Women with preeclampsia, independent of obesity and glucose intolerance, exhibit insulin resistance during pregnancy. The purpose of the present study is to determine whether early diagnosis of insulin resistance during pregnancy can predict preeclampsia. Through a case-control study, 675 pregnant women were selected and their first trimester blood was taken. Their fasting blood glucose and insulin were also measured after diagnosis of preeclampsia by 20 weeks of pregnancy. Based on the experiments conducted on 675 women who were 20 weeks past their pregnancy, 375 cases with preeclampsia were selected and assigned to the case group. 35 other pregnant women were put in the control group. Diagnosis criteria for the participants included blood pressure above 140/90 and proteinuria above 300 mg or above +1. Both groups were matched according to age, parity, gestational age, and BMI. Homa-Irand rate of insulin resistance was calculated by HOMA-IR and patients were followed up. Homeostatic model assessments (HOMA-IR) revealed that the average insulin resistance increased during pregnancy among both the case and control groups. There was a significant difference between insulin resistance of these two groups in both first trimester and third trimester and after developing preeclampsia (P< 0.001,P= 0.021). Insulin-resistance of the group with preeclampsia was higher in first trimester prior to diagnosis as well as the third trimester after diagnosis compared to natural pregnancy under similar conditions. Measurement of insulin resistance in first trimester may be useful in predicting the risk of preeclampsia.

Abstract Objectives During pregnancy, systemic physiological alterations lead to some changes in the oral cavity, which could prepare the mouth environment for oral and dental problems. This study was aimed to investigate salivary α-amylase, sialic acid levels, and pH levels in pregnant and nonpregnant females. Materials and Methods In this analytical, case–control study, unstimulated saliva samples were collected with spiting method from 35 pregnant women (case group) and 35 nonpregnant women (control group) and transferred to the laboratory to assess salivary α-amylase, sialic acid, and pH levels. Data were analyzed by SPSS (version: 19) software through statistical methods of independent t-test and analysis of variance. Results The mean sialic acid levels were 2.285 ± 1.230 mg/dL in pregnant and 2.744 ± 1.326 in nonpregnant women without any significant difference (p = 0.138). The mean salivary α-amylase concentrations were 2.461 ± 1.869 U/L and 2.439 ± 2.058 U/L, respectively, in pregnant and nonpregnant women, with no significant difference (p = 0.963).The mean salivary pH in nonpregnant women was significantly more than that in pregnant women (7.845 ± 0.430 and 6.868 ± 0.413, respectively) (p < 0.001). Also, the mean salivary pH levels in pregnant women were 7.474 ± 0.420 in the first trimester, 6.868 ± 0.413 in the second trimester, and 6.568 ± 0.387 in the third trimester, which were significantly different (p < 0.001). Conclusion Salivary sialic acid and α-amylase levels among pregnant women were no different from those of other subjects. During pregnancy, the salivary pH significantly reduced, and the mean salivary pH during pregnancy had a decreasing trend from the first trimester to the third trimester.

Abstract Aim: To assess the effect of psychotropic drugs on fetal behavior using four-dimensional (4D) ultrasound in the third trimester of pregnancy. Methods: Fetal behavior was assessed using Kurjak’s antenatal neurodevelopmental test (KANET) using 4D ultrasound between 28 and 36 weeks of gestation. Thirty healthy (control group) and 10 psychotropic-drug-administered pregnant (case group) women were studied. The total value of the KANET score and values of each parameter (eight parameters) were compared between the two groups. Results: The total KANET score was normal (except for one fetus in the case group: total score of 9) in both groups, and there was no significant difference in the total KANET score. When individual KANET parameters were compared, no significant differences were noted in any of the eight parameters. Conclusion: Our results showed that there is no difference in fetal behavior between fetuses of normal pregnant women and those of psychotropic-drug-administered pregnant women in the third trimester of pregnancy. These results suggest that psychotropic drugs may not affect fetal behavioral development in utero. However, the data and their interpretation in the present study should be taken with some degree of caution because of the small number of subjects studied. Further studies involving a larger sample size are needed to assess the effect of psychotropic drugs on fetal neurobehavior during pregnancy.

