Background. Serum ratio of leptin to adiponectin has been reported to be a useful index of insulin resistance. Since these two adipokines could also be involved in many immunoinflammatory reactions, this ratio might be associated with some inflammatory diseases such as periodontitis. The present study aims to explore the difference in serum ratio of leptin/adiponectin among patients with diabetes and periodontitis, patients with periodontitis only, and healthy people and find out the correlation between this ratio and periodontal parameters. Method. 15 patients with chronic periodontitis and type 2 diabetes, 15 patients with chronic periodontitis only, and 15 healthy controls were selected. Clinical indexes and periodontal parameters were recorded and serum leptin and adiponectin levels were tested by ELISA kits. Result. The ratio of leptin/adiponectin is significantly different among three groups; the T2 DM + CP group has the highest ratio (1.91±0.48) and the healthy group has the lowest ratio (0.65±0.13). What is more, the ratio of leptin/adiponectin has strong positive correlation with periodontal parameters (P<0.001). Conclusions. Chronic periodontitis could influence the level of adipokines in serum and change the ratio of leptin/adiponectin, and the effect would be enhanced combining with type 2 diabetes.

Abstract Background Patients with type 2 diabetes (T2DM) have an increased or normal BMD; however fragility fractures represent one of the most important complications of T2DM. Aims This study aimed to evaluate whether the use of the Radiofrequency Echographic multi spectrometry (REMS) technique may improve the identification of osteoporosis in T2DM patients. Methods In a cohort of 90 consecutive postmenopausal elderly (70.5 ± 7.6 years) women with T2DM and in 90 healthy controls we measured BMD at the lumbar spine (LS-BMD), at femoral neck (FN-BMD) and total hip (TH-BMD) using a dual-energy X-ray absorptiometry device; moreover, REMS scans were also carried out at the same axial sites. Results DXA measurements were all higher in T2DM than in non-T2DM women; instead, all REMS measurements were lower in T2DM than in non T2DM women. Moreover, the percentage of T2DM women classified as “osteoporotic”, on the basis of BMD by REMS was markedly higher with respect to those classified by DXA (47.0% vs 28.0%, respectively). On the contrary, the percentage of T2DM women classified as osteopenic or normal by DXA was higher with respect to that by REMS (48.8% and 23.2% vs 38.6% and 14.5%, respectively). T2DM women with fragility fractures presented lower values of both BMD-LS by DXA and BMD-LS by REMS with respect to those without fractures; however, the difference was significant only for BMD-LS by REMS (p < 0.05). Conclusions Our data suggest that REMS technology may represent a useful approach to enhance the diagnosis of osteoporosis in patients with T2DM.

Objective: To examine potential associations between neutrophil/lymphocyte ratio, platelet/lymphocyte ratio, mean platelet volume (MPV), HbA1c and microvascular complications in diabetic patients from a cost-effectiveness perspective. Methods: One hundred patients with type 2 diabetes attending our outpatient unit between May 2018 and October 2018 were included, and 100 healthy individuals served as the control group. A retrospective file search was performed to collect information on hemoglobin, mean platelet volume (MPV), glycosylated haemoglobin (HbA1c), hematocrit (Hct), neutrophil and lymphocyte count, neutrophil/lymphocyte ratio (NLR), platelets (Plt), platelet/lymphocyte ratio (PLR), and microvascular complications (neuropathy, retinopathy, nephropathy). Results: Demographic and laboratory data were retrospectively controlled between diabetes (n=100) and healthy control (n=100) groups. The mean age in diabetic patients and healthy controls was 56.34 and 36.68 years, respectively. The mean NLR in diabetics and healthy controls was 2.48 and 2.11, the difference in NLR being significant (p=0.002). MPV in diabetics and controls was 8.54 and 8.53, respectively, and the difference was not significant (p=0.93). PLR was also similar, i.e. 149.7 and 145.3 in diabetics and healthy controls (p=0.067). With respect to microvascular complications, retinopathy was found to be significantly associated with MPV and NLR (p=0.015, and p=0.051), and nephropathy showed a significant association with NLR (p=0.027) among diabetics. In contrast with the two other microvascular complications, no significant association between neuropathy and NLR could be detected, while PLR and neuropathy was significantly associated (p=0.003). Conclusion: Microvascular complications may be associated with certain hematologic parameters, as suggested by comparisons both between diabetics and healthy individuals and within the group of diabetic individuals. We believe that hematologic parameters such as hematocrit, MPV, NLR, and PLR, which can be obtained through a simple complete blood count, may be utilized as cost-effective predictors of diabetic microvascular complications. Background: Anaemia is increasingly recognized entity in the patients with diabetes mellitus and constitutes an additional burden in patients. The prevalence of anaemia in the patients with diabetes is two or three times higher than in patients with comparable renal impairment and iron stores in the general population. As India is foreseen a diabetic capital of the world, it becomes imperative to recognize co-morbidities such as anaemia at the earliest. Hence this study is being conducted with the aim to determine the prevalence and various causes of anaemia in diabetics.Methods: After obtaining informed written consent, all diabetics patients were subjected to detailed history, through clinical examination and investigation with CBC, Renal function test including creatinine clearance. The difference of mean between anaemic and non anaemic diabetic patients was evaluated by unpaired student t test. Finally, correlation between the level of haemoglobin and index of renal damage (albumin-creatinine ratio) was accessed by Pearson correlation. Statistical software of SPSS 10 ver. and EXCEL (office 9) was used to analyse the data.Results: In the present study, nearly two third patients of type 2 diabetes mellitus were anaemic. The maximum number of anaemic patients with type 2 diabetes mellitus had microcytic hypochromic type of anaemia.Conclusions: It is therefore concluded that anaemia is a prevalent finding in patients with type 2 diabetes mellitus and represents significant unrecognised burden. The anaemia may be attributed to variable contribution of iron deficiency state and chronic inflammation as result of the disease itself.

