Objectives.The aim is to evaluate the survival and success rates, as well as the marginal bone loss (MBL) and periodontal indexes of zirconia implants positioned in the esthetic jaw areas.Materials and Method.13 patients were selected and 20 one-piece zirconia implants were used for the rehabilitation of single tooth or partially edentulous ridge in the esthetic jaw areas. Six months after surgery and then once a year, a clinical-radiographic evaluation was performed in order to estimate peri-implant tissue health and marginal bone loss.Results.The survival and success rates were 100%. The average marginal bone loss from baseline to 48 months after surgery was +2.1 mm. Four years after surgery, the median and the mode for visible Plaque Index and Bleeding On Probing resulted 1 whereas Probing Pocket Depth amounted to 3 mm (SD = ±0.49 mm).Conclusion.One-piece zirconia dental implants are characterized by high biocompatibility, low plaque adhesion, and absence of microgap that can be related to the clinical success of these implants even in the esthetic areas.

Objectives. The aim was to evaluate survival and success rates, soft tissue health, and radiographic marginal bone loss (MBL) of zirconia implants placed in the esthetic and posterior areas of the jaws and in association with multiple or single implant restorations after at least 6 months of definitive restoration.Material and Methods. 35 one-piece zirconium implants were utilized for single or partially edentulous ridges rehabilitation. All implants received immediate temporary restorations and six months after surgery were definitively restored. Every 6 months after implant placement, a clinical-radiographic evaluation was performed. For each radiograph, the measurements of MBL were calculated.Results. The results showed that the mean MBL at 48-month followup was 1.631 mm. The mean MBL during the first year of loading was not more significant for implants placed in the first molar regions than for those positioned in other areas. Moreover, no differences in marginal bone level changes were revealed for multiple and single implants, whereas MBL in the first year was observed to be slightly greater for implants placed in the maxilla than for those placed in the mandible.Conclusion. Zirconia showed a good marginal bone preservation that could be correlated with one-piece morphology and characteristics of zirconia implants.

ABSTRACT Background The use of endosseous dental implants has led to more sophisticated fixed options when considering treatment of patients with distal extensions. The use of narrow-diameter implants may reduce the necessity for bone augmentation. The mechanical strength of titanium is limited, so titanium alloys with greater tensile and fatigue strength may be preferable. Purpose The purpose of this study was to evaluate clinically and radiographically the performance of narrow-diameter bone level implants made from titanium-zirconium alloy (TiZr, RoxolidTM) in restoring unilateral atrophic mandibular distal extensions with fixed dental prostheses. Materials and methods Twenty partially edentulous patients with unilateral atrophic mandibular distal extensions received two 3.3 mm diameter bone level TiZr implants (Straumann AG, Basel, Switzerland). The two implants were restored with 3-units ceramo-metal fixed partial dentures. Standardized clinical and radiographic parameters (survival rate, probing pocket depth and marginal bone loss) were evaluated at the time of the completion of the prosthetic treatment (baseline) and after 3, 6 and 12 months of functional loading. Prosthetic complications were also assessed. Results The survival rate for narrow-diameter bone level TiZr implants was 100% after 1 year of functional loading. There were no statistically significant differences between the values of probing pocket depth over the follow-up period. All implants showed less than 1 mm of marginal bone loss at the end of the follow-up period. Conclusion Within the limitations of this 1-year pilot study, the use of narrow-diameter bone level TiZr implants appears to be predictable in restoring the unilateral atrophic mandibular distal extensions. Abstract In the last decade, some investigations have reported that the resorbable blast media surface (also named CaPO4 blasted implants [CaPO4-BIs]) has achieved excellent results. However, no report regarding CaPO4-BIs inserted into fresh frozen bone (FFB) is available. Thus, we planned a retrospective study on a series of CaPO4-BIs inserted into FFB to evaluate their clinical outcome. In the period between December 2003 and December 2006, 16 patients (10 females and 6 males, median age of 55 years) were operated on, and 76 CaPO4-BIs were inserted. The mean implant follow-up was 23 months. Implant diameter and length ranged from 3.25 to 4.5 mm and from 11.5 to 15 mm, respectively. Implants were inserted to replace 7 incisors, 11 cuspids, 31 premolars, and 27 molars. Only 1 out of 76 implants was lost (ie, survival rate [SVR] = 98.7%), and no differences were detected among the studied variables. When peri-implant crestal bone resorption was used as an indicator of clinical success (ie, success rate), it was possible to identify some variables that correlated with a better clinical outcome. Specifically, Cox regression showed that removable prosthetic restoration and longer implant length correlated with a statistically significant lower delta implant abutment junction (IAJ; ie, reduced crestal bone loss) and thus a better clinical outcome. In this study, CaPO4-BIs had high survival and success rates, similar to those reported in previous reports of 2-stage procedures in nongrafted bone. CaPO4-BIs inserted into FFB are reliable devices, although greater marginal bone loss occurs when fixed prosthetic restorations and short implants are used.

