Background. Macroprolactinemia is defined as predominance of high molecular weight prolactin forms in the circulation. Although macroprolactin is considered as a biologically inactive molecule, some authorities suggest treatment in symptomatic cases. Gigantomastia is defined as excess breast tissue and most cases in the literature were treated by surgical intervention.Case. A 44-year-old woman was admitted to our clinic with gigantomastia and galactorrhea. The patient had a demand for surgical therapy. In laboratory examination, she had hyperprolactinemia and macroprolactinemia. Pituitary imaging revealed 6 mm microadenoma in right side of the hypophysis. Since she was symptomatic, cabergolin treatment was started. Macroprolactin became negative, breast circumference decreased significantly, and galactorrhea resolved after treatment.Conclusion. Gigantomastia might be the presenting symptom in patients with macroprolactinemia. In these patients medical treatment with cabergoline may be used initially as an alternative to surgical approach.

Crohn's disease may affect any segment of the gastrointestinal tract; however, isolated duodenal involvement is rather rare. It still remains a complex clinical entity with a controversial management of the disease. Initially, patients with duodenal Crohn' s disease (DCD) are managed with a combination of antiacid and immunosuppressive therapy. However, medical treatment fails in the majority of DCD patients, and surgical intervention is required in case of complicated disease. Options for surgical management of complicated DCD include bypass, resection, or stricturoplasty procedures. In this paper, we reported a 33-year-old male patient, who was diagnosed with isolated duodenal Crohn’s diseases, and reviewed the surgical options in the literature.

Introduction The occurrence of isolated spheno-choanal polyps are not very common and can be very well confused with antro-choanal polyps or adenoids in adolescents and children. Appropriate diagnosis and prompt surgical intervention are essential for its removal.   Case Report We report one such case of a 16 year old female who presented with complaints nasal obstruction and reduced hearing. Endoscopic polypectomy was done with complete removal of the polyp.   Discussion Sphenochoanal Polyp is often misdiagnosed and proper investigations are thus essential for its diagnosis. Surgical Approach is the mainstay form of treatment with complete excision of the polyp.

We report an uncommon ssevere soft-tissue infection of the thighs in a male child with acute lymphoblastic leukemia. Early and aggressive medical treatment and the conservative surgical approach were successful. Necrotizing fasciitis should be suspected in any soft-tissue infection until it can be definitely ruled out, since prompt deliver of medical and surgical intervention is essential.

Abstract The rationale behind endodontics is to thoroughly disinfect the root canal system, and to provide hermetic seal which in turn ceases the progress of the periapical infection. Most of these periapical infections attributed directly or indirectly to oral microbes. Surgical interventions in larger periapical infection are sometimes accompanied by certain complications. Hence, non-surgical approach can be considered. This article presents non-surgical healing of a periapical lesion of endodontic origin associated with maxillary left central and lateral incisors, observed over a period of 6 months, using calcium hydroxide as an intracanal medicament. Abstract Osteomas are benign tumours that involve paranasal sinuses more than other parts of body. Frontal sinus is most common while sphenoid is least one. They are usually asymptomatic and diagnosed incidentally on imaging for other reasons. Giant variety of frontoethmoid osteoma is very rare and only few cases are described in literature. Orbital involvement in giant frontoethmoid osteoma is even rarer. Due to threat to vision, this variety is an indication for surgical intervention followed by reconstruction of defect. As these tumours are very large, endoscopic approaches are less indicated and there is no clear consensus which open surgical approach is better. We are reporting such a rare case of giant frontoethmoid osteoma causing proptosis and restriction of eye movement that was treated with open surgical approach.

Background. Postpartum haemorrhage (PPH) is a significant contributor to worldwide maternal morbidity and mortality. When PPH continues despite aggressive medical treatment, early consideration should be given to surgical intervention. Various surgical interventions may be used but conservative interventions are recommended primarily.Case. This case report describes laparoscopic coagulation of hypogastric artery technique in a patient with PPH.Conclusions. Laparoscopic ligature of the hypogastric artery for PPH treatment can be a valid alternative to laparotomy in patients with vaginal delivery.
