Phlegm is one of the most common patterns of coronary artery disease (CAD) in Chinese medicine. Our research was aimed at investigating the association between phlegm syndrome of CAD and coronary angiography (CAG) by meta-analysis. According to inclusion criteria, a total of 30 studies involving 5,055 CAD patients were included. The meta-analysis showed that phlegm syndrome patients were prone to multivessel disease (28 studies, OR = 1.53, 95% CI, 1.24 to 1.88,P<0.01) and higher Gensini score (2 studies, OR = 5.90, 95% CI, 1.86 to 9.94,P=0.004), but not obviously relevant to severe stenosis (≥75%) of coronary arteries (13 studies, OR = 1.20, 95% CI, 0.63 to 2.27,P=0.57). We concluded that the coronary arteries lesions of CAD patients with phlegm syndrome were more severe than those with nonphlegm syndromes. Phlegm syndrome should, therefore, be regarded as a dangerous pattern of CAD with worse prognosis.

Abstract Objectives To investigate the association of pericoronary adipose tissue mean attenuation (PCATMA) with coronary artery disease (CAD) characteristics on coronary computed tomography angiography (CCTA). Methods We retrospectively investigated 165 symptomatic patients who underwent third-generation dual-source CCTA at 70kVp: 93 with and 72 without CAD (204 arteries with plaque, 291 without plaque). CCTA was evaluated for presence and characteristics of CAD per artery. PCATMA was measured proximally and across the most severe stenosis. Patient-level, proximal PCATMA was defined as the mean of the proximal PCATMA of the three main coronary arteries. Analyses were performed on patient and vessel level. Results Mean proximal PCATMA was −96.2 ± 7.1 HU and −95.6 ± 7.8HU for patients with and without CAD (p = 0.644). In arteries with plaque, proximal and lesion-specific PCATMA was similar (−96.1 ± 9.6 HU, −95.9 ± 11.2 HU, p = 0.608). Lesion-specific PCATMA of arteries with plaque (−94.7 HU) differed from proximal PCATMA of arteries without plaque (−97.2 HU, p = 0.015). Minimal stenosis showed higher lesion-specific PCATMA (−94.0 HU) than severe stenosis (−98.5 HU, p = 0.030). Lesion-specific PCATMA of non-calcified, mixed, and calcified plaque was −96.5 HU, −94.6 HU, and −89.9 HU (p = 0.004). Vessel-based total plaque, lipid-rich necrotic core, and calcified plaque burden showed a very weak to moderate correlation with proximal PCATMA. Conclusions Lesion-specific PCATMA was higher in arteries with plaque than proximal PCATMA in arteries without plaque. Lesion-specific PCATMA was higher in non-calcified and mixed plaques compared to calcified plaques, and in minimal stenosis compared to severe; proximal PCATMA did not show these relationships. This suggests that lesion-specific PCATMA is related to plaque development and vulnerability. Key Points • In symptomatic patients undergoing CCTA at 70 kVp, PCATMAwas higher in coronary arteries with plaque than those without plaque. • PCATMAwas higher for non-calcified and mixed plaques compared to calcified plaques, and for minimal stenosis compared to severe stenosis. • In contrast to PCATMAmeasurement of the proximal vessels, lesion-specific PCATMAshowed clear relationships with plaque presence and stenosis degree.

Background: Epicardial fat increase has not yet a clear correlation with coronary artery disease (CAD). Aim: This study had as goal to demonstrate a relationship between an increase of epicardial fat thickness (EFT) and CAD. Materials & methods: In this observational study, we included 234 patients who underwent invasive coronary angiography. Before invasive coronary angiography, all patients underwent echocardiographic-2D for evaluation of EFT and they were divided into groups based on Gensini score and also on Syntax score. Results: EFT was significantly correlated to the presence and severity of CAD assessed by Gensini score with a cut-off value of 5.2 mm (sensitivity of 90.9%-specificity of 87.3%- area under the ROC curve = 92.1%). Conclusion: EFT increase (fat index ≥5.2 mm) evaluated by echocardiographic-2D could be considered as a risk factor for predicting CAD.

Drug-eluting stents (DESs) have a low prevalence of in-stent restenosis. However, we describe a patient with coronary artery disease with rapid progress, which might have been triggered by implantation of a DES. The patient was a 72-year-old woman who was first admitted to hospital with non-ST-segment elevated myocardial infarction and had a DES implanted after coronary angiography showed severe stenosis of the left circumflex artery. However, although she kept taking dual antiplatelet therapy, her condition deteriorated and she was admitted to hospital three more times. Angiography showed that the coronary stenosis had become more severe and was more severe not just in the stent-implanted segments, but also in other coronary arteries. Another DES and drug-eluted balloon were used. However, the stent-implanted and balloon-dilated segments became severely stenosed within 1 month. Tests for auto-immune diseases and allergies were negative. We speculate that the first DES triggered an unknown response of the coronary arteries and led to severe stenosis from the stent-implanted segment to the distal segment and other arteries.