Background: More than one million children throughout the world are born by using fertility techniques. In this process, human intervention and laboratory conditions may have an impact on the growth and development of the fetus. Objectives: The present study aimed to consider the effect of assisted reproductive techniques on the results of embryonic health screening tests. Methods: In this case-control study, among clients who went to the Infertility Center of the Ome Leila Hospital in Bandar Abbas Province, we selected 200 pregnant women who were impregnated via assisted reproductive techniques as the case group and 200 natural pregnant women as the control group by sample random sampling. Checklists were filled out and data analyzed by the SPSS version 21 and chi-square tests by using an interview with pregnant women and investigating the embryonic screening tests. Results: Most of the participants in the study were housewives and had primary infertility. Their BMI was normal. There was no significant difference between the final results of the embryonic screening test in the two groups at the first trimester (P > 0.05). However, in the second trimester, there was a significant difference between them. The positive results were higher in the case group in comparison with the control group (P = 0.001). The mean of all types of screening tests in the first trimester in both groups was not different, significantly (P < 0.05), although in the second trimester, the mean of all the testis, including BHCG (P = 0.006) and AFP (P = 0.018) in both case and control groups, was significantly different. The mean of BHCG and AFP was higher in the case group, while the mean of Estradiol and Inhibin, NB, and NT were not really different (P < 0.05). Conclusions: Our study showed that embryonic screening tests, particularly the BHCG and AFP tests, would be affected by applying the assisted reproductive techniques. For the couples who had a history of infertility and used reproductive methods, screening tests are essential in the first and second trimester.

Background: Pregnancy is a major endocrine event in the female lifespan, involving wide-ranged and often dramatic changes in the metabolism of various hormones. Cross sectional, case control, analytical quantitative study was conducted in Sudan, Khartoum state in Yastabsheron obstetric hospital during the period from March to August 2011. Analytical and statistical methods were applied to measure the concentration of A1c% in healthy pregnant women as well as in healthy non-pregnant women to assess the difference in the results.Methods: Blood samples were taken from a total of 90 healthy pregnant women (case group) and 30 healthy non-pregnant women (control group), then samples were analyzed for A1c% by using affinity chromatography technique, and results were recorded in addition to their age, body mass index and the number of pregnancies.Results: showed that, the mean concentration of the A1c% in cases group was (4.407±1.054%) in first trimester, (4.797±0.631) % in second trimester and (4.833±0.626) % in third trimester, and (5.670±0.471%) in control group with a P value of 0.00, indicating the highly significant difference between the two groups. Others finding showed that the mean concentration of A1c% of the first trimester is lower than that of the second and third trimesters, also there was no significant difference between the mean concentration of the second and third trimester. A significant weak positive correlation between A1c% concentration with body mass index and the age of pregnant women.Conclusions: Healthy normal pregnant women have lower A1c% concentrations than non-pregnant women which can be impute to the reduce in plasma glucose values and to the shortened erythrocyte life span that can occur during pregnancy. The body mass index and age affect the concentration of A1c% c, but it is not affected by gravida.