Background: Once weekly (OW) semaglutide 0.5-1.0 mg is a glucagon-like type-1 receptor agonist (GLR-1 RA) approved for treatment of type 2 diabetes and is currently under evaluation for treatment of obesity at a higher dose of 2.4 mg OW. Objective: to provide an appraisal of WO semaglutide 2.4 mg for treatment of obesity. Methods: Pubmed research up to March 22. Randomized trials, pertinent animal studies, and reviews are included. Search terms were glucagon-like type 1 receptor agonists, weight loss, obesity, semaglutide, safety, efficacy. Results: WO semaglutide 2.4 mg was evaluated as a weight loss agent in 3 well-designed clinical trials of 68 week-duration. In one trial including patients with type 2 diabetes, the difference in weight loss from baseline to week 68 between OW semaglutide and placebo was - 6.2 percentage points (95% CI, -7.3 to -5.2; P<0.0001). In the other 2 studies that excluded patients with diabetes, the difference in weight loss between OW semaglutide and placebo ranged between -10.3% and -12.4%. A significantly higher proportion of participants in the semaglutide groups vs placebo groups achieved at least 5% of weight loss. The most common adverse effects of semaglutide were related to the gastrointestinal (GI) system. Across these 3 trials, premature discontinuation of OW semaglutide occurred in 6-7% vs 3% in placebo groups. Conclusions: OW semaglutide may be a promising agent for treatment of obesity irrespective of presence of type 2 diabetes. Further studies are needed to establish its long-term safety and efficacy.

Diabetes – a disease in which the effect of the treatment substantially depends on the patient. Known a study showed that the use of glucometers with the technology of three-color display of test results facilitates self-monitoring of blood sugar and leads to a decrease in glycated hemoglobin (HbAlc). Purpose of the study: to modeling the impact of using of a glucometer with a color-coded display on the clinical outcomes of diabetes mellitus and calculating, the potential economic benefits of reducing the hospitalization rate of patients with diabetes. Material and methods. Based on data from two studies (O. Schnell et al. and M. Baxter et al.) simulation of the reduction in the number of complications with the use of a glucometer with a color indication. In a study by O. Schnell et al. a decrease of HbA1c by 0.69 percent is shown when using the considered type of glucometers, which was the basis of the model. Results. In the model, the use of a glucometer with a color-coded display for type 1 diabetes led to a decrease in the total number of complications by 9.2 thousand over 5 years per a cohort of 40 thousand patients with different initial levels of HbA1c. In a cohort of 40 thousand patients with type 2 diabetes, the simulated number of prevented complications was 1.7 thousand over 5 years. When extrapolating these data to all patients with diabetes included in the federal register of diabetes mellitus (FRD), the number of prevented complications was 55.4 thousand cases for type 1 diabetes and 67.1 thousand cases for type 2 diabetes. The possible economic effect from the use of the device by all patients with a diagnosis of diabetes, which are included in the FRD, estimated at 1.5 billion rubles for a cohort of patients with type 1 diabetes and 5.3 billion rubles for patients with type 2 diabetes. Conclusion. Improving the effectiveness of self-monitoring, which is the result of the use of glucometers with color indicators, can potentially significantly reduce the incidence of complications in diabetes and thereby provide significant economic benefits to society.

