Benign tumors of the esophagus are uncommon, representing <0.5% of esophageal tumors. Fibrolipomas are a subset of benign fibrovascular tumors, which present with dysphagia, odynophagia, and substernal fullness. These intraluminal tumors can become elongated and molded into a long pedunculated polyp by constant peristaltic movements. They can cause esophageal obstruction if large and long enough and can cause asphyxiation if they become lodged into the glottis. A barium swallow is the main diagnostic tool; treatment is surgical via a transoral, transcervical, or transthoracic approach. We report the excision of a large esophageal fibrolipoma through a transoral laser microsurgical approach.

Esophageal leiomyoma is uncommon. However, this tumor is the most common subepithelial tumor affecting the esophagus, comprising approximately two-thirds of benign esophageal tumors. Leiomyomas of the esophagus rarely cause symptoms when they are single and &#x3c;5 cm. The mainstay of treatment is esophagectomy for symptomatic patients. A 68-year-old male patient presented with progressive dysphagia for 4 months. The degree of dysphagia and chest discomfort was more severe on solid rather than liquid diet. The CT scan of the chest showed multiple well-defined, submucosal nodules, up to 1.9 cm in diameter located at the middle esophagus. The barium swallow study illustrated multiple, well-defined, smooth, semilunar filling defects along the mid to distal esophagus. Meanwhile, esophagogastroduodenoscopy revealed 8 smooth subepithelial masses. Moreover, the radial EUS showed multiple hypoechoic masses arising from the 4th layer, with some of the tumors connected to others as a horseshoe-like shape causing narrowed lumen. Last, high-resolution esophageal manometry revealed ineffective esophageal motility. We report a rare case of numerous esophageal leiomyomas which caused dysphagia as a result of both mechanical obstruction and hypomotility disorder. The histopathology confirmed the diagnosis of esophageal leiomyoma. Symptoms improved significantly after lifestyle modifications and adherence to dietary advice on the part of the patient.

A case of esophageal lipoma resembling a paraesophageal omental herniation is described herein. A 68-year-old male with symptoms of epigastric gas pains was noted. A barium swallow examination showed a large mass in the lower esophagus, which could move up and down along the esophagus easily and intermittently. Computed tomography scan of the chest and upper abdomen demonstrated a mass with fatty like density at the lower thoracic esophagus. A left posterior-lateral thoracotomy was performed. Histological examination revealed large mature lipocytes and lipoma of the esophagus was diagnosed. Diagnosis and treatment of lipoma of the esophagus were discussed. Lipomas of the esophagus are extremely rare benign tumors with an overall incidence of only 0.4%[1]. The leading clinical presentations of esophageal lipoma included dysphagia, mass regurgitation and signs of compression of the tumor. Preoperative diagnosis can be confirmed by CT, magnetic resonance imaging (MRI) and endoscopic ultrasonography. The present report describes a patient who suffered from an esophageal lipoma with symptoms of epigastric gas pains with a burning retrosternal sensation, which resembling a paraesophageal omental herniation. We also discuss the associated diagnosis and treatment of lipoma of the esophagus.

A 40 year old female presented with complaints of dysphagia, and regurgitation. On examination abdomen was soft, non tender, not distended and had no palpable organomeagaly. Patient was diagnosed of hiatus hernia after undergoing upper gastro intestinal scopy and barium swallow. Patient was planned for nissens laproscopic fundoplication. Intra-operatively patient was found to be having a large splenic hemangioma. Postoperatively patient had deep vein thrombosis for which intra-venous heparin was given. Post heparinization patient went into shock and ultrasound revealed a large collection in peri-splenic, spleeno-renal spaces. Spleenic hemangiomas are the 2nd common benign tumors of the spleen. Incidence of spleenic hemangioma is 0.02-0.16%. Spontaneous rupture of splenic hemangioma occurs rarely and is dreadly complication of splenic hemangioma. They may represent a congenital nevus. Patients present with spleenomegaly, abdominal pain, hypotension and dyspnea. A ultrasound would show a fluid collection in peri splenic areas. Emergency splenectomy is the treatment of choice for these patients.

The objectiveof the study is to evaluate the safety and efficacy of the using of minimally invasive endoscopic methods in the treatment of submucosal esophageal tumors.Material and methods.The study included 89 patients with benign tumors of the esophagus. Endoscopic ultrasonography (EUS) was performed to determine the esophageal wall layer from which the tumor grows the size of the tumor and its relationship with the surrounding organs.Results.Submucosal tunneling resection was performed successfully in 73 patients. The average operation time was 84.5 minutes. In 16 patients, the tumor was removed by submucosal endoscopic dissection (SED). The average duration of ESD was 39.4 minutes. Among this group of patients we faced nonstandard cases of practical interest: leiomyomas of the upper part of the esophagus, plexiform leiomyomas and multiple leiomyomas. Each of the above types had its own specificity in diagnosis, treatment and postoperative management of patients.Conclusion.Submucosal esophageal tumors vary in types of growth, sizes, number of tumors, localization and its relationship with the surrounding organs. Scrupulous diagnosis of each case is necessary which will allow choosing the most appropriate treatment tactics for the patient.