Abstract Background Dyslipidemia is one of the most important factors for CAD. Atherogenic index of plasma (AIP) is a new indicator involved in dyslipidemia. However, its relation with CAD severity in diabetic patients is not well established yet. Aim of the Study To study the relationship between atherogenic index of plasma and severity of coronary artery disease (CAD) in the diabetic patients. Methods Our study was conducted on 150 diabetic patient with stable CAD undergoing elective coronary angiography in Al-Agoza Hospital from april 2016 to august 2018. Atherogenic index was calculated from the logarithmically transformed ratio of concentrations of TGs to HDL-C obtained from the patient’s lipid profile. correlation of the AIP and the severity of CAD according to Gensini scoring system was done. Results AIP had a statistically significant positive relation with IDDM (p = 0.033), NIDDM (p = 0.033), HTN (p = 0.024), dyslipedemia (p = 0.042). And a highly significant positive correlation with FBS (p &lt; 0.001, r = 0.479) and HbA1c (p &lt; 0.001, r = 0.538), proximal LAD lesions (p = 0.002), LCX (p &lt; 0.001), and the number of vessls affected (p = 0.005). AIP Also had a highly significant positive correlation with severity of CAD in terms of Gensini score (p &lt; 0.001, r = 0.692). Conclusion AIP is considered a good predictor of CAD severity and multivessel affection in diabetic patients.

Myocardial Infarction with Non-obstructive Coronary Arteries-MINOCA is a clinical syndrome that encompasses a subgroup of heterogeneous patients who present with myocardial infarction yet do not have any significant coronary artery obstruction on angiogram. From several studies it is understood that MINOCA has a 8.8% prevalence of all Myocardial Infarction (MI) presentations, with no characteristic distinguishing clinical features when compared with MI-CAD( Coronary artery disease), except for patients being younger with a female preponderance & less likely to have hyperlipidemia. The prognosis is extremely variable, depending on the causes of MINOCA. Clinical history, echocardiography, coronary angiography, and left ventriculography represent the first-level diagnostic investigations. Ibrahim Card Med J 2015; 5 (1&2): 61-66

Abstract: Acute Coronary Syndrome (ACS) is a set of manifestations or symptoms of coronary artery disease and thrombosis which can threaten the patien’s life due to interfering the blood supply of heart muscle. The infarct size is determined by assessing the coronary arteries lesion, stenosis. This study was aimed to determine the relationship between levels of leukocytes and the severity of coronary arteries lesions in patients with ACS. This was a descriptive analytical study with a cross-sectional retrospective design. The modified Gensini score was used to assess the severity of coronary artery lesion. The first leukocyte count examined when the patients was admitted to the hospital was obtained from the medical records period July to September 2015. Data were analyzed by using SPSS 20.0. The Pearson correlation test showed no significant relationship between the levels of leukocytes and the severity of coronary arteries lesions in patients with ACS (p >0.05). Conclusion: There was no significant correlation between the levels of leukocytes and the severity of coronary arteries lesions in patients with ACS.Keywords: leukocyte, severity of lesion, acute coronary syndrome, modified Gensini score Abstrak: Sindrom koroner akut (SKA) merupakan sekumpulan manifestasi atau gejala akibat penyakit arteri koroner dan trombosis yang dapat mengancam kehidupan pasien karena mengganggu pasokan darah ke otot jantung. Penilaian luas infark dilakukan dengan menilai lesi pembuluh darah koroner yang mengalami stenosis. Penelitian ini bertujuan untuk mengetahui hubungan antara kadar leukosit dengan severitas lesi pembuluh darah koroner pada pasien SKA. Jenis penelitian ialah deskriptif analitik retrospektif dengan desain potong lintang. Penilaian severitas pembuluh darah yang mengalami lesi menggunakan skor modifikasi Gensini. Hitung leukosit yang digunakan ialah saat pasien pertama kali masuk Rumah Sakit yang diperoleh dari rekam medis periode Juli-September 2015. Data dianalisis menggunakan SPSS 20.0. Hasil uji korelasi Pearson memperlihatkan tidak terdapat hubungan bermakna antara kadar leukosit dan severitas lesi pembuluh darah koroner pada pasien SKA (p >0,05). Simpulan: Tidak terdapat hubungan bermakna antara kadar leukosit dan severitas lesi pembuluh darah koroner pada pasien Sindrom Koroner Akut.
