Background. High quality reporting of endoscopic procedures is critical to the implementation of colonoscopy quality assurance programs.Objective. The aim of our research was to (1) determine the quality of colonoscopy (CS) reporting in “usual practice,” (2) identify factors associated with good quality reporting, and (3) compare CS reporting in open-access and non-open-access procedures.Methods. 557 CS reports were randomly selected and assigned a score based on the number of mandatory data elements included in the report. Reports documenting greater than 70% of the mandatory data elements were considered to be of good quality. Physician and procedure factors associated with good quality CS reporting were identified.Results. Variables that were consistently well documented included date of the procedure (99.6%), procedure indication (88.9%), a description of the most proximal anatomical segment reached (98.6%), and documentation of polyp location (97.8%). Approximately 79.4% of the reports were considered to be of good quality. Gastroenterology specialty, lower annual CS volume, and fewer years in practice were associated with good quality reporting.Discussion. CS reporting in usual practice in Ontario lacks quality in several areas. Almost 1 in 5 reports was of poor quality in our study.Conclusions. Targeted interventions and/or use of mandatory fields in synoptic reports should be considered to improve CS reporting.

AbstractThe loss of a pet can be particularly distressing for owners, whether the method of death is euthanasia or is unassisted. Using primary-care clinical data, this study aimed to report the demographic and clinical factors associated with euthanasia, relative to unassisted death, in dogs. Method of death (euthanasia or unassisted) and clinical cause of death were extracted from a random sample of 29,865 dogs within the VetCompass Programme from a sampling frame of 905,544 dogs under UK veterinary care in 2016. Multivariable logistic regression modelling was used to evaluate associations between risk factors and method of death. Of the confirmed deaths, 26,676 (89.3%) were euthanased and 2,487 (8.3%) died unassisted. After accounting for confounding factors, 6 grouped-level disorders had higher odds in euthanased dogs (than dogs that died unassisted), using neoplasia as the baseline. The disorders with greatest odds included: poor quality of life (OR 16.28), undesirable behaviour (OR 11.36) and spinal cord disorder (OR 6.00). Breed, larger bodyweight and increasing age were additional risk factors for euthanasia. The results highlight that a large majority of owners will face euthanasia decisions and these findings can support veterinarians and owners to better prepare for such an eventuality.

An online survey was conducted to identify factors associated with financial insecurity, food insecurity and poor quality of daily lives of adults in Nigeria during the first wave of the COVID-19 pandemic. The associations between the outcome (experience of financial loss, changes in food intake and impact of the pandemic on daily lives) and the explanatory (age, sex, education level, anxiety, depression, HIV status) variables were determined using logistic regression analysis. Of the 4439 respondents, 2487 (56.0%) were financially insecure, 907 (20.4%) decreased food intake and 4029 (90.8%) had their daily life negatively impacted. Males (AOR:0.84), people who felt depressed (AOR:0.62) and people living with HIV -PLHIV- (AOR:0.70) had significantly lower odds of financial insecurity. Older respondents (AOR:1.01) had significantly higher odds of financial insecurity. Those depressed (AOR:0.62) and PLHIV (AOR:0.55) had significantly lower odds of reporting decreased food intake. Respondents who felt anxious (AOR:0.07), depressed (AOR: 0.48) and who were PLHIV (AOR:0.68) had significantly lower odds of reporting a negative impact of the pandemic on their daily lives. We concluded the study findings may reflect a complex relationship between financial insecurity, food insecurity, poor quality of life, mental health, and socioeconomic status of adults living in Nigeria during the COVID-19 pandemic.

