Hepatitis B virus (HBV) infection is a worldwide public health problem. In this paper, we study the dynamics of hepatitis B virus (HBV) infection which can be controlled by vaccination as well as treatment. Initially we consider constant controls for both vaccination and treatment. In the constant controls case, by determining the basic reproduction number, we study the existence and stability of the disease-free and endemic steady-state solutions of the model. Next, we take the controls as time and formulate the appropriate optimal control problem and obtain the optimal control strategy to minimize both the number of infectious humans and the associated costs. Finally at the end numerical simulation results show that optimal combination of vaccination and treatment is the most effective way to control hepatitis B virus infection.

AbstractInfection of hepatitis B virus (HBV) is a global health problem. We provide the study about hepatitis B virus dynamics that can be controlled by education campaign (awareness), vaccination, and treatment. Initially we bring constant controls in considerations for treatment, vaccination, and education campaign (awareness). In the case of constant controls, we study the stability and existence of the disease-free and endemic equilibria model’s solutions. Afterwards, we take time as a control and formulate the suitable optimal control problem, acquire optimal control strategy in order to reduce the number of humans that are infected and the costs associated. At the end, results of numerical simulations show that the optimal combination of education campaign (awareness), treatment, and vaccination is the most efficient way to control the infection of hepatitis B virus (HBV) infection.

Hepatitis B virus (HBV) infection is a challenging public health problem in China and worldwide. Mother-to-child transmission is one of the main transmission routes of HBV in highly endemic regions. However, the mechanisms of HBV perinatal transmission in children have not been clearly defined. The aim of this study was to demonstrate the association between single-nucleotide polymorphisms (SNPs) in IFN-γ signaling pathway and HBV infection or breakthrough infection in children. Two hundred and seventy-four HBV-infected children defined as test positive for hepatitis B surface antigen (HBsAg) and 353 controls defined as negative for HBsAg in China were recruited from October 2013 to May 2015. SNPs in IFN-γ signaling pathway including IFNG, IFNGR1, IFNGR2, and IL12B were genotyped. Rs2234711 in IFNGR1 was significantly associated with HBV infection in children (OR = 0.641, 95% CI: 0.450–0.913). In addition, rs2234711 was also significantly associated with HBV breakthrough infection in children born to HBsAg-positive mothers (OR = 0.452, 95% CI: 0.205–0.998). Our study confirmed that genetic variants in IFN-γ signaling pathway have significant associations with HBV infection, especially with HBV breakthrough in children. This study provides insight into HBV infection in children and could be used to help design effective strategies for reducing immunoprophylaxis failure.

Despite worldwide vaccination campaigns, hepatitis B virus (HBV) infection remains a major public health problem. The natural history ranges from asymptomatic infection to severe liver injury or failure, chronic complications or reactivation episodes. The effects of HBV on the organism are immunomediated, possibly triggering extrahepatic manifestations. Since 1971, only a few cases of pleural effusion related to HBV infection have been described. We report HBV-associated pleural effusion occurring during a viral reactivation episode. Antiviral treatment directed towards pleural effusion related to HBV infection should be dictated by underlying liver disease severity and not pleural effusion severity.

Background: Hepatitis B virus (HBV) infection is a serious public health problem in Africa and worldwide. Barbers are regularly in contact with the blood fluid of their clients, who could develop skin cuts and abrasions during shaving practices. There is scarcity of information about HBV among barbers in Cameroon. This study presents seroprevalence and associated risk factors for HBV infection among barbers and their clients in two cities (Yaounde and Douala) in Cameroon.Methods: Three millilitres of venous blood was collected into EDTA tubes from each of the 262 consenting participants. Plasma was obtained and transported to the Military Health Research Centre (CRESAR) for analysis. Sociodemographic data were collected using a structured questionnaire. The prevalence of Hepatitis B surface antigen (HBsAg) was determined by a rapid diagnostic test (DiaSpot HBsAg) and confirmed with an ELISA sandwich test. Data were analysed using SPSS, version 21.Results: Thirty-three participants tested positive, giving an overall prevalence of 12.6%. A seroprevalence of 15.0% and 10.6% was obtained for barbers and their clients respectively. In Yaounde, the frequency of HBsAg among barbers and their clients was 14.9% and 10.4% respectively, while in Douala it was 15.4% and 11.1% respectively. However, no statistically significant difference was observed in these groups. There was also no statistically significant association between the knowledge of the modes of HBV transmissions and HBV status.Conclusion: The present study reveals that the rate of HBsAg among the study population is high in Cameroon and most people are not still properly sensitised regarding prevention measures.

