Mesiodens is a midline supernumerary tooth commonly seen in the maxillary arch, and incidence of molariform mesiodens in the maxillary midline is rare in permanent dentition and extremely uncommon in primary dentition. A midline supernumerary tooth in the primary dentition can cause ectopic or delayed eruption of permanent central incisors which will further alter occlusion and may compromise esthetics and formation of dentigerous cysts. This paper reports a rare case of the presence of a molariform mesiodens in the primary dentition. On clinical and radiographic examination, flaring of the primary central incisors was seen, with a molariform mesiodens consisting of multiple lobes or tubercles on the occlusal surface with the well-formed root. The treatment plan consisted of the extraction of the supernumerary tooth and regular observation of permanent central incisors for proper eruption and alignment.

Mesiodens is a midline supernumerary tooth commonly seen in the maxillary arch and the talon cusp is a rare dental developmental anomaly seen on the lingual surface of anterior teeth. This paper presents a rare clinical case of development of talon cusp in a mesiodens with multiple lobes, which interfered with both occlusion and appearance of an 11-year-old patient. During clinical interview, the patient reported difficulty on mastication. Clinical and radiographic examination revealed that a supernumerary tooth with completely formed root was causing an occlusal interference. The supernumerary tooth was diagnosed as multi-lobed mesiodens associated with a palatal talon cusp. The treatment plan consisted in the extraction of the supernumerary tooth followed by orthodontic treatment for diastema closure and tooth alignment.

ABSTRACT Dens invaginatus is a dental anomaly caused by an invagination of enamel that affects in most of the cases the crown of the tooth. The treatment depends on the degree of invagination and morphological involvement. This report describes a dental anomaly affecting the permanent lower lateral incisor of a 13-year-old patient, which resulted in swelling, pain, and dental impaction. The initial radiographic examination suggested a gemination, a fusion with a supernumerary tooth or a dens invaginatus extending to the root. For more accurate diagnosis and treatment plan of this anomaly, computed tomography scan was performed, which revealed the presence of enamel in the root. Due to pain and the impossibility of performing an orthodontic traction, the treatment of choice in this case was dental extraction. The final diagnosis was confirmed by histopathological examination and revealed the rare occurrence of a radicular dens invaginatus. A dentigerous cyst or follicular cyst is a form of odontogenic cyst. It is believed that it forms during the development of the tooth and is associated with pressure exerted by the crown of an unerupted (or partially erupted) tooth on the fluid within the follicular space. Typically, dentigerous cysts are painless and discovered during routine radiographic examination. However, they may be large and result in a palpable mass. Additionally, as they grow they displace adjacent teeth. They almost exclusively occur in permanent dentition. The cyst is lined by stratified squamous non-keratinizing epithelium. About 70% of dentigerous cysts occur in the mandible and 30% in the maxilla. Dentigerous cysts associated with ectopic teeth within the maxillary sinus are very rare. We report radiologic and pathologic features in a rare case of infected dentigerous cyst of maxillary sinus arising from an ectopic third molar in a 21-year-old female patient.

ABSTRACTSupernumerary teeth occur frequently in permanent dentition, but they are rarely found in primary dentition. Supernumerary teeth of orthodox shape and size that resemble normal dentition are called ‘supplemental teeth’. Supplemental teeth are less common than supernumerary teeth and are often overlooked because of their normal shape and size. Supplemental teeth may cause esthetic problems, delayed eruption and crowding, and they require early diagnosis and treatment to prevent complications. The case reported here is one of bilateral supplemental teeth impeding the eruption of permanent maxillary lateral incisors, and it emphasizes the importance of early diagnosis and treatment during early mixed dentition. Double tooth is a developmental anomaly that is seen more frequently in the primary than the permanent dentition. Double tooth involving primary canines and lateral incisors is found only in the mandible. The succeeding permanent lateral incisor is often found missing congenitally in the same region. This article reports a rare case of a double tooth involving a mandibular primary canine and lateral incisor. Instead of agenesis, the double tooth has been succeeded by a permanent supernumerary tooth in the same region.

