Background. Our purpose was to investigate the association between the surgical approach for Todani type IV cysts and subsequent malignancy rate.Methods. The records of patients who received cyst excision from 1994 to 2013 were analyzed retrospectively for the following data: demographics, presenting symptoms, postoperative outcomes, malignant transformation, and follow-up reexaminations, including imaging, laboratory, and tumor marker tests.Results. Seven of the 196 patients initially treated at our hospital developed postoperative biliary malignancy, and the surgical approaches were extrahepatic bile duct cyst resection combined with hilar cholangioplasty and Roux-en-Y cystojejunostomy (n=5), and intra- and extrahepatic bile duct cyst resection and Roux-en-Y hepaticojejunostomy (n=2). The overall malignancy rate was 3.6% (7/196). Forty-eight patients initially treated at other hospitals developed malignancy postoperatively: 15 (31.2%) remained untreated and 33 (68.8%) had undergone incomplete resection procedures. Because Todani type IV cysts were seen in 268 patients, the postoperative malignancy rate of this group of patients was 12.3% (33/268).Conclusions. Radical resection of both intra- and extrahepatic cysts combined with hepatic resection and Roux-en-Y hepaticojejunostomy is associated with a reduced risk of subsequent cancer development. Procedures in which radical cyst excision is not performed are associated with a greater risk of subsequent malignancy.

Spontaneous rupture of the common bile duct cyst is a rare cause of acute abdomen in children. In our case, we present a 3-year-old patient with no history of trauma, whose diagnosis was confirmed by laparotomy, with a possible suspected common bile duct cyst due to massive ascites and a cystic lesion in the portal area. Although common bile duct cysts are mostly asymptomatic, various studies have shown that malignant transformation can cause important complications such as pancreatitis.Numerous causes have been suggested in the etiopathogenesis of rupture, such as parietal necrosis associated with pancreatic reflux irritation.Since rupture of cysts may require urgent laparotomy such as biliary peritonitis, it is important to know imaging findings that may be warning for early diagnosis. Because of the increased risk of cholangiocarcinoma after cyst excision, cholecystectomy and Roux-en-Y hepaticojejunostomy are the most common treatment procedures.

Objectives: To study the clinical and subclinical characteristics, and the treatment results of the laparoscopic choledochal cyst excision. Subjects and Methods: patients with bile duct cyst were diagnosed and treated by laparoscopic excision at the Hue Central Hospital from 6/2013 to 9/2017. Result: The common bile duct cyst was type I with highest ratio (90.77%), in which type Ic occupied the rate was 53.84%. The most common clinical manifestation was abdominal pain which occupied 87.69% of cases. The average time of opreation was 197.58 ± 46.57. Drain pipe was removed within 1-4 days after surgery. The average time of postoperative treatment was 8.7 ± 3.8 days. The rate of early complications after surgery was 3.07%. The rate of postoperative complications was 1.54%. Conclusion: Laparoscopic treatment of choledochal cyst is safe, feasible and efficient in medical high-tech center Key words: laparoscopic, bile duct cyst
