Background. Critical illness is associated with muscle weakness leading to long-term functional limitations.Objectives. To assess the reliability of a novel method for evaluating fatigability of the quadriceps muscle in noncooperating healthy subjects.Methods. On two occasions, separated by seven days, nonvoluntary isometric contractions (twitch and tetanic) of the quadriceps femoris muscle evoked by transcutaneous electrical muscle stimulation were recorded in twelve healthy adults. For tetanic contractions, the Fatigue Index (ratio of peak torque values) and the slope of the regression line of peak torque values were primary outcome measures. For twitch contractions, maximum peak torque and rise time were calculated. Relative (intraclass correlation, ICC3.1) and absolute (standard error of measurement, SEM) reliability were assessed and minimum detectable change was calculated using a 95% confidence interval (MDC95%).Results. The Fatigue Index (ICC3.1, 0.84; MDC95%, 0.12) and the slope of the regression line (ICC3.1, 0.99; MDC95%, 0.03) showed substantial relative and absolute reliability during the first 15 and 30 contractions, respectively.Conclusion. This method for assessing fatigability of the quadriceps muscle produces reliable results in healthy subjects and may provide valuable data on quantitative changes in muscle working capacity and treatment effects in patients who are incapable of producing voluntary muscle contractions.

Ten volunteers (19-23 years old) performed 9 sets of 12 bilateral knee-extension exercises at 60% 1RM. Following exercise, 4 ultrasound treatments (5-cm transducer head, 1.0-MHz frequency, pulsed mode at 1.0 W/cm2) were applied for 8 min daily to the quadriceps muscle of a randomly selected treatment leg. The placebo leg received similar treatment with the ultrasound apparatus turned off. Knee-extension peak torque values and delayed onset muscle soreness (DOMS) were assessed on each leg prior to exercise and at 20 min and 24, 48, 72, and 96 hr postexercise. Postexercise peak torques declined to 60-70% of preexercise values and returned to normal by 96 hr. DOMS sensation peaked 24 hr postexercise and diminished thereafter. No significant differences in peak torque or DOMS were noted between ultrasound- or placebo-treated legs at any time postexercise. Hence ultrasound, as applied in this study, does not appear to be effective in enhancing postexercise muscle strength recovery or in diminishing DOMS.

Although it is generally accepted that human performance must be assessed in a manner specific to the training, previous studies have violated this principle. In order to determine the validity of evaluating short-term resistance training programs with isometric and isokinetic measures, 23 recreationally active males participated in an 8-week training program. Subjects were randomly divided into barbell squat, hip sled, leg extension, and control groups. Pre- and posttesting of quadriceps strength was performed with a Cybex isokinetic dynamometer. Six angle-specific torques (N.m) were determined at 0 rad-s-1 and 1.05 rad-s-1. Ten RM training loads increased significantly for all groups that trained. Isometric torque values differed significantly from isokinetic torque values at 30, 60, 75, and 90° of leg flexion for all groups. No significant torque increases from pre- to posttest were observed for any group at any limb angle for either isometric or isokinetic testing, or for isokinetic peak torque. This indicates that strength increases during short-term dynamic external resistance exercise are not adequately assessed with either isometric or isokinetic evaluations.

Background Commonly used spasticity scales assess the resistance felt by the evaluator during passive stretching. These scales, however, have questionable validity and reliability. The tonic stretch reflex threshold (TSRT), or the angle at which motoneuronal recruitment begins in the resting state, is a promising alternative for spasticity measurement. Previous studies showed that spasticity and voluntary motor deficits after stroke may be characterized by a limitation in the ability of the central nervous system to regulate the range of the TSRT. Objective The study objective was to assess interevaluator reliability for TSRT plantar-flexor spasticity measurement. Design This was an interevaluator reliability study. Methods In 28 people after stroke, plantar-flexor spasticity was evaluated twice on the same day. Plantar-flexor muscles were stretched 20 times at different velocities assigned by a portable device. Plantar-flexor electromyographic signals and ankle angles were used to determine dynamic velocity-dependent thresholds. The TSRT was computed by extrapolating a regression line through dynamic velocity-dependent thresholds to the angular axis. Results Mean TSRTs in evaluations 1 and 2 were 66.0 degrees (SD=13.1°) and 65.8 degrees (SD=14.1°), respectively, with no significant difference between them. The intraclass correlation coefficient (2,1) was .851 (95% confidence interval=.703, .928). Limitations The notion of dynamic stretch reflex threshold does not exclude the possibility that spasticity is dependent on acceleration, as well as on velocity; future work will study both possibilities. Conclusions Tonic stretch reflex threshold interevaluator reliability for evaluating stroke-related plantar-flexor spasticity was very good. The TSRT is a reliable measure of spasticity. More information may be gained by combining the TSRT measurement with a measure of velocity-dependent resistance.

Objectives. The effects of Kinesio Taping (KT) on muscular performance remain largely unclear. This study aimed to investigate the acute effects of KT on the maximum concentric and eccentric quadriceps isokinetic strength.Study Design. This is a single-blinded, placebo crossover, repeated measures study.Methods. Maximum isokinetic concentric/eccentric extension torque, work, and power were assessed by an isokinetic dynamometer without taping (NT) and with KT or placebo taping (PT) in 17 healthy young men. Repeated measures one-way analysis of variance (ANOVA) was used for statistical analyses.Results. Testing concentric contractions at 60°/s or 180°/s isokinetic speed, no significant differences in peak torque (Nm), total work (J), or mean power (W) were noted among the application modes under different conditions. Testing eccentric contractions at 30°/s or 60°/s isokinetic speed, no significant differences in mentioned parameters were noted, respectively. KT on the quadriceps neither decreased nor increased muscle strength in the participants.Conclusion. KT application onto the skin overlying the quadriceps muscle does not enhance the strength or power of knee extensors in healthy men.

