant. Cortisol and CRP levels slightly increased after bonding, but the difference was small without statistical significance. Changes in the presence of proteins and blood were also insignificant. Conclusions: Exposure to fixed orthodontic appliances did not show a significant effect on salivary parameters related to inflammation or stress, with the exception of a significant but transient pH decrease after bonding.

ABSTRACT Transposition of maxillary teeth is an eruptive disturbance occurring in approximately 1 of every 300 orthodontic patients. Such cases are frequently very challenging in terms of treatment planning and orthodontic management. The canine is one of the most commonly transposed teeth, ectopically positioned with either the lateral incisor or the first premolar. This case report illustrates unique orthodontic treatment, describes treatment procedures, and presents the final outcome of bilateral maxillary canine-lateral incisor complete transpositions in which the involved teeth were moved to their clinically normal position in the dental arch without extracting premolars.

Introduction: Maxillary lateral incisor is a common congenitally missing tooth in the oral cavity. Maxillary lateral incisor is also the most common varying tooth in form than any other tooth. The objective of the study is to estimate the prevalence of congenitally missing maxillary lateral incisors and peg laterals in patients receiving orthodontic treatment. Materials & Method: This is a cross sectional retrospective study carried out among 204 orthodontic patients. Panoramic radiographs taken during the diagnostic procedure of orthodontic treatment was used to confirm the presence or absence of lateral incisors. The presence of peg laterals was determined by dental casts. Result: Prevalence of missing maxillary lateral incisors and peg laterals was found 2.5% and 3.9% respectively. Prevalence of missing lateral in male was 5.7% while among female no missing laterals were observed. Prevalence of peg laterals in male and female was 1.1% and 6% respectively. Among the missing lateral incisors, prevalence of bilaterally missing lateral incisors was 60% while prevalence of both right and left unilateral missing laterals was 20%. Among the peg laterals, bilateral peg laterals were present in 50% patients, followed by left unilateral 37.5% and right unilateral 12.5% peg laterals. Conclusion: The present study shows higher prevalence of peg laterals than missing lateral incisors. Prevalence of missing lateral incisors was higher in male and prevalence of peg laterals was higher in female. Prevalence of bilaterally missing laterals and bilateral peg laterals was higher than unilateral presentation.

Objectives: To evaluate the level of knowledge, understanding, and impact of the coronavirus disease 2019 (COVID-19) pandemic on adult patients regarding their ongoing fixed orthodontic treatment. Subjects and Methods: A cross-sectional and descriptive survey was conducted on 108 adult patients undergoing fixed orthodontic treatment at GSL Dental College (GSLDC), India. All the candidates were aged 18 years and above, who were in lockdown due to the COVID-19 pandemic and devoid of treatment. Results: Among the patients 43.5% were male and 56.5% were female; 22.3% were not at all aware that the COVID-19 virus spreads quickly in a dental setup; 64.8% were definitely willing for their status disclosure and to undergo pretreatment screening; 71.3% were definitely willing to adhere to strict appointment timings for adequate sanitization of the clinical area; 60.2% thought that missed appointments during the COVID-19 pandemic would prolong their overall orthodontic treatment and affect their treatment outcome; and 51.9% were financially affected due to the pandemic. Conclusions: Not all adult orthodontic patients were in knowledge of COVID-19 cross infection. The majority were in a state of understanding and willing to follow infection-eradicating protocols. Patients’ view on overall orthodontic treatment was altered due to the psychological and financial impact of COVID-19.

Abstract Background The present study aimed to investigate the patients’ perception of the dental practice during the COVID-19 outbreak, and whether the pandemic will affect the attendance of orthodontic patients at the dental practice. An online questionnaire, including the Patient Health Questionnaire-4 (PHQ-4), was submitted to Italian dental patients with items about their perceived risks when going to the dentist, concerns about continuing orthodontic treatment, and the onset of temporomandibular disorders (TMD). Data were analyzed with a chi-square test and logistic regression analysis. The level of significance was set at P < 0.05. Results A total of 1566 subjects completed the survey, including 486 who were under orthodontic treatment or who had a child in orthodontic treatment. A total of 866 participants (55.3%) thought the risk of contracting the COVID-19 infection was higher in a dental practice; this perception was associated with gender (women more than man), age (over 60 years old) and high levels of distress (P<0.001). However, 894 patients (57.1%) felt comfortable going back to the dentist. Most of the orthodontic patients (84%) would continue their treatment. After the lockdown, there was a slight increase in the frequency of TMD pain (356 versus 334). Conclusions Most of the participants believed that the dental practice is a place at greater risk of contracting COVID-19, even if they continue to go to the dentist. Gender, age, and the level of distress were associated with the increase in the fear of going to the dentist due to COVID-19. Because of the pandemic, 16% of patients undergoing orthodontic treatment would not return to the dental practice to continue their orthodontic treatment after the lockdown. The prevalence of TMD pain in the population increased due to the pandemic.

