Endodontic treatment of mandibular molars is challenging because of variable root canal morphology. The nonsurgical endodontic management of a mandibular first molar presenting an independent middle mesial canal is reported. After coronal access, additional clinical inspection of the mesial canals’ orifices and their interconnecting groove using an endodontic explorer and 4.5× loupes enabled the identification of the middle mesial canal orifice. All root canals were chemomechanically prepared and filled. The tooth was asymptomatic and functional after 4 years of followup. Cone beam computed tomography (CBCT) images revealed normal periapical status and three-dimensional (3D) anatomical aspects of the root canal system.

Abstract Background The purpose of this study was to determine the anatomical variations of the root canal system of mandibular second molars using cone-beam computed tomography (CBCT). Methods 190 mandibular second molars cone-beam computed tomography images were reviewed. The evaluation was performed by a radiologist with endodontic experience and two endodontists trained with CBCT technology. Tooth position, number of root and root canals, C-shaped root canal system configuration, presence of extra root (radix), and radicular grooves were assessed. Data was statistically analyzed using The Chi-square test (α = 0,05) to determine any significant difference between gender and the total number of root and root canals, and any significant difference between gender and root canal anatomical variation. Results Overall, 85.5% showed two separated roots, 12.1% a single root, 2.6% three roots or radix. 87.7% showed three root canals, 12.1% two root canals, 2.6% four root canals, and 1.6% a single root canal. 10% showed a single foramen, 75.3% two foramina, 13.6% three foramina and 1% showed four foramina.19.5% showed C-shaped anatomical variation, 51.4% in male patients, 48.6% in female patients. According to Fan classification: C1 13.6% in cervical third, C2 10% in the middle third, C3 17.3% in middle third, 15.5% in apical third, and C4 12.7% in the apical third. Root canals number in these samples were 5.4% a single canal, 21.6% two canals, 70.3% three canals, and 2.7% four canals. The root showed 46% with one foramen, 46% two foramina, and 8% three foramina. Radicular grooves 83.3% were found in the lingual area and 16.2% towards the buccal area. Conclusions The most prevalent anatomic presentation of the evaluated sample was a mandibular second molars with two roots, three root canals, and two apical foramina. Their variation was C-shaped root canals and Radix Paramolaris.

The meticulous knowledge of anatomic characteristics and their variations is essential for the clinician. Radix entomolaris (RE) is one such anomaly where an extra root is present on the distolingual aspect of the mandibular first molar. 18-year-old patient was referred for the root canal treatment of mandibular right and left first molars. Intraoral periapical radiograph revealed additional periodontal spacing crossing distal root of 36. A CBCT was advised and it confirmed the presence of extra roots both in 36 and 46. CBCT is useful in endodontics as it aids in the identification of essential anatomic structures and determination of radius and angle of root canal curvature which is linked to fracture of the instrument. The classical triangular access cavity was modified to a trapezoidal form to locate the extra canal. All canals were instrumented with controlled memory nickel titanium instruments and obturation was done with single cone technique.

ABSTRACT Aim To evaluate the root canal morphology of maxillary first premolar using Cone beam computed tomography (CBCT). Materials and methods A total of 125 teeth were selected and CBCT scans were taken and following factors were recorded – number of roots, number of root canals, anatomy, and configuration of canals. Results Overall 88% teeth had two roots. Most frequent canal configuration was type I in 93.6%; 94.4% teeth had single canal exiting in single apical foramen. Conclusion Within the limitation of the study, it was concluded that among the study population, there is a high incidence of two-rooted maxillary first bicuspid with two canals with type I Vertucci canal configuration. Introduction: A few studies investigated the numerous potential endodontic uses of CBCT, including the examination of root canal morphology and presumed that CBCT was effective for the initial identification of such morphology; moreover CBCT is a reliable method for the detection of the MB2 canal when compared with the gold standard of physical sectioning of the specimen. The aim of this study was to identify the root and canal morphology of the maxillary first molars among Malaysians analysed by cone-beam computed tomography (CBCT) images. Materials and Methods: Maxillary first (n = 421) molars from Malaysian patients (n = 241) of Malay, Chinese and Indians inceptions were examined by two Endodontists using in vivo CBCT methods. The number and configuration of roots, the number of root canals, and the canal configuration according to Vertucci’s classification were determined. Results: Single roots were not found in maxillary first molars. The incidence of fused roots was 1.995% in the first molars between mesiobuccal and distobuccal roots. In (421) 3-rooted maxillary first molars, additional canals were found in 45.6% of the mesiobuccal (MB) roots and 0% of the distobuccal (DB) roots. Bilateral symmetry of the MB roots was found in 82.36% of the first molar. Only one tooth was found to have pulp stone inside the pulp chamber. Conclusions: The root and canal configuration of a Malaysian population showed different features from those of other populations. CBCT scans can enhance the understanding of root canal anatomy, with the potential of improving the outcome of endodontic treatment.