Historically, pipeline operators have tended to place more weight on inline inspection tool specifications than on the inherent design and reporting capabilities of the service providers themselves. While internal collection of integrity data is very important, it’s imperative that vendors, also, have high levels of expertise and effective quality control systems in place to successfully analyze exceedingly high volumes of inspection data. The quality of inspection information is vital to assessing if a pipeline is fit for purpose now and/or into the future. Integrity managers attempting to reduce overall operating risk by making decisions based on inaccurate or poor quality reporting are in fact exposing their networks to greater safety and financial risk. Recognizing these risks and that inline inspection (ILI) is an overall system that needs to be formally qualified, operators and ILI service providers have collaborated to develop several international standards. The most recent is the umbrella API-1163 industry consensus standard, which is now being widely adopted, primarily in USA. This standard provides requirements and recommended practices for qualification of the entire ILI process. Two companion standards: ASNT In-line Personnel Qualification and Certification Standard No. ILI-PQ and NACE Recommended Practice In-Line Inspection of Pipelines RP0102 combine to address specific requirements for personnel who operate and analyze the results of ILI systems. In Europe, the Pipeline Operators Forum (POF) has, also, established specific requirements for ILI reporting processes and data formats. However, these standards do not define how operators and vendors must meet these requirements. To follow will be a story about how an ILI service provider embraced a holistic approach to address these standards’ requirements, in particular in the areas of data analysis, reporting, and dig verification due to their significant importance in assuring the final quality of its deliverables. A key outcome desired will be to provide operators with greater insight into what best practices and technologies ILI service providers should have embraced and invested in to insure reliable service delivery.

The chapter examines the opportunities associated with open access (OA) initiatives from the perspective of research students in selected Nigerian universities. A survey design was adopted to conduct the study, whereby the questionnaire was the instrument of data collection. The findings revealed that OA is perceived by the research students as a good idea, easy, beneficial in terms of access and use and usually makes research more interesting. OA most importantly provides opportunities such as enabling readers with the access to publish articles, the future of scholarly research, and publishes faster than the traditional outlets, along with wider dissemination of research and opportunity of free access to all. Incentives of OA include users' accessibility, copyright retention by authors, high quality publications, among others; while the disincentives of OA include lack of reach, cost of publishing, and inadequate quality of some OA platforms. A considerable number of respondents have the continuous intention to use OA instead of traditional platforms, for study and research, as an autonomous tool for improving research and to recommend to colleagues. Significant challenges associated with OA include procrastination, inadequate ICT infrastructure, high cost of internet, and slow speed of internet, restrictions placed on using storage devices like discs or flash drive for downloading contents from OA platforms, among others. The chapter made recommendations based on the findings.

Abstract Background Erectile dysfunction (ED) has a negative impact on ones’ relationships with poor quality of life as inevitable result. The effects of ED maybe worse in developing countries setting like Tanzania because men’s sexual health has been forgotten. Men’s sexual and reproductive health needs are not in the national reproductive health strategic. This study aimed to determine the prevalence and factors associated with erectile dysfunction among adult men in Moshi municipality, northern Tanzania. Results The mean age of the 381 men was 39.6 (SD ±16.8) years. The overall prevalence of ED on this study was 29.7%. The severity of ED among study participants was; 13.4% (51), 9.7% (37), 3.7% (14), 2.9% (11) had mild, mild to moderate, moderate and severe erectile dysfunction respectively. Age 40–54 years (Adjusted OR 5.0, 95% CI 2.5–9.9), > 55 years (aOR 11.7, 95% CI 5.8–23.7) and hypertension (aOR 2.6, 95% CI 1.1–6.4) were independent predictors of ED respectively. Conclusion The prevalence of ED is high among men in Moshi municipal as 1 out of 3 men have ED. Age and hypertension were independent predictors of ED. These results point to the need of community awareness and education programs to raise awareness among men about existence of ED problem, its consequence and where they can get advice and care in this setting. Further, health providers taking care of hypertensive and men with DM should be equipped with knowledge and skills on early detection for ED and how to counsel and where to refer patients for help.

Abstract High-quality satellite orbits and clocks are necessary for multi-GNSS precise point positioning and timing. In undifferenced GNSS solutions, the quality of orbit and clock products significantly influences the resulting position accuracy; therefore, for precise positioning in real time, the corrections for orbits and clocks are generated and distributed to users. In this research, we assess the quality and the availability of real-time CNES orbits and clocks for GPS, GLONASS, Galileo, and BeiDou-2 separated by satellite blocks and types, as well as the product quality changes over time. We calculate the signal-in-space ranging error (SISRE) as the main orbit and clock quality indicator. Moreover, we employ independent orbit validation based on satellite laser ranging. We found that the most accurate orbits are currently available for GPS. However, Galileo utmost stable atomic clocks compensate for systematic errors in Galileo orbits. As a result, the SISRE for Galileo is lower than that for GPS, equaling 1.6 and 2.3 cm for Galileo and GPS, respectively. The GLONASS satellites, despite the high quality of their orbits, are characterized by poor quality of clocks, and together with BeiDou-2 in medium and geosynchronous inclined orbits, are characterized by SISRE of 4–6 cm. BeiDou-2 in geostationary orbits is characterized by large orbital errors and the lowest availability of real-time orbit and clock corrections due to a large number of satellite maneuvers. The quality of GNSS orbit and clock corrections changes over time and depends on satellite type, block, orbit characteristics, onboard atomic clock, and the sun elevation above the orbital plane.

