Anaphylaxis is a life-threatening event that can occur anytime during pregnancy. It has been reported following administration of various substances with adverse maternal and neonatal consequences. It should be considered in the differential diagnosis of intrapartum collapse. We encountered a case of severe anaphylactic reaction following a routine cesarean section. It is very important that all members of the perinatal team are aware of early recognition and management of anaphylactic reaction. We think that it is important to highlight this as a further case report of severe anaphylactic reaction to a colloid solution and discuss the pathophysiology and management.

Anaphylactic reaction is a common problem encounter by the emergency physician. Although the drug treatment and management of anaphylaxis had been well published in the literature, the emergency management of anaphylaxis is still unsatisfactory. Severe anaphylactic reaction and death is not uncommonly reported. This article reviews the pathophysiology, aetiology, clinical manifestation, diagnostic approach, management algorithm and prevention of anaphylaxis with particular emphasis on the emergency physician's perspective. The clinical manifestation of anaphylaxis is significantly variable in terms of severity, onset and progression, which imposed a diagnostic challenge to emergency physician. An awareness that severe anaphylaxis can be rapidly fatal is essential for those working in the emergency room. Life threatening clinical features such as laryngeal oedema, bronchospasm and circulatory collapse must be recognize early and treated aggressively as favourable prognosis is well documented. The key to success in managing anaphylaxis emergency depends on the early recognition of anaphylaxis reaction, initial assessment, anticipation of deterioration and finally prompt and aggressive support of airway, oxygenation, ventilation and circulation. Adrenaline is the cornerstone of the treatment modality in anaphylaxis. This is currently underused, although, it is safe and almost always effective. At last prevention of subsequent episode of anaphylaxis should be considered a priority for emergency physician before discharging patient.

Smooth muscle tumors arising in the vulva are rare. Leiomyosarcoma is the most common variant of vulvar sarcoma, and very few cases have been reported during pregnancy. A 36-year-old woman presented with a progressively enlarging vulvar mass during pregnancy, diagnosed as a Bartholin's gland cyst. The lesion was resected at 38 weeks of gestation during cesarean section and diagnosis of myxoid leiomyosarcoma of the vulva was made. Six weeks later the patients were referred to our center and submitted to wide vulvar excision with groin lymph node dissection that revealed the presence of a small residual focus of leiomyosarcoma. At 30 months of follow-up the patient was well without any sign of recurrent disease. Leiomyosarcoma should be included in the differential diagnosis of vulvar masses; progressively enlarging vulvar lesion should be biopsied even during pregnancy. Leiomyosarcoma should be considered in the differential diagnosis of vulvar mass.

Dieulafoy lesion is a rare cause of massive GI bleeding. It’s an abnormal sub-mucosal artery protruding from a minute mucosal defect (≤3 mm). A 31 yearold male presented with complaints of hematochezia. Preliminary investigations failed to locate the exact source of bleed. Enteroscopy suggested distal ileal bleed. At laparotomy, an ulcerated nodular lesion, approximately 0.5 cm was identified in distal ileum. 30 cm of ileum along with mesentery was resected. Histology revealed it to be Dieulafoy lesion. Dieulafoy lesion is uncommon but one of the causes of obscure gastrointestinal bleeding that could result in treacherous and life-threatening gastrointestinal haemorrhage. This lesion is difficult to identify and high index of suspicion is required to make diagnosis. Hence, it should be considered in the differential diagnosis of active GI bleeding. The definitive diagnosis is based only on histopathology.

Life-threatening hyperkalemia is usually the result of renal or adrenal disorders although on some occasions it is an iatrogenic mishap. An alternate etiology of hyperkalemia is an unusual dietary regimen followed by the patient which may or may not be known to his physician. The present case occurred when the parents of a newborn infant followed the advice given for treatment of colic in the popular nutrition book, Let's Have Healthy Children.1 CASE REPORT This 27-day-old boy weighed 4.36 kg at birth, born to a healthy white gravida 2, para 0, abortus 1 by cesarean section for face presentation.

Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening medical condition characterized by hyperphagocytosis secondary to an inappropriate over-activation of macrophages and lymphocytes that driven by excessive cytokines production which resulted in cellular destructions. It can arise de novo as a result of an autosomal recessive genetic disorder, or in the background of an infection, malignancy or autoimmune disease. Dengue fever is one of the uncommon causes of infection related secondary HLH. Here, we present a case of a Dengue associated HLH which was successfully treated with intravenous methylprednisolone and immunoglobulin G. In conclusion, the purpose of this case report is to illustrate the importance of early recognition and prompt initiation of the appropriate treatment for HLH suspected patient whom otherwise has high mortality rate. Background Pemphigoid nodularis is a rare clinical variant of bullous pemphigoid characterized by overlapping clinical features of both prurigo nodularis lesions and bullous pemphigoid blisters. The condition appears to be more common in females and is often resistant to treatment. Objective To raise awareness of this rare variant of bullous pemphigoid. Methods Case report. Results and Conclusion Dermatologists should include this variant in differential diagnosis of prurigo nodularis because early recognition can lead to an effective treatment for the prurigo component.

Hematological abnormalities are frequently encountered during treatment with antipsychotic drugs. Most of these are mild and of no clinical significance, but in a small minority of patients, hazardous, potentially life-threatening hematological effects, including leucopenia, neutropenia, agranulocytosis, thrombocytopenia, anaemia , leucocytosis, thrombocytosis, eosinophilia and altered platelet function. (1). Clozapine is associated with several well-known abnormalities of blood cell count (2), but some case reports associate novel antipsychotics, such as risperidone with leucopenia and agranulocytosis (3). This report describes a case of leucopenia under treatment with Oral Risperidone , suggesting the necessity of early recognition of leucopenia in order to prevent occurrence of potentially life-threatening agranulocytosis. ( 1) .