Hepatitis B virus (HBV) infection is a major health problem with an important biological and a significant socio-economic impact all over the world. There is a high pressure to come up with a new and more efficient strategy against HBV infection, especially after the recent success of HCV treatment. Preventing HBV infection through vaccine is currently the most efficient way to decrease HBV-related cirrhosis and liver cancer incidence, as well as the best way to suppress the HBV reservoir. The vaccine is safe and efficient in 80-95% of cases. One of its most important roles is to reduce materno-fetal transmission, by giving the first dose of vaccine in the first 24 hours after birth. Transmission of HBV infection early in life is still frequent, especially in countries with high endemicity.Successful HBV clearance by the host is immune-mediated, with a complex combined innate and adaptive cellular and humoral immune response. Different factors, such as the quantity and the sequence of HBV epitope during processing by dendritic cells and presenting by different HLA molecules or the polymorphism of T cell receptors (TOL) are part of a complex network which influences the final response. A new potential therapeutic strategy is to restore T-cell antiviral function and to improve innate and adaptive immune response by immunotherapeutic manipulation.It appears that HBV eradication is far from being completed in the next decades, and a new strategy against HBV infection must be considered. Hepatitis B virus (HBV) infection represents a serious global health problem and persistent HBV infection is associated with an increased risk of cirrhosis, hepatocellular carcinoma and liver failure. Recently, the study of the role of microRNA (miRNA) in the pathogenesis of HBV has gained considerable interest as well as new treatments against this pathogen have been approved. A few studies have investigated the antiviral activity of vitamin E (VE) in chronic HBV carriers. Herein, we review the possible role of tocopherols in the modulation of host miRNA with potential anti-HBV activity. A systematic research of the scientific literature was performed by searching the MEDLINE, Cochrane Library and EMBASE databases. The keywords used were ‘HBV therapy’, ‘HBV treatment’, ‘VE antiviral effects’, ‘tocopherol antiviral activity’, ‘miRNA antiviral activity’ and ‘VE microRNA’. Reports describing the role of miRNA in the regulation of HBV life cycle,in vitroandin vivoavailable studies reporting the effects of VE on miRNA expression profiles and epigenetic networks, and clinical trials reporting the use of VE in patients with HBV-related chronic hepatitis were identified and examined. Based on the clinical results obtained in VE-treated chronic HBV carriers, we provide a reliable hypothesis for the possible role of this vitamin in the modulation of host miRNA profiles perturbed by this viral pathogen and in the regulation of some cellular miRNA with a suggested potential anti-HBV activity. This approach may contribute to the improvement of our understanding of pathogenetic mechanisms involved in HBV infection and increase the possibility of its management and treatment.

This review aimed to provide an update on the impact of pregnancy on the natural course of hepatitis B virus (HBV) infection and also on the impact of HBV infection on adverse pregnancy outcomes, including mother-to-child transmission (MTCT). For the literature review, original research articles, review articles, and guidelines were narratively reviewed and comprehensively validated. The databases of PubMed, EMBASE, and CINAHL were carefully searched for articles in English on topics related to HBV infection, pregnancy, and vertical transmission from 1960 to May 2021. Immunological changes during pregnancy such as suppression of Th1 response and induction of Th2 immunity lead to an impaired immune reaction to HBV and stimulate viral activity along with the reduction of CD8 T cells to escape immune detection. The impact of pregnancy on the natural course of chronic HBV infection seems to be minimal, while pregnancy can increase morbidity and mortality in the case of advanced HBV hepatitis or cirrhosis. Importantly, hepatitis flare or alanine aminotransferase (ALT) flare can occur during pregnancy and is more common during the postpartum period due to the interaction between HBV and the immune response. Interestingly, the impact of HBV infection on adverse pregnancy outcomes is more serious than ever thought. Updated evidence indicates that pregnancies with chronic HBV infection increase the risk of preterm birth and gestational diabetes, especially in cases of positive hepatitis e antigen (HBeAg).