Nowadays, machine engraving of photos into solid materials such as marble or granite is becoming very popular. Relatively cheap CNC machines are available. The problem is that high quality photos are essential to obtain good results. The first part of the paper describes a model of a CNC machine used for engraving and puts down the principles of image processing applied to poor quality photos in order to get the best results, as well as the fundamental image processing methods necessary for achieving satisfactory results when using an electromagnetic diamond percussion tool for engraving. The second part of the paper describes a very simple method of data coding and the algorithm of engraving tool movement for image engraving process by means of a control system based on ATmega16 microcontroller. The quality of the engraved images is comparable, or even better, than that of manually engraved images or images engraved by other competitive CNC machines.

Abstract Background Direct observation is necessary for specific and actionable feedback, however clinicians often struggle to integrate observation into their practice. Remotely audio-monitoring trainees for periods of time may improve the quality of written feedback given to them, and may be a minimally disruptive task for a consultant to perform in a busy clinic.Methods Selected faculty used a wireless audio receiver during the second half of students' oncology rotations to listen to encounters during clinic in real time. They then gave written feedback as per usual practice, as did faculty who did not use the listening-in intervention. Feedback was de-identified and rated, using a rubric, as strong/medium/weak according to consensus of 2/3 rating investigators.Results Monitoring faculty indicated that audio monitoring made the feedback process easier and increased confidence in 95% of encounters. Most students (19/21 respondents) felt monitoring contributed positively to their learning, and included more useful comments. 101 written evaluations were completed by 7 monitoring and 19 non-monitoring faculty. 22/23 (96%) of feedback after monitoring was rated as high quality, compared to 16/37 (43%) (p<0.001) for monitoring faculty before using the equipment (and 20/78 (26%) without monitoring for all consultants (p<0.001)).Conclusions Using live audio monitoring improved the quality of written feedback given to trainees, as judged by the trainees themselves and also using an exploratory grading rubric. The method was well received by both faculty and trainees. Although there are limitations compared to in-the-room observation (body language), the benefits of easy integration into clinical practice and a more natural patient encounter without the observer physically present lead the authors to now use this method routinely while teaching oncology students.

Background: Diarrhoea is a major public health problem in children worldwide. It continues to be a major health challenge, especially in developing countries, despite the availability of regularly updated standard treatment guidelines. Non-compliance to such guidelines by the physicians has been a long standing story. The treatment is often marred with incapacitating prescription of drugs besides neglecting even the basic tenets of good prescribing. As a result, the quality of such prescriptions for diarrhoeal disorders in children remains poor. To gauge the magnitude of this problem in this setup towards possible corrective measures, the study was aimed to audit prescription practices in the management of acute and persistent diarrhoea in hospitalised children up to five years of age.Methods: An observational study was conducted in 100 patients of either gender in the age group up to 5 years admitted with acute and persistent diarrhoea. A detailed medical history from the parents/guardians and the details of prescription from the time of admission till the discharge of the patient were obtained. Quality of prescriptions was analysed using prescription quality index (PQI) tool, a validated comprehensive tool described by Hassan et al in 2010. Based on this tool, prescription with the total PQI score of ≤ 31 were interpreted as poor quality, scores with 32 to 33 as medium quality and scores 34 to 43 as high quality with a possible maximum score of ‘43’.Results: Based on the PQI tool for 100 children, 60 prescriptions were found to be of poor quality. Only 2 prescriptions were of medium quality, whereas 38 prescriptions were in high quality range. Average mean±SD score of prescriptions with poor quality was 25.2±1.48, ranging from 21 to 31. The mean±SD of prescriptions with medium quality was observed to be 32±0 and for prescriptions of high quality was 38.07±2.28. The total average mean score of all prescriptions was 30.23±6.50. Poor quality prescriptions were particularly observed for the patients with the diarrhoea with No dehydration.Conclusions: Prescription appropriateness in spite of available guidelines continues to be a big challenge in the adequate management of patients with diarrhoeal disorders under the age group of five years in a tertiary care centre in India.
