Research shows that living with illness can be a distressing experience for the family and may result in suffering and reduced health. To meet families’ needs, family systems intervention models are developed and employed in clinical contexts. For successful refinement and implementation it is important to understand how these models work. The aim of this study was therefore to describe the dialogue process and possible working mechanisms of one systems nursing intervention model, the Family Health Conversation model. A descriptive evaluation design was applied and 15 transcribed conversations with five families were analyzed within a hermeneutic tradition. Two types of interrelated dialogue events were identified: narrating and exploring. There was a flow between these events, a movement that was generated by the interaction between the participants. Our theoretically grounded interpretation showed that narrating, listening, and reconsidering in interaction may be understood as supporting family health by offering the families the opportunity to constitute self-identity and identity within the family, increasing the families’ understanding of multiple ways of being and acting, to see new possibilities and to develop meaning and hope. Results from this study may hopefully contribute to the successful implementation of family systems interventions in education and clinical praxis.

The 15-minute family interview is a condensed form of the Calgary Family Assessment and Intervention Models (CFAM and CFIM) that aims to contribute to the establishment of a therapeutic relationship between nurses and family and to implement interventions to promote health and suffering relief, even during brief interactions. This study investigated the experience of nurses from the Family Health Strategy (FHS) who used the 15-minute interview on postpartum home. The qualitative research was conducted in three stages: participants' training program, utilization of the 15-minute family interview by participants, and interviews with nurses. The data were collected through semi-structured interviews with eight nurses. The thematic analysis revealed two main themes: dealing with the challenge of a new practice and evaluating the assignment. This work shows that this tool can be used to deepen relationships between nurses and families in the Family Health Strategy.

Objectives: The objective of this study was to review literature focused on the introduction of Brazilian oral healthcare service models, incorporation of Oral Health Teams (OHTs) within the Family Health Strategy (FHS), changes occurring in the work process of the professionals that make up OHTs, difficulties for successful implementation of this new healthcare model and impacts on indicators of oral health. Methods: A review of the literature was conducted through an electronic consultation of the Pubmed and Virtual Health Library (VHL) databases published between 2001 and 2016, using a combination of descriptors. We selected 44 studies for review. Results: Oral healthcare in Brazil evolved from exclusionary and curative models to those aimed at prevention and promotion of health. The increase in government resources dedicated to oral healthcare in Brazil represented an improvement in access to dental treatment, however, some studies demonstrated that OHTs within the FHS still operate by the traditional curative models. Conclusion: Lack of proper planning for hiring OHTs and precariousness of their work contracts were also reported. The need for better managerial planning for implementation of OHTs within the scope of the FHS was demonstrated.

Background: Having a family member living in a residential home affects the entire family and can be hard to handle. Family members require encouraging and open communication support from nurses during and after relocation to a residential home. A Family Systems Nursing intervention, “Family Health Conversations” (FamHC) was conducted in order to strengthen the health of families having relatives at residential home for older people. Objectives: The aims of this study were to evaluate the responses to the Family Health Conversations in families with a member living at a residential home for older people and to integrate the empirical results with a theoretical assumption upon which the intervention was based. Methods: A mixed methods research design was used. The Swedish Health-Related Quality of Life Survey and the Family Hardiness Index were administered before and 6 months after the intervention. Qualitative data was collected by semi-structured interviews with each family 6 months post-intervention. The sample included 10 families comprising 22 family members. Result: Main finding was that FamHCs helped family members process their feelings about having a member living at a residential home and made it easier for them to deal with their own situations. FamHCs helped to ease their consciences, improve their emotional well-being, and change their beliefs about their own insufficiency and guilt. Seeing problems from a different perspective facilitated the families’ thinking in a new way. Conclusion: These findings showed that FamHC could be an important type of intervention to improve family functioning and enhance the emotional well-being.

Currently, there are few studies which examine targeted family-focused support when a family member is diagnosed with breast cancer. Thus, the aim of this study was to explore families’ experiences of participating in a family nursing intervention identified as Brief Family Health Conversations (BFamHC) following the diagnosis of breast cancer. Semi-structured family interviews were conducted with nine families (including 29 family members) 2 weeks following the family-focused intervention of three sessions of BFamHC. Thematic analysis was used to analyze the data. Families reported the BFamHC as positive and as a unique kind of family health conversation, one that afforded them the opportunity to communicate and share their experiences as a family group. A family conversation, even one as time-limited as BFamHC, offered a sense of relational sharing and togetherness, thus preventing feelings of isolation and vulnerability. Therapeutic family-focused conversations, such as BFamHC, hold promise as a useful family nursing intervention following the diagnosis of breast cancer.

RESUMO Objetivo: avaliar a sobrecarga dos cuidadores informais de idosos frágeis e analisar sua associação com as variáveis socioeconômicas e inerentes ao acompanhamento pela Estratégia Saúde da Família (ESF). Método: trata-se de estudo quantitativo, transversal, realizado por meio de formulário para a caracterização socioeconômica e clínica e do instrumento de Zarit Burden Interview para a avaliação da sobrecarga. Compôs-se a amostra por 51 cuidadores informais de duas Estratégias de Saúde da Família (ESF’s), e os resultados apresentaram-se por meio de dados estatísticos em figuras e tabelas. Resultados: encontrou-se alta prevalência de sobrecarga entre os cuidadores (82%), a qual apresentou associação estatística significante com a idade (p- 0,003) e o apoio da Estratégia de Saúde da Família ao cuidador (p- 0,028). Conclusão: comprova-se, pelos resultados, a importância dos profissionais no apoio formal ao cuidador, assim subsidiando o planejamento de ações de intervenção de Enfermagem para o binômio idoso/cuidador.