The records of 200 orthodontic patients accepted for treatment by the Orthodontic Department of the Bristol Dental Hospital in 1977 were compared with 200 taken on in 1985 in order to determine whether there had been any change in the proportion of referred cases requiring more complex procedures. Within each sample, cases were categorized as follows: (a) suitable for removable appliance treatment by an undergraduate or general practitioner; (b) requiring simple one arch fixed appliance treatment such as might he attempted by a general practitioner after a period of further training; (c) needing specialist treatment such as full multibracketed fixed appliances or orthognathic surgery. It was found that there had been no change in the proportions of simple and complex cases referred during the 8-year period although the proportion of patients now receiving complex treatment had increased greatly. Possible explanations and implications are discussed.

Introduction: Anxiety is the state of feeling nervous or worried that something bad is going to happen. Dental anxiety is defined as a patient’s response to stress that is associated with a dental procedure. The aim of our study is to investigate the anxiety status of dental patients visiting Orthodontic department at Nobel Medical College Teaching Hospital, Biratnagar. Materials and Method: Total 80 ongoing orthodontic patients (M =21 F=59) who completed modified Dental Anxiety Scale questionnaire were included in the study. Result: Majority of patients (65%) had moderate anxiety where as 25% had mild anxiety and around 9% had severe to extreme anxiety. Patients’ age and education level had significant association with the level of dental anxiety. Conclusion: Dental anxiety in orthodontic patients is unavoidable but needs appropriate counseling. Orthodontist’s role is crucial in bridging the gap between patients’ perception towards orthodontic treatment and the actual treatment.

Crossbite is one of the most prevalent malocclusion, posterior crossbite occurs in 8% to 22% of orthodontic cases and anterior crossbite has been seen in Class III cases, which accounts for 3.4% of orthodontic cases. The etiology of posterior crossbite can include any combination of dental, skeletal, and neuro muscular functional components, but the most frequent cause is reduction in width of the maxillary dental arch. Patients/cases seeking comprehensive orthodontic treatment in between 5 to 35 years were diagnosed for crossbite with diagnostic model and care record file. Out of 300 cases 163(54.3%) had crossbite, 90(30%) had anterior crossbite and 109(36.3%) had posterior cross bite. Among posterior crossbite 60(20%) had unilateral and 49(16.3%) had bilateral crossbite. Posterior crossbite was more prevalent than anterior crossbite. Cases with Class I molar relation showed more crossbite. Crossbite was more prevalent in cases with congenitally missing teeth.Bangladesh J Medicine Jan 2015; 26 (1) : 9-12

Introduction: Root resorption is the loss of apical root tissue leading to the shortness of root which is often evident in orthodontic tooth movement. Proper management during orthodontic treatment however can minimize this undesirable outcome. The present article attempts to review the etiology of root resorption, methods of diagnosis, and strategies for prevention.Materials & Method: A scoping review was done with the purpose to carry out the narrative integration of the relevant evidences on root resorption and orthodontic treatment from the published literatures. The resulting papers were studied and reviewed thoroughly for the key explanation of root resorption in orthodontic patients. A total of 41 published research articles were reviewed.Discussion: According to the literatures root resorption is common iatrogenic outcome in orthodontic treatment. Biological, mechanical, and combined biological and mechanical factors result in external root resorption. Though most clinicians diagnose root resorption by conventional radiography, researches have clearly shown that CBCT is the promising tool. The clinicians need to counsel orthodontic patients and their parents that the root resorption might be a potential consequence of the treatment lasting for long time. In case of severity; it is essential to reassess the patient and minimize the underlying cause. It is necessary to understand the role of orthodontist in preventing root resorption
