In the field of psychogeriatrics, the differential diagnosis of depression and dementia, as well as the treatment of depression and comorbid dementia, is an important issue. In this paper, the authors present the case of a 72-year-old woman with Cotard’s syndrome and frontotemporal dementia (FTD) who was admitted to a psychiatric hospital with delusions of negation accompanied by depressive symptoms. Pharmacotherapy over a 2-year hospitalization was unsuccessful, and she was subsequently transferred to our university hospital. A total of 18 sessions of electroconvulsive therapy released her from psychomotor inhibition, appetite loss, and Cotard’s delusions. The indication for electroconvulsive therapy in patients with dementia is discussed.

IntroductionThe liaison psychiatry (LP) is a feature used by the psychiatrist in order to improve the management of patients with mental suffering and/or mental disorder admitted to general hospital.ObjectivesTo characterize the epidemiological profile of hospitalized patients at the university hospital of the federal university of Sergipe (HU-UFS) submitted to LP.Methodsretrospective and observational study, through analysis of medical records of patients admitted in the wards of clinical medicine and surgery from the HU-UFS, in the period from January to December 2015, submitted to LP. The information collected fed a specific questionnaire developed by the authors, intended for research of socio-demographic data and clinical profile.Resultsthe frequency of request for IP was of 3.5%, with the majority of applications was performed by clinical medicine (71.2%), while the surgical clinic was responsible for 28.8%. The main reason for the request of LP was the presence of depressive symptoms (49.1%). There was a predominance of females (52.5%) and the mean age was 45.9 ± 14.6 years.ConclusionsThe frequency of request for LP was very low, suggesting a difficulty in the early detection of mental disorders by physicians. This finding points to an underreporting of cases, since the prevalence of depressive symptoms in hospitalized patients is over 50% in this institution.Disclosure of interestThe authors have not supplied their declaration of competing interest.

Objective: To demonstrate a case of anorexia nervosa in the elderly and to highlight the need for broadening of current diagnostic criteria. Clinical picture: First onset of anorexia nervosa in a 72-year-old woman following bereavement of her husband. Treatment: Nine treatments of electroconvulsive therapy. Outcome: Treatment resulted in remission of the depressive symptoms and improvement of eating behaviour. Conclusions: Anorexia nervosa does occur in the elderly and can be difficult to detect. Where comorbid depression exists it requires aggressive treatment.

Abstract Background SWI provides information and helps in the differential diagnosis of different pediatric neurological diseases as it accentuates the paramagnetic properties of intravascular hemoglobin in the brain, and is sensitive for other extravascular blood products such as de-oxy-hemoglobin, hemosiderin, calcifications and air. Objectives The purpose of this article is to present and discuss the susceptibility weighted imaging signal characteristics of the normal pediatric brain and those of a variety of pediatric brain pathologic abnormalities. We aimed to evaluate the influence of the SWI signal pattern in current pediatric neurological diseases we will make a comprehensive review and discuss the utility of SWI in pediatric neuroimaging in establishing the diagnosis, differential diagnosis, and also understanding the pathomechanism of various paediatric brain pathologies Materials and Methods This study was held in MRI imaging unit of radiology department of Ain Shams university hospital and included 34 patients pediatric age group with different neurological diseases focusing on stroke, haemorrhage, tumour, calcifications and vascular malformation. Results We included 34 pediatric patients with different neurological diseases that commonly seen in our department and in which 26 (76.5 % )of them gained more detailed information from SWI sequence than conventional sequences, we found that SWI shows better sensitivity 100 % compared to conventional sequences that were 79.4 % in other means SWI was clinically and statistically significant . Conclusion Its high susceptibility for blood products, iron depositions, and calcifications makes susceptibility-weighted imaging an important additional sequence for the diagnostic workup of pediatric brain pathologic abnormalities that should be routinely added in our department. Compared with conventional MRI sequences, susceptibility-weighted imaging may show lesions with better detail and higher sensitivity. Familiarity with the pediatric susceptibility-weighted imaging signal variance is essential to prevent misdiagnosis.

Caecal volvulus is an infrequent cause of acute surgical abdomen, where an abnormally mobile cecum twists on its own axis. It can lead to the development of closed-loop obstruction, small bowel ischaemia and perforation. Early recognition and prompt treatment is key; however, due to the rarity of this pathology, it is seldom listed as a differential diagnosis. Here, we present a single-centre case series of two patients presenting with caecal volvulus to an Emergency Surgery Unit at a University Hospital.

Electroconvulsive therapy is an efficacious treatment for some acute or drug resistant psychiatric disorders. However, there is some concern with memory impairment and brain damage and in order to decrease these complications, some changes to the technical aspect of electroconvulsive therapy were implemented during the last few decades. One of the major changes is the use of unilateral electrical stimulation on the non-dominant hemisphere which leads to less memory impairment and faster recovery time of cognitive functions. However, for optimal efficacy several variables have to be understood and integrated: electrodes placement, interelectrodes distance, waveform stimuli.

In an investigation of white, coloured, and black patients admitted to a psychiatric hospital, the prevalence and treatment of depression in schizophrenia was assessed and found to be 30% in group of acute, nuclear schizophrenics. While the prevalence was similar in the three groups, depression was clinically under-detected in black patients.
