BACKGROUND: This study examined the epidemiology, antibiotic susceptibility and serotype distribution ofStreptococcuspneumoniaeassociated with invasive pneumococcal disease (IPD) in British Columbia.METHODS: Six hospitals and one private laboratory network participated in a prospective, sentinel laboratory based surveillance study of IPD, between October 1999 and October 2000. At each site,S pneumoniaeisolates were collected and epidemiological data were gathered using a structured questionnaire, for all cases of IPD meeting the study case definition. Isolates were serotyped and tested for antimicrobial susceptibility. Bivariate associations were analyzed and multivariate logistic regression was used to identify independent risk factors associated with hospitalization or death.RESULTS: One hundred three reports and isolates were collected. Seventy-nine per cent of cases were community-acquired, 64% required hospitalization and 5% died. Cases with one or more assessed risk factor for IPD and of female sex were independent variables associated with hospitalization or death. One-third of isolates had reduced penicillin susceptibility and 96% of these represented serotypes contained in the 23-valent pneumococcal polysaccharide vaccine (PPV-23). Overall, 89% of serotypes identified are included in the PPV-23 vaccine and 88% of isolates from children under five years of age are found in the 7-valent pneumococcal conjugate vaccine (PCV-7). Forty-one per cent of cases qualified for publicly funded pneumococcal vaccine and 34% of eligible persons were vaccinated.CONCLUSIONS: Overall, pneumococcal serotypes associated with IPD in this study closely matched serotypes included in PPV-23 products currently licensed in Canada. Most serotypes associated with IPD in children under five years of age are included in a recently licenced PCV-7. One third of isolates demonstrated reduced penicillin susceptibility, most involving serotypes included in PPV-23. Effective delivery of current public health immunization programs using PPV-23 and extending protection to infants and young children using the PCV-7 will prevent many cases of IPD.

AbstractDiseases caused by Streptococcus pneumoniae are mostly preventable infections by current immunization programs. The objective of this study was to evaluate the impact of the introduction of the heptavalent and the 13-valent pneumococcal conjugate vaccines (PCV7 and PCV13) on the burden of pneumococcal disease and on the serotype distribution of S. pneumoniae causing invasive pneumococcal diseases (IPDs) in the pediatric age over a 5-year study (from January 2008 till December 2012). We observed a decrease in IPD rate in children after PCV13 introduction despite increases in nonvaccine serotype (NVS) rates in 2011. Nevertheless, from 2012, an increase in IPD rates due to non-PCV13 serotypes was observed.

The introduction of pneumococcal conjugate vaccines PCV7 and PCV13 led to decreases in incidence of pediatric invasive pneumococcal disease (pIPD) and changes in serotype distribution. We evaluated the consequences of higher vaccine uptake after the introduction of PCV13 in the National Immunization Plan (NIP) in 2015. Besides culture and conventional serotyping, the use of molecular methods to detect and serotype pneumococci in both pleural and cerebrospinal fluid samples contributed to 30% of all pIPD (n = 232) in 2015–2018. The most frequently detected serotypes were: 3 (n = 59, 26%), 10A (n = 17, 8%), 8 (n = 16, 7%) and 19A (n = 10, 4%). PCV13 serotypes still accounted for 46% of pIPD cases. Serotypes not included in any currently available conjugate vaccine (NVT) are becoming important causes of pIPD, with the increases in serotypes 8 and 33F being of particular concern given the importance of serotype 8 in adult IPD and the antimicrobial resistance of serotype 33F isolates. This study highlights the importance of using molecular methods in pIPD surveillance since these allowed a better case ascertainment and the identification of serotype 3 as the leading cause of pIPD. Even in a situation of vaccine uptake >95% for 3 years, PCV13 serotypes remain important causes of pIPD.

Diarrhea is still a public health problem in the world and the mostly occur in children under five years. At the moment the number of diarrhea-caused mortality was 3.8 per 1000 per year by 3.2 episodes per year in children under five years. This research aims to analyze the association between hand-washing habits and toilet use with the incidence of diarrhea in children under five years. This is an observational analytic with cross sectional study. Population of the research was a children's mother took her children to the public health center of Sekardangan Sidoarjo Subdistrict. The number of samples taken as many as 58 mother of children under five years. Data collection was done with interviews to parents of children and the observations of the toilets. Technique of data analysis using chi-square test or Fisher's exact test. On the results of the research there is a significant association the habit of hand washing (p = 0.006) and toilet use(p = 0.014) with the incidenceof diarrhea in children under five years. Conclusions of the research is the habit of hand washing and toilet use had a significant association with the incidenceof diarrhea in children under five years. Advice that can be given to the mother of a children under five years is to wash the hands before and after defecating and before preparing food for children. Keywords: diarrhea, hand washing, toilet use, observational, children under five years

Abstract Background: Intimate partner violence (IPV) against women is a pervasive and significant public health problem. The phenomenon is linked to adverse health effects for women and children. Mothers of young children in Malawi can be particularly at risk because of gender-based power imbalances. The objectives of this study were to examine the prevalence and the risk factors of IPV perpetrated by the current or recent partner against mothers of children under five years of age in rural Malawi.Methods: A multistage cross-sectional study design was used. A sample of 538 mothers of young children was randomly selected from postnatal clinics in Dowa district. The WHO's Violence against women instrument was used to collect data. Logistic regressions were used to determine risk factors that were associated with IPV against mothers. Results: The prevalence of IPV controlling behavior, psychological, physical, and sexual violence were 74.7%, 49.4%, 43.7% and 73.2% respectively. In multivariate analyses, mothers whose partners had extra marital affairs were more likely to experience controlling behavior (AOR: 4.97, 95% CI: 2.59-8.55, P<0.001), psychological (AOR: 2.14, 95% CI: 1.486-3.472, P<0.001) and physical (AOR: 2.29, 95% CI: 1.48-3.94, P<0.001) violence than mothers whose partners did not have extra marital affairs. Mothers whose partners consume alcohol were more likely to experience sexual violence (AOR: 2.00, 95% CI: 1.17-3.41, P<0.001) than mothers whose partners did not drink. Finally, mothers who spent more than 30 minutes drawing water were at greater risk of experiencing IPV than mothers who spent less than 30 minutes.Conclusion: This study found a significantly higher prevalence of IPV against mothers in rural Malawi compared to the national prevalence. Programs aimed at reducing the partners’ potential risk behaviors identified in this study are recommended. Public health programs that support increased household access to safe water are also recommended to help mitigate IPV against mothers.
