Ectopic eruption is a developmental disturbance in which the tooth fails to follow its normal eruption pathway. Ectopic eruption of the second molar is relatively rare. This paper presents the case of thirteen-year-old male with an ectopic mandibular second permanent molar. The condition was corrected with surgical exposure and placement of elastic separators. This case report lays emphasis on the practice of basic methods to obtain acceptable results rather than extensive surgical or orthodontic corrections. It is advised that ectopic teeth should not be neglected especially when it concerns developing caries and malocclusion.

Migration describes the movement of an unerupted tooth within the bone when normal eruption is prevented and the tooth leaves its normal site of development. This report describes a case of distal migration and ectopic eruption of the mandibular first premolar in an 8.5-year-old boy. Following early extraction of the primary second molar, the first premolar migrated distally through the extraction site of the primary second molar and erupted into occlusion just mesial of the permanent first molar.

Background: Information on tooth emergence is a key indicator for demonstrating maturity in the diagnosis of certain growth disturbances and an estimation of the chronological age of the children with unknown birth records in forensic dentistry. The association of dental and skeletal maturity with chronologic age among different populations has been investigated by several researchers. Early eruption of permanent molar appears to be a unique finding at such an early chronological age. The present report aimed to present a case of early eruption of mandibular second permanent molar in a seven-year-old girl. Case Report: A seven-year-old girl was referred to the department of pediatric dentistry of Hamadan University with the chief complaint of an extra palatal tooth. Apart from the supernumerary tooth, mandibular second molars and premolars were fully erupted. Radiographic evaluation revealed a closure of the apex of the maxillary and mandibular incisors along with the first molars. For further investigations, the patient was referred to pediatric endocrinologist in order to rule out any systemic disease; however, patient’s test results did not show any systemic or hormonal problems. This case is one of the rare cases of early eruption of mandibular second molars at seven with no underlying problems. To our knowledge, no case of early eruption of second permanent molar has been reported in a seven-year-old child and early eruption of second molar appears to be a unique finding at such an early chronological age. Conclusions: Any change in sequence or timing of the normal tooth eruption is not common, and it needs prepared eyes and adequate knowledge to diagnose and examine it in a timely manner.

Resumen: La agenesia es una alteración común en el desarrollo del hombre con una prevalencia de 3.5 a 8.8 %; excluyendo a los terceros molares, se presenta generalmente en pacientes femeninos y su etiología es multifactorial. Informe de caso: paciente femenino ASA I de 10 años de edad con ausencia de segundos premolares permanentes en ambos maxilares. Se realizó la rehabilitación integral y de acuerdo a la valoración por parte del área de Ortodoncia se efectuará el desgaste selectivo de los segundos molares temporales para conseguir la migración del primer molar permanente y con ello conservar la integridad del arco dental. Conclusión: Un diagnóstico preciso y oportuno puede ser realizado por medio de la radiografía panorámica; las opciones de tratamiento dependerán de la edad y tipo de oclusión con la finalidad de conservar la integridad de los arcos, brindar un desarrollo oclusal óptimo y una mejor calidad de vida.Palabras clave: Agenesia, premolares, diagnóstico, tratamiento.Abstract: Agenesis is a common alteration in the development of men with a prevalence of 3.5 to 8.8%; excluding third molars, usually occurs in female patients and the etiology is multifactorial. Case report: female patient ASA I of 10 years of age with absence of permanent second premolars in both maxillaries. Integral rehabilitation was performed and according to the assessment by the orthodontic area was carried out the selective grinding of the deciduous second molar to achieve the migration of the first permanent molar for preserve the integrity of the dental arch. Conclusion: A reliable and timely diagnosis can be made by means of panoramic radiography; the treatment options will depend on the age and type of occlusion whit the aim of preserve the integrity of the arches, provide an optimal occlusal development and a better quality of life.Key words:  Agenesis, premolars, diagnosis, treatment.

Ectopic eruption of permanent maxillary first molar can cause root resorption of adjacent primary second molar, thus leading to early loss of primary maxillary second molar. Therefore, it is necessary to correct ectopic maxillary first molar. This case report demonstrates that K-loop can be used as a simple, comfortable, and easier method to correct ectopic eruption of permanent maxillary first molar in existing severe root resorption on adjacent primary second molar.

Ectopic eruption of the permanent molar may absorb the distal root of the primary second molar and may result in a decreased arch length or delayed eruption of the permanent tooth, requiring timely treatment. Therefore, we devised an effective and convenient method to unlock the entrapped tooth using a novel device called a “piston-elastic”. This case report aims to explain the design and clinical application of this piston-elastic and to describe successful cases. Three patients (aged 6, 13, and 16 years) with ectopically erupted maxillary and mandibular molars, respectively, were treated with a piston-elastic. It was bound to the locked molar to improve the eruption path. After a certain time period, the repulsive force pushed the surface of the adjacent tooth, improving the eruption path of the entrapped tooth. The piston-elastic is a novel device that simply and effectively changes the direction of eruption of ectopically entrapped molars. As it can be manufactured and attached to the chair side, impression acquisition on a model cast and laboratory procedures are unnecessary. Compared to existing methods, the piston-elastic can be easily produced and delivered, causes little irritation, and is inexpensive.

Ectopic eruption of a tooth is the process in which a tooth emerges in an abnormal position. The first permanent molar is the tooth that most frequently presents ectopic eruption, mostly because it is retained below the distal contour of the second temporary molar.  The ectopic eruption may be reversible or irreversible, the latter of which requires corrective treatment to achieve correct eruption of the permanent molar in the arch. Treatments can be minimally invasive, such as the use of interproximal wedging, or more invasive orthodontic devices, such as brass wire, k-loops, Halterman devices, or wedging springs. The aim of this case report is to present two clinical cases where the ectopic eruption of permanent first molars is corrected using elastic separators. Conclusion:The elastic separators could be used successfully in cases of moderate or severe ectopic eruptions, not only in mild cases, as reported in most of the literature. The use of this technique does not require great cooperation from the patient, and it is low in cost.

Objective: To report the case of a 5-year-old patient diagnosed with ectopic eruption of the lower left first permanent molar that was mesially inclined, initially based on the clinical examination and followed by periapical radiographic examination. Case report: The treatment consisted of the placement and maintenance of an elastic separator for one week between the space of the lower second primary molar and the lower first permanent molar. Subsequently, the elastic separator was removed, allowing the tooth to erupt freely. After 1 week without the elastic separator, the patient returned and the tooth was observed in adequate eruption. Final considerations: The treatment of ectopic eruption was effective, since it repositioned the first permanent molar and allowed its vertical eruption in a short time using a simple technique. Moreover, the early diagnosis of this eruption disorder of permanent molar in mixed dentition is important to avoid more complex treatments and enable normal occlusion development.