Abstract We report on a case of ileal lipoma that prolapsed into the ascending colon and was resected by laparoscopy-assisted surgery. A 31-year-old male Japanese patient was admitted to our hospital because of hematochezia and anemia. Colonoscopy revealed a pedunculated polyp arising from the ileum. The surface was covered with slightly edematous mucosa. Abdominal computed tomography showed a low-density mass in the ascending colon. A diagnosis of pedunculated ileal lipoma with intussusception was made, and laparoscopy-assisted surgery was performed. The intussusception was reducted by resection of the lipoma. The surgical specimen was a 40 × 30 × 25 mm round tumor with a long stalk 11 cm in length. Microscopic examination of the specimen revealed ileal lipoma. Laparoscopic surgery is recommended for benign tumors of the small intestine because it is minimally invasive.

Lipomata of the larynx and the hypopharynx are very rare benign lesions; as they look macroscopically like retention cysts, their diagnosis is usually made after surgery. Sometimes, lipomata of the hypopharynx become very large and life-threatening. Plain fluoroscopy and fluoroscopy aided with barium often fail to depict the lesion if it is localized subepiglottically; standard barium swallow examination and computed tomography allow an accurate diagnosis to be made in many cases, although magnetic resonance imaging is still more accurate and allows not only a more specific diagnosis of the lesion, but also a better depiction of the origin of the frequently pedunculated tumor and its extension into the parapharyngeal space. This paper adds 6 cases of hypopharyngeal and 1 case of laryngeal lipoma to the literature and discusses modern diagnostic and therapeutic strategies.

Aim: Evaluate clinical pattern, diagnosis, surgical management and outcome of Gastric outlet obstruction in children post accidental ingestion corrosive (sulfuric acid of battery) , also role  of continuing public education to reduce and avoid  incidence these unfortunate events. Patients & method: prospective study for 20 patients’ children with isolated gastric outlet obstruction post corrosive sulfuric acid ingestion was admitted and operated in Saudi hospital at Hajjah Yemen in period April 2015_April 2021.  The study evaluates patients’ demographic data, clinical presentation, diagnosis and management, also included in this study the Comparison proportions between two surgical procedures used for management this patient (group (A) pyloric resection and gastroduodenostomy and group (B) gastrojejunostomy with Braun anastomosis) with P value >0.05 was non-significant. Results: Twenty patients with gastric outlet obstruction (12 boys and 8 girls), (Mean age was 4.5). Main presenting symptom recurrent attacks vomiting and loss of weight. The interval between acid ingestion and presentation ranged from 3_4weeks(mean=3.5weeks). The History, Barium swallow and meal used as diagnostic tool because safe, inexpensive. Surgical procedure included group (A)pyloric resection and gastroduodenostomy (n=10) and group (B) gastrojejunostomy with Braun anastomosis (n=10). Comparison between the two surgical procedures show all safe, less morbid, no mortality and no significant difference in end outcome result follow-up period up 2 years, all patients are symptoms free and gained adequate weight. Conclusion:  for patients children post corrosive accident ingestion with gastric outlet obstruction The History, barium swallow and meal used as diagnostic tool. Early surgery between 4_6 weeks by resection or bypass remains the treatment of choice with best result. To prevent these unfortunate events by encourage the Family education about clear labelling of dangerous substances and keeps this substance Far from reaching the kids. Within the last few years, adrenal cortical tumors with features concordant with the diagnostic criteria attributed to oncocytomas have been reported. To date, only nine reported cases exist in the literature. This report is the tenth case presentation of a presumptively benign neoplasm of the adrenal gland with a rare differentiation. Oncocytomas are well recognized benign tumors of the thyroid, parathyroid, and salivary glands and of the kidney. Other organs also give rise to these types of tumors, however with less frequency than the former sites. The characteristics generally used to classify a tumor as an oncocytoma include the following criteria: the tumor is 1) usually a solitary circumscribed mass with no gross nor microscopic evidence of metastasis (no tissue nor vascular invasion), 2) fairly bland in terms of mitotic activity and nuclear morphology, and 3) composed of large eosinophillic cells in which the cytoplasm is packed full of mitochondria (Figure 1).

Elicited imitation tasks are frequently used as a diagnostic tool in evaluating children with communication handicaps. This article presents a scoring procedure that can be used to obtain an in-depth descriptive analysis of responses produced on elicited imitation tasks. The Elicited Language Analysis Procedure makes it possible to systematically evaluate responses in terms of both their syntactic and semantic relationships to the stimulus sentences presented by the examiner. Response quality measures are also included in the analysis procedure.
