Introduction. A paraoesophageal hernia (PH) may be one reason for iron-deficiency anaemia (IDA) but is often overlooked as a cause. We aimed to assess the incidence and resolution of transfusion-dependent IDA in patients presenting for hiatal hernia surgery. Methods. We analysed a prospective database of patients undergoing laparoscopic hiatal repair in order to identify patients with severe IDA requiring red cell/iron transfusion. Results. Of 138 patients with PH managed over a 4-year period, 7 patients (5.1%; M : F 2 : 5; median age 62 yrs (range 57–82)) with IDA requiring red cell/iron transfusion were identified. Preoperatively, 3/7 patients underwent repetitive and unnecessary diagnostic endoscopic investigations prior to surgery. Only 2/7 ever demonstrated gastric mucosal erosions (Cameron ulcers). All patients were cured from anaemia postoperatively. Discussion. PH is an important differential diagnosis in patients with IDA, even those with marked anaemia and no endoscopically identifiable mucosal lesions. Early recognition can avoid unnecessary additional diagnostic endoscopies. Laparoscopic repair is associated with low morbidity and results in resolution of anaemia.

BackgroundOptic neuritis is recognised by the international classification of headache disorders as a painful cranial nerve lesion. A lumbar puncture may be performed in the investigation of optic neuritis. Postdural puncture headache (PDPH) due to intracranial hypotension is a frequent complication of this procedure. In contrast, cerebral venous thrombosis (CVT) is a rare but potentially fatal complication of dural puncture. A few studies have identified an association between iron deficiency anaemia and venous thrombosis. There are no reports linking CVT with lumbar puncture and iron deficiency anaemia.Methods and resultsWe present a 32-year-old woman with optic neuritis and iron deficiency anaemia complicated by a PDPH and CVT.ConclusionCVT should be considered in a patient with persistent headache, recent lumbar puncture and iron deficiency anaemia. Early recognition and treatment of this condition are vital to avoiding mortality and morbidity.

Iron deficiency Anaemia is the most common case of nutritional Anaemia worldwide and is a public health problem in developing countries; characterized by abnormal red cell count (MCV, MCHC). Iron deficiency Anaemia associated with fatigue, skin paleness, weakness etc. The aim of the study is to determine the risk factors for iron deficiency Anaemia among population of Ujjain district (M.P.). Both male and female iron deficient patients were studied from the SS hospital and CHL applo hospitals during Jan. 2012 to Dec. 2012. A group of 6828 patients were studied for iron deficiency Anaemia in different age groups. A total of 4549 female and 2279 males were studied. Information was collected about sex, socioeconomic level. It is inferred that the percentage of anaemia is more in women compared to men.

Both iron deficiency and thalassaemia trait can present with hypochromic microcytic morphology. Naked eye single tube red cell osmotic fragility (NESTROF) is an inexpensive and simple test which can effectively differentiate iron deficiency anaemia from thalassaemia trait. Our study was an opportunity to observe the role of NESTROF in screening of thalassaemia trait among our study population. Initially NESTROF was carried out in 677 patients (N) population of four groups having both positive and negative NESTORF; for economical issue, auto-analyzer study and haemoglobin electrophoresis were done among only 100 randomly selected subjects, who were finally included in our study as final study population (n). In our study, NESTROF was 94.23% sensitive and its specificity was 92.08%. The predictive value of a positive test was 88.13% and predictive value of a negative test was 96.24%. Our study showed, NESTROF can effectively differentiate iron deficiency anaemia (IDA) from thalassaemia trait but, is not effective in co-existent IDA and thalassaemia.J Bangladesh Coll Phys Surg 2014; 32: 190-193

SUMMARYThe relationship between varying intensities ofTrichuris trichiurainfection and iron status was examined in Jamaican schoolchildren, aged 7 to 11 years. A total of 409 children was identified withT. trichiura(epg > 1200). A control group comprised 207 uninfected children who were matched by school and class to every pair of infected subjects. Blood samples were obtained from 421 children: 264 infected and 157 controls. Compared to the rest of the children, those with heavy infections (epg > 10 000) had significantly lower (P< 0·05) Hb (11·5 ± 1·3vs. 12·1 ± 1·1 g/dl), MCV (78·6 ± 6·3vs. 81·2 ± 5·5 fl), MCH (26·2 ± 2·9vs. 27·5 ± 2·5 pg) and MCHC (33·2 ± 1·5vs. 33·9 ± 1·4 g/dl). Similarly, the prevalence of anaemia (Hb < 11·0 g/dl) amongst heavily infected children (33%) was significantly higher (P< 0·05) than the rest of the sample (11%). These differences remained significant after controlling for confounding variables including socio-economic status, age, gender, area of residence and the presence ofAscarisinfections. Differences in red cell count, ferritin, and free erythrocyte protoporphyrin were not statistically significant and showed no association with the infectious load. These results suggest that in the Jamaican children studied, iron deficiency anaemia is associated withTrichurisinfections over 10 000 epg, but not with less intense infections.

Cicatricial pemphigoid is a chronic mucosal blistering disorder with a predilection for subsequent scar formation. Many physicians may be unaware of the various presentations and sequelae of this uncommon disease. This report of the largest series to date focuses on the upper aerodigestive tract manifestations of this disease. During the years 1975 to 1985, 142 patients with cicatricial pemphigoid were seen at the Mayo Clinic. There were 93 women and 49 men; the age range was 21 to 92 years. Mucosal lesions occurred most often in the mucous membranes of the oral cavity and conjunctiva. Involvement of the pharynx, larynx, and esophagus was less common. Stenosis of the nasopharynx or larynx necessitated surgical repair in several persons and caused obstructive sleep apnea in two. The otolaryngologist can make an important contribution to the early recognition, diagnosis, and management of the complications of cicatricial pemphigoid.

The most commonly encountered disorders with microcytic anemia are iron deficiency anemia (IDA) and β-thalassemia trait (BTT).It is important to distinguish between IDA and BTT to avoid unnecessary iron therapy and the development of hemosiderosis.Various parameters obtained on automated haematology analyser by calculating discriminant functions have been helpful to differentiate iron deficiency anemia and β-thalassemia trait. Materials and Methods: This study was carried out in the pathology department of Gujarat Adani Institute Of Medical Science.We had studied 100 cases of differentiating Iron deficiency anaemia and β-Thalassaemia trait from patient's case records and laboratory reports irrespective of age and sex over a period of one year (January 2020 to December 2020).Tests for serum iron and ferritin were carried out in individuals showing low hemoglobin (Hb). All the selected subjects' samples were subjected to blood morphology, comparison of mean cell volume (MCV), RBC count, red cell distribution width (RDW), Red cell distribution width Index (RDWI),Srivastav Index(SI),Green and king Index (GI) and Mentzer's Index(MI). Results: Out of 100 cases seventy five were diagnosed having iron deficiency anaemia, whereas twenty five were having βthalassemia trait. The RBC count was higher and MCV was much lower in β-thalassemia trait as compared to iron deficiency anaemia.Both groups were subjected to RDW,RDWI,SI,MI and GI. Conclusion:Among various discriminant function calculated from different haematological parameters and blood indices,Mentzer index and Srivasta index are more sensitive and specific for differentiating iron deficiency anaemia and β-thalassemia trait.