Introduction: Hyperuricemia is common Type 2 diabetes at very high cardiovascular risk. Objective: Evaluate the relationship between hyperuricemia and diabetes type 2, and determine its predictive factors in this population. Patients and Methods: Retrospective study cross including 190 patients with diabetes type 2 hospitalized Service of Endocrinology of CHU Ibn Rushd Casablanca from January 2015 to December 2017. Hyperuricemia was defined as a serum uric acid concentration> 70 mg/L (men) and> 60 mg/L (women). The variables studied were the anthropometric measurements), cardiovascular factors (tobacco, hypertension, dyslipidemia), and degenerative complications (retinopathy, neuropathy, kidney failure, ischemic heart disease). The analyzes were performed by SPSS software. Results: Hyperuricemia was found in 26.5% of patients with a female predominance (76%), an average age of 55.9 years, and an average age of 12.4ans diabetes. The glycemic control was found in 84.6% of cases with mean glycated hemoglobin 8.6%. Factors associated al hyperuricemia were the blood pressure in 86% (p <0.05), dyslipidemia in 76.3% of cases (p <0.001) with hypertriglyceridemia in 48.3% of cases (p <0.02), and a hypoHDLémie 28% (p <0.001). The age, obesity, smoking, and glycemic control were associated significantly n al hyperuricemia. The research of degenerative complications of hyperuricemia has objectified renal impairment (GFR between 15 and 60ml / min) chez47% (p <0.001), it was kind of moderate in 35.8% (p <0.01) and severe in 5.1% (p <0.02), ischemic heart disease was found in 34% of cases (p <0.01). Conclusion: In our study, hyperuricemia in type 2 diabetes is common in female patients, especially with hypertension, dyslipidemia, and renal failure. Other factors such as age, obesity, smoking is not associated with hyperuricemia in type 2 diabetics.

Background: Self-care is essential for the prevention of complications in patients with diabetes, but several authors report that even with health education programs, the incidence of complications in patients with diabetes continues to increase. Objective: We aimed to examine adherence to self-care strategies and the repercussions of adherence on the clinical profiles of individuals with type 2 diabetes. Methods: We performed a systematic review according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines. We searched for related studies in 4 databases: PubMed, Web of Science, Scopus, and Latin American and Caribbean Health Science Literature (LILACS). We included observational studies in English and Portuguese that evaluated the effects of selfcare in individuals with type 2 diabetes. Results: The search resulted in the identification of 615 articles, of which 34 met all the inclusion criteria. General self-care was considered unsatisfactory. Physical exercise was classified as the self-care activity that was performed less frequently by individuals with type 2 diabetes mellitus; adherence to medication was the most frequent behavior among volunteers. Conclusion: The studies indicated poor adherence of the population to good self-care practices, reflected by the increase in complications related to DM.

Background: Diabetes Mellitus (DM) is a progressive metabolic disorder. Objective: The aim of this study was to investigate the relationship between antioxidant and oxidative stress markers in the saliva of patients with type 2 DM and a healthy control group. Methods: In this study, 20 patients with diabetes and 20 healthy individuals were evaluated. Salivary antioxidants markers consisted of total antioxidant capacity (TAC), uric acid (UA), peroxidase and catalase. Oxidative stress markers included total oxidant status (TOS), malondealdehyde (MDA) and total thiol (SH). Sialochemical analysis was performed with spectrophotometric assay. All the statistical analyses were conducted using STATA software. Results: TAC decreased significantly in patients with diabetes. Although salivary UA and peroxidase were lower in patients with diabetes compared to the control group, the difference was not significant. Salivary catalase in patients with diabetes was significantly lower than that in the control group. MDA and TOS exhibited significantly higher levels in type 2 DM. SH levels were slightly higher in DM. Conclusions: According to the results of the present study, there were some changes in the salivary levels of some antioxidants and oxidative stress markers in patients with type 2 DM and could be measured as an indicator of serum changes..