Single tooth implant restorations in the aesthetic area are a demanding challenge. If a complete osseointegration is mandatory, the final result has to result in a higher standard of biomimetic and soft tissue health among natural teeth. This outcome is traditionally pursued by cementing crowns over individualized abutments. However, in recent years, the need for controlling peri-implant health and the preference towards a retrievable solution has led to an increase in screw-retained crowns, which is not always applicable when the implant axis is not ideal. In the aesthetic area, the use of a novel technical solution represented by the angled screw channel (ASC) of the abutment has been proposed in order to match the advantages of the screwed solution with the aesthetic demands. The aim of this study was to compare ASC crowns to cemented crowns (CC) in single implant restorations using the white esthetic score (WES) and pink esthetic score (PES) at the crown delivery and at a follow-up of a minimum of 2 years. Peri-implant health and marginal bone loss (MBL) were also evaluated. The mean follow-up was 44.3 months, with a mean MBL of 0.22 mm in the ASC group and 0.29 mm in the CC group. The total WES/PES score was 16.6 for ASC, compared with 17.3 for CC at baseline, and 16.2 and 17.1, respectively, at follow-up. Both of the groups reached a high WES/PES, and this was maintained over time, without signs of peri-implant diseases or bone loss, regardless of the choice of connection. In conclusion, ASC can be adopted in cases where the implant axis is not ideal, with aesthetic and functional results that are comparable to implants restored by cemented crowns.

The aim of this prospective cohort observational field trial was to examine 1-year survival and success rates of a recently introduced tapered implant system with switched platform conical abutments and to evaluate patient related outcomes of therapy. Partially edentulous patients aged between 18 and 75 years, with available bone height for dental implants ≥10 mm desiring to restore the missing tooth/teeth with implant supported restoration, were recruited by 7 periodontists in their respective private practices. Dental implants were installed according to standard implant therapy protocol. Three to 6 months postoperatively, after evaluating interim implant success, implants were restored by the referring dentists. Patient, Ramfjord teeth, and implant data, including baseline and 1-year postoperative, were collected. A total of 60 patients were recruited and received 117 implants. Complete 1-year clinical and radiographic data were available for 83 and 65 implants, respectively. Two implants failed during the first year, resulting in a 1-year survival rate of 98.3%. Mean implant probing pocket depth was 2.29 ± 0.84 mm. Mean radiographic bone distance from implant's shoulder at the mesial and distal sites at 1 year was 0.66 ± 0.5 and 0.79 ± 0.64mm, respectively, resulting in a success rate of 95.4%. Patient subjective evaluation of therapy exhibited a median pain experience of 1 and median esthetics, function, and general satisfaction evaluation of 10 on a scale of 1 to 10. The tapered conical connection dental implant system, used in private dental practices, shows good 1-year survival and success rates that are similar to other implant systems on the market.

AIM: To evaluate and compare the marginal bone loss and aesthetic outcomes of zirconia implants with titanium implants in randomized controlled trials (RCTs). MATERIAL AND METHODS: Electronic [PubMed] and hand searches were performed to identify randomized controlled trials that were published between January 2008 to April 2020 which investigated and compared various outcomes between zirconia and titanium dental implants. Outcomes included assessment of marginal bone loss and aesthetics using spectrophotometric measurements. Meta-analysis was performed to estimate the above parameters among various studies. RESULTS: A total of 58 articles were screened for titles and abstracts. Subsequently 8 articles were selected for data extraction and evaluation. Zirconia implants were investigated and compared to titanium implants for marginal bone loss [MBL]. Customized zirconia and titanium abutments seated over implants were analyzed for aesthetic outcomes using spectrophotometric method using CIE-Lab measurements. Meta-analysis estimated that zirconia implants exhibited marginal bone loss reduction of 0.179mm (95% CI, 0.02 to 0.33) and -0.242mm (95% CI, -4.026 to 3.542) in aesthetic measurements than titanium implants. CONCLUSIONS: No heterogeneity was observed among studies analyzed for marginal bone loss and significant differences were noticed between two groups. Noticeable heterogeneity was observed among studies assessing aesthetics using spectrophotometry and CIE-Lab measurements and results revealed no many significant differences between the two groups.

Abstract Introduction Marginal bone level is the criterion for implant success. Patient expectations for more natural looking implant restorations created the need to restore implants with more esthetically pleasing materials like Zirconia rather than conventional porcelain-fused to-metal (PFM) crowns. The aim of this study was to evaluate marginal bone loss around dental implants clinically and radiographically when restored with Zirconia and PFM prosthesis. Materials and Methods Two groups (control and test) were formed with 14 patients each. In the control group, the subjects were rehabilitated with PFM crowns and in the test group, the subjects were rehabilitated with Zirconia crowns. Rehabilitation was done after the healing period of 3 months. Radiographic evaluation was done at regular (baseline, 3rd, 6th, and 12th month) intervals. Results The results were statistically analyzed. Keeping in mind the limitations of the study, it was revealed that the difference in the crestal bone resorption in both the groups was not significant.

Periodontitis is a chronic inflammatory condition that may contribute to diabetogenesis. The aim was to investigate the levels of glycated proteins and their correlation with periodontal and systemic inflammation. Fifty-one patients with periodontitis and 20 healthy subjects underwent probing pocket depth (PPD) measurements. PPD total and PPD disease with and without tooth adjustment were used as continuous indices. Marginal bone loss (MBL) for mandibular premolars and molars was measured digitally. Body mass index (BMI) and waist circumference (WC) were also analyzed. Glycated hemoglobin (HbA1c) and fructosamine (FrAm) levels were measured in all subjects. A multiplex proximity extension assay (PEA) was used to analyze the serum samples for simultaneous measurement of 92 proteins. Both HbA1c and FrAm inversely correlated with IL-10, FGF-21, MCP-1, and TNF beta amongst 16 proteins. HbA1c correlated directly with OPG. Parameters of disease severity were consistently significant for HbA1c. Adjusted PPD total and number of missing teeth were increased in diabetes whereas levels of RANKL and RANKL to OPG ratio were the highest in nondiabetic periodontitis patients. Hyperglycemic conditions in periodontitis patients are associated with reduced levels of anti-inflammatory proteins as well as dysregulated bone resorption.