The incidence of preeclampsia is high in the developing countries. Since this condition is preventable if detected and treated at an early stage, it is essential to diagnose the disease at an early stage, and to institute proper medical care on time. The present study carried out in the Department of Obstetrics and Gynaecology, BIRDEM, and Dhaka Medical College Hospital during January 2009 and June 2010, was aimed to find out concentration of serum C reactive protein (CRP) in preeclamptic women. The study included 60 pregnant women; 30 normal (control) and 30 preeclamptic (case) pregnant women in their third trimester. Estimation of CRP was done by immunoprecipitation assay turbulometry method for both groups. The mean (±SD) age was 23.23±4.58 (control) and 23.90±3.20 (case) years (no significant difference). However, BMI, SBP and DBP were significantly (P<0.001) high in case compared to control group (BMI: 23.37±1.47 and 21.81±1.45 kg/m2; SBP: 148.33±13.41 and 108.00±7.14 mmHg; DBP: 106.67±6.99 and 69.67±5.56 mmHg). C reactive protein concentration (mg/dl) was significantly higher (P<0.001) in case group (10.57±6.71) compared to control group (0.63±0.49). In control and case group, respectively, CRP was normal (£0.8 mg/dl) in 25 (83.3%) and 2 (6.7%), and raised (>0.8 mg/dl) in 5 (16.7%) and 28 (93.3) (P<0.001). This study shows that maternal CRP concentration tends to be significantly high in women with preeclampsia. Background: Exact aetiology of this potentially fatal disorder remains poorly understood. A number of theories have been put forward where different biochemical markers have been implicated in the causal association of preeclampsia. This study was intended to find the association between serum b-hCG level and preclampsia Methods: This cross-sectional, case-control study was conducted on 74 pregnant women with preeclampsia (cases) who were admitted in the Eclampsia ward of Dhaka Medical College Hospital, Dhaka between January and July of 2013. A total of 76 normotensive pregnant women were also taken from the Obstetrics & Gynaecology Out-patient Department of the same hospital as control. The study subjects were selected on the basis of predefined eligibility criteria. The serum levels of b-hCG were compared between case and control groups as well as between mild and severe preeclampsia. Result: The case and control groups were almost similar in terms of all the baseline demographic and obstetric characteristics except past history of PET which was significantly higher in the former group than that in the latter group. Majority (97.1%) of the cases had severe hypertension (74.3%) with mean systolic and diastolic blood pressures being 162.6 and 110.8 mmHg respectively. The mean serum ?-hCG was much higher in the case group than that in the control group (p<0.001). The mean serum ?-hCG was the highest in severe preeclampsia and the lowest in the control group, while that in mild preeclampsia lie in between the two (p<0.001). The serum ?-hCG exhibits a significantly linear correlation with systolic and diastolic blood pressures (p<0.001 respectively). Conclusion: There was a significant difference between the ?-hCG level in the preeclamptic women compared to the normotensive pregnant women and the severity of preeclamsia increases with further rise of ?-hCG level. Background: The term gestational diabetes mellitus (GDM) is becoming a major health problem in developing countries undergoing rapid changes in lifestyle, dietary habits and body mass index. GDM is associated with an increased incidence of congenital abnormalities which is also aggravated by mother’s zinc and copper deficiency. Zinc and copper are essential trace elements for normal embryogenesis and fetal growth and their deficiency increase mortality and morbidity of mothers, embryos and neonates. This study was designed to evaluate the association of serum zinc and copper with GDM in second and third trimester.Methods: It was a case-control study. This study was conducted in Mymensingh Medical College Hospital during the period from July 2013 to June 2014 to evaluate the association of zinc and copper levels of pregnant women with GDM. A total induded of 172 subjects were participated in this study; among them 86 women diagnosed with GDM were selected as case (Group-I) and 86 healthy pregnant women were control (Group- II).The case group was again subdivided as Group Ia and Ib according to second and third trimester respectively. Control group was also subdivided as Group IIa and IIb according to second and third trimester respectively. Student’s unpaired ‘t’ test was used to analyse the data between groups. For analytical purpose 95% confidence limit (p<0.05) was taken as level of significance.Results: There was significant difference in serum zinc and copper levels in cases compared to control group. Highly significant difference (p<0.001) was found when serum zinc was compared between women with GDM and normoglycemic pregnant women in second and third trimester. Serum copper level was significantly increased in cases compared to control group in second trimester and the difference was found highly significant (p<0.001) and significant difference (p<0.01) was found in GDM compared to normoglycemic pregnant women in third trimester.Birdem Med J 2018; 8(1): 52-55

Objective: India is the second country after China having the highest population prevalence of diabetes. Several research studies investigating diabetes have been done, but not much work has been done on prediabetes. The purpose of this study was to investigate the effect of nutrition and lifestyle modification on prediabetic females. Methods: A total of 120 prediabetic females from Ludhiana city were divided into 2 matched groups: control group (n = 60) and experimental group (n = 60). Impact of nutrition intervention for dietary and lifestyle modification (for 3 months) was assessed on the anthropometric, dietary, biochemical parameters, and diabetes risk score of the experimental group and control group (no intervention). Results: All the selected 120 subjects completed the study (experimental group = 60; control group = 60). There was significant difference in the changes between the 2 groups throughout the study. The fasting blood glucose and glycated hemoglobin A1c levels of the experimental group subjects reduced significantly ( P ≤ .01). However, no change was observed among the control group subjects. The lipid profile of the experimental group showed a significant improvement ( P ≤ .01). Conclusion: Nutrition counselling of the prediabetics regarding dietary and lifestyle modification is recommended so as to improve their metabolic control, thus preventing them from being diabetics.