Background: Certain characteristics of the primary dentition have been described that may be required in the smooth transition from primary to permanent dentition and therefore development of ideal occlusion in permanent dentition. The objectives of this study were to assess different occlusal characteristics of primary dentition among school going children of Dharan, Nepal and to compare these occlusal characteristics based on gender and race. Methods: A community based cross-sectional study was conducted among 625 school children of Mongolian and Caucasian race, aged 3 to 7 years, and having complete set of primary dentition. A close-ended questionnaire was developed to gather demographic information followed by the examination of occlusal and soft tissue parameters. Results: The result showed 77.2% of the children had developmental spaces and 83.1% had primate spaces. Flush terminal plane molar and class I canine relationships were present in 68.8% and 81.6%, respectively with normal overjet (53.5%), normal overbite (41.1%), straight facial profile (86.7%) and competent lips (98.6%). Crowding was present in 35.4%, midline discrepancy in 11.3%, crossbite in 7.2%, scissor bite in 0.6% and open bite in 2.1% of the children examined. Conclusions: Most of the occlusal characteristics were desirable but with the existence of some malocclusion traits, necessitating timely recognition and correction through early screening. Gender and racial influences were also observed and it emphasizes the need for treatment plan based on them. Keywords: Gender; occlusal characteristics; primary dentition; race

A mesiodense is a most common supernumerary tooth which is one of the most significant dental anomalies at the primary and early mixed dentition stages. The reported prevalence of mesiodense in general population ranges between 0.15% to 1.9% and more frequently found in the permanent dentition but rarely found in primary dentition. Mesiodense are primarily responsible for eruption disturbances or delay of the maxillary anterior permanent teeth emergence. Therefore the dentists and parents are great concern about malocclusion and esthetic problems which is caused by mesiodense. Early detection, diagnosis and treatment of mesiodense is important to prevent further complications in permanent dentition. A case of mesiodense of 8 years old child in upper anterior region in mixed dentition and their management have been discussed. Update Dent. Coll. j: 2020; 10 (2): 26-28

A longitudinal study was undertaken to assess the persistance of occlusal features of the primary dentition at five years into the permanent dentition at 12 years. It was found that although there was a broad measure of predictability there was variation in detail which made forecasting unreliable in the individual patient. Generally speaking, incisal overjet and overbite changed very little between the two dentitions, but those changes which occurred were in both directions, and the direction of change could not be predicted. The sagittal relationships of the dental arch were also relatively stable, but where change occurred it tended to be in a Class II direction, with the mandibular arch becoming more retroposed in relation to the maxillary arch. The prediction of crowding of the permanent dentition from a simple count of spaces in the primary dentition, while generally reliable, was not accurate in every individual patient.

ABSTRACT Supernumerary tooth, also known as hyperdontia, is the condition where more than the normal number of teeth is present. By definition, the supernumerary teeth are the extra teeth present in the oral cavity. They can be found in the region of oral cavity, but are common in the maxilla than mandible.   Supernumerary teeth may be encountered by the dental practitioner as a chance finding on a radiograph or as the cause of an impacted central incisor. Their presence may give rise to a variety of clinical problems. Detection of supernumerary is best achieved through clinical and radiographic examination. This article shows the cases of supernumerary teeth in mandibular premolar region and maxillary third molar region along with its etiology, frequency, classification, complication, and management. Fusion is a developmental anomaly characterized by the union of two adjacent teeth. In this paper we report a rare case of fusion involving permanent mandibular second molar with supernumerary tooth. The rarity with which this entity appears, along with its complex characteristics, often makes it difficult to treat. The use of high-end diagnostic imaging modalities such as spiral computed tomography can help the clinician in making a confirmatory diagnosis and determining the treatment plan before undertaking the actual treatment. In the present case, we have used spiral computed tomography (SCT) for better understanding the complicated root canal morphology of the fused tooth and successful management of this rare case.