The strength of the shoulder internal and external rotators incricket bowlers, may not be sufficient to cope with the demands of bowling.As very little research has been done on cricketers, this study was done to establish the isokinetic strength profile of the shoulder internal andexternal rotators in cricket bowlers.Isokinetic, shoulder rotational strength was evaluated in thirty malecricket volunteers with a mean age of 23.9 years and mean body weight of 70.3 kgs. The Cybex 340 dynamometer multi joint system was used to collect data on shoulder rotation strength in a standing neutral position. Data were collected at four different speeds (60,90,180 and 300deg/sec) and were computed for peak torque values for internal and external ratios for both dominant and non dominant shoulders.The results showed no statistically significant difference in the mean shoulder rotational torque between the bowlingand non-bowling shoulders for external rotation (p>0.05), and indicated statistically significant differences in themean shoulder rotational torque between the bowling and non-bowling shoulders for internal rotation (p<0.05). Therewas a significant decrease in isokinetic peak torque production for the external/internal rotator muscles as the speedof contraction increased (p<0.05). The peak torque ratio for the external/internal rotator muscles of the bowling armwere significantly less than of the non-bowling arm (p<0.05). These findings suggest that the strength ratios of thebowling arm need to be considered when managing young cricketers and their injuries.

ABSTRACT Introduction: Isokinetics is a tool commonly used in professional soccer. There is ongoing debate among researchers as to the isokinetic reference values a player should have. Objectives: To determine the absolute peak torque (PT) and average work of professional soccer players in relation to their positions on the field, and to establish the reference values for these variables. Methods: Purposeful sampling was used to select 289 professional soccer players. The sample included 32 goalkeepers, 100 defenders, 98 midfielders, and 59 strikers. The participants were measured preseason. The players were asked to perform a 10-minute warm-up on an exercise bike, and then to perform 5 repetitions at low speed, 10 at medium speed, and 25 at high speed, with 30 to 40-s of rest between each set of repetitions. The contraction method was concentric-concentric in a dynamometer Isomed 2000. Results: The average age, weight, and height of the players was 21.9 years, 74.3 kilograms, and 1.8 meters, respectively. The goalkeepers presented higher PT at the 3 measured speeds, and the higher average work at 180°/s and 240°/s in relation to defenders and midfielders. The strikers presented higher average work at 240°/s in relation to midfielders, and higher PT in relation to the defenders and midfielders. Absolute values were shown and reference values were established. Conclusions: The goalkeepers and strikers were the players that showed the greatest differences in their favor in relation to the other positions. The peak torque values and average work were described in relation to the player's position on the field. This study resulted in the creation of a tool for health professionals working with professional soccer players, providing reference values for these players in relation to their position on the field that can be used as benchmarks, by health professionals, to optimize soccer players’ performance. ObjectivesTo study muscle biopsy tissue from patients with juvenile dermatomyositis (JDM) in order to test the reliability of a score tool designed to quantify the severity of histological abnormalities when applied to biceps humeri in addition to quadriceps femoris. Additionally, to evaluate whether elements of the tool correlate with clinical measures of disease severity.Methods55 patients with JDM with muscle biopsy tissue and clinical data available were included. Biopsy samples (33 quadriceps, 22 biceps) were prepared and stained using standardised protocols. A Latin square design was used by the International Juvenile Dermatomyositis Biopsy Consensus Group to score cases using our previously published score tool. Reliability was assessed by intraclass correlation coefficient (ICC) and scorer agreement (α) by assessing variation in scorers’ ratings. Scores from the most reliable tool items correlated with clinical measures of disease activity at the time of biopsy.ResultsInter- and intraobserver agreement was good or high for many tool items, including overall assessment of severity using a Visual Analogue Scale. The tool functioned equally well on biceps and quadriceps samples. A modified tool using the most reliable score items showed good correlation with measures of disease activity.ConclusionsThe JDM biopsy score tool has high inter- and intraobserver agreement and can be used on both biceps and quadriceps muscle tissue. Importantly, the modified tool correlates well with clinical measures of disease activity. We propose that standardised assessment of muscle biopsy tissue should be considered in diagnostic investigation and clinical trials in JDM.

AbstractThis study investigated the acute effects of seated and supine knee extension exercise on muscle swelling, torque, and work output. Twelve resistance-trained men performed two isokinetic concentric-only knee-extension training protocols at different hip positions in a counter-balanced order. They completed the knee extension exercise in the seated (hip angle at 85°) and supine (hip angle at 180°) positions. The torque and work output were assessed during each set. Moreover, muscle thickness of the middle and proximal vastus lateralis and rectus femoris were evaluated before and after each protocol and used as an indicator of muscle swelling. Middle rectus femoris and proximal vastus lateralis thickness increased significantly (p=0.01) with no difference between exercise variations. However, the middle vastus lateralis thickness increased (p=0.01) only after the seated knee extension exercise (~7%). Knee extensors’ peak torque and work output were approximately 8% higher (p=0.04) in the seated when compared to the supine hip position. There was a similar decrease in torque and work output throughout both protocols (p=0.98). In conclusion, seated knee extension exercises produced greater torque, work output, and muscle swelling in the vastus lateralis when compared to the supine knee extension exercise.