Abstract BackgroundPhysical activity during pregnancy period is one of the issues with priority during pregnancy period. Researches show that women reduce their physical activity during this period and are unaware of the benefits on the health of the mother and embryology. Although researchers regarding physical activity during pregnancy have prepared many guidelines; it is not however clear why pregnant women do not perform physical activity and the effectiveness factors that facilitate the desired behavior. MethodsThe research population included all pregnant women aged 18 to 40 years with gestational age of 12-38 weeks referring to the healthcare centers in District 5 of Tehran Municipality, from 22 districts of the city who had eliminated their physical activity during pregnancy. Questionnaires of physical activity assessment questionnaire after educational intervention in pregnant women (PPAQ) and a questionnaire designed based on the results of the needs assessment and the dimensions of the PEN-3 model were used for assessment. This study is a Research Clinical Trial (RCT). Due to the nature, a quasi-experimental research design (pre-test, post-test) with the control group was used in this research. ResultsBased on the obtained results, the intervention based on E-learning has a significant effect of enabling factors to increase physical activity in pregnant women in the experimental group. In addition, the intervention based on E-learning has a significant effect on the training factors in order to increase physical activity in pregnant women in the experimental group. ConclusionBased on the research results, it can be said that there is a statistically significant difference between the experimental and control groups in the scores obtained from training factors in the pretest and the experimental group. Furthermore, it can be said that there is a statistically significant difference between the experimental and control groups in the scores obtained from the enabling factors in the pre-test and the experimental group.

Abstract Background The goal of our study is to investigate whether the methylation levels of AHCY and CBS promoters are related to the risk of cerebral infarction by detecting the methylation level of AHCY and CBS genes. Methods We extracted peripheral venous blood from 152 patients with cerebral infarction and 152 gender- and age-matched healthy controls, and determined methylation levels of AHCY and CBS promoters using quantitative methylation-specific polymerase chain reaction. We used the percentage of methylation reference (PMR) to indicate gene methylation level. Results We compared the promoter methylation levels of two genes (AHCY and CBS) in peripheral blood DNA between the cerebral infarction case group and the control group. Our study showed no significant difference in AHCY promoter methylation between case and control. Subgroup analysis by gender showed that the methylation level of AHCY in males in the case group was lower than that in the control group, but the difference was not statistically significant in females. In a subgroup analysis by age, there was no significant difference in the AHCY methylation level between the case and control in the young group (≤44 years old). However, the level of AHCY gene methylation in the middle-aged group (45–59 years old) was significantly higher and the aged group (≥60 years old) was significantly lower than that in the control groups. However, CBS promoter methylation levels were significantly lower in the case group than in the control group (median PMR: 70.20% vs 104.10%, P = 3.71E-10). In addition, the CBS methylation levels of males and females in the case group were significantly lower than those in the control group (male: 64.33% vs 105%, P = 2.667E-08; female: 78.05% vs 102.8%, P = 0.003). We also found that the CBS levels in the young (23–44), middle-aged (45–59), and older (60–90) groups were significantly lower than those in the control group (young group: 69.97% vs 114.71%; P = 0.015; middle-aged group: 56.04% vs 91.71%; P = 6.744E-06; older group: 81.6% vs 119.35%; P = 2.644E-04). Our ROC curve analysis of CBS hypomethylation showed an area under the curve of 0.713, a sensitivity of 67.4%, and a specificity of 74.0%. Conclusion Our study suggests that hypomethylation of the CBS promoter may be closely related to the risk of cerebral infarction and may be used as a non-invasive diagnostic biomarker for cerebral infarction.
