Recent research has led to the hypothesis that events which unfold in time might be spatially represented in a left-to-right fashion, resembling writing direction. Here we studied fourteen right-hemisphere damaged patients, with or without neglect, a disorder of spatial awareness affecting contralesional (here left) space processing and representation. We reasoned that if the processing of time-ordered events is spatial in nature, it should be impaired in the presence of neglect and spared in its absence. Patients categorized events of a story as occurring before or after a central event, which acted as a temporal reference. An asymmetric distance effect emerged in neglect patients, with slower responses to events that took place before the temporal reference. The event occurring immediately before the reference elicited particularly slow responses, closely mirroring the pattern found in neglect patients performing numerical comparison tasks. Moreover, the first item elicited significantly slower responses than the last one, suggesting a preference for a left-to-right scanning/representation of events in time. Patients without neglect showed a regular and symmetric distance effect. These findings further suggest that the representation of events order is spatial in nature and provide compelling evidence that ordinality is similarly represented within temporal and numerical domains.

Abstract Objectives: The aim of this systematic review was to identify the presence and nature of relationships between specific forms of aprosodia (i.e., expressive and receptive emotional and linguistic prosodic deficits) and other cognitive-communication deficits and disorders in individuals with right hemisphere damage (RHD) due to stroke. Methods: One hundred and ninety articles from 1970 to February 2020 investigating receptive and expressive prosody in patients with relatively focal right hemisphere brain damage were identified via database searches. Results: Fourteen articles were identified that met inclusion criteria, passed quality reviews, and included sufficient information about prosody and potential co-occurring deficits. Twelve articles investigated receptive emotional aprosodia, and two articles investigated receptive linguistic aprosodia. Across the included studies, receptive emotional prosody was not systematically associated with hemispatial neglect, but did co-occur with deficits in emotional facial recognition, interpersonal interactions, or emotional semantics. Receptive linguistic processing was reported to co-occur with amusia and hemispatial neglect. No studies were found that investigated the co-occurrence of expressive emotional or linguistic prosodic deficits with other cognitive-communication impairments. Conclusions: This systematic review revealed significant gaps in the research literature regarding the co-occurrence of common right hemisphere disorders with prosodic deficits. More rigorous empirical inquiry is required to identify specific patient profiles based on clusters of deficits associated with right hemisphere stroke. Future research may determine whether the co-occurrences identified are due to shared cognitive-linguistic processes, and may inform the development of evidence-based assessment and treatment recommendations for individuals with cognitive-communication deficits subsequent to RHD.

Understanding the relationship between symbolic numerical abilities and individual differences in mathematical competencies has become a central research endeavor in the last years. Evidence on this foundational relationship is often based on two behavioral signatures of numerical magnitude and numerical order processing: the canonical and the reverse distance effect. The former indicates faster reaction times for the comparison of numerals that are far in distance (e.g., 2 8) compared to numerals that are close in distance (e.g., 2 3). The latter indicates faster reaction times for the ordinal judgment of numerals (i.e., are numerals in ascending/descending order) that are close in distance (e.g., 2 3 4) compared to numerals that are far in distance (e.g., 2 4 6). While a substantial body of literature has reported consistent associations between the canonical distance effect and arithmetic abilities, rather inconsistent findings have been found for the reverse distance effect. Here, we tested the hypothesis that estimates of the reverse distance effect show qualitative differences (i.e., not all participants show a reverse distance effect in the expected direction) rather than quantitative differences (i.e., all individuals show a reverse distance effect, but to a different degree), and that inconsistent findings might be a consequence of this variation. We analyzed data from 397 adults who performed a computerized numerical comparison task, a computerized numerical order verification task (i.e., are three numerals presented in order or not), a paper pencil test of arithmetic fluency, as well as a standardized test to assess more complex forms of mathematical competencies. We found discriminatory evidence for the two distance effects. While estimates of the canonical distance effect showed quantitative differences, estimates of the reverse distance effect showed qualitative differences. Comparisons between individuals who demonstrated an effect and individuals who demonstrated no reverse distance effect confirmed a significant moderation on the correlation with mathematical abilities. Significantly larger effects were found in the group who showed an effect. These findings confirm that estimates of the reverse distance effect are subject to qualitative differences and that we need to better characterize the underlying mechanisms/strategies that might lead to these qualitative differences.

AbstractThe abstract concept of time is mentally represented as a spatially oriented line, with the past associated with the left space and the future associated with the right. Although the line is supposed to be continuous, most available evidence is also consistent with a categorical representation that only discriminates between past and future. The aim of the present study was to test the continuous or categorical nature of the mental timeline. Italian participants judged the temporal reference of 20 temporal expressions by pressing keys on either the left or the right. In Experiment 1 (N = 32), all words were presented at the center of the screen. In Experiment 2 (N = 32), each word was presented on the screen in a central, left, or right position. In Experiment 3 (N = 32), all text was mirror-reversed. In all experiments, participants were asked to place the 20 temporal expressions on a 10-cm line. The results showed a clear Spatial–TEmporal Association of Response Codes (STEARC) effect which did not vary in strength depending on the location of the temporal expressions on the line. However, there was also a clear Distance effect: latencies were slower for words that were closer to the present than further away. We conclude that the mental timeline is a continuous representation that can be used in a categorical way when an explicit past vs. future discrimination is required by the task.

AbstractIn the current study, we investigated whether standard assessment techniques of visuospatial neglect are sensitive to detecting dissociable subtypes. We administered a battery of tasks commonly used to detect the presence of visuospatial neglect to 120 patients with unilateral right hemisphere infarcts and, in most cases, performed a systematic analysis of their lesions to quantify and localize brain damage. Using a factor analysis, we discovered seven relatively independent constructs, three of which were specifically related to the presence of left hemispatial neglect: Left Attentional Processing, Line Bisection. and Word Reading. Impairments in two of these factors. Left Attentional Processing and Line Bisection, occurred together in most cases but also occurred independently in 38 cases. There were no cases in whom Word Reading was present without concomitant deficits in one or the other two factors. These three factors could not be distinguished neuroanatomically; that is, lesions were equally likely in the temporal/parietal cortex, dorsolateral frontal cortex, or in deep frontal structures. These data confirm the notion that hemispatial neglect is a complex and multifaceted disorder composed of cognitively independent processes. These processes, however, cannot be dissociated neuroanatomically based on currently available assessment techniques. (JINS, 1996, 2, 441–451.)

Hemispatial neglect has been conceptualized as having dissociable and potentially clinically relevant subtypes. However, the question of whether patient performance on neglect subtype measures is consistent over time remains largely unanswered. We examined changes in performance over time on measures of motor, perceptual, and personal neglect in 21 patients with neglect from acute right hemisphere stroke. Patients were assessed on three occasions, separated by at least one week, using a lateralized target test, lateralized response test, and modified fluff test. Across three testing timepoints, 18 (85.7%) patients changed subtype performance patterns at least once. In 13 (61.9%) of these patients, inconsistency between timepoints was not adequately accounted for by recovery. On initial testing, seven, patients (33.3%) demonstrated more than one neglect subtype symptom; by the third testing timepoint none of the patients demonstrated multiple symptoms. In the setting of acute stroke, performance on three measures of neglect symptoms is inconsistent across time. However, the distribution of neglect subtype symptoms appears to become more discrete over time. These findings complicate our understanding of the pathophysiology and potential prognostic value of neglect subtypes, and suggest that treatment decisions based on subtype performance assessed at a single timepoint, may be of limited utility.