Knowledge of dental anatomy through the assessment of the anatomic variations of each tooth’s root canal system is essential to undertake endodontic therapy. The aim of this systematic review was to analyze the different studies on the internal morphology of permanent mandibular incisors where Cone-Beam Computed Tomography (CBCT) X-ray imaging is used. Pubmed, CENTRAL, Wiley Library and Web of Science electronic databases were searched for scientific studies included until March 2020. The terms used in the search were: “permanent mandibular incisors”, “root canal morphology” and “cone-beam computed tomography”. The search was limited to studies whose aim was the analysis of the morphology of the root canal system evaluating the parameters of methodology, population, sample, number and configuration. A total of 19 studies met the inclusion criteria. There was a noticeable lack of unanimity in the setting adjustments of each of the CBCT devices used. The presence of two root canals varied from 0.4% to 45%. The most frequent configurations were Vertucci’s Types I, III, II, V, IV, VII and VI. Type VIII configuration was non-existent. CBCT revealed the existence of anatomical symmetry patterns, and there was no unanimity of criteria regarding the presence of a second root canal. Results concerning the presence of a second root canal in the mandibular incisors differ widely, with a possible influence of the geographic area where the study was conducted. The prevalence of a second canal is higher in mandibular lateral incisors than in mandibular central incisors. There was no direct relationship between voxel size (0.125–0.3 mm) and increased prevalence of a second canal.

Thorough knowledge of root canal morphology is essential for the endodontic therapy. There are rare variations in canal number and configuration in maxillary molars, which could affect treatment outcome. This paper presents the endodontic management of a maxillary first molar with two palatal canals in one root (Vertucci type IV) and a single buccal canal. In this paper cone-beam computed tomography was made to asses this morphology. This paper is intended to reinforce clinician’s awareness of the rare morphology of root canals.

Objective: The complex root canal anatomy is inherently colonised by microbial flora. Endodontic treatment success is always related to adequate disinfection of the root canal space, which ultimately affects the treatment outcome. A thorough understanding of the external and internal root canal anatomy by using adequately imaging modalities is essential before planning any treatment. The aim of this study was to investigate the number and morphology of the root canals of maxillary and mandibular premolars in Chennai population. Material and Methods: Full-size cone-beam computed tomographic images were randomly collected from 100 patients, resulting in a total of 200 first and 200 second maxillary premolars as well as 200 first and 200 second mandibular premolars. All the eight premolars were analysed in single patients, who underwent cone-beam computed tomography scanning during pre-operative assessment (before implant surgery, orthodontic treatment, diagnosis of dental-alveolar trauma or difficult root canal treatment). Total number of roots and root canals, frequency and correlations between men and women were recorded and statistically analysed by using chi-square tests. The root canal configurations were rated according to the Vertucci’s classification. Results: In the maxillary first premolar group (n = 200), 36.3% had 1 root, 56.7% had 2 roots and 7.0% had 3 roots, with most exhibiting a type IV canal configuration. In the maxillary second premolar group (n = 200), 60% had 1 root, 29.8% had 2 roots and 10.2% had 3 roots, with the majority of single-rooted second premolars exhibiting a type I canal configuration. In the mandibular first premolar group (n = 200), 80.5% had 1 root, 9.8% had 2 roots and 5% had 3 roots. In the mandibular second premolar group (n=200), 90.1% had 1 root, 6.4% had 2 roots and 3.5 % had 3 roots, with most exhibiting a type I canal configuration. No statistical correlation was found between number of roots, gender and tooth position. Conclusion: This cone-beam computed tomographic study confirmed previous anatomical and morphological investigations.
