The objective of this cross-sectional study was to investigatethe possibility that the saccule may contribute to human hearing. The forty participants included twenty healthy people and twenty other subjects selected from patients who presented with benign paroxysmal positional vertigo to Audiology Department of Hazrat Rasoul Akram hospital (Tehran, Iran). Assessments comprised of audiological evaluations, cervical vestibular evoked myogenic potentials (cVEMPs), recognition of spoken phonemes in white noise (Rsp in wn), and auditory brainstem response to 500 Hz tone burst (ABR500 HZ). Twenty affected ears with decreased vestibular excitability as detected by abnormal cVEMPs revealed decreased scores of Rsp in wn and abnormal findings ofABR500 HZ. Both unaffected and normal ears had normal results. Multiple comparisons of mean values of cVEMPs andABR500 HZbetween three groups were significant (P<0.05, ANOVA). The correlation between RSP in wn and p13 latencies was significant. The peak-to-peak amplitudes showed significant correlation to RSP in wn. The correlation between RSP in wn and the latencies of n23 was significant. In high-level of noisy competing situations, healthy human saccular sensation can mediate the detection of low frequencies and possibly help in cochlear hearing for frequency and intensity discrimination. So, all human hearing is not cochlear.

Since saccular projection is sound sensitive, the objective is to investigate the possibility that the saccular projections may contribute to auditory brainstem response to 500 HZ tone burst (ABR500 HZ). During the case-control research, twenty healthy controls compared to forty selected case groups as having chronic and resistant BPPV were evaluated in the audiology department of Hamadan University of Medical Sciences (Hamadan, Iran). Assessment is comprised of audiologic examinations, cervical vestibular evoked myogenic potentials (cVEMPs), and ABR500 HZ. We found that forty affected ears of BPPV patients with decreased vestibular excitability as detected by abnormal cVEMPs had abnormal results in ABR500 HZ, whereas unaffected ears presented normal findings. Multiple comparisons of mean p13, n23 latencies, and peak-to-peak amplitudes between three groups (affected, unaffected, and healthy ears) were significant. In conclusion, the saccular nerves can be projective to auditory bundles and interact with auditory brainstem response to low frequencies. Combine the cVEMPs and ABR500 HZ in battery approach tests of vestibular assessment and produce valuable data for judgment on the site of lesion. Regarding vestibular cooperation for making of wave V, it is reasonable that the term of ABR500 HZ is not adequate and the new term or vestibular-auditory brainstem response to 500 HZ tone burst is more suitable.

Background and Aim: Tinnitus is defined a phantom sound percept. Few studies have examined the occurrence of synaptopathy in tinnitus patients utilizing a battery of tests that indicate synaptopathy. This study aimed to investigate the role of synaptopathy in tinnitus production and compare the various characteristics of the auditory brainstem response (ABR) test and electrocochleography (ECochG) in normal-hearing people with and without tinnitus. Methods: This cross-sectional study was conducted on 34 normal-hearing individuals, 20 without tinnitus as controls (11 females and 9 males) and 14 with tinnitus (8 females and 6 men). The test components (amplitude, growth and slope of wave I, V/I ratio, action potential (AP) amplitude, and summating potential (SP)/AP) ratio were recorded during the ABR and ECochG tests for each subject. Results: The control group had higher mean values of amplitude, growth and slope of wave I, and AP amplitude compared to the tinnitus group, and this difference was statistically significant (p<0.05). The mean V/I ratio and SP/AP ratio were lower in the control group than in the tinnitus group, and this difference was statistically significant (p<0.05). Conclusion: The significant difference in the parameters of ABR and ECochG tests between normal-hearing people with and without tinnitus indicates that these parameters can be used to evaluate the presence of synaptopathy in tinnitus patients. These findings suggest the need for proper interpretation of the results of ABR and ECochG tests in tinnitus patients with a focus on the parameters indicating synaptopathy.

PURPOSE: To follow up the maturation of the auditory pathway in preterm infants small for gestational age (SGA), through the study of absolute and interpeak latencies of auditory brainstem response (ABR) in the first six months of age.METHODS: This multicentric prospective cross-sectional and longitudinal study assessed 76 newborn infants, 35 SGA and 41 appropriate for gestational age (AGA), born between 33 and 36 weeks in the first evaluation. The ABR was carried out in three moments (neonatal period, three months and six months). Twenty-nine SGA and 33 AGA (62 infants), between 51 and 54 weeks (corrected age), returned for the second evaluation. In the third evaluation, 49 infants (23 SGA and 26 AGA), with age range from 63 to 65 weeks (corrected age), were assessed. The bilateral presence of Transient Evoked Otoacoustic Emissions and normal tympanogram were inclusion criteria.RESULTS: It was found interaural symmetry in both groups. The comparison between the two groups throughout the three periods studied showed no significant differences in the ABR parameters, except for the latencies of wave III in the period between three and six months. As for the maturation with tone burst 0.5 and 1 kHz, it was found that the groups did not differ.CONCLUSION: The findings suggest that, in the premature infants, the maturational process of the auditory pathway occurs in a similar rate for SGA and AGA. These results also suggest that prematurity is a more relevant factor for the maturation of the auditory pathway than birth weight.

Background and Objectives: The present study aimed to compare thresholds of direct bone-conduction (BC direct) with those of behaviorally measured BC pure-tone audiometry (PTA) and objectively measured BC auditory brainstem response (ABR) to confirm the clinical feasibility of their relationships.Subjects and Methods: Young adults with normal hearing participated in the study to determine the thresholds from three measurements at four testing frequencies. In the BC direct, the vibrator of a bone-anchored hearing aid softband was placed on the right mastoid of each subject. In both PTA and ABR, a B71 bone oscillator was placed on the subject’s right mastoid. While the subject’s thresholds of BC direct and BC PTA were determined with a clinically routine 5-dB step procedure, BC ABR was conducted to determine the individual’s hearing sensitivity by a peak V of the waveform using tone-burst and click stimuli.Results: The BC direct showed a different pattern between low and high frequencies. Precisely, its thresholds were 13.25 and 12.25 dB HL at 0.5 and 1 kHz, respectively, but 19 and 19.75 dB HL at 2 and 4 kHz, respectively. A significant positive correlation existed between BC direct and PTA at 1 kHz, which was also correlated with ABR.Conclusions: Based on the current data, the thresholds of BC direct were similar to BC PTA at low frequencies and BC ABR at high frequencies. The thresholds of BC direct might be predictable at approximately 5 dB higher (or lower) than that in PTA, although a large data set is required for standardization.

<p class="abstract"><strong>Background:</strong> Chronic suppurative otitis media (CSOM) emerging as common hearing problems in the recent years with high prevalence requiring immediate attention. Hence, our study includes assessment of adult patients with CSOM using electrophysiological test, auditory brainstem response (ABR) and analysing the latency of ABR peaks in order to find the effect of CSOM on ABR latency with respect to shift in latency.</p><p class="abstract"><strong>Methods:</strong> The study followed cross sectional study design where data collected from March to September 2019 were used. A total of 50 subjects with unilateral CSOM were analysed. Descriptive statistics and paired t-test was used for statistical analysis of the data.  </p><p class="abstract"><strong>Results:</strong> The data was divided into 3 groups based on degree of hearing loss (mild, moderate and moderately severe). The Mean ABR peak latency was analysed and subjects showed a significant latency shift. Also, it was found that the magnitude of latency shift increased with increase in degree of hearing loss.</p><p class="abstract"><strong>Conclusions:</strong> Hence the study concludes that as the amount of conductive component increases the pure tone threshold deteriorates and ABR latency gets affected. Also, the morphology of ABR peaks on comparison to the normal hearing ear gets affected due to constant conductive pathology in the pathological ear.</p><p class="abstract"> </p>

Background: Tibial tubercle–trochlear groove (TT-TG) distance is often used as a measure of lateralization of the TT and is important for surgical planning. Purpose: To investigate if increased TT-TG distance measured on axial magnetic resonance images is due to lateralization of the TT or medialization of the TG. Study Design: Cross-sectional study; Level of evidence, 3. Methods: A total of 84 knees (28 normal [NK], 28 with trochlear dysplasia [TD], and 28 with patellar dislocation without TD [PD]) were examined. The medial border of the posterior cruciate ligament (PCL) was chosen as the central anatomic landmark. The distance from the TT to PCL (TT-PCL) was measured to examine the lateralization of the TT. The distance from the TG to the PCL (TG-PCL) was measured to examine the medialization of the TG. Between-group differences were investigated by use of 1-way analysis of variance. Results: The mean values for TT-TG distance were 8.7 ± 3.6 mm for NK, 12.1 ± 6.0 mm for PD, and 16.7 ± 4.3 mm in the TD group ( P < .01). The mean values for TT-PCL distance were 18.5 ± 3.6 mm for NK, 18.5 ± 4.5 mm for PD, and 21.2 ± 4.2 mm in the TD group ( P = .03). The mean values for TG-PCL distance were 9.6 ± 3.0 mm for NK, 7.1 ± 3.4 mm for PD, and 5.1 ± 3.3 mm in the dysplastic group ( P < .01). Conclusion: The present results indicate that increased TT-TG distance is due to medialization of the TG and not lateralization of the TT. Knees with TD had increased TT-TG distance compared with the knees of the control group and the knees with PD. The TT-PCL distance did not differ significantly between groups, whereas the TG-PCL distance declined with increased TT-TG.
